Objective To assess the value of acute physiology and chronic health evaluationⅡ (APACHE-Ⅱ) score and extrapancreatic inflammation on abdominal computed tomography (EPIC) score in predicting outcome of patients with acute pancreatitis (AP).Methods One hundred and forty-seven patients with AP who underwent abdominal computed tomography scan within 24 h and acute physiological examination after admission were studied.To assess their value in predicting severe AP and complications,the APACHE Ⅱ score and EPIC score was calculated and evaluated by calculating receiver operator characteristic (ROC)curves,the sensitivity,the specificity,the Youden index and the area under the ROC curve (AUC).Results Forty-three of 147 patients developed severe AP.The mean scores of APACHE-Ⅱ score and EPIC score two criteria systems was significantly higher in severe AP than that in mild AP (P< 0.01).EPIC score had 0.893 sensitivity,0.870 specificity and the largest AUC for predicting severe AP.EPIC score had the largest AUC in predicting local complications.But the EPIC score was equal to the APACHE-Ⅱ score in AUC to predict systemic complications.Conclusion In patients with AP.EPIC score and APACHE-Ⅱ score combination will estimate the prognosis of disease accurately within 24 h of admission.

4.Relationship between expression of Syk,VEGF-C and lymph node metastasis in breast carcinoma

Purpose To investigate the relationship between the expression of Syk,VEGF-C and lymph node metastasis in breast carcinoma.Methods Immunohistochemical EnVision and SP methods were used to detect the expression of Syk,NFκB(p65)and VEGF-C in 55 cases of breast carcinoma.Results The positive rates of Syk, VEGF-C and NFκB(p65)in 55 cases of breast carcinoma were 50.9%,56.4% and 81.8%,respectively.Lower positive rate of Syk was obtained in the group of positive lymph-node metastasis than that in the group of non-lymph-node metastasis (P<0.05);Higher positive rate of VEGF-C was obtained in the group of positive lymph-node metastasis than that in the group of non-lymph-node metastasis (P<0.05);But no significant differences was found in p65(P>0.75).Higher positive rate of p65 nuclear expression was obtained in the group of positive lymph-node metastasis than that in the group of non-lymph-node metastasis (P<0.025).The expression of Syk was negatively associated with VEGF-C (r=-0.620,P=0.000);the nuclear expression of p65 was associated with the low expression of Syk(r=0.448,P=0.002)and the high expression of VEGF-C (r=0.310,P=0.036).Conclusions In breast carcinoma, Syk may downregulate the expression of VEGF-C by inhibiting the activation of NFκB, which suppresses the lymph node metastasis of the cancer.

Objective The automated capillary zone electrophoresis (CE) is used for human serum protein analysis and compared with the results of agarose gel electrophoresis (AGE) for human serum protein analysis.Discuss its′ practical application in clinical.Methods With the Capillarys ?1-?2+reagent set, proteins in serum were separated at 7 kV for 4 min in 15.5 cm?25.0 ?m fused-silica capillaries (n=8) at 35.5℃ in a pH 10 buffer with online detection 200 nm.Results Capillary zone electrophoresis compared with agarose gel electrophoresis(AGE;Hydrasys-Hyry; Hydragel protein(e)15/30/54 reagent set;Sebia)in precision, relatives, linearity and potential interferences were analyzed.A samples without Para-protein (n=165) were performed relation test. Objective To investigate and analyze the iodine nutritional status of school-age children six months after implementation of a new standard of iodized salts in northern area of Jiangsu Province,and to provide a basis for control of iodine deficiency disorders.Methods According to the water iodine content,northern area of Jiangsu Province was divided into low,moderate and high iodine areas (the water iodine contents were < 10,l0-150 and > 150 μg/L,respectively).Two towns were selected in each area; one elementary school was selected in each town; and 120 children aged 8-10 were selected in each school.The samples of urine,household salt,and water in each town were collected.Urinary iodine content was tested by arsenic cerium catalytic spectrophotometry;iodine content of household salt sample was determined by direct titration; iodine content of water sample was tested by cerium sulfate catalytic spectrophotometry.Results In low,moderate and high water iodine areas,240,242 and 242 urine samples of 8-10 years old children were tested; urinary iodine median was 135.49,183.62 and 448.99 μg/L,respectively,and the difference between groups was statistically significant (x2 =165.56,P < 0.05).In low,moderate and high water iodine areas,100,131 and 117 household salt samples were tested,and the median of salt iodine was 21.88,22.01 and < 5 mg/kg,respectively.Water samples collected in low,moderate and high iodine areas were 12,14 and 10 copies,respectively,and the median of water iodine was 8.89,38.52 and 189.25 μg/L,respectively,and the difference between groups was statistically significant (x2 =22.27,P < 0.05).Conclusions The iodine nutritional condition of 8-10 years old children in low,moderate water iodine areas in northern area of Jiangsu Province is appropriate.The new standard is suitable for iodine nutritional needs in the area.The iodine nutritional condition in high water iodine area is excess,iodine intake should be reduced.

