1.Study in application of nurse security scale in elderly inpatients for security precaution

Objective To seek the best method for security precaution of elderly inpatients.Methods Elderly patients(296 cases) from January to August in 2006 and elderly patients(336 cases) from January to August in 2007 were divided into the control group and the improved group.The improved group received selective safety precaution measures after assessment of crisis degree by nurse security scale.The control group was given routine security nursing measures.The hospitalization time and incidence of unsafe events were compared in the two groups. Results The two groups were different in the following aspects:hospitalization time, incidence of unsafe events,satisfaction degree of family members,shift time of nurses,satisfaction degree of nurses. Conclusions To supply selective security nursing for elderly patients after assessment of crisis degree by nurse security scale at the admission day could decrease the incidence of un-safe events and hospitalization time.At the same time it shortened the shift time and increased shift efficien-cy to realize the win-win relations between nurses and patients.
2.Clinical Observation of Aconite-cake-partitioned Moxibustion at Different Frequencies for Benign Prostatic Hyperplasia Due to Kidney-yang Deficiency

ObjectiveTo comparethetherapeutic efficaciesofaconite-cake-partitionedmoxibustionat different frequencies in treatingbenign prostatic hyperplasiadue tokidney-yangdeficiency.MethodEightypatientswere randomized into four groups:control group,treatment group1(moxibustiononceper day),treatmentgroup2(moxibustiontwiceper day), andtreatmentgroup3 (moxibustiononce every other day).TheInternational prostate symptom score (I-PSS), TCM syndrome score, maximum flow rate of urine (Qmax) and bladder residual urine volume (PVR) were observedbefore and after intervention.ResultAfter treatment,the improvement of theInternational prostate symptom scoreandTCM syndrome score,increase of Qmax and decrease of PVR were found in the three treatment groups, while the therapeutic efficacy in treatment group 1 was more significant than that in the rest groups. The total effective rate was 70.0% in the control group, 85.0% in treatment group 1, 80.0% in treatment group 2, and 65.0%in treatment group 3.ConclusionAconite-cake-partitionedmoxibustionat different frequenciescan produce therapeutic efficacies to different extent in treatingbenign prostatic hyperplasiadue tokidney-yangdeficiency, and the comprehensive analysis shows that the optimal frequency is once per day.
3.Test-retest Reliability of Active Static and Dynamic Balance Test in Elderly People:KorebalanceTM System

ObjectiveTo determine the test-retest reliability of active static and dynamic balance ability with the KorebalanceTM System in elderly people.Methods20 ambulatory elderly persons who can walk self-helping participated in this study. Measurements were performed through KorebalanceTM System for active static and dynamic balance ability. All tests were repeated 7 d after the initial test, the intra-class correlation coefficients (ICC) were used to determine the test-retest reliability of each balance test.ResultsThe ICC indicated that active static and dynamic test had good reliability (ICC=0.804~0.923).ConclusionThe KorebalanceTM System had good reliability in active static and dynamic balance test.
4.DISTRIBUTION OF NEUROGLOBIN IN SPINAL CORD OF ADULT RAT

Objective In order to explore the localization of NGB protein in spinal cord of adult rat. Methods Spinal cord of adult rat was used to make frozen section. The localization of NGB protein in the spinal cord of adult rat was examined by immunohistochemistry method. Results NGB protein immunoreactive cells were mainly distributed in the gray matter of spinal cord, including in the ventral horn, intermediate zone and dorsal horn. NGB-immunoreactive product located in the plasm of neurons.Conclusion The results indicated that NGB widely expressed in the neurons of spinal cord, suggested that NGB might play an important role in the physiological function of the spinal cord.
5.Geographical distribution rule of normal reference value of old female’s hemoglobin based on factor analysis

