1. Comparison of long-term result of endovenous laser ablation vs high ligation and stripping for varicosity of great saphenous vein: A meta analysis

Objective Varicosis of great saphenous vein has a gradually increased morbidity rate in the troop. We conducted a meta-analysis of the result of two main treatment modalities, namely EVLA (endovenous laser ablation) and HL/ S (high ligation with stripping), in order to provide a better reference for the surgical option. MethodsLiterature published before November 2014 was selected and screened on the basis of inclusion and exclusion criteria for PRISMA statement (preferred reporting items for systematic reviews and meta-analyses), and Epidata (3.1 version) were used for data extraction and RevMan (5.2 version) used for meta-analysis. ResultsA total of seven randomized controlled trials in patients with 1402 cases involving 1455 legs were included in this study. Meta-analysis indicated that: technical success rate was 98.5% and 98.4% for HL/S and EVLA, respectively and OR was 1.06 (95% CI 0.45-2.52), P=0.89; long-term recurrence rate was 15.8% in HL/S and 15.5% in EVLA, OR 1.11 (95% CI 0.18-1.52), P=0.53; long-term reflux rate was 12.5% for HL/S and 15.7% for EVLA, OR 0.65 (95% CI 0.34-1.22), P=0.18. The mean difference of AVVSS (Aberdeen Varicose Vein Symptoms Severity Score) was 0.17 (95% CI -0.63-0.96), P=0.68. ConclusionThere is no significant difference between EVLA and HL/S in the success rate, long-term recurrence rate, long-term reflux rate, and long-term postoperative AVVSS, and both treatment modalities could be the appropriate option for the treatment of varicosity of great saphenous vein.
2.Evaluation of the immunogenicity of recombinant replicative DNA vaccines expressing multiple anti-gens of hepatitis C virus in a mice model

Objective To investigate the immunogenicity and cross protective effects of two novel HCV DNA vaccines in a mice model.Methods Two self-replicating alphavirus vector-based HCV DNA vaccines, pSCK CE1E2Y and pSCK H155, were constructed based on the genes encoding the structural pro-teins (Core, E1 and E2) and structural and NS3 fusion proteins (Core, E1 , E2 and NS3) of a HCV strain isolated from a Chinese patient (genotype 1b, Hebei strain), respectively.Western blot analysis was per-formed to detect the expression of fusion antigens.The BALB/c mice were intradermally immunized with the recombinant DNA vaccines by using electroporation.The immune responses induced in mice and the cross protective effects of the recombinant DNA vaccines were evaluated.Results The DNA vaccines effectively expressed the target antigens in vitro.The antigen-specific antibody responses and specific T cell immune re-sponses were induced in mice by the immunization of replicative DNA vaccines.However, no effective cross protection was provided by either of the DNA vaccines in the surrogate challenge model based on a recombi-nant heterologous HCV (JFH1, 2a) vaccinia virus strain.Conclusion Although no effective cross protec-tion was observed, both of the two replicative DNA vaccines could induce strong humoral and cellular im-mune responses against multi-target antigens of HCV strains.This study has paved the way for further inves-tigation on the development of novel HCV vaccines.
3.Research progress of a new immune checkpoint inhibitor T-cell immunoglobulin and immunoreceptor tyrosine-based inhibitory motif domain in anti-tumor immunotherapy

The T-cell immunoglobulin and immunoreceptor tyrosine-based inhibitory motif domain (TIGIT) is an inhibitory receptor mainly expressed on active T-cells, or natural killer cells (NK cells) that activate negative stimulus signals in immune cells by combining with multiple ligands on the surface of target cells including tumor cells and infected cells. TIGIT plays an important regulatory role in the immune pathogenesis of tumors, viral infections and various autoimmune diseases by inhibiting the over activation of cells and the over secretion of proinflammatory cytokines. Recent researches show that TIGIT is highly expressed in T cells and NK cells of cancer patients, and is related to disease progression and poor clinical prognosis. Researchers try to enhance the activity of T cells or NK cells by blocking the binding of TIGIT and its ligand for therapeutic intervention. At present, there have been many reports about the use of anti-TIGIT monoclonal antibody treatment in different mouse tumor models leading to tumor regression, TIGIT has received extensive attention in cancer immunotherapy as a promising target for next generation cancer immunotherapy. Several clinical trials are currently evaluating the efficacy of anti-TIGIT monoclonal antibodies (mAbs) in patients with several cancers. The most advanced candidate, tiragolumab, has exhibited remarkable efficacy in programmed cell death ligand 1 (PD-L1)-positive non-small cell lung carcinoma (NSCLC) patients in phase Ⅱ clinical trials, in combination with PD-L1 blockade. However, the specific mechanism of TIGIT blockade remains to be fully elucidated.

