1.Clinical observation of treatment for menopause induced by perimenopausal uterine leiomyoma with different dose of mifepristone

Objective To analyze the different doses of mifepristone in treatment of menopause induced by perimenopausal uterine leiomyoma , and to explore the clinical application of the effective dose of mifepristone.Methods 141 cases with perimenopausal uterine leiomyoma were divided into group A, group B and group C randomly, 47 cases in each group.Three group of patients were given mifepristone.Group A were treated with dose of 12.5 mg/d, group B was treated with 10.0 mg/d, group C was given with 5.0 mg/d.Three groups of patients continued treatment for three months. Uterine volume and leiomyoma volume were observed after treatment, and recorded status of induced menopausal after six months.Results Three groups with uterine volume and leiomyoma volume post-treatment were significantly less than pre-treatment (P<0.05);induction of menopause success rate of group A was 89.4%, menstruation incidence was 6.4%, vaginal bleeding was 2.1%, which was significantly better than the other two groups ( P<0.05) .Conclusion Different doses of mifepristone orally in treatment of perimenopausal uterine leiomyoma has a significant effect, but with 12.5mg/d induced menopause can effectively enhance the success rate , it could be used as the optimal dose of clinical treatment.
2.The effect of mirror therapy supplemented with motor imaging training on the upper limb function of hemiplegic stroke patients

Objective To investigate the effects of mirror therapy supplemented with motor imaging training on the upper extremity functions of hemiplegic stroke patients and on their ability in the activities of daily living (ADL).Methods Forty-four stroke patients were divided into a control group and an experimental group with 22 patients in each.All were given routine rehabilitation treatment.In addition,the experimental group was given 30 minutes of mirror therapy daily supplemented with 15 minutes of motor imaging training.The control group was given routine occupational therapy.All the patients were assessed with Brunnstrom staging,the functional test for the hemiplegic upper extremity-Hong Kong (FTHUE-HK) and the Barthel index (BI) scale before therapy and after 6 weeks.Results After 6 weeks of treatment,both groups showed significant improvements in upper limb function,hand function,and ADL ability.There were significant intra-group differences and significant differences between the experimental and control groups in terms of Brunnstrom staging,FTHUE-HK scores and BI scores.Conclusions Mirror therapy supplemented with motor imaging training can benefit patients suffering hemiplegia in improving their upper limb functioning and their ADL ability.
3.The effect of aerobic exercise of middle intensity on cognitive and motor functions of patients with Alzheimer's disease

Objective To evaluate the effects of aerobic exercise of middle intensity on cognitive and motor functions of elderly people with Alzheimer's disease (AD).Methods Forty-eight patients with AD were studied.The subjects were randomly divided into a treatment group and a control group.The subjects in treatment group were treated with cycle ergometer exercise for 3 months.The control group accepted dementia education.All the patients performed the mini-mental state examination (MMSE) and Alzheimer's disease assessment scale-cognitive subscale (ADAS-Cog) at the beginning and after 3 months of treatment.Results Forty-one participants completed the whole treatment and observation program.After treatment,the scores of MMSE and ADAS-Cog improved significantly more in treatment group than those in control group (P < 0.05).Conclusion Middle intensity of aerobic exercise can improve cognitive and motor function in patients with AD.
4.Clinical effect of intraperitoneal perfusion of verapamil and chemotherapeutic drugs on treating patients with gastric cancer after operation