Objective To reveal the geographical distribution regularity of the normal reference value of old female’s hemoglobin in China,and provide scientific basis for making its unified standards.Methods The normal reference values of 15 690 examples of old female’s hematocrit by the method of hemoglobin cyanide(HiCN)and five geographical factors in 157 areas in China,its distribution regularity and the complex relationship characteristic between geography environment elements were analyzed with the methods of factor analysis,spatial analysis of GIS and correlation analysis.Results By the method of factor analysis,the five factors of geography environment were combined into two common factors F1and F2;the original data was replaced with its scores to infer the regression equation :y^=127.9-0.01009F1-0.01543F2?8.8.And this equation was used to figure out the reference value of 1 288 observation points in China.With support of the model of spatial analysis of GIS,the distribution regulation of old female’s hemoglobin reference value in China and its distribution regularity was west-higher,east-lower,north-higher and south-lower.Conclusion If some area’s altitude(X1),sunshine hours yearly(X2),mean relative humidity yearly(X3),mean temperature yearly(X4),precipitation yearly(X5) are known,this place’s reference value of hemoglobin can be calculated out by using this equation.The reference value of hemoglobin in any areas in China can be gotten from the trend distribution regularity map.
6.Comparison of standard tangential field and multiple fileld radiotherapy in axillary lymph node after conserving surgery

Objective To investigate the difference of standard tangential field and multiple fileld radiotherapy in axillary lymph node after conserving surgery,to provide a reference for clinical treatment.Methods 20 cases of left breast cancer patients were selected,all patients underwent breast -conserving surgery,postoperative radiothera-py.All patients used standard radiotherapy wild tangent based and reverse -intensity modulated radiation therapy optimization.Armpit I,Ⅱ lymph nodes were used to calculate the extent of coverage of the original plan,target dose uniformity HI,dose conformal CI,dosimetric parameters were calculated.Results Under standard tangential field irradiation,I stand mean dose was (33.95 ±8.24)Gy,which was significantly higher than Ⅱ station (22.13 ± 6.67)Gy,and V50,V45,V40 were (22.45 ±6.32)%,(39.83 ±7.54)%,(49.65 ±8.31)%,which were higher than Ⅱ stand,I stand irradiation was significantly more,the differences were statistically significant (t =5.632,P =0.009;t =7.214,P =0.000;t =8.954,P =0.000;t =6.121,P =0.002).The whole breast irradiation armpit IMRT plan,19 patients (95.00%)reached 95% by volume APTV≥50Gy,coverage was better;but HI,CI were (1.11 ± 0.03),(1.36 ±0.07)respectively,which were significantly worse than the standard wild tangent,the differences were statistically significant (t =6.584,P =0.001;t =9.144,P =0.000).Under Hatano IMRT irradiation ipsilateral lung,heart suffered an average dose of (1 694.58 ±102.31)cGy,(645.54 ±74.44)cGy,which were significantly more than the standard tangential field irradiation,the differences were statistically significant (t =7.654,P =0.000;t =6.654,P =0.001).Conclusion Standard tangential field irradiation has poor coverage in Ⅱ axillary lymph node station,irradiation intensity modulated radiation has better coverage,but significantly increased the dose to normal tissues.
7.LncRNA ZBED3-AS1/miR-339-5p/Notch 1 axis regulates osteoblast proliferation and differentiation in osteoporotic rats

Objective:To investigate the role of LncRNA ZBED3-AS1 in osteoblast proliferation and differentiation in osteoporotic rats through regulating miR-339-5p/Notch 1.Methods:The rat models of sham operation (Sham) group and model (Model) group were established, and the bone mineral density (BMD) of rats was examined. Rat osteoblasts were isolated and the expression of ZBED3-AS1 and miR-339-5p was detected by qRT-PCR. The osteoblasts of rats in Sham group and Model group were divided into different groups and transfected. CCK8, alizarin red (AR-S) staining and alkaline phosphatase (ALP) staining were used to detect the proliferation and differentiation ability of cells in each group. The distribution of ZBED3-AS1 in cells was determined by FISH assay. Double luciferase report confirmed the relationship between ZBED3-AS1 and miR-339-5p as well as miR-339-5p and Notch 1.Western blot was used to detect the expression of Notch pathway related factors.Results:The bone mineral density of femur in Model group was significantly lower than that in Sham group ( P=0.0057) . Compared with Sham group, the expression of ZBED3-AS1 in osteoblasts of Model group was lower, while that of miR-339-5p was higher (all P<0.05) . Overexpression of ZBED3-AS1 could promote the proliferation and differentiation of osteoblasts, while knockdown of ZBED3-AS1 could inhibit the proliferation and differentiation of osteoblasts (all P<0.05) . ZBED3-AS1 could regulate miR-339-5p as ceRNA (all P<0.05) . Overexpression of miR-339-5p can inhibit the proliferation and differentiation of osteoblasts, which can be partially saved by overexpression of ZBED3-AS1. Notch 1 was confirmed as a target of miR-339-5p, at the same time, interfering with the expression of ZBED3-AS1/miR-339-5p can affect the expression of Notch-1 protein, and the regulation of ZBED3-AS1/miR-339-5p on osteoblasts may be realized through Notch pathway. Conclusion:ZBED3-AS1 can be used as ceRNA to regulate miR-339-5p, and then affect the proliferation and differentiation of osteoblasts in osteoporotic rats.
8.Test-retest Reliability and Measurement Error in Using A Jamar Dynamometer to Determine Grip Strength for Elderly Men