Objective:
To understand the prevalence of hepatitis C virus ( HCV ) infection in sentinel surveillance in Liaoning Province from 2015 to 2019, in order to provide evidence for the precise prevention and control of HCV.
Methods:
The HCV sentinel surveillance data of 5 kinds of populations, including dialyzed patients, patients receiving invasive diagnosis and treatment, family planning outpatients, people receiving physical examination and unpaid blood donators in Liaoning Province from 2015 to 2019 were retrieved, and the HCV positive rates of different years, gender and age groups in the five populations were analyzed.
Results:
There were 26 000 persons surveyed and 61 HCV positive cases were detected from 2015 to 2019. The HCV positive rate was 0.23%. The HCV positive rates of dialyzed patients, patients receiving invasive diagnosis and treatment, family planning outpatients, people receiving physical examination and unpaid blood donators were 0.40%, 0.70%, 0.25%, 0.24% and 0.14%, respectively. The HCV positive rates of dialyzed patients, patients receiving invasive diagnosis and treatment, and unpaid blood donators had significant downward trends ( P<0.05 ). The HCV positive rates of males and females were 0.22% and 0.26%, without any significant difference ( P>0.05 ). The HCV positive rate of people aged above 70 years old was 1.15%, the highest among different age groups.
Conclusions
The prevalence of HCV infection among the patients with hemodialysis and with invasive diagnosis and treatment is relatively high. The level of HCV antibody of older age groups should be monitored regularly.
6.Clinical observation of sinew-regulating and bone-setting manipulation combined with functional exercise to treat rotator cuff injury

Objective:To explore the clinical efficacy of sinew-regulating and bone-setting manipulation combined with functional exercise to treat mild to moderate rotator cuff injury (RCI).Methods:Forty patients with mild to moderate RCI were randomly divided into a treatment group and a control group,with 20 cases in each group.The treatment group was given sinew-regulating and bone-setting manipulation combined with functional exercise,and the control group was given the same manipulation as the treatment group.The therapeutic effect on the functional improvement was assessed after treatment according to pain threshold,shoulder range of motion (ROM) including flexion,abduction,internal rotation and external rotation,and the University of California at Los Angeles (UCLA) shoulder rating score.Results:Compared with the same group before treatment,the pain threshold,shoulder flexion,abduction,internal and external rotations increased after the treatment in the two groups (all P<0.05).The UCLA shoulder rating score increased (both P<0.05).The improvement in the treatment group is significantly better than that in the control group with a statistical significance (P<0.05).The total effective rate was 95.0% in the treatment group,was higher than 65.0% in the control group (P<0.05).Conclusion:The sinew-regulating and bone-setting manipulation combined with functional exercise relieve the pain of patients with mild to moderate RCI,increase the motion of shoulder joints,and improve the quality of life of patients.The curative effect is better than the treatment of simple sinew-regulating and bone-setting manipulation.
8.Role of Overexpressed Proinflammatory Cytokines in the Pathogenesis of Cerebral Palsy

ObjectiveTo investigate the role of overexpressed proinflammatory cytokines in the pathogenesis of cerebral palsy (CP).MethodsLevels of tumor necrotic factor-α (TNF-α) and interleukin-6 (IL-6) in serum of 31 CP children, 20 healthy children (as controls), 37 neonates with CP risk factors such as hypoxic-ischemic injury and/or perinatal infection, and 20 healthy neonates (as controls) were measured by enzyme-linked immunosorbent assays (ELISA) retrospectively.ResultsLevels of TNF-α and IL-6 of CP children and neonates with CP risk factors were significantly higher than that of healthy controls (P<0.05). TNF-α level of CP children was significantly higher than that of neonates with CP risk factors (P<0.05), but there was no significant difference in IL-6 level between two groups.ConclusionOverexpressed proinflammatory cytokines play an important role in the pathogenesis of CP and may be an independent risk factor of CP.
9.Clinical analysis of vitrectomy in treatment of 48 eyes with metallic foreign bodies

AIM:To investigate the effect of vitrectomy in treatment of metal intraocular foreign bodies and the factors affecting visual prognosis.
METHODS:Fourty seven cases ( 48 eyes ) with foreign bodies from January 2010 to June 2013 in our hospital underwent vitrectomy combined with intraocular foreign body removal were retrospectively analyzed. The changes of visual acuity, the size of foreign body, the complications of preoperative, and the relationship between treatment time and visual acuity were recorded.RESULTS: Totally 46 eyes were successfully removed the complete foreign body, 2 eyes of orbital foreign bodies had not been removed. After 6mo of followed-up, the best corrected visual acuity were all improved in varying degrees (P<0. 05), intraocular foreign body size combined with retinal detachment was the risk factor for visual impact (P<0. 05).
CONCLUSION:Vitrectomy has obvious advantages in the treatment of metal intraocular foreign bodies. Timely and appropriate vitrectomy is very important to the recovery of visual function.