Objective To explore the clinical efficacy and side effects of intraperitoneal perfusion of verapamil and chemotherapeutic drugs in treating patients with gastric cancer after operation.Methods Totally 184 gastric cancer patients accepted radical surgery with postoperative classification (pT1/2/3/4N2/3M0) were randomly divided into control group (n =58) and treatment group (n =126).For the post-gastrectomy chemotherapeuric treatment,1 g of 5-fluorouracil and 70 to 90 mg of cisplatin were administrated for the patients in the control group,while 1 g of 5-fluorouracil,70 to 90 mg of cisplatin and 25 mg of verapamil were given to the patients in the treatment group.Patients in both groups received intravenous administration of 20 to 30 mg of doxorubicin 3 ~ 5 hours before the intraperitoneal chemotherapy,and 0.1 g of leucovorin 30 minutes prior to the intraperitoneal chemotherapy.On the second day,intraperitoneal chemotherapy of 0.5 to 0.75 g 5-fiuorouracil was given to all patients for 3 to 5 days.The disease-free survival rate,overall survival rate,disease-free survival (DFS),overall survival (OS),peritoneal recurrence rate,liver metastasis rate,gastric remnant recurrence rate,adverse reactions were examined and recorded at the time points of 12 and 24 months after the chemotherapy treatment.Results In the treatment group,disease-free survival and overall survival rates at 12 and 24 months were significantly higher than those in the control group,and the median DFS and OS were significantly longer than the control group (P < 0.05).In patients with regional lymph node staging (N2,N3),low differentiated adenocarcinoma,tumor diameter greater than 4-cm,the disease free survival rate and overall survival rate at 12 and 24-months were significantly higher than the control group,and the median DFS and OS were significantly longer than the control group (P <0.05).In the treatment group,intraperitoneal recurrence rate and liver metastasis rate at 12 and 24-months were significantly lower than the control group (P < 0.05).There was no significant difference in recurrence rate of gastric stump between the control group and treatment group.Application of verapamil did not increase the incidence rate of adverse reactions.There were 8 patients with mild abdominal pain and 3 patients with incomplete intestinal obstruction,and all of them were cured after one week of treatment.Conclusion The intraperitoneal perfusion of verapamil and chemotherapeutic drugs can significantly increase the disease-free survival rate and overall survival rate,prolong the median DFS and OS,reduce the recurrence rate of intra-abdominal and liver metastasis rate in the gastric cancer patients at 12 and 24 months after chemotherapy.

6.The effects of dynamic support-inducing exercise on the motor function of a hemiplegic lower extremity after stroke

Objective To study the effects of dynamic,support-inducing exercise on the support,balance and gait ability of patients with moderate-to-severe hemiplegia after stroke.Methods Fourteen stroke patients were randomly assigned to an experimental or a control group (7 cases to each).The patients in the experimental group received both dynamic,support-inducing exercise and routine exercises,while the patients in the control group received routine exercises only.Before training and after 40 and 60 days of training,their functional capacity was evaluated with the Chinese stroke scale (CSS) for neurological deficits,Berg's balance scale (BBS) and using functional ambulation categories (FACs).Results Before training there was no inter-group difference in average CSS or BBS scores or in FACs.For the experimental group there were significant intra-group differences compared with 0th day in all three items at both time points.At days 40 and 60 there were also significant intra-group differences in BBS scores and FACs in the control group,and CSS scores improved significantly only in the experimental group.At day 40 there were significant inter-group differences in average CSS,BBS and FAC results.However,by day 60 a significant difference persisted only in average CSS scores.Conclusions Dynamic,support-inducing exercise can improve support,balance and gait in patients with moderate-to-severe hemiplegia after stroke.
7.Experimental study of verapamil mediated apoptosis of esophageal cancer cells and multi-drug resistance

Objective To investigate the effect of verapamil on chemosensitivity and apoptosis of esophageal cancer cells.Methods The IC50 values (IC501) of three kinds of chemotherapy drugs:paclitaxel,cisplatin,and 5-fluorouracil were detected by CCK-8 method in 4 kinds of esopha geal cancer cell lines (EC109,EC9706,KYSE-450,TE-1).Each cell was processed by verapamil (4.91 μg/mL) combined with the chemotherapy treatment above,IC50 (IC502) value was detected by CCK8 assay combined with chemotherapy for the verapamil of the 4 esophageal carcinoma cell lines (EC109,EC9706,KYSE-450 and TE-1).A pair of esophageal cancer cells with the most significant reversal of the resistance to chemotherapy by VER detected by the previous experiments were included as research objects,and Annexin V-PI double staining method was used to detect the apoptosis of esophageal cancer cells.Results The IC50 values of paclitaxel in KYSE-450 and EC9706 cell lines were significantly higher than that in EC109 and TE-1.The IC50 values of cisplatin in TE-1 cells were significantly less than those in the other three cells,while the IC50 values of 5-fluorouracil in EC109 were significantly higher than that in other three cell lines.After adding verapamil,the IC50 values of paclitaxel,cisplatin and 5-fluorouracil in four esophageal cancer cell lines (EC109,EC9706,KYSE-450 and TE-1) all decreased to different degrees,and the above three kinds of chemotherapeutic drugs had increased their sensitivity to varying degrees.Among them,the cisplatin group before adding verapamil had different degrees of resistance compared with EC109,EC9706,KYSE-450,TE-1.After joining verapamil,IC50 value change for the EC9706 multiplied for 15.2 times,suggesting that the reversal was sensitive;IC50 value change for EC109 was 1.6 times smaller,suggesting that the reversal was tolerant.Conclusion Verapamil reversal of EC9706 adriamycin has highest efficiency index (15.2 times),there was significant difference compared with the EC109 cells (1.6 times).Tipthe EC9706 (DDP) sensitivity to the reversal of verapamil,and EC109 (DDP) reverse resistance of verapamil.Verapamil may enhance the ability to reverse chemotherapy tolerance by promoting cell apoptosis,thereby enhancing the sensitivity of chemotherapeutic drugs.

