1.Navelbine in the Treatment of Recurrent and Metastatic Breast Cancer.

Objective To evaluate the efficacy and adverse reactions of Navelbine(NVB)in the treatment of recurrent and metastatic breast cancer.Methods 34 patients with recurrent and metastatic breast cancer were treated with Navelbine in this clinical study,and 16 patients were treated by the NP(DDP+NVB)regimen,12 patients by the NA(ADM+NVB)regimen,6 patient by single-agent NVB.34 patients received above regimens for two cycles(3~4 week a cycle).Result The overall response rate(CR+PR)was 35.3%,clinical benefit rate(CR+PR+SD≥6 months)44.1%,disease contrrol rate(CP+PR+SD)58.8%,the median time to failure 3.4 months(range:0.7~11 months),the median time to progression(TTP)3 months(range:0.8~11 months),and the median duration of response(CP+PR)5 months(range:2~11 months).The main side effect of the therapies is bone marrow depression,and the grade Ⅲ~Ⅳ leucopenia is 35.3%.Conclusion Single Navelbine and Navelbine plus DDP(or ADM)are effective and well tolerated treatments in patients with recurrent and metastatic breast cancer.
3.Recent advances in diabetic cataract advanced glycation end products and its inhibitors

Diabetic cataract is an metabolic-type cataract. Its pathogenesis mechanism is not completely clear. Researches demonstrated that glycosylation plays an increasingly important role in the formation of diabetic cataract,and increased blood glucose level promote the pathogenesis and progression of diabetic disease by accelerate the glycosylation response. Therefore, more researches on advanced glycation end products(AGEs) and its inhibitors is being concerned. The advance in the study of AGEs concept, its effect on diabetic cataract formation, the influence of AGEs inhibitor on diabetic cataract are summarized in this article.
4.Effect of high mobilityg group box-1 protein on interleukin-2 and interleukin-2 receptor α expressions in human T lymphocytes

Objective To investigate the effect of high mobility group box-1 protein (HMGB1) on inter-leukin-2 (1L-2) and interleukin-2 receptor α (IL-2α) expressions in human T lymphocytes and its potential regulat-ing mechanism in vitro. Method Human T lymphocytes were isolated and suspended, the cells were cultured with 20 μg/mL phytohemagglutinin (PHA) in 5% CO_2 at 37 ℃, recombinant human HMGB1 (rhHMGB1, 0, 10, 100, 1000 ng/mL) was added with the PHA and cultures were centrifuged at 12 and 48 hours for cell collect-ing. Reverse transcription polymerase chain reaction (RT-PCR) amplification was perfomed to determine gene ex-pressions of IL-2, IL-2Rα. IL-2, sIL-2R protein levels in cell culture supematants were measured by ELIZA. Re-sults After coincubated with rhHMGB1 (10, 100, and 1000 ng/mL) for 12 hours, IL-2 levels in cell culture su-pernatants respectively were 0 . 064 ± 0. 017 μg/L, 0.076±0.033 μg/L, and 0.061 ±0.02 μg/L, which were significantly higher compared with the untreated cells (0.045±0.011 μg/L, P < 0.05 or P < 0.01). Mean-while, IL-2 mRNA expression was markedly up-regulated following rhHMGB1 stimulation in various doses (F = 4.6872, P < 0.01). At 48 bourn, however, both IL-2 mRNA expression and protein production tended to de-crease along with an increased dose of dd-IMGB1 stimulationn. IL-2/sIL-2R ratio in 1000 ng/mL rhHMGB1 was markedly lower than that in 10 ng/ml rhHMGB1 (0.036±0.015 vs.0.055±0.017, P <0.05), together with down-regulation of IL-2Rα mRNA expression(P <0.01). Conclusions These data indieated that HMGBI could marked influence the IL-2/IL-2R expression in human T lymphocytes. With the increase in stimulating doses and prolongation of time, HMGBI might down-regulate T cell-mediated immune response of human lymphocytes.
5.Endoscopic Thyroidectomy via Breast Areola Approach:Report of 18 Cases

Objective To investigate the feasibility and cosmetic results of endoscopic thyroidectomy via breast areola approach.Methods From January 2004 to January 2006,endoscopic thyroidectomy was performed through the breast areola approach in 18 patients with benign thyroid mass.Results The surgery was accomplished successfully in all the 18 cases.The operation time ranged 45-120 min(mean,62 min) for unilateral resection and 90-180 min(mean,98 min) for bilateral resection.The intraoperative blood loss ranged 6-20 ml(mean,11 ml) for unilateral resection,and 15-70 ml(mean,29.5 ml) for bilateral resection.Postoperative hospital stay was 3-5 days.No complication occurred after the operation.The patients were followed up for 6-12 months(mean,9 months).The wound healed well in all the patients,inconspicuous scars or no recurrence was observed during the follow-up.Conclusions Endoscopic thyroidectomy via breast areola approach is a safe and feasible procedure with satisfying neck cosmetic outcome.
6.Interleukin-18 mRNA expression in vital organs in septic rats and its relation to endotoxemia

