1.Influence of psychological nursing on perioperative irritated response of patients undergoing gyne-cological surgeries

ObjectiveTo observe influence of psychological nursing on perioperative irritated response of patients undergoing gynecological surgeries. MethodsOne hundred and twenty gynecological surgical pa-tients were randomly divided into the psychological nursing group and the control group, each group included 60 cases. Patients in the control group were given routine nursing. In addition of routine nursing, patients in the psychological nursing group were given perioperative psychological nursing intervention. The change of psycho-logical reaction, heart rate and blood pressure of the two groups were compared. ResultsCompared with the control group, number of patients with 3 to 4 grade of anxiety and terror in the psychological nursing group was significantly lower. Compared with that of preoperative day, heart rate and blood pressure in the control group evidently increased 5 minutes after entering the operation room, while in the psychological nursing group, except heart rate increasing, blood pressure was more stable. ConclusionsPsychological nursing can alleviate anxi-ety of patients undergoing gynecological surgeries, maintain normal hemodynamics and promote early recovery of patients.
2.Laparoscopic Hysterectomy for Large Uterus:Experience of 200 Cases

Objective To explore the feasibility and safety of laparoscopic resection of large uterus . Methods We performed laparoscopic hysterectomy in patients who had more than 3 months pregnant huge uterus in our hospital from January 2010 to December 2013.The mirror trocar was introduced at 5 cm above fundus .Surgical key steps were appropriate management of uterine blood vessels, electrical coagulation of the ascending uterine artery , and dissection of uterine vessels from front , rear, and lateral sides. Results The laparoscopic surgery was completed in all the 200 cases, without conversion to laparotomy .The operation time was 80-170 min,with a mean of 120 min;the blood loss was 20-250 ml,with a mean of 60 ml.No blood transfusion was required due to hemorrhage during operation .Two cases developed deep vein thrombosis at the third day after surgery , and were cured by anticoagulant therapy .Two patients with vaginal stump inflammation at 15 postoperative day were cured by applying the metronidazole powder at vaginal .One patient were sutured by 2 stitches for vaginal stump bleeding at the first month after the operation .The remaining patients had no serious complications .Postoperative hospital stay was (5 ±2) days.Follow-up reviews in 144 patients for 6 months found satisfactory heal of the vaginal stump , normal ultrasonographic results of bilateral ureters , and no abnormity in the pelvic cavity. Conclusion Laparoscopic hysterectomy of large uterus with bipolar coagulation is safe and feasible .

5.Comparative study on the effects of automatic and semi-automatic hemodialyzer reuse

Objective To explore the influence of automatic and semi-automatic hemodialyzer reuse method on hemodialyzer reuse effect. Methods 1728 dialyzers were randomly divided into automatichemodialyzer reuse group and semi- automatic hemodialyzer reuse group with 864 dialyzers in each group. Thetime of douching and testing, the cost of sterilization,the frequency of the pyrphgen reaction,the broken dialyzer membrane and re-examined dialyzer between the two groups were measured. Results The time of douching dialyzer, testing of total cell volume and pressure in the semi- automatic hemodialyzer reuse group was (26.443±3.237), (2.172±0.128) and (2.157±0.090) minutes respectively,while the automatic hemodialyzer reuse group was (5.793±0.193), (1.257±0.118) and (1.110±0.076) minutes respectively. The frequency of re-examined dialyzer in testing total cell volume and pressure was 499(57.755%), 243(28.125%) respectively. At the same time, all dialyzer in semi-automatic hemodialyzer reuse group could be examined successfully at a time. The cost of sterilization in automatic henmdialyzer reuse group was (9.330±0.138)yuan. No pyrogen reaction and broken dialyzer membrane happened. Conclusions The semi-automatic bemodialyzer reuse group can retrench cost during perfusion,but consumes long douching time, lacks matching detection equipment, difficult to detect, and is not easy to read data and has high re- examination rate. while in the automatic hemodialyzer group, it is convenient of douching and detection, but the cost of sterilization and equipment is high, and clinical demand can be fulfilled only when the dialysis center can allocate reasonable number of the machines.
6.Effects of shoutai pills on expression of Th1/Th2 cytokine in maternal-fetal interface and pregnancy outcome.

OBJECTIVETo evaluate its mechanism of inducing the maternal-fetal immune tolerance by studying the effects of Shoutai pills on the expression of Th1/Th2 cytokine and pregnancy in maternal-fetal interface of mice with recurrent spontaneous abortion (RSA).
METHODThe normal pregnancy and RSA model were respectively induced with CBA/J x BALB/c and CBA/J x DBA/2. The mice with RSA were randomly divided into model group and low, middle and high dose groups of Shoutai pills. The mice were killed in 14 days after administration and embryo resorption rate was counted and their decidual and placental tissues were co-cultured to detect the expressions of IL-4, IL-10, IFN-gamma and TNF-alpha with ELISA.
RESULTThe embryo resorption rate of the model group was significantly higher than the normal pregnancy, middle and high dose groups of Shoutai pills could decreased the embryo resorption rate of the mice with RSA (P < 0.05). All the doses in 3 groups of Shoutai pills could decreased the expression of IFN-gamma and TNF-alpha (P < 0. 05) and there was no obvious difference between normal pregnancy group and all groups of Shoutai pills. Middle and high doses of Shoutai pills could increased the expression of IL-4 and IL-10 (P < 0.05) and there was no obvious differences between normal pregnancy and high dose group of Shoutai pills.
CONCLUSIONThe mechanism about Shoutai pills can change Th1 /Th2 cytokine towards Th2 bias, which induced the maternal-fetal immune tolerance.

