OBJECTIVE:To provide reference for clinical rational drug use.METHODS:The content of Levocarnitine Injection from two manufactories in 3 different dissolvants(5%Glucose,0.9%Sodium Chloride Injection,and Fructose Injection) within 6 h was determined by HPLC.The change of microparticle was measured by light blockage method in accordance with China Pharmacopeia(2005 edition),and the appearance and pH of the mixtures were observed as well.RESULTS: The changes in appearance,turbidance,pH value,content and microparticle were not significant.CONCLUSION: Levocarnitine Injections from two manufactories were stable within 6 h after mixing with 3 different dissolvants(5%Glucose, 0.9%Sodium Chloride Injection,and Fructose Injection).

With a long duration of the CHB, patients who have economic pressures and physical problems will get changes in their psychological status, which are involved in the development of the disease. Moreover, poor thera-py compliance and insight of their disease, negative mood of anxiety and depression also have effects the prognosis of disease. Therefore, mental factors play a vital important role in the development and prognosis of CHB patients, health education and mental intervention are the most effective way to avoid the psychological disorder of patients.

Objective To investigate the association between matrix metalloproteinase 9(MMP 9) and type 2 diabetic macroangiopathy and explain the mechanism of Simvastatin protective effects on blood vessel.Methods The serum MMP 9 was determined by ELISA in 40 healthy controls and 80 type 2 diabetes(including 40 type 2 diabetic macroangiopathy and 40 type 2 diabetes without complications) before and after Simvastatin treatment.Results The serum MMP 9 in type 2 diabetic macroangiopathy was higher than that in type 2 diabetes without complications and healthy controls.Serum MMP 9 was correlated positively with TC,LDL C,Ox LDL and blood pressure and correlated negatively with HDL C. After Simvastatin treatment,the levels of serum MMP 9 were reduced and serum TC,LDL C and Ox LDL were also decreased.Conclusion MMP 9 may play an important role in the development and progression of type 2 diabetic macroangiopathy.The protective effects of Simvastatin on blood vessel partly attribute to its lowering of serum MMP 9.

Objective:To investigate the clinical effects of health and safety management in elderly patients receiving gastroenteroscopy under sedation.Methods:From February 2018 to November 2019, 126 elderly patients admitted to the cadre medicine and gerontology department of our hospital to undergo gastroenteroscopy under sedation were included as research subjects.According to the digital random number method, they were divided into the control group and the test group(n=63, each group). The control group was given routine management, while the test group was given routine management plus health and safety management.Clinical effects and safety were analyzed and compared between the two groups.Results:There was no significant difference in scores of the Hamilton anxiety scale(HAMA)and the Hamilton depression scale(HAMD)between the two groups before intervention.After intervention, the scores of HAMA and HAMD were lower in the test group than in the control group[(39.2±3.1)scores vs.(48.6±2.1)scores, (40.1±4.1)scores vs.(52.6±5.5)scores, t=19.926 and 14.463, both P=0.000]. The incidence of postoperative complications was lower in the test group than in the control group(7.94% or 5/63 vs.19.05% or 12/65, χ2=14.209, P=0.002). The differences between maximum and minimum values of heart rate(HR), diastolic blood pressure(DBP), respiratory rate(RR)and systolic blood pressure(SBP)were lower in the test group than in the control group(all P<0.05). The length of the procedure was shorter and the anesthetic dose was lower in the test group than in the control group(9.11±2.56 min vs.13.56±3.05 min, 15.35±2.66 ml vs.21.67±6.15 ml, t=8.870 and 7.486, both P=0.000). Patient satisfaction was higher in the test group than in the control group( χ2=16.383, P=0.014). Conclusions:Health and safety management can improve emotional health, stabilize vital signs and enhance satisfaction in elderly patients undergoing gastroenteroscopy under sedation.

Radiation?induced heart disease ( RIHD) is a common type of radiation?induced damages in chest radiotherapy. There are no obvious short?term symptoms in patients with RIHD. However, RIHD causes irreversible permanent damages to the heart over time, which undermines the quality of life. Patients with severe RIHD even have a risk of death from myocardial infarction caused by coronary atherosclerosis. This paper summarizes the research advances in epidemiology, diagnosis, mechanisms of radiation?induced injury in various parts of the heart, radiotherapy techniques, and treatment. Reduction in radiation range and dose, early diagnosis, and early treatment are recommended for patients to reduce heart injury and improve the quality of life.

Objective:To explore the influence of different blood glucose level at hospitalization on no-reflow and prognosis in patients with acute myocardial infarction (AMI),but without diabetes after emergency percutaneous coronary intervention (PCI).Methods:A total of 678 non-diabetic AMI patients undergoing PCI were selected.Ac-cording to blood glucose level at hospitalization,patients were divided into normal group (n=312),blood glucose level higher group (higher group ,n=201)and blood glucose level extremely high group (extremely high group,n=165).Clinical characteristics,no-reflow after PCI and incidence of cardiovascular events within 30d after PCI were compared among three groups.Results:Compared with normal group and higher group,there were significant rise in age,percentages of >1 location of myocardial infarction and Killip's ≥ class II,white blood cell count instant af-ter hospitalization,peak value of creatine kinase (CK)and CK-isoenzyme (CK-MB)in extremely high group,P <0.01 all;Compared with normal group,the age,peak value of CK and CK-MB significantly rose in higher group (P<0.01 all).Along with blood glucose rose,incidence rate of no-reflow significantly increased (17.0% vs.38.8%vs.49.7%),left ventricular ejection fraction (LVEF)on 30d after PCI gradually reduced [(68.6± 5.7)% vs. (62.2±6.4)% vs.(55.6±6.9)%]in above-mentioned 3 groups in turn,there was significant difference between any two groups,P <0.05 or <0.01.Conclusion:Stress hyperglycemia possesses great influence on no-reflow and prognosis in non-diabetic AMI patients after PCI.

