1.Investigation and Thinking on Nursing Ethics Teaching in Local Medical Universities and Colleges: Take an Example of Medical School in Xi'an

Objective:To understand the effect and existing problems of teaching reform about primary health care demands in local medical universities and colleges.Methods:With sampling survey method,150 students from Xi'an Medical College were investigated using questionnaires.Results:Totally 93.1% of the freshmen approved of setting up the courses,72.6% of the students thought that nursing ethics and skills were equally important,43.2% of the students thought learning this course made themselves have a new understanding of nursing ethics.Totally 84.2% of the students were satisfied with the teacher's teaching process,while 69% of the students were not satisfied with the existing teaching materials.Conclusion:The teaching method oriented by practical problems and the teachers' own efforts are prerequisites for students' acceptance of this course.Content setting based on post needs is an important link of nursing ethics teaching effect.Setting up sustainable teaching time is the basis of the teaching effect.High quality teaching materials are necessary conditions to achieve the teaching orderly.
2.The relationship of anxiety and feelings of control during labour among primiparae

Objective To know the relationship of anxiety and feelings of control during labour among primiparae.Methods Selected 90 primiparae from the July 2007 to September 2007,investegated anxiety condition and feelings of control during labour of them by related questionnaire and scales,and then analyzed the relationship between them.Results The anxiety score among the 90 primiparae was significant higher than that of in normal women.There was a negative correlation between the anxiety and feelings of control during labour among the primiparae.Conclusions Lower feelings of control indicats higher anxiety in labour among primiparae.
3.Comparison of ticagrelor versus clopidogrel in effect of bleeding in elderly patients after stenting

Objective To compare effect of ticagrelor and clopidogrel on bleeding in elderly patients after stenting.Methods 48 elderly patients after stenting who were diagnosed with acute coronary syndrome in the hospital were collected.All patients were divided into clopidogrel group and ticagrelor group according to different therapy, 24 cases in each group,and were given corresponding drug treatment in each group.After treatment, the platelet inhibition rate, cardiovascular events and bleeding events were observed.Results After treatment, compared with clopidogrel group, the platelet inhibition rate was higher in ticagrelor group(P<0.05), the incidence of cardiovascular events was lower in ticagrelor group (4.16%vs.25.00%;P<0.05), but there was no obvious difference in incidence of bleeding events of ticagrelor group (4.16% vs.8.32%).Conclusion In elderly patients after stent implantation, ticagrelor could inhibit platelet aggregation better than clopidogrel, prevent the occurrence of cardiovascular events, and would not increase the risk of bleeding.
4.Application of decision tree method in evaluation of nursing quality

Objective To establish a reasonable method to evaluate nursing quality with both the quantity and quality of nursing taken into account.Methods Quantity indexes of nursing were analyzed with decision tree method (CHAID)in SPSS18.0,which included the number of patients admitted to wards,first grade' s nursing days,critically ill patients,emergencies,operations,intravenous infusions,intramuscular injections,oxygen inhalations,urinary tract catheterizations,gastric tube catheterizations,enemas and the average turnover rate of beds.After the quantity analysis,the nursing units were divided into different groups according to their workload.Then,we made a comprehensive evaluation of nursing quality with 12 indicators on the same group.Results Decision tree analysis can identify hierarchy relationship of various nursing quantity indicators and high-priority work quantity variables.Furthermore,it can establish the hierarchical rule based on the distribution of high-priority variables,and serve the purpose of classifying nursing units based on nursing workload.Conclusion The decision tree method can provide a same-level nursing workload platform for quality examination.This has minimized the influence of nursing workload discrepancies on nursing quality examination,and made nursing quality more comparable with nursing workload units of the same level,thus a scientific method for comprehensive nursing quality evaluation.
5.Efficiency and distribution of gene delivery to the injured tendons by microinjection and tissue re-actions caused by vectors

