2.Diagnosis and treatment of rectal stromal tumor

Gastrointestinal stromal tumor (GIST) is a group of tumors which is independently derived from the mesenchymal stem cells of the digestive tract,and consists of undifferentiated or multi-functional spindle or epithelioid cells.Most of the rectal stromal tumor located at the middle or lower part of the rectum,and presented with exogenous growth outside of the rectum cavity.Rectal stromal tumor accounted for 5% of GIST.Rectal stromal tumor is easily missed or misdiagnosed because of insidious onset and non-typical clinical manifestations.The diagnosis of rectal stromal tumor relies on immunohistochemical staining (CD117 and CD34) and pathological examination.Surgical resection is the first choice for the treatment of local primary rectal stromal tumor.Protection of the integrity of the tumor is important during operation,and routine lymph node dissection is not recommended.Molecular targeted drug mesylate imatinib is adopted in the pre-and postoperative adjuvant therapy.However,due to the rarity of rectal stromal tumor,the treatment strategies need further investigation.
3.Application of functional magnetic resonance imaging in medulloblastoma

Medulloblastoma was a common malignant, invasive neoplasm of the central nervous system occurring in children. This disease had the characteristics of easy recurrence and the prognosis remained relatively poor, which heavily influenced healthy and quality of life of children. In recent years, advanced magnetic resonance imaging (MRI) techniques such as diffusion weighted imaging (DWI), diffusion tensor imaging (DTI)and 1H- magnetic resonance spectroscopy (1H- MRS) were the new noninvasive technique of diagnostic imaging to be used in the diagnosis and prognosis of medulloblastoma, which showed very significant clinical value and made some new progress. In this study, we talked about the DWI, DTI and 1H-MRS used in medulloblastoma.
4.Perioperative biochemical markers and early postoperative mortality in senile patients with femoral intertrochanteric fracture

Objective To investigate the associations between levels of perioperative biochemical markers of the liver,kidney,lung and heart and death within 3 months postoperatively in the senile patients with femoral intertrochanteric fracture.Methods A retrospective study was conducted on the 153 senile patients with femoral intertrochanteric fracture who had been treated at Nanfang Hospital from January 2010 to January 2012.They were 71 men and 82 women,with an average age of 83.1 years.There were 70 stable and 83 unstable fractures.At preoperation (within 24 h after admission),and 24 h,25 to 48 h and 72 h postoperation,all the patients had blood examinations of biochemical markers of the liver [alanine aminotransaminase (ALT)],kidney (creatinine and urea nitrogen),lung (PaO2) and heart [brain natriuretic peptide (BNP)].The levels of biochemical markers at all time points and clinical data were compared between the living patients and those who died within 3 months postoperatively.Univariate and multivariate logistic regression analyses were conducted to estimate the independent risk factors for mortality within 3 months postoperatively.Results Of the 153 patients,32 (20.9%) died within 3 months postoperatively,including 10 men and 22 women.In the dead and surviving patients,there were respectively 25 and 17 cases who were rated as level Ⅲ or Ⅳ by ASA (American Society of Anesthesiologists).The levels of ALT,creatinine,urea nitrogen,and BNP increased to different extents while the level of PaO2 decreased postoperatively in both the dead and surviving patients.The differences between the dead and surviving patients were statistically significant (P < 0.05).Multivariate logistic regression analysis showed poor preoperative physical condition (ASA level Ⅲ or Ⅳ),increased creatinine level at 25 to 48 h postoperation,decreased PaO2 at 72 h postoperation,and increased BNP level at 24 h postoperation were independent risk factors for early mortality in senile patients with femoral intertrochanteric fracture.Conclusions Femoral intertrochanteric fracture in senile patients may lead to changes in the liver,kidney,lung and heart.Prompt and dynamic monitoring of the levels of PaO2,creatinine and BNP may provide timely prediction of the poor prognosis.
5.Investigation of the health related quality of life in patients with chronic hepatitis C