Objective:
To investigate human enterovirus (HEV) infection and clinical characteristics of viral encephalitis patients in Pingdingshan, Henan Province.
Methods:
Cerebrospinal fluid specimens and epidemiological information were collected from 274 viral encephalitis patients in the departments of pediatrics and neurology in hospitals in Pingdingshan, Henan Province, from April 2011 to August 2012. Patients with bacterial infections were excluded from the study. Demographic information was collected by questionnaires and clinical information was mainly obtained from hospital examinations. Objective To develop a computer-aided-controlling and analysis system for light/dark box in mice and rats with a high degree of automation and intelligence.Methods Video recording and image processing were applied to develop the computer-aided-controlling and image analysis system for light/dark box test in mice and rats. The artificial environment was developed. The stability and reliability of the system was validated by male rats. Results The percentage of time spent in the lit chamber in total time was above 79.40%. The data showed that the artificial environment was successful. When the threshold was set at 18 cm/s, the data showed a high correlation coefficient of movement time between the computer and manual recordings(r > 0.99). Classical indexes including transition and time spent in both the lit and dark chambers also showed a high correlation. The model group showed a significantly decrease in the transitions and time spent in the lit chamber compared with the control group, indicating a high stability and reliability of the light/dark box test. Conclusions A stable and highly intelligent computer-aided-controlling and image analysis system for light/dark box test of mice and rats has been developed,and it could be used for pathological mechanism studies of anxiolytics.
9.Research in application of the ultraviolet fluorescence detection technology in personal protective equipment training

Objective:To study the role of ultraviolet fluorescence detection technology in personal protective equipment education (PPE) and training.Methods:A study was designed to inspect the risk of self-contamination during PPE doffing between 77 healthcare workers. Used a fluorescent tracer slurry which put on the hands, chest, abdomen, knees to simulate the contaminations. Self-contamination of scrubs and skin was measured using ultraviolet light visualization respectively.Results:According to the uv-fluorescer simulating study, 43 (55.8%) of the medical staff had contamination after the removal of PPE, and the main sites of contamination included: left side of the abdomen 11 (11.70%), left side of the chest 9 (9.57%), left forearm 6 (6.38%), left foot instep 6 (6.38%), neck 6 (6.38%), right shoulder 5 (5.32%), etc. Among them, the frequency of simulated fluorescence pollution in the group with working years less than 6 years was less than that in the other groups, and the difference was statistically significant compared with the group of 11-15 years ( t value was -3.685, P value was 0.001 ). Conclusion:Ultraviolet fluorescence labeling detection technology can directly, quickly and effectively evaluate and feedback the key contaminated parts in the process of using PPE, which can provide detailed evidence for redesigning PPE and improve the PPE training process to reduce the contamination.
10.Diagnosis and clinical characteristics of patients with non-allergic rhinitis.

OBJECTIVETo explore a step-by-step exclusive diagnosis and analyze the clinical characters of non-allergic rhinitis (NAR).
METHODSPatients with symptoms (nasal itching, sneezing, rhinorrhea, nasal congestion) were selected to take four-step exclusive diagnosis for NAR and we tried to eliminate the false NAR and retain the true NAR. First step was to exclude the patients who were not suitable for skin prick test (SPT, such as during pregnancy, breastfeeding, asthma, oral antihistamine medication in 7 day, severe skin diseases). The second step was to exclude the patients with positive SPT and the third step was to exclude the patients with 1 level or above of specific sero-immunoglobulin E (sIgE). The fourth step was to exclude the patients with infection rhinitis, clear abnormal nasal structure, drug-induced rhinitis, nasal neoplasm. The remained patients were finally diagnosed as NAR and who were further differential diagnosed as vasomotor rhinitis (VMR) or non-allergic rhinitis with eosinophilia syndrome (NARES) according to the eosinophilia counts in nasal secretion and venous blood. The common characters of patients with NAR were analyzed and their symptoms and quality of life were evaluated by visual analogue scale (VAS) and rhino-conjunctivitis quality of life questionnaire (RQLQ) separately.
RESULTSOne thousand four hundred and thirty-seven patients were included after first step exclusion and 735 cases with negative SPT were remained after second step exclusion. Of 735 patients, 302 were tested in vitro for sIgE and 93 cases with 0 level of sIgE and total IgE were remained after third step exclusion. Sixty-two patients were finally diagnosed as NAR after fourth step exclusion. The NAR diagnosis rate was 51.15% (735/1 437) with negative SPT alone and the NAR diagnosis rate was 29.06% (93/302) with combination of negative SPT and sIgE. Of 62 patients with NAR, 47 patients (75.81%) were diagnosed as VMR and 15 cases (24.19%) as NARES. There were 23 males and 39 females in the 62 patients aged 11 - 77 years. The history was 11-47 months. The biggest numbers of patients with VMR or NARES were among 41-50 years. Their onset ages were among 21-30 years in both two groups. VAS scores of nasal congestion in VMR patients were the highest with significant difference among nasal symptoms (F = 3.958 0, P = 0.009 1). VAS scores of sneezing in NARES patients were the highest but without significant difference among nasal symptoms. There were no difference in seven domain scores of RQLQ and the total mean scores between VMR group and NARES group but the nasal symptoms got the highest scores with significant difference among the seven domains in each group (VMR group, F = 9.771 2, P = 0.000 0;NRAES group, F = 3.226 9, P = 0.006 2).
CONCLUSIONSSPT combined with sIgE may exclude much more patients with AR. Females with NAR are much more than males. Patients with NAR aged 21-30 years. The characters of NAR are helpful to improve our knowledge about NAR. VAS and RQLQ may be a suitable tool in assessment of NAR.
