1.Analysis of blood glucose monitoring during pregnancy and pregnancy outcome in patients with gestational diabetes mellitus

Objective To discuss the effect of blood glucose monitoring during pregnancy and pregnancy outcome in patients with gestational diabetes mellitus (GDM). Methods One hundred and fifty-three GDM patients from July 2007 to July 2008 were analysed.The cases were divided into satisfactory group (74 cases)and unsatisfactory group (79 cases) based on the level of bleod glucose and compared the pregnancy outcome between the two groups. Results The incidences of pre-eclampsia, polyhydramnios, macrosomia, neonatal hypoglycemia, hyperbilirubinemia of satisfactory group were lower than those of unsatisfactory group(5.41% vs 17.72%,4.05% vs 20.25%, 12.16% vs 31.65% , 5.41% vs 20.25%,8.11% vs 30.38%). The difference is statistically significant (P <0.05). The incidence of premature rupture of membranes, premature birth, postpartum hemorrhage, fetal distress of satisfactory group were lower than those of unsatisfactory group, but there was no significant difference between the two groups. Conclusion GDM can cause great harm to maternal and child health, and satisfactory blood glucose control during pregnancy may reduce maternal and child complications.
2.Effects of low-frequency ultrasound combined with Tongluo Zhitong herbs on urokinase-type plasminogen activator system and inflammatory cytokines in knee osteoarthritis

BACKGROUND: Low-frequency ultrasound combined with Tongluo Zhitong herbs can effectively relieve pain and improve joint function of patients with osteoarthritis, but the underlying mechanisms are still unclear.OBJECTIVE: To investigate the effects of low-frequency ultrasound combined with Tongluo Zhitong herbs on urokinase-type plasminogen activator (uPA) and inflammatory cytokines of osteoarthritis rabbits and the related mechanisms.METHODS: Forty rabbits were randomly divided into four groups: normal, model, control, and experimental groups. The normal group was treated with normal feeding, not for any processing. For the other three groups, papain was injected into the rabbit knee articular cavity to establish osteoarthritis models. One week later, the experimental group was treated with low frequency ultrasound combined with Tongluo Zhitong herbs for 3 weeks, the control group treated with the same volume of normal saline, and the model group received no intervention. The levels of inflammatory cytokines in the serum and synovial fluid were measured, the histological changes of synovial tissues and articular cartilage were evaluated using Mankin's score, and the expression levels of uPA and its receptor were determined by western blot assay.RESULTS AND CONCLUSION: The levels of inflammatory cytokines in the model group were significantly higher than those in the normal and experimental groups (P < 0.05). The Mankin's scores of synovial tissue in the experimental group were significantly lower than those in the model and control groups (P < 0.05), while Mankin's scores of the cartilage tissue had no significant difference between the experimental and model groups (P > 0.05). Compared with the model group, in the experimental group, the expression levels of uPA and its receptor were significantly downregulated,and the expression of uPA inhibitor was notably upregulated. These data show that low-frequency ultrasound combined with Tongluo Zhitong herbs attenuates inflammatory reaction in the synovial tissues, by inhibiting the formation of joint fluid and production of inflammatory cytokines, which is an effective treatment for knee osteoarthritis.
3.Effect on fibula-tibia fracture treatment by two different intramedullary nails

[Objective]To evaluate and compare the effects of fibial-tibial fracture(FTF) treated by interlocking intramedullary nail (I I N group A) and intramedullary expand expanded self-lock nail (IESN group B).[Method]From June 2000 to September 2003,one hundred sixteen cases of FTF were treated by I I N(N=60)or IESN (N=56) and followed-up for 12~24 months (average 17.9 months).Results were compared between group A and group B.[Result]In group A,the satisfactory rate,union rate,close reduction rate,medullary expanding rate,complication rate and antirotation ability were 89.5%, 96.7%, 71.6%, 51.6%,21.6% and +++ respectively.In group B,that were correspondently 91.0%,98.5%,89.3%,5.3%,3% and +.[Conclusion]Adopting the biological osteosynthesis either the I I N or IESN for treatment of FTF could get a result of more satisfactory rate and higer union rate.Applying a stable fixation,the IESN is more suitable for instable FTF.Such as multisegmental or comminuted targe oblique FTF,while of fering a flexible fixation,IESN is preferable for transverse fractures of middle and lower third tibia and fibula.
4.Closed reduction and trans-cuteneous screw and Kirschner's wire fixation to treat intercondylar fracture of humerus