Objective To observe the change in interleukin-18 (IL-18) mRNA expression in cecal ligation and puncture (CLP) induced sepsis, and to investigate its potential role in the pathogenesis of endotoxemia. Methods Fifty-four Wistar rats were randomly divided into normal group (n=6), CLP group (n=36) and recombinant bactericidal/permeability increasing protein (rBPI 21) treatment group (n=12). 8.CT characteristic findings of pancreatic rupture caused by closed abdominal trauma

Objective To study CT characteristic findings of pancreatic rupture caused by closed abdominal trauma.Methods The clinical and CT data of 6 patients with pancreatic rupture caused by closed abdominal trauma were analyzed retrospectively.CT plain scan and the tripple-phased enhancement scan were performed,multi-planar reformation(MPR)and maximum intensity projection(MIP)were used after CT scanning.Thin slice of CT scan and post-processing technique of imaging were used furthermore.Results The rupture of pancreatic neck and tail were shown in 2 cases,the rupture of pancreatic head and body in 1 case.Main CT findings were such as following:(1)Hematomas in pancreas or in pancreatic surrounding tissues were shown in 6 cases,they were hyperintensity or slight hyperintensity in plain scan and were not enhanced in three-phase dynamic scan of pancreas.(2)Pancreatic splits,which were relative hypointensity after administration of CT contrast agent in the pancreatic phase,were found in 6 cases,they were like slit-shaped or wedge-shaped or pancreas separated completely in situs.In the pancreatic phase,pancreas parenchyma enhanced obviously but hematoma in the splits did not enhanced,and this drew the outline of pancreatic splits clearly.(3)Contrast agent in the pancreatic hematomas,which leaked from vessels of pancreas,were shown in two patients,like dot or nodule or pond,were highly intensity in artery phase,and there were more leaking of contrast agent in the pancreatic phase,whose intensity was same as abdominal aorta in the same slice image.In the delayed phase,the volume of leaking contrast agent enlarged and they had higher intensity while intensity of abdominal aorta in same slice image decreased obviously.(4)Dilation of the Wirsung duct,which was like curved-tube,was shown in one patient.(5)Meanwhile the contusion of liver,kidney or spleen with pancreatic rupture were found in 4 patients.Conclusion The pancreatic rupture has typical CT findings.It is helpful to improve the accuracy rate of diagnosis with reasonable scanning techniques and understanding correctly CT appearances of pancreatic rupture.
9.Effect of high mobility group box-1 protein on immune function of peritoneal macrophages of BALB/c mice

Objective To investigate the effect of high mobility group box-1 protein (HMGB1) on immune function of peritoneal macrophages in mice. Methods The peritoneal macrophages were isolated from female BALBc mice in vitro. The macrophages were stimulated with different doses of HMGB1 (1, 10, 100, 1 000ng/ml) for 6 hours, or with 100ng/ml HMGB1 for different duration (0, 6, 12, 24, 48, 72 hours). The phagocytosis ability of macrophages was determined by the neutral red dye uptake assay and optical densities of samples were read at 540nm. The cytotoxicity to L1210 cells was measured with MTT assay and was expressed by absorbance at 570nm. The chemotaxis was assayed by using the Costar Transwell cell culture chamber inserts and the cells on the lower surface of the transwell membrane were fixed and counted. The cells were harvested at the indicated time points, fixed, and stained for indirect immunofluorescence, then the expressions of I-Ak MHC class Ⅱ alloantigen on macrophages were determined with flow cytometry. Objective To investigate the effects of antibody against heat shock protein (HSP) B of Helicobactor pylori (H. pylori) on H.pylori associated gastric disease for the study of the relationship between serum antibody to HSP B of H.pylori and the pathologic changes of the gastric mucosa. Methods 1 036 patients from Shandong province were collected. Six hundred patients were infected with H.pylori. Of those, three hundred received triple therapy for eradication of H.pylori and the other received placebo. Five years later, patients with H.pylori positive chronic superficial gastritis, chronic atrophic gastritis, peptic ulcer, and H.pylori negative chronic superficial gastritis underwent gastroscopy and collected serum specimens again, each group had 20 patients.
