1.Clinical characteristics of 13 cases of neonatal acute osteomyelitis

Objective To study the clinical and imaging features of neonatal acute osteomyelitis.Method From 2011 to 2016,the clinical feature,laboratory results,pathogen,imaging features,treatment and prognosis of neonates who were diagnosed with acute osteomyelitis in NICU of People's Hospital of Xinjiang Uygur Autonomous Region were retrospectively studied.Result A total of 13 cases were enrolled during the study period,5 males and 8 females,with onset age of 7 ~30 d.8 cases had different degrees of fever and 5 of them showed dyspnea;8 cases had elevated skin temperature and swelling at the affected area;7 cases presented with irritability;6 cases had anorexia;all cases had elevated white blood cell count.Blood culture were taken immediately after their admission,and culture from the pus were taken for some patients.5 cases had positive blood culture and Staphylococcus aureus was the main pathogen.6 cases had positive pus culture,and 4 cases showed gram positive (G +) bacteria and 2 cases gram negative (G-) bacteria.Humerus and tibia were the main foci of the infection.4 cases had X-ray examination within 7 days of onset showing soft tissue swelling and no sign of bone destruction.13 cases had X-ray examination at 7 to 14 days of disease.Among them only 1 case showed soft tissue swelling,and the rest 12 cases had different levels of bone destruction and periosteal reaction.2 cases had X-ray examination after 14 days of disease,showing stiffening ring,inflammatory changes and periosteal reaction.All patients were treated with broad-spectrum antibiotics after admission,and the affected limbs were immobilized.3 cases had further surgical treatment.11 patients recovered and were discharged,1 patient was treated at local hospital after diagnosis and 1 patient refused further treatment due to serious complications after 35 d of hospital stay and was lost on follow-up.Conclusion The clinical manifestation of neonatal osteomyelitis is atypical,the radiological and pathogen examination should be carried out once osteomyelitis is suspected.Appropriate antibiotics and timely surgery can prevent further damage and long-term sequela.
2.P-selectin and ischemic cerebrovascular disease

P-selectin is also known as CD62. It promotes the occurrence of ischemic cerebrovascular disease by mediating the activation of endothelial cells and platelets as well as the processes of the formation and development of atherogenesis. A number of studies have confirmed that P-selectin plays important roles in the occurrence and development of the risk factors (such as hypertension, diabetes, hypercholesterolemia, heart disease, smoking, alcohol abuse and hyperfibrinogenemia) for ischemic cerebrovascular disease. It remains to be confirmed by further studies whether P-selectin can be used as an independent risk factor for predicting the occurrence of ischemic cerebrovascular disease.
3.Analgesic effect analysis of preemptive analgesia with oxycodone hydrochloride in abdominal operation

Objective To evaluate the analgesic effect of oxycodone hydrochloride and morphine in preemptive analgesia for abdominal surgery.Methods The 200 patients who received abdominal operation were randomly divided into the oxycodone hydrochloride group (group O) and the morphine group(group M) with 100 patients in each group,and patients of the two groups were given oxycodone hydrochloride and morphine in preemptive analgesia respectively .The VAS, OAA/S and BCS 48 hours after surgery , the frequency of the use of PCIA in 24 hours,the satisfaction of patients ,and the occurrence rate of complications were compared between the two groups .Results There was no significant difference between the two groups in VAS ,BCS,OAA/S,frequency of the use of PCIA and patients satisfaction (P>0.05).The occurrence rate of nausea,vomit,and itch in group O were much lower than that in group M (P<0.05).The occurrence rate of drowsiness in group O was higher than that of group M(P<0.05).Conclusion Compared with morphine,oxycodone hydrochloride has the same analgesia efficiency and less side effect occurrence rate for patients with abdominal operation .
4.THE INFLUENCES OF VITAMIN A DEFICIENCY ON CYTOKINES RESPONSE OF INTESTINAL MUCOSA IN RATS