Objective To investigate efficiency and distribution of gene delivery to the injured tendons by microinjeetion and tissue reactions caused by different vectors. Methods By using a mi-croinjection technique, 10μl of pCMV-EGFP, pCAGGS-EGFP, AAV2-EGFP and Ad5-EGFP harboring enhanced green fluorescence protein (EGFP) gene were respectively injected to two sites of the proximal stump of 48 transected digital flexor tendons in 18 chickens. At days 3, 7, 14 and 21, the tendons wereharvested for observing distribution and expression of EGFP under a fluorescence microscope by using fro-zen tissue sections. The tendon sections were also stained with hematoxylin and eosin to examine inflam-mation caused by these vectors. The other 24 normal flexor tendons were served as controls. ResultsCompared with normal tendon tissues, the EGFP expression was observed in tendons at days 3, 7, 14 and21 after injection. The EGFP expression was observed at day 3 and reached peak at day 7 for all vectors.The EGFP expression was decreased at day 14 but seldom seen at day 21. EGFP was distributed evenly inthe injected tendon segment adjacent to the cut level. The EGFP expression in the tendons injected withAAV2-EGFP and Ad5-EGFP was higher than that with pCMV-EGFP and pCAGGS-EGFP injection, withinsignificant statistical difference upon the EGFP expression between AAV2-EGFP and Ads-EGFP vec-tors. Tissue reactions of the tendons caused by the liposome-plasmid vector ( including pCMV-EGFP and pCAGGS-EGFP) were the most prominent among all vectors. Infiltration of inflammatory cells, chiefly lymphocytes and neutrophilic granulocytes, were found. The tendons injected with AAV2 vectors presen-ted gentle inflammatory reactions. Conclusions Mieroinjection to two sites of each tendon stump deliv-ers the transgene to the entire tendon segment adjacent to the cut. Gene delivery by the AAV2 and Ad5 vectors has the highest cfficiency among four vectors tested, when expression level peaks at day 7 after in-jection and AAV2 vector causes the slightest tissue reactions in the tendons, indicating that the AAV2 vector is a promising gene delivery vector and microinjection is practical for tendon gene therapy.
6.Influence of fasudil on the epithelial-mesenchymal transdifferentiation of renal tubular epithelial cells in diabetic rats

Objective To investigate the influence of fasudil on the epithelialmesenchymal transdifferentiation of renal tubular epithelial cells in diabetic rats and to explore the mechanism. Methods Wistar mts were randomly divided into three groups:control,diabetes and fasudil-treatment.All the rots were sacrificed after three months of feeding with or without fasudil.Pathological changes of the glomeruli and renal interstitium were studied by periodic acidSchiff'S staining and Masson staining,respectively.Expression of ROCKI,α-SMA,E-cadherin and the distribution of β-catenin was examined by immunohistochemistry.Changes in the MYPT1 phosphorylation profile and α-SMA,E-cadherin and membrane β-catenin expression were detected bv Western blot.Changes in the levels of ROCKI,E-cadherin and total β-eatenin mRNA expression were analyzed by real-time PCR. Results Fasudil treatment notably attenuated renal interstitial fibrosis in diabetic rats.Compared to the control rats.diabetic rats showed an elevated phosphorylation of MYFF1(P<0.01),increased expression of α-SMA(P<0.01),decreased expression of E-cadherin and membrane β-catenin(P<0.01,respectively)and increased expression of ROCKI,total β-catenin mRNA(P<0.01,respectively),decreased expression of E-cadherin mRNA(P<0.01 ). Fasudil treatment for diabetic rats attenuated MYPT1 phosphorylation (P<0.01), decreased α-SMA expression (P<0.01), increased E-cadherin and membrane (β-catenin expression (P<0.01, respectively), and reduced ROCKI, total β-catenin mRNA expression (P <0.01, respectively), increased expression of E-cadherin mRNA (P<0.01). Conclusions Fasudil may reduce the epithelial-mesenchymal transdifferentiation and renal interstitial fibrosis in diabetic rats through the inhibition of ROCK activity. Such effect further facilitates the recovery of the cell-cell adhesion among renal tubular epithelial cells and the formation of adhesion complex.