[Objective]To explore the methods and effects of closed reduction and percutaneous internal fixation to treat intercondylar fractures of humerus.[Method]From 2001,3 to 2004,9 we selectively treated 6 cases(group A) of Rise-Borough Ⅱ,Ⅲ type of humeral intercondylar fractures with closed reduction and percutaneous internal fixation with hollow screw and Kirschner's wire.Others(group B) treated with opened reduction and internal fixation with plates and screws.[Result]Following up 6~12 months,the outcome were evaluated with Aitken and Rorabeek rating system.Group A:5 patients were graded as excellent,1 as good.Group B:7 patients were graded as excellent,7 as good,1 as fair,1 as poor.[Conclusion]To treat Rise-Borough Ⅱ,Ⅲ type of humeral intercondylar fractures with closed reduction and percutaneous internal fixation with hollow screw and Kirschner's wire,can decrease iatrogenic impairment,receive reliable fixation and early functional exercise.The fracture union quickly,and the patient has excellent joint function and less postoperative complication.
5.Conservative and operative therapies in treatment of proximal humeral fractures

Objective To evaluate fixation of anatomic plate by comparing ou tc omes of surgery and conservative therapy for proximal humeral fractures. Methods 24 cases of proximal humeral fractures were fixed with anatomic plate, lag scre ws and kirschner wire. 19 cases were managed with conservative therapy of manual reduction and external wood splint fixation. The results were evaluated by Neer classification and Constant-murley scoring system. Results All the case were f ollowed up 10 to 19 months(averaging 11.5 months). In the 24 operated cases, 11 patients were graded as excellent, 8 as good, 4 as fair, 1 as poor. In the 19 ca ses of conservative therapy, 6 patients were graded as excellent, 7 as good, 6 a s fair, most of which were Neer types Ⅲand Ⅳ. Objective To investigate the relationship between serum pregnancy associated plasma protein A (PAAA‐A) ex‐pression and gene polymorphism with the severity of coronary lesions in the patients with coronary heart disease (CHD) .Methods Ninety‐eight patients with CHD in the Nanyang Municipal Central Hospital were selected as the observation group and divided into single vessel lesion group and multiple vessel lesion group according to coronary angiographic results .Ninety‐eight individuals un‐dergoing healthy physical examination were selected as the control group .The venous blood was collected at the visiting hospital in the observation group and at the physical examination in the control group for detecting the serum PAPP‐A protein level by ELISA .PAPP‐A gene and IVS6+ 95 polymorphism were analyzed by adopting polymerase chain reaction restriction fragment length polymorphism(PCR‐RELP) .Results Compared with the control group ,peripheral blood PAPP‐A protein level in the obser‐vation group was significantly increased(P<0 .05) ,moreover the PAPP‐A protein level in the multiple vessel lesion group was sig‐nificantly higher than that of the single vessel lesion group (P<0 .05) .The peripheral blood PAPP‐A level was positively correlated with the severity of CHD .Three genotypes existed in PAPP‐A gene IVS6+95 locus ,including CG heterozygous ,homozygous CC and GG homozygous type .The CC homozygous allele frequency in the patients with multiple vessel lesion was higher than that in the patients with single vessel lesion (P<0 .05) .Conclusion The PAPP‐A protein level and IVS6+95 polymorphism have a close relation with the severity of coronary lesions in patients with CHD .CC genotype may be one of genetic susceptibility gene markers in the patients with CHD .
7.Clinical study of leflunomide combined with clopidogrel in the treatment of IgA nephropathy

Objective To investigate the efficacy and feasibility of combination of leflunomide and clopi-dogrel in the treatment of IgA nephropathy. Methods 84 patients with primary IgA nephropathy were randomly divided into four groups: Control group (group 1) received valsartan; test group (group 2) received valsartan +clopidogrel; group 3 received valsartan + leflunomide; group 4 received valsartan + clopidogrel + leflunomide. The level of 24 h urinary protein,serum creatinine, glomerular filtration rate and therapeutic efficacy, adverse reactions were detected and recorded during the 48 weeks follow-up. Results The 24 h urinary protein of 84 patients showed a downward trend during the course of treatment , but there were statistical differences between groups since 8 weeks after treatment (P < 0.05). The level of urine protein in group 3 and group 4 decreased significantly to 61.48% and 67.23% respectively in 48 weeks after treatment. At the end of the follow up , the serum creatinine levels in group 1 and group 2 significantly increased while the glomerular filtration rate de-creased obviously (P < 0.05). Conclusion The combination of leflunomide with clopidogrel could reduce the level of urinary proteins and slow renal function deterioration in the treatment of IgA nephropathy with less ad-verse reactions. This study provides a new idea for treatment of IgA nephropathy.
8.Restoration of cardiac function in failing beagle hearts by recombinant adeno-associated viral gene transfer of sarcoplasmic reticulum Ca~(2+)-ATPase

9.Evaluation of clinical efficacy of continuous gastrointestinal decompression after gastric lavage with edible oil for treatment of patients with oral aluminum phosphide poisoning