Objective To understand the mechanism of vitamin A deficiency(VAD) impairing the mucosal immunity and the influences of VAD on cytokines response of intestinal mucosa in rats. Method The rat model with VAD was established,and intestinal infection was induced by typhoid salmonella orally administered. The fresh ileum tissue samples were taken,and ELISA and RT-quantity PCR were used to detect the production of cytokines,including IFN-?,IL-2,IL-4,IL-6,IL-10 and IL-12,by intestinal mucosa in protein and mRNA levels. Results IL-12,the cytokine produced by dendritic cells,was markedly increased in VAD rats,and the level was further raised when the VAD rats were complicated by intestinal infection. The Th1 cytokines,IFN-? and IL-2,were reduced in VAD rats. IL-4,the main Th2 cytokine,was significantly decreased after intestinal infection,especially in VAD rats. IL-6 mRNA was lower in VAD rats and after intestinal infection compared with that of the VA normal control. IL-10 mRNA was also declined in VAD rats,and more markedly reduced after intestinal infection. Conclusion The regulation of VAD onproduction of cytokines in local intestinal mucosa is likely to be an important mechanism of VAD affecting intestinal mucosal immunity.
5.Design of portable 12-lead digital ECG with low power consumption.

The design of portable and low power consumption 12-lead ECG is based on the digital signal processor TMS320C5515 and the analog front end ADS1298. The ADS1298 collects the ECG signals and deliver them to TMS320C5515. The preprocessed ECG signals are displayed real-time on a LCD and can be stored without compression for a long time. The ECG signals can also be sent to an up computer by a USB connector so that ECG data can be analyzed offline. The system has small volume, high precision and low power consumption.

6.Evaluative merit of brain blood perfusion image of single photon emission computed tomography on local brain blood distribution in neonatal hypoxic ischemic encephalopathy

BACKGROUND: Neonatal hypoxic ischemic encephalopahty(NHIE) is a common neonatal disease, which could cause cerebral palsy, inferior intelligence and convulsion. At present, usual examinations for this disease are CT.MRI and single photon emission computed tomography(SPECT), etc.OBJECTIVE: To investigate the character of SPECT image of NHIE, and compared with CT and MRI as well to provide a theoretical gist for function surveillance and early intervention of NHIE.DESIGN: An observatory comparative study based on NHIE patients and normal neouates as control.SETTING: Department of nuclear medicine in a medical university hospital.PARTICIPANTS: Thirty neonates hospitalized in the Department of Pediatrics, Affiliated Hospital of Jining Medical College between September 1998 and October 2000 were selected as observation group with 23 males and 7 females. Another 10 normal neonates including 7 males and 3 females were randomly selected from same period into control group.METHODS: SPECT brain blood perfusion was conducted in the subjects of two groups and the results were compared with that of CT and MRI.MAIN OUTCOME MEASURES: ① comparison of SPECT brain blood image features between NHIE and normal neonates; ② comparison of SPECT images and CT, MRI results. RESULTS: In 22 NHIE cases who received SPECT, CT and MRI simultaneously, mild group had 7 cases, and positive cases of SPECT, CT or MRI was 7, 3 or 5; moderate group had 10 cases, and positive cases of three methods was 10, 8 or 9; severe group had 5 cases, the all of them were positive in all three examinations. The total positive detectable rate of SPECT, CT or MRI was 100, 72.73% or 86. 36%. There was no significant difference of sensitivity between SPECT and MRI in mild group, while there was significant difference between SPECT and CT(x2 = 3.95, P < 0.05);there was no significant difference among three methods in either moderate group or severe group(x2 =0. 83, P > 0.05).CONCLUSION: SPECT brain blood perfusion image is a quite sensitive method for the surveillance of NHIE, which can provide imageological reference for the evaluation of its prognosis and early intervention.
7.Investigation on hemorheologic changes in patients with chronic hepatitis B and its correlation with HBV genotype