9.Application of Canadian Occupational Performance Measure combined with activity analysis in clients with hand injuries

ObjectiveTo explore the occupational needs of hand injury patients and the effects of activity-based analysis and occupation-based interventions. MethodsFrom January, 2021 to June, 2022, 212 patients who accepted occupational therapy for wrist-hand injury in Rehabilitation Center of the First Affiliated Hospital of Nanjing Medical University were interviewed using Canadian Occupational Performance Measure (COPM) to collect the activities that they wanted to do, needed to do, were expected to do, but unable to do. Two senior occupational therapists conducted the activity analysis of the summarized needs, identified occupational components, designed a purposeful occupation-based intervention program, and evaluated the performance and satisfaction using COPM. ResultsA cumulative total of 84 occupational needs were proposed, covering five major domains: work, basic activities of daily living, instrumental activities of daily living, leisure and sleep. The activities mostly impaired involved using computer (8.28%), using chopsticks (6.42%), wringing towels (6.25%), travelling on transport (4.90%) and lifting heavy objects (4.73%). The performance and satisfaction improved after treatment (t > 16.572, P < 0.001). ConclusionThe occupational needs are culturally specific for local hand injury patients; the use of an activity-based analysis and occupation-based intervention is effective on meeting the occupational needs.
10.Investigation on personality traits and related factors of job satisfaction for pediatric therapists

ObjectiveTo investigate the current situation of personality traits and job satisfaction of pediatric therapists in Jiangsu province, and to explore the correlation between them and the related factors of job satisfaction. MethodsFrom June, 2021 to February, 2022, random cluster sampling method was adopted after field investigating several pediatric hospitals and institutions in Jiangsu. Online questionnaires were sent out to pediatric therapists in different settings in eleven cities in Jiangsu. The questionnaires included Minnesota Satisfaction Questionnaire (MSQ), Eysenck Personality Questionnaire-Revised Short Scale for Chinese (EPQ-RSC) and self-made scale of demographic characteristics and working status. ResultsAll of the questionaires were taken back, in which 165 were valid (83.33%). Among the three dimensions of psychoticism, extraversion and neuroticism, the intermediate type accounted for 66.06%, 38.79% and 40.00%, respectively. The overall satisfaction of job was (3.80±0.55), the internal satisfaction was (3.88±0.53), and the external satisfaction was (3.59±0.69). There was significant difference in the satisfaction among different types of work institution, monthly incomes, levels of parental support and neuroticism (P < 0.05). Multiple linear regression showed that the internal satisfaction and overall satisfaction were related with neuroticism, monthly income and parental support (P < 0.05), while external satisfaction was related with the neuroticism and parental support (P < 0.05). The level of neuroticism was negatively correlated with the internal satisfaction, the external satisfaction and overall satisfaction (|r| > 0.337, P < 0.001), and the level of extraversion was postively correlated with the external satisfaction (r = 0.155, P < 0.05). ConclusionThe job satisfaction of pediatric therapists is fine in Jiangsu, and is ralated with the level of neuroticism, the monthly income and the level of parental support. The levels of neuroticism and extraversion correlate with the job satisfaction.