Objective:To evaluate the effect of different dietary pattern and soybean oligosac-charides supplementation on the amount and proportion of short-chain fatty acids (SCFA). Method: Twelve healthy students aged 20 to 25 years old were selected in the medical college. The study included 3 periods. In every period the students accepted different dietary patterns in 1st week [1. low animal food diet (LAFD),2. balanced food diet (BD), 3. high animal food diet (HAFD)]. Adopting the cell model of multilayer spherical symmetry and the circuit analysis, the present paper gives the calculated results of the voltages on each of several parts of malignant Tonsillar B-cells and Jurkat T lymphocytes when the first-order Gaussian pulses at different central frequency apposed on them. The relationship between the central frequency and the transmembrane voltages of plasma membrane is also given. The optimum frequency causing electroporation in nuclear envelope is given as well. The paper discusses the reasons of electroporation in membrane and DNA degradation in nuclear. The work provides a reference for usage of transient bipolar electric pulses in cancer treatment.

9.Short-term prognostic value of the 40 Hz auditory steady state response in patients with brainstem stroke

Objective To investigate the predictive value of the 40 Hz auditory steady state response (40-Hz ASSR) in the short-term prognosis of patients with brainstem stroke.Methods The 40-Hz ASSR and brainstem auditory evoked potentials (BAEP) examinations were recorded and graded from 36 patients with brainstem stroke admitted in the Neurological Intensive Care Unit (NICU). Tue end point of short-term prognostic evaluation was at the time of leaving the NICU. Tue patients with brainstem stroke were divided into the survival group (n =21) and the death group (including brain death,n = 15). The correlation between the grade of the 40-Hz ASSR or BAEP and the short-term prognosis was analyzed. Results The grade of the 40-Hz ASSR (r=0.571,P =0.000) or BAEP (r =0.441,P =0.001 ) was significantly correlated with the short-term prognosis in patients with brainstem stroke. The sensitivity,specificity and accuracy of the 40-Hz ASSR were 60.00%,100% and 83.33%,respectively,and those for BAEP were 66. 67%,71.43% and 69. 44%,respectively. Conclusions Tue 40-Hz ASSR has a certain short-term prognostic value in patients with brainstem stroke. The higher the grade of 40-Hz ASSR is,the more serious the illness,and the worse the prognosis.
10.The role of transoralsonography guiding fine-needle aspiration biopsy in the diagnosis of retropharyngeal or parapharyngeal masses identified on the images of treated patients with malignant carcinoma

Objective To investigate the role of transoralsonography guiding fine?needle aspiration biopsy in the diagnosis of retropharyngeal or parapharyngeal masses identified on the CT, MRI or PET?CT images of treated patients with malignant carcinoma. Methods From 2002 to 2013,this study recruited fifty?five patients with a history of cancer, of which 50 were treated with radiation treatment, including 46 nasopharyngeal carcinoma, 3 esophagus squamous cell carcinoma and1 lung apex carcinoma. There were 4 patients with a history of 1 thyroid papillary carcinoma, 1 buccal mucosa squamous cell carcinoma,1 glottis squamous cell carcinoma and 1 sigmoid colon adenocarcinoma treated with surgery. The rest one patient with nasal olfactory neuroblastoma was treated by postoperative radiation. The enlarged retropharyngeal lymph nodes in 44 cases and parapharyngeal masses in 10 cases were identified on CT or MRI imges. The enlarged retropharyngeal lymph node in the rest case was identified on PET?CT. With transoral ultrasound examination, all lesions were with hypo?intensity echo. Cystic areas were noted on occasion. Biopsy was performed in all cases. Results After cytology examination, carcinoma cells were detected in 37 retropharyngeal lymph nodes, with a detection rate of 82% (37/ 45). In the 10 parapharyngeal masses, carcinoma cells were detected in 3 lesions, with a detection rate of 30%. Conclusions Transor alsonography guiding fine?needle aspiration biopsy can be useful in the cytopathology diagnosis of retropharyngeal or parapharyngeal masses identified on the CT, MRI or PET?CT images of treated patients with malignant carcinoma,which facilitates, early diagnosis and treatment for patients.
