1.Analysis of the appearances of multiple MRI sequences and the assessment of resectability in caput pancreatic carcinoma

Objective:To analyze the appearances of multiple MRI sequences and the accuracy of the assessment of resectability in caput pancreatic carcinoma.Methods:MRI appearances were analyzed retrospectively in nineteen patients with histopathology or clinical examination proved carcinoma in the head of pancreas,and the assessment of resectability was compared with the findings of surgical and histopathology.Plain MRI sequences included:coronal scanning with T2-weighted 2D true-fast imaging with steady-state free precession,axial scanning using T1-weighted fast low angle single shot with fat suppression,T2-weighted half-Fourier single shot turbo spin-echo with fat suppression,and MRI cholangiopancreatography.The contrast-enhanced scanning procedures were axial scanning using T1-weighted 3D volume interpolated breathhold examination with fat suppression,the imaging were obtained during arterial phase,portal vein and delayed phase scanning.Results:Of 19 lesions in these patients,17 lesions showed hypo-intensity on T1WI of plain sequences,and normal pancreats showed hyper-intensity.15 lesions showed hyper-intensity or immingled-intensity on T2WI,one appended with cyst.During pancreatic arterial phase,18 lesions did not show enhancement,contrasting with the normal pancreas showing evidently enhancement,so the borderline of carcinoma was clear.During portal vein and delayed phase,17 lesions showed inhomogeneous slight enhancement,peripheral vas of pancreas in 17 cases were contacted or involved by tumor.3 transfer lesions in hepar showed slight enhancement.13 cases showed dilatation at both common bile duct and pancreatic duct as a typical double duct sign on MRCP.According to the assessment of MRI,7 cases could be resected,and 12 cases could not be resected.The assessment of 14 cases agreed with the surgical and histopathological findings.Conclusion:Carcinoma of pancreatic head is shown with many features on multiple MRI sequences.MRI examination has a high accuracy for the diagnosis and the assessment of resectability in carcinoma of pancreatic head,which is helpful in choosing project of therapy.

Post-resuscitation multiple organ dysfunction syndrome(PR-MODS) is a major cause of low survival rate in patients with cardiac arrest.To fully understand the concept of PR-MODS is beneficial for estimating the state of illness,adjusting the therapy and eventually increasing the survival rate.This article reviews the development of PR-MODS concept and compares it with MODS of other causes,including their pathogeny,pathology,mechanism,diagnosis and therapy.

5.Autophagy and energy metabolism

Eukaryotic cells maintain cellular homeostasis by regulating the process of protein degradation and organelle turnover.Autophagy is a ubiquitin-like degradation process,and plays an essential role in cellular development,differentiation,and elimination of damaged organelles and protein.Previous studies mostly focused autophagy on tumor and neurodegenerative diseases,whereas studies about autophagy and energy remain not clear.Since 2009,an increasing number of studies have showed light on the importance of autophagy in regulating lipid metabolism.The review focuses on the relationship between autophagy and differentiation of adipocytes,lipogenesis,and lipolysis.
6.Research progress in aberrant sphingolipid metabolism and obesity and obesity-mediated insulin resistance

Sphingolipids are essential components of cellular membrane , regulating important physiologic function of cells .Al-terations of sphingolipid metabolism are closely related to the pathogenesis of several chronic diseases .Ceramide biogenesis , the center of sphingolipid metabolism , includes de novo ceramide synthesis , ceramide formation via breakdown of complicated sphingolipids and so on . Recent studies found that aberrant sphingolipid metabolism is strongly linked to insulin resistance associated with obesity .The review fo-cuses on recent advances about aberrant sphingolipid metabolism in the pathogenesis of insulin resistance associated with obesity .
7.Percutaneous coronary Intervention for ostial occlusion lesion of an anomalous right coronary artery

Ostial lesions present many challenges for percutaneous coronary intervention (PCI). Coronary anomaly will further increase difficulties in performing PCI for the patient. We present such a case as ostial occlusion of an right coronary artery with high takeoff. A 77-year-old male was referred to our institution with a diagnosis of non-ST elevated acute myocardial infarction. Selective coronary angiography and nonselective ascending aortography could not identify the origin of the right coronary artery. Multi-slices computed tomography showed RCA ostial totally occluded. A successful PCI was performed and a perfect final result was achieved utilized with many tips and tricks, including buddy wire technique and focused-force angioplasty.

9.Role of urotensin Ⅱ gene in the genetic susceptibility to gestational diabetes mellitus in northern Chinese women

Objective To investigate the relationship between the polymorphism of site rs228648 in urotensin Ⅱ gene and the genetic susceptibility to gestational diabetes mellitus in northern Chinese women. Methods Genotyping was conducted to investigate the polymorphism of site rs228648 (G—A) in urotensin Ⅱgene among 70 unrelated gestational diabetes mellitus(GDM) subjects and 70 normal controls. DNA samples isolated from leucocyte of the control and study groups were analyzed for single-nucleotide polymorphisms of the urotensin Ⅱ gene at positions rs228648 using polymerase chain reaction and restriction analysis. Results (1)The distribution of genotype frequencies of site rs228648 accorded with Hardy-Weinberg′s equation law, being colony representative.(2)The frequency of G allele of site rs228648 was 70.7% in GDM group, significantly higher than that in the control group (57.9%, P0.05). (3) Women in control group were more likely to be homozygous for the allele A of site rs228648 than women with GDM. The frequency of A/A genotype of rs228648 was negatively correlated with GDM group. By the logistic procedure, after adjustment by age and gestational weeks, the odds ratio was 0.312 , and Wald Confidence Limites were 0.108 to 0.900 (P= 0.031). Conclusion Urotensin Ⅱ gene may contribute to the genetic susceptibility to gestational diabetes mellitus in northern Chinese population. G allele of site rs228648 is related to GDM possibly, and that homozygosis A of site rs228648 is likely to be an important protecting factor for GDM.
