1.A histologic study on osteophytes in severe osteoarthritis by Masson staining and immunohistochemical staining for collagen type Ⅱ

Objective To establish a histological classification system for osteophytes by Masson staining and immunohistochemical staining for collagen type II. Methods Osteophytes were collected from knee joints in os?teoarthritis patients after joint replacement surgery. To develop an osteophyte classification ,serial tissue sections were identified using histological(hematoxylin and eosin,Masson ' s trichrome,toluidine blue)and immunohisto?chemical staining(collagen typeⅡ). Joint function was assessed by WOMAC score and radiological findings was measured by Kellgren?Lawrence grading. The consistency between the osteophyte classification and WOMAC/Kell?gren?Lawrence grading were evaluated. Results 31 patients and 101 osteophyte samples were included in this study. Based on the histological and immunohistochemical evaluation,osteophytes can be divided into type A (4.9%),type B(4.0%),type C(26.7%)and type D(64.4%). This histological classification system had better consistency with joint function score(χ2 = 0.108,P = 0.947)and Kellgren?Lawrence grading(χ2 = 3.205,P =0.201). Conclusions This trial establishes a histological classification system for osteoarthritis osteophytes ,types A and B indicating early osteophyte and types C and D indicating advanced osteophyte. The system is helpful in clini?cal and basic research on osteoarthritis.
2.Significance of young females’ different vertical facial types on the aesthetic evaluation of facial profiles

BACKGROUND:It is confirmed that the characteristics of soft and hard tissue development vary largely in different vertical facial types, which reflect the development trend of the patients and the control ing degree on anchorage.
OBJECTIVE:To evaluate the influence of different vertical facial types on the aesthetic evaluation of facial profiles in young females, and analyze the perception of facial attractiveness from different people.
METHODS:Three beautiful females, with high, average and low mandibular angle, respectively were selected according to the measurement data of projection and the orthodontist’s aesthetic appreciation. They were yel ow race, bilateral facial appearance was basical y symmetrical, and no other maxil ofacial disease was found. They also received no orthodontic or orthognathic treatment. The profile digital photographs and cephalograms of three females were col ected. The positions of soft tissue lip and soft tissue chin were altered incremental y with Dolphin Imaging and Management software. Twenty-five new pictures were generated by the smal scale (2 mm per unit) modification in the sagittal direction. By using“acceptable facial appearance”as a filter, 15 orthodontists selected 11 from 25 pictures. Fifty parents of orthodontic patients, 50 orthodontic patients, and 50 amateurs were taken as the evaluators for subjective aesthetic ratings and for the assessment of the influence of vertical facial types on the facial profile attractiveness.
RESULTS AND CONCLUSION:With low and average angle, straight profile, slightly protrude chin was perceived to be beautiful. With high angle, straight profile, slightly protrude profile was perceived to be beautiful, and evaluation results are consistent between men and women. Evaluators are in agreement when it comes to evaluating the perception of facial attractiveness of three different vertical facial types profile in young females. They also have certain difference in aesthetic tendencies, but when it comes to straight profile they share the same preference. Although different vertical facial types profile has an impact on the profile of lip and chin, but it can’t represent the aesthetic preference of al evaluators.
3.Imaging features in children with Kallmann syndrome

Objective To summarize imaging features of Kallmann syndrome (KS) in children and to improve diagnostic level of the disease.Methods The imaging manifestations of 13 patients with clinically proved KS were retrospectively studied.MRI of rhinencephalon and left wrist X-ray examinations were performed in all the 13 children.Ultrasound of abdomen and pelvis was implemented in the 8 of 13 patients.Results All the patients had abnormalities of olfactory system.Bilateral olfactory bulbs and tracts were absent in 9 patients.Unilateral olfactory bulbs and tracts were absent in 4 patients,in which contralateral olfactory bulbs and tracts were hypoplastic.These patients presented 2 aplastic,17 hypoplastic and 7 normal olfactory sulci.Anterior pituitary was hypoplastic in 3 patients.Bone age showed retardation in 6 of 13 patients.Bilateral testicles were small in all the 8 cases by ultrasound.Conclusions Aplastic or hypoplastic olfactory bulbs and tracts and sulci are seen in children with KS.Anterior pituitary dysplasia is present in part of patients and bone age backward happens in approximate half of the children.
4.Burdens of Spouse Caregivers of Patients with Dementia at Home

