1.Drug-resistance of Klebsiella pneumoniae in Mountain Hospitals

OBJECTIVE To investigate the drug-resistance status of Klebsiella pneumoniae(KPN) in mountain hospitals and provide the reference for rational application of antibiotics.METHODS Referring to Rules of Operation in Clincal Laboratory of Nationwide,146 KPN isolates were cultured and identified and the drug-sensitivity test was performed in our hospital.RESULTS 100% of KPN isolates were sensitive to imipenem and meropenem.The resistant rates to amikacin,piperacillin/tazobactam,cefoperazone/sulbactam and cefoxitin were all below 22.0%.And that to the other 11 antibiotics were higher and all above 30%.The detection rate of the extended-spectrum-?-lactamases(ESBLs) producing KPN was 28.8%.CONCLUSIONS The drug-resistance of KPN in mountain hospitals is obviously lower than that in general hospitals of developed cities.We should strengthen reasonable management of antibiotics to prevent and control the occurrence and dissemination of drug resistant strains.
2.Drug Resistance of Lower Respiratory Tract Infection with Gram-negative Bacilli in Grass-roots Hospitals

OBJECTIVE To investigate drug resistance of hospital acquired lower respiratory tract infection with Gram-negative bacilli in grass-roots hospitals and to provide the scientific reference for clinically reasonable use of antibiotics. METHODS Referring to Nationwide Rules of Operation in Clinical Laboratory,the strains were isolated and identified. The antibiotic susceptibility test was performed by K-B method. RESULTS Among 199 strains of Gram-negative bacilli,the top 4 of isolating rate were Pseudomonas aeruginosa (PAE),Klebsiella pneumoniae (KPN),Acinetobacter baumannii (ACB) and Escherichia coli (ECO). The drug-resistance supervision in vitro manifested ascending tendency of drug resistance in four kinds of important pathogenic bacteria. The susceptibility of Enterobacteriaceae to imipenem and meropenem was 100%,while the resistance rate of PAE and ACB arrived at 8.0-10.0%. The detection rate of extensive-spectrum beta-lactamases (ESBLs) producing ECO and KPN was 29.2% and 30.6%,respectively. CONCLUSIONS The drug resistance of lower respiratory tract infection with Gram-negative bacilli in grass-roots hospitals show ascending tendency. We should strengthen the management of hospital infection.
3.Progress in the development of metabolic and breast cancer

With the social development,the number of obese people is increasing.Recent studies have shown that obese women to have breast cancer disease's risk is higher than normal body weight,and etabolic disorders growing threat to human health.Estrogen,aromatase have contributed to the occurrence and development of breast cancer after menopause.AMPK is a negative regulator of aromatase,it can inhibit proliferation of cancer cells by metabolic pathways.What's more metformin,resveratrol inhibited proliferation by this route has been confirmed in breast cancer cells.Here,the author do an overview of the role of obesity-related factor for postmenopausal breast tissue estrogen synthesis and metabolic abnormalities affect aromatase expression in breast.
4.Determination of Bifenthrin in Tea by Ultra Performance Convergence Chromatography and Gas Chromatography-Mass Spectrometry

A method was developed for the determination of bifenthrin in tea by ultra performance convergence chromatography ( UPC2 ) . The samples were extracted with petroleum ether and cleaned up with Waters Sep-Pak:Carbon NH2 , and then detected by UPC2 . The mobile phase was the mixture of supercritical CO2 and acetonitrile at a flow rate of 1. 5 mL/min. The separation was conducted on a column of ACQUITY UPC2 TM BEH (100 mmí3. 0 mm,1. 7 μm). The UV detector was set at a wavelength of 220 nm. The detection limit was 20 μg/L. The linear range of bifenthrin was 0. 32-10. 30 mg/L. The recoveries ranged from 88. 7% to 98. 2%. The relative standard deviations (RSD) were from 1. 4% to 2. 8%. The result showed that the UPC2 was more efficient, rapid and low-cost than GC-MS. The method can meet the testing requirements of bifenthrin in tea. The efficacy of UPC2 on testing the concentration of bifenthrin in tea was compared with GC-MS based on National Proficiency Testing and the results were satisfactory.
5.Clinical efficacy of CalliSpheres drug-loaded microspheres versus blank microspheres in the treatment of advanced non-small cell lung cancer by transarterial chemoembolization