ObjectiveTo examine the test-retest reliability and measurement error in using a Jamar dynamometer to determine grip strength for elder men. Methods40 elder men with a mean age of 82 years old volunteered to participate in this study. The grip strength was administered twice using a Jamar dynamometer by specially trained testers; subjects were re-tested at the same time of day a week later by the same testers. ResultsThe infraclass correlation coefficient(ICC) was 0.99. The standard error of measurement (SEM and SEM%) were 1.18~1.69 and 4.11%~6.41%. The smallest real difference (SRD and SRD%) were 3.27~4.68 and 11.39%~17.75%. The Bland-Altman analysis revealed no systematic errors between test and retest. ConclusionThe Jamar dynamometer is reliable and with acceptable measurement errors to determine grip strength for elder men.
9.Assessment of the right ventricular function after pulmonary thromboendarterectomy by Doppler echocardiography

Objective To evaluate the right ventricular function of chronic thromboembolic pulmonary hypertension (CTEPH) after pulmonary thromboendarterectomy (PTE) by Doppler echocardiography. Methods In 16 patients with CTEPH,end-diastolic left ventricular diameter(LVDd), end-diastolic right ventricular areas (RVEDA), end-systolic right ventricular areas (RVESA), right ventricular fractional area change (RVFAC), pulmonary accelerative time (Pact), pressure gradient of tricuspid valve regurgitation (PGT1), area of tricuspid valve regurgitation (AT1) were measured by echocardiography. Results Fifteen and thirty days after surgery,the LVDd and Pact had increased,RVEDA, RVESA,PGT1 and AT1 had decreased while RVFAC had increased in all cases. Conclusions PTE may effectively improve the right ventricular function of patients with CTEPH.
10.Robot-assisted training can improve the bladder and intestinal functions of paraplegic patients

Objective To explore the clinical effect of training assisted by a lower limb rehabilitation robot on the bladder and intestinal function of paraplegic spinal cord injury survivors. Methods Thirty-eight paraplegic patients with spinal cord injury were divided according to their admission order into an experimental group ( n=19) and a control group (n=19). Both groups were given conventional rehabilitation training, while the experimental group was additionally provided with robot-assisted lower limb training in three stages:adaptation, training and con-solidation. It lasted 30 minutes daily, 5 days per week for 12 weeks. Before and after the training, an urodynamics examination system was used to evaluate the maximum urine flow, bladder capacity, residual urine volume, bladder pressure and detrusor pressure. Colon transit time, mean rectal pressure and intestinal function were measured using the colon transit test, a mean rectal pressure test, and the Functional Independence Measure ( FIM) scale respective-ly. Results The average bladder volume, maximum urine flow rate, average urine flow rate, detrusor pressure, bladder compliance, average rectal pressure and intestinal FIM score of the robot training group after training were all significantly better than before the training, as were the average residual urine volume and colon transit time. After the training, the average bladder volume, maximum urine flow rate, average urine flow rate, detrusor pressure, bladder compliance and average rectal pressure of the robot training group were all significantly higher than those of the control group, while the average residual urine volume and colon transit time were significantly smaller. Then, 32% of the patients in the experimental group achieved no less than 6 points for their average FIM score, significantly higher than in the control group. Conclusion Robot-assisted lower limb training combined with comprehensive rehabilitation training can effectively improve the bladder and intestinal function of paraplegic patients after a spinal cord injury.