Objective To evaluate the role of nicotinamide adenine dinucleotide phosphate oxidase (NADPH oxidase) in the spinal cord in the maintenance of diabetic neuropathic pain in rats.Methods Pathogenfree male Sprague-Dawley rats,aged 2 months,weighing 180-220 g,were used in the study.Diabetes mellitus was induced by intraperitoneal streptozotocin (STZ) 60mg/kg and confirmed by blood glucose > 16.7 mmol/L at 72 h after STZ injection.Twenty diabetic rats were randomly allocated to diabetic neuropathic pain group (DN group,n =10) and apocynin (specific NADPH oxidase inhibitor) group (A group,n =10).Another 10 agematched normal rats served as control group (C group,n =10).Twenty-eight days after STZ injection,apyconin 5 mg/kg was injected intraperitoneally once a day for 7 consecutive days in A group.Paw withdrawal threshold to yon Frey filament stimulation (PWT) was measured before STZ injection (T1) and at 7,14,21,28 and 35 days after STZ injection (T2-6).The rats were sacrificed after PWT was measured at T6 and L4.5 segments of the spinal cord were removed for determination of NADPH oxidase subunits gp91phox and p47phox expression,MAD content and SOD activity.Results Compared with C group,PWT was significantly decreased at T3-5,gp91phox and p47phox expression was up-regulated,MAD content was increased,and SOD activity was decreased in DN and A groups.Compared with DN group,PWT was significantly increased at T6,gp91phox and p47phox expression was downregulated,MAD content was decreased,and SOD activity was increased in A group.Conclusion NADPH oxidase in the spinal cord is involved in the maintenance of diabetic neuropathic pain in rats.
8.Effect of contracted learning on rehabilitation training to patients with stroke

Objective To explore the effect of contract learning on rehabilitation training to patients with stroke . Methods According to the random number table, 120 stroke patients were randomly divided in equal number into control group and observation group. The patients in the control group were treated with conventional nursing during hospitalization and health guidance during follow-up. The patients in the observation group were given rehabilitation training according to contracted learning during the period of hospitalization and follow-up. The time was 6 months. The quality of life was compared between the two groups of patients before and after the intervention, 1 months after discharge, 6 months after discharge. Results After six months of intervention, the 7 dimensions of quality of life, including daily life ability, hand function, mobility, strength, emotion, communication and participation in the intervention group were significantly better than those of the control group (P<0.001). The 6 dimensions of quality of life, including daily life ability, hand function, mobility, strength, emotion, and communication of the control group were significantly better than before the intervention (P<0.001). Conclusion For stroke patients, the rehabilitation training under the learning contract can improve the effect of rehabilitation training so as to improve the quality of life and the prognosis.

OBJECTIVETo study the effect of the synthetic peptide RGDSY-CTTHWGFTLC on the biological behavior of breast cancer MCF-7 cells in vitro.
METHODSMCF-7 cells were incubated with different concentrations of the synthesized peptide RGDSY-CTTHWGFTLC (RGDSY-CTT), the positive control peptide CTTHWGFTLC (CTT), or the negative control peptide STTHWGFTLS (STT) in fibronectin-coated 96-well plates for different time lengths, and the changes in cell adhesion, invasiveness, proliferation, apoptosis and cell cycle were detected using Transwell chamber assay, MTT assay, and flow cytometry.
RESULTSIncubation of the cells with 50, 100 and 200 µg/ml of RGDSY-CTT caused a significant concentration- dependent inhibition of the cell adhesion (cell adhesion rates of 85.1%, 74.1% and 63.8%, respectively) with stronger effects than CTT (P<0.05). At 100 and 200 µg/ml, RGDSY-CTT significantly inhibited the invasion (with inhibition rate of 41.8% and 63.9%, respectively) of MCF-7 cells with an effect similar to that by CTT (P>0.05). At 50, 100 and 200 µg/ml, RGDSY-CTT concentration-dependently suppressed MCF-7 cell proliferation (with cell proliferation rates of 98.8%, 82.4% and 63.0%, respectively), and this inhibitory effect was stronger than that of CTT at 100 and 200 µg/ml (P<0.05). The results of flow cytometry also demonstrated a stronger apoptosis-inducing effect of RGDSY-CTT (76.7%) than that in CTT, STT and the blank control groups (P<0.05).
CONCLUSIONSRGDSY-CTT can inhibit cell invasion, suppress adhesion and proliferation, and induce apoptosis in MCF-7 cells.