Objective: To explore the burden of spouse caregivers of patients with dementia at home and factors having influence.Methods:Forty-two patients with dementia were collected from psychiatric out-clinic consecutively. Trained investigators did home visit to collect data on severity of the disease (CDR, MMSE), pathological behavior (BEHAVE-AD), activity of daily living (ADCS-ADL) and burdens of their spouse as caregiver (CBI). The caregivers' anxiety, depression (Zung's Anxiety Scale and Depression Scale) and knowledge about dementia were also collected.Results:The caregivers felt big burden (BDI total score: 30.7?15.1). Nearly half of them (42.9%) were in depression and one quarter (23.8%) were anxious. They were lack of knowledge about dementia and unskillful in care-giving. 26.2% caregivers thought dementia is curable or equal to normal aging, 57.1% of them did not believe it is an improving disease. 5.Nursing of replantation of amputated finger patients accepting brachial plexus block analgesia and patient-controlled sedation

Objective To probe into nursing management methods of replantation of amputated finger patients accepting brachial plexus block analgesia and patient-controlled sedation (PCS) after operation.Methods 48 patients of replantation of amputated fingers accept continuous brachial plexus block analgesia and PCS after operation,Analgesia was 0.2% ropivaeaine,sedation was 0.05% midazolam or compositive liquid mixed 0.05% midazolam and 0.0005% fentanyl,the visual analogue scale (VAS),sedation scores and the changes of vital signs and side effocts after operation were recorded before the operation and at time intervals of 6,12,24,48,72 hours after the operation.Results The visual analogue scale(VAS)in the post-operation was obviously lower than the pre-operation,the Ramsay scores of PCS were higher than the pro-operation.There was no inadequate or excessive sedation.No influence was seen on respiration,circulation system and no other side effects happened.Conclusions Brachial plexus block analgesia and patient-controlled sedation (PCS) can offer a good Analgesia and sedation effect for patients of amputated fingers replantation,in period of analgesia and sedation,we should strengthen nursing and monitoring of vital signs to avoid accident.
6.The Current Status and Influence Factors of Indoor Air Pollution in Urban Area in Beijing

Objective To investigate the current status of the indoor air pollution in urban area in Beijing and find the influence factors for the concentrations of formaldehyde and nitrogen oxides (NOx) indoors. Methods 463 persons were chosen randomly from Xicheng district of Beijing to complete the questionnaires about their indoor environment condition. The levels of formaldehyde and NOx were monitored in the bedrooms and the kitchens of 91 households. And we used the multi-regression model to find out some influence factors that related to formaldehyde and NOx concentration. Results The average daily concentration of formaldehyde in the bedrooms was about 0.049 mg/m3 and the rate of excess standard was 13%. The formaldehyde pollution in the decorated rooms (0.058 mg/m3) was more serious than that in the control rooms (0.034 mg/m3) and there was statistically significant difference between them (P
7.Self-expandable metal stents for gastric outlet and duodenal obstruction

ObjectiveTo investigate the efficacy and safety of the self-expandable metal stent for treatment of gastric outlet and duodenal obstruction.MethodsData of 24 patients with symptomatic gastric outlet and duodenal obstruction diagnosed from January 2005 to February 2011 were retrospectively studied.All patients were measured by gastric outlet obstruction scoring system ( GOOSS ) before and one week after the placement of self-expandable metal stent.ResultsStent placement was succeeded in all patients with a mean operation time of 25.90 ± 11.03 min.Twenty patients (83.3%) were able to intake liquid food after the procedure.The GOOSS (P50 =0) before and after stent placement were 0.14 (P50 =0) and 1.42( P50 =2),respectively,with significant difference (P <0.01 ).Minor digestive bleeding occurred in 3 patients,but no severe complications including perforation or aspiration were observed.ConclusionPlacement of a self-expandable metal stent in patients with gastric outlet or duodenal obstruction is safe and effective for resolving obstructive symptoms.