Objective To evaluate the health related quality of life (HRQOL) of patients with chronic hepatitis C(CHC) and the improvement after interferon therapy. Methods Using the SF 36 questionnaire and the hepatitis specific scale derived from the Medical Outcomes Study (MOS), the HRQOL in 67 non cirrhotic patients with chronic HCV infection were evaluated, 38 patients of which were treated with IFN ?2a 3MU( n=31) and CIFN 15 ?g ( n =7), three times a week for 6 months. 12 patients had a sustained viral response (undetectable serum HCV RNA and normal ALT level at 24 weeks post treatment) to interferon therapy. 26 patients had no response. Live biopsies were performed in 23 patients and the diagnosis of chronic hepatitis proved. These scales were self administered by patients at baseline and at 24 weeks post treatment. Patients with chronic hepatitis C were compared with healthy volunteers ( n =40) who were selected as normal controls, the age, sex, education profession were matched between them. 7.The application and research of Xueshuantong combined with radiofrequency thermocoagulation on the treatment of low back pain caused by lumbar degeneration

Objective To investigate the value of Xueshuangtong combined with radiofrequency thermocoagulation in the treatment of lumbar degenerative lumbar pain.MethodsFrom January 1, 2014 to December 30, 2015, 60 patients with lumbar degenerative low back pain admitted in Jiashan Traditional Chinese Medicine Hospital.All of them were randomly divided into observation group and control group, and each with 30 cases.All patients received radiofrequency thermocoagulation, while the observation group received Xueshuantong for further treatment.All of the patients were assessed by the international Pain Association recommended the use of visual analogue scale (VAS) and McGill pain questionnaire (SF-MPQ) and CRP when treatment before starting and after 1, 3, 7, 14, 30 and 90d of the treatment.ResultsThere were no significant differences between the two groups in VAS, SF-MPQ and CRP before treatment (P<0.05).However, when compared with the control group, patients in the observation group got significantly lower levels of VAS at 1, 3, 7 and 14d after the operation;significantly lower levels of SF-MPQ at 1, 3, 7, 14 and 30d after the operation;and significantly lower levels of CRP at 14, 30 and 90d after the operation (P<0.05).And we found no serious complications occurred in the two groups.ConclusionThe Xueshuantong combined with radiofrequency thermocoagulation treatment of lumbar facet joint is a safe, effective and inexpensive for the low back pain which caused by lumbar degeneration.

10.Arthroscopic partial meniscectomy for medial meniscal tear in late middle-aged adults.

OBJECTIVETo study the results of arthroscopic partial meniscectomy for medial meniscal tear in late middle-aged adults.
METHODSFrom October 2010 to October 2012,49 patients meeting inclusion criteria were treated with arthroscopic partial meniscectomy for medial meniscal tear. There were 31 males and 18 females,with an average age of (55.3±2.8) years old. There were 22 left knees and 27 right knees. The average flexion angle was (116.01±12.03)°. The pre-operative HSS Knee Score was 48.73±8.43, and the Lysholm score was 63.95±5.45. The patients were followed up and evaluated. All the patients received a standard surgery, in which the torn tissue was removed, and the anterior part of the meniscus was left in situ.
RESULTSAll the surgeries were successful with no serious complications. All the patients were followed up,and the duration ranged from 12 to 36 months. All the patients had 5 degrees of muscle strength. The average flexion angle was (136.77±18.56)°. There was significant difference between the two angles before operation and after operation. The pre-operative HSS Knee Score was 90.17±4.10, and the Lysholm score was 87.84±5.16. Statistically, the difference between preoperative score and the postoperative score was significant.
CONCLUSIONArthroscopic partial meniscectomy is a good option for medial meniscal tear in late middle-aged adults. Operative indication is crucial and the excellent surgical technique is also critical for the good clinical outcome. All the patients could get good clinical results, although there are some patients with motion restrictions in the early stage after operation.