Objective To discuss hemorheologic changes and hepatic function in patients with chronic hepatitis B(CHB)and its correlation with HBV genotype.Methods Relative index of hemorheologic changes,ALT,HBV-DNA and HBV genotype were examined in 96 CHB patients and 24 healthy control people.The results underwent correlation analysis.Results Low-shear blood viscosity and RBC aggregation index were significantly higher in CHB patients than those in the healthy control people(P<0.05).Low-shear blood viscosity and RBC aggregation index were significantly higher in CHB patients with abnormal ALT than those CHB patients with normal ALT and the healthy control people(P<0.05).No statistical difference was seen when CHB patients with normal ALT were compared with the healthy control people and patients with positive HBV-DNA were compared with patients with nesative HBV-DNA(P>0.05).No statistical difference was seen in hemorheologic changes in CHB patients with different genotype(P>0.05).Conclusions Microcirculation disorder of CHB patients is correlated with damage degree of hepatic function.Hemorheologic changes are not correlated with HBV-DNA and HBV genotype,so it can be a relatively independent index for clinical evaluation of disease.
8.Artificial cycle therapy of acupuncture and moxibustion for irregular menstruation.

Through the discussion on TCM physiological characters of females in follicular, ovulatory, luteal and menstrual phases and treatment principles, the clinical application of artificial cycle therapy of acupuncture and moxibustion was introduced for irregular menstruation and the typical cases were attached. It is suggested that the menstrual cycle follows the growth-consumption rule of yin, yang, qi and blood. The corresponding treatment principles should be applied in accordance with the change rule of menstrual cycle. Hence, it is worth to adopt the artificial cycle therapy of acupuncture and moxibustion for irregular menstruation in clinical application.

9.Changes of inflammatory factor level in 2-diabetes patients before and after improving glycemic control

Objective To investigate the inflammation levels of 2-diabetes patients before and after 3 months of improving glycemic control.Methods A longitudinal study was performed in a subgroup of 48 subjects with T2D and poor glycemic control.Forty-four healthy individuals were taken as control group.The serum concentration of C-reactionprotein (CRP),interleukin-6 (IL-6),interleukin-6 (IL-8),transforming growth factor-β1 (TGF-β1) and transforming growth factor-β1 (MCP1) in all participants were measured simultaneously by multiplexed Luminex assay.Results The serum levels of CRP,MCP-1 of 2-diabetes patients were 3.96 (3.45,5.58) mg/L and (195.0± 129.8) ng/L,significant higher than those in control group (2.25 (1.24,3.22) mg/L,(148.5±85.7) ng/L),and the differences were significant(t=-2.580,P=0.010;t=-2.118,P =0.047).No significant difference was found in the serum levels of IL-6,IL-8,TGF-β lbetween the two groups (P>0.05).TGF-β1 level in patients with good glycemic control decreased to 26.85 (23.17-31.12) ng/l,significant lower than that before glycemic control (43.5(26.5-62.25) g/L;Z=-2.191,P=0.028),and there were no significant differences among the other 4 kinds of inflammatory factors before and after blood glucose control(CRP:Z =-0.937P =0.372;IL-6:Z =-0.875,P =0.396;IL-8:Z =-1.215,P =0.286;MCP-1:t =-1.846,P=0.065).Conclusion Low grade systemic inflammation status in T2D patients.Improvement of glycemic control reduces TGF-β1 levels and plays a key role in delaying the development of diabetic nephropathy.
10.Synthesis and biocompatibility of ethynylated open ring derivatives of polyasparamide

Objective To synthesize a new ethynylated open ring derivatives of polyasparamide as functional drug carrier.Methods L-phenylalanine methyl ester hydrochloride was prepared using L-phenylalanine and then was used for ring opening reaction of polysuccinimide.To synthesize the target product of PSI-Phe-OMe-PA, the obtained polyasparamide-g-phenylalanine derivatives ( PSI-Phe-OMe) was further ring opened by propargylamine.The structure of PSI-Phe-OMe-PA was confirmed by 1 H NMR.The biocompatibility of PSI-Phe-OMe-PA was evaluated by MTT method, inverted microscope observation and cell cycles analysis ( propidium iodide staining ) .Results The ring-opening rate of polyasparamide by L-phenylalanine methyl ester and propargylamine was 40%and 100%, respectively.All results of biocompatibility studies indicated that PSI-Phe-OMe-PA may be a good candidate for functional drug carrier.Conclusion Based on the ring-opening capability of amino-group and the specificity of click reaction, L-phenylalanine methyl ester hydrochloride and propargylamine were used successively to react with polyasparamide.PSI-Phe-OMe-PA is a biocompatible functional drug carrier.