Objective To analyze and compare the clinical efficacy of CalliSpheres drug-eluting micro-spheres and blank microspheres in the treatment of advanced non-small cell lung cancer by bronchial arterial chemoembolization.Methods Fifty patients with advanced non-small cell lung cancer who had failed or relapsed after radiotherapy,chemotherapy,targeting and immunotherapy were collected and treated with super-selective bronchial artery chemoembolization.A retrospective analysis was conducted to compare the tumor response rate and survival between CalliSpheres drug-eluting and blank microspheres.Results The PR,ORR and DCR in the drug-eluted microsphere group were higher than those in the blank microsphere group,and there was a statistical difference in DCR between the two groups 1 month after surgery(χ2 = 4.08,P = 0.04).PD in the drug-eluted microsphere group was lower than that in the blank microsphere group.The CEA,CYF and SCC in the drug-eluted microsphere group after surgery were lower than those in the blank microsphere group,and the CEA,CYF and SCC in the two groups after surgery were lower than those before surgery,and there were statistical differences in CEA and CYF 1 month after surgery between the two groups.The PFS and OS in drug-eluted microsphere group were higher than those in blank microsphere group.Conclusion CalliSpheres drug-eluting microspheres could improve the effective rate of tumor treatment and prolong the survival time more effectively than the blank micro-spheres via arterial chemoembolization,providing reliable clinical practice basis for the treatment of advanced non-small cell lung cancer.

We conducted a prospective study to assess the non-inferiority of adjuvant chemotherapy alone versus adjuvant concurrent chemoradiotherapy (CCRT) as an alternative strategy for patients with early-stage (FIGO 2009 stage IB-IIA) cervical cancer having risk factors after surgery. The condition was assessed in terms of prognosis, adverse effects, and quality of life. This randomized trial involved nine centers across China. Eligible patients were randomized to receive adjuvant chemotherapy or CCRT after surgery. The primary end-point was progression-free survival (PFS). From December 2012 to December 2014, 337 patients were subjected to randomization. Final analysis included 329 patients, including 165 in the adjuvant chemotherapy group and 164 in the adjuvant CCRT group. The median follow-up was 72.1 months. The three-year PFS rates were both 91.9%, and the five-year OS was 90.6% versus 90.0% in adjuvant chemotherapy and CCRT groups, respectively. No significant differences were observed in the PFS or OS between groups. The adjusted HR for PFS was 0.854 (95% confidence interval 0.415-1.757; P = 0.667) favoring adjuvant chemotherapy, excluding the predefined non-inferiority boundary of 1.9. The chemotherapy group showed a tendency toward good quality of life. In comparison with post-operative adjuvant CCRT, adjuvant chemotherapy treatment showed non-inferior efficacy in patients with early-stage cervical cancer having pathological risk factors. Adjuvant chemotherapy alone is a favorable alternative post-operative treatment.

7.Targeting cAMP in D1-MSNs in the nucleus accumbens, a new rapid antidepressant strategy.

Studies have suggested that the nucleus accumbens (NAc) is implicated in the pathophysiology of major depression; however, the regulatory strategy that targets the NAc to achieve an exclusive and outstanding anti-depression benefit has not been elucidated. Here, we identified a specific reduction of cyclic adenosine monophosphate (cAMP) in the subset of dopamine D1 receptor medium spiny neurons (D1-MSNs) in the NAc that promoted stress susceptibility, while the stimulation of cAMP production in NAc D1-MSNs efficiently rescued depression-like behaviors. Ketamine treatment enhanced cAMP both in D1-MSNs and dopamine D2 receptor medium spiny neurons (D2-MSNs) of depressed mice, however, the rapid antidepressant effect of ketamine solely depended on elevating cAMP in NAc D1-MSNs. We discovered that a higher dose of crocin markedly increased cAMP in the NAc and consistently relieved depression 24 h after oral administration, but not a lower dose. The fast onset property of crocin was verified through multicenter studies. Moreover, crocin specifically targeted at D1-MSN cAMP signaling in the NAc to relieve depression and had no effect on D2-MSN. These findings characterize a new strategy to achieve an exclusive and outstanding anti-depression benefit by elevating cAMP in D1-MSNs in the NAc, and provide a potential rapid antidepressant drug candidate, crocin.