Objective To observe the curative effect of continuous gastrointestinal decompression after gastric lavage with edible oil on saving patients with oral aluminum phosphide poisoning.Methods Seventy-eight patients with oral aluminum phosphide admitted to the Department of Internal Emergency of the Second People's Hospital of Yunnan Province from October 2009 to October 2016 were divided into a mild poisoning group (39 cases), a moderate poisoning group (26 cases) and a severe poisoning group (13 cases) according to clinical manifestations and laboratory examinations, all the patients were treated with continuous gastrointestinal decompression after early gastric lavage with edible oil, including scavenging toxicant, correcting intracellular oxygen intake and metabolic disturbance, and inhibiting and eliminating inflammatory mediators. The difference of remission times of clinical symptoms, recovery times of abnormal indexes and hospitalization times were compared among patients with different disease severities. Results With the aggravation of disease, the remission times of clinical symptoms (hours: from mild to severe were 24±12, 54±18, 84±12), recovery times of abnormal indexes (hours: from mild to severe were 18±6, 72±0, 108±12) and hospitalization times (hours: from mild to severe 48±24, 120±24, 144±24) were all gradually extended. Of the 13 patients with severe poisoning, 2 patients died of multiple organ functional failure (MOF) after 28 hours of treatment because they were incapable of cooperating with continuous gastrointestinal decompression. There were 76 patients were clinically cured, the cure rate being 97.4%. In the follow-ups at 1 month and 6 months after the treatment, no abnormalities were seen.Conclusion Continuous gastrointestinal decompression after early gastric lavage with edible oil for saving patients with oral aluminum phosphide poisoning is an effective therapy worthwhile to be popularized.
10.Analysis of recurrent and prognosis factors of primary retroperitoneal liposarcoma

Objective:To investigate the factors related to recurrence and prognosis of retroperitoneal liposarcoma.Method:The clinical data of patients with primary retroperitoneal liposarcoma who underwent surgical treatment in the First Affiliated Hospital of Zhengzhou University from June 2011 to January 2020 were analyzed retrospectively. There were 42 males and 47 females and patients’median age was 53 (26-78). Sixty-five cases were treated by operation in our hospital, and 24 cases were primarily treated by the operation in another hospital. The clinical manifestations of the initial diagnosis included retroperitoneal mass in 41 cases, abdominal distension in 12 cases, abdominal pain in 10 cases, fever in 11 cases, nausea, vomiting and poor appetite in 8 cases, frequent urination and dysuria in 6 cases, and bilateral lower limb edema in 1 case. Preoperative CT imaging showed that the tumor body was located in the retroperitoneal kidney area in 58 cases, while in the retroperitoneal space or the pelvic extraperitoneal space in 31 cases. There were 55 single cases and 34 multiple cases. The median tumor length was 20(3-52) cm. Among the primarily treated 65 patients, 47(72.3%) were considered as primary retroperitoneal liposarcoma by preoperative imaging examination. Among the 89 patients treated by surgery, 78 underwent endoscopic surgery, among which 21 underwent laparoscopic surgery, 38 cases of retroperitoneal laparoscopic surgery, 19 cases of Da Vinci robot-assisted laparoscopic surgery. Open operation was performed in 11 cases. There were 87 patients undergoing radical resection and 2 patients undergoing palliative resection. Forty-two patients underwent intraoperative combined resection of the adjacent organs. The recurrence and survival status of patients were followed up.Results:All the 89 patients underwent the operation successfully, with the median operative blood loss of 200 (10-2000) ml. There were 23 cases being diagnosed of well differentiated liposarcoma, 40 cases of dedifferentiated, 20 cases of myxoid/round, 5 cases of myxoid liposarcoma, and 1 cases of mixed type. Pathologically, there 42 cases with low grade histology and 47 cases with high grade histology. In this study, 89 patients were followed up for 3 to 108 months, and the median follow-up time was 28 months. The 5-year recurrence free survival rate, disease-free survival rate and overall survival rate of the patients were 16.7%, 16.1% and 52.6%, respectively. There were 57 patients presenting local recurrence, 1 patient of lung metastasis, and 1 patient of liver metastasis, and the median disease-free survival time was 24 months. There were 42 patients died of the disease, with a median survival time of 64 months. Univariate analysis showed that intraoperative blood loss( P<0.01), whether multiple cases( P<0.01), pathologic types( P<0.01), and histological grades ( P<0.01) were related to disease-free survival.The intraoperative blood loss( P<0.01), multiple cases( P<0.05), pathologic types ( P<0.05), and recurrence ( P<0.01)were related with overall survival. Gender, age, tumor size, tumor location, whether primary surgery, radical resection or combined resection of adjacent organ had no effect on the prognosis of patients ( P>0.05). Cox regression model multivariate analysis revealed that surgical bleeding ( RR=2.360, 95% CI 1.313-4.241, P=0.004), multiple tumor ( RR=1.899, 95% CI 1.068-3.375, P=0.029), and pathological type ( RR=4.976, 95% CI 1.622-15.264, P=0.005) were independent factors affecting disease-free survival. The recurrence was an independent factor affecting the overall survival of patients ( RR=31.495, 95% CI 1.062-933.684, P=0.046). Conclusions:Retroperitoneal liposarcoma is a rare disease with high recurrence rate. The intraoperative blood loss, whether multiplicity and pathological type are independent factors affecting the disease-free survival, and recurrence is independent factors affecting the overall survival.