Aim To investigate the effect of resveratrol (Res)on septic mice and LPS-insulted H9c2 cells,as well as its involved mechanisms.Methods By use of a mouse cecal ligation and puncture-induced septic model (CLP),the survival of septic mice was evalua-ted after resveratrol treatments.H9c2 cells were insul-ted by LPS and then treated with resveratrol,the mR-NA expressions of TNF-α,SIRT1 and other class III HDAC members were detected using RT-PCR and real-time PCR,Finally,the protein levels of nuclear p65, an important subunit of NF-κB,were measured in H9c2 cells using Western blot assay,to reveal the effect of resveratrol on LPS-induced nuclear transloca-tion of NF-κB.Results Compared with the control septic animals,intraperitoneal injection of resveratrol (1 or 5 mg·kg -1 ·d -1 )significantly increased the survival of septic mice.Furthermore,resveratrol signif-icantly increased mRNA expressions of SIRT1,SIRT2, SIRT6 and SIRT7 in LPS-insulted H9c2 cells.Res-veratrol also remarkably inhibited LPS-induced nuclear translocation of NF-κB.Conclusion An appropriate dose of resveratrol protects septic mouse hearts from the injury induced by LPS through the activation of SIRT family members and the inhibition of NF-κB pathway.

Objective To discuss the relationship between serum folic acid (Fa),homocysteine (Hcy),Vitamin B12 (VB12) in pregnent women as well as neonatal umbilical cord blood and fetal growth restriction.Methods We selected 44 delivery mothers and their infants inpatient from February to November in 2010.Serum Hcy,Fa,VB12 were measured by enzyme-linked immunosorbent assay.According to the birth weight neonates were classified into fetal growth restriction (FGR),appropriate for gestational age (AGA),large for gestational age (LGA).We recorded fetal birth weight,height,head circumference,abdomen circumference,placenta weight,body mass index (BMI),Rohrer index,delivery method,gender and evaluated neonatal growth state.Results ( 1 ) Serum Fa,VB12 of pregnant women in FGR group were less than those in AGA and LGA group (P ＜0.05).Hcy of pregnant women in FGR group was higher than that in AGA,LGA group (P ＜0.05).Serum VB12 of umbilical cord blood in FGR group was less than that in AGA,LGA group.Hcy in FGR group was higher than that in AGA,LGA group ( P ＜ 0.05).Serum Fa in pregnant women was correlated positively with Fa,VB12 in umbilical cord blood ( P ＜0.01 ),as well as serum VB12 in pregnant women was correlated positively with that in umbilical cord blood ( P ＜ 0.01 ).( 2 ) Serum Hey in pregnant women and umbilical cord blood was negatively correlated with neonatal growth parameters (P ＜0.01 ).Serum Fa in pregnant women was positively correlated with birth weight ( P ＜ 0.05 ).Serum VB12 in pregnant women was positively correlated with neonatal head circumference and abdomen circumference ( P ＜0.05).(3) Placenta weight in FGR group was less than that in AGA,LGA group (P ＜0.01).There wasn't correlation between placenta weight and birth weight,height,head circumference,abdomen circumference,BMI in FGR group (P ＞ 0.05 ).There is a significant positively correlation between placenta weight and birth weight,height,head circumference,abdomen circumference,BMI in AGA,LGA group (P ＜0.01 ).(4)Rohrer's body index: In FGR group the index was less than 2.00 at the gestational age of ≤37.The index was less than 2.20 at the gestational age of ＞ 37.The ratio of height and head circumference was more than 1.36 in three groups.Conclusion Lack of the Fa and VB12 may induce hyperhomocysteinemia in period of pregnancy.FGR may be corelated with the increasing Hcy level.Serum Fa,VB12 and Hcy had important influence on the fetal physical condition at the later period of pregnancy.Placenta weight may play an important role in the fetal growth and development.

OBJECTIVE:To promote standard management of drugs in ICU area. METHODS:The change of work mode in ICU area of our hospital was analyzed and compared after the application of automated drug dispensing system(ADDS)as well as the change of related indicators 3 months before and after the application of ADDS;the effect of ADDS was evaluated. RESULTS& CONCLUSIONS:After the application of ADDS,the management of base drugs changed from open management to intelligent, closed-off and authority management;drug storage and cost management became more scientific,and staff allocation became more optimal. Under the condition of zero difference in the number of medical orders,initial medication time shortened from 170 min to 131 min (P<0.05),time of daily drug preparation and drug repercussion shortened from (81.20 ± 3.56)min to (30.04 ± 7.55) min,and (9.18 ± 2.79)min to (6.47 ± 4.66) min (P<0.05). The application of ADDS can improve work efficiency,standardize drug management,promote medical safety,and provide reference for initiating a new mode of drug management in wards.
