1.Effect of three dimensional conformal radiotherapy combined with chemotherapy for advanced esopha-geal carcinoma

Objective To evaluate the clinical efficacy and toxicities of three dimensional conformal radiotherapy combined with chemotherapy for advanced esophageal carcinoma. Methods We retrospectively reviewed 127 patients with advanced esophageal squamous carcinoma in our department from January 2008 to January 2011,who were divided into two groups:conformal radiotherapy combined with chemotherapy(RCT) group,69 patients and radiotherapy(RT)group,58 patients. The side effects,quality of life and effective rates of them were compared. Results The severe rate of the marrow toxic effect and the incidences of radia-tion-induced esophagitis were higher in the RCT group(29. 0% vs 12. 1% ,χ2 = 5. 387,P = 0. 020;24. 6% vs 10. 3% ,χ2 = 4. 341,P = 0. 037). Patients in the RT group had better quality of life than those in the RCT group in gastrointestinal side effects(nausea,vomiting,diarrhea)and food intake(73. 9% vs 12. 1% ,χ2 =48. 574,P = 0. 000;43. 5% vs 25. 9% ,χ2 = 4. 274,P = 0. 039). In the RCT group,the effective rate was 91. 30%(63 / 69)and 72. 41 %(42 / 58)in the RT group,with a statistical significance(χ2 = 7. 852,P =0. 005). Conclusion Three dimensional conformal radiotherapy combined with chemotherapy for advanced esophageal carcinoma can improve the short-term curative effects,but it also increase the acute toxic effect. Further follow-up studies are needed for its long-term effect.
2.HRCT Analysis of Peripheral Pulmonary Hamartoma

Objective To study the CT and high resolution CT (HRCT) features of peripheral pulmonary hamartoma, and improve the diagnostic accuracy. Methods CT and HRCT materials of fifteen peripheral pulmonary hamartomas with pathologically proved were analyzed retrospectively. Results In the 15 cases, 6 and 9 were located in right and left lung respectively. All of the 15 cases were solitary lesion. The sizes of the lesions were from 0.9 cm?1.0 cm to 3.8 cm?4.5 cm. Most of the lesions were under the pleura. The shapes of the lesions were round and oval. The margins of the lesions were clear. The average CT density was (39.6?13.2) HU,and fat was shown in 6 cases(40%), calcification was shown in 8 cases(53.3%).Conclusion Peripheral pulmonary hamartoma with specific CT and HRCT features can be diagnosed correctly.
3.Attributional analysis on the relation between the mental health and personality characteristic of hyperplasia of breast

Objective To analyze the mental health status and personality characteristic of hyperplasia of breast,so as to provide sound scientific basis for enhancing the quality of life of those patients.Methods Assessments were made by using Symptoms Checklist 90 (SCL-90)and 16 Personality Factor Questionnaire (16-PF) for the mammary gland hyperplasia patients and the normal,and data were analyzed with SAS 9.2.Results (1)Significant differences were found in the scores of somatization,compulsion,sensitivity,depression,terror and paranoid between the byperplasia of breast and heahhy subjects.(2)Multivariate and unconditional Logistic regression analysis revealed that the protective factor of psychosomatic symptoms is the warmth stability and perfectionism which factor regression coefficient was negative; the risk factor of psychosomatic symptoms was the dominance,suspicion,apprehension and tension which factor regression coefficient was negative.Conclusions There are various psychological problems which affected by personality characteristic in the patients with breast hyperplasia,treatment should be optimized based on the personality characteristics of patients with breast hyperplasia,including changing negative attribution model,disseminating preventive knowledge,and finally improve the mental health condition of patients with breast hyperplasia.
4.Factors affecting therapeutic effect of ADHD treated by optimal dose of methylphenidate

Objective:To explore the therapeutic effect,security and influencing factors of methylphenidate titration program,try to find the potential predicting index of therapeutic effect.Methods:This study was openly designed.The patients who were fit for DSM-Ⅳ diagnosis criteria of attention deficit hyperactivity disorder(ADHD) underwent systematic methylphenidate titration treatment until they attained the most optimal dose with best therapeutic effect and minimal side effects.The primary evaluative index of therapeutic effect was ADHD rating scale filled by teachers(ADHD-RS-Ⅳ),and secondary index included abbreviated Conners questionnaire filled by teachers,ADHD rating scale and Conners questionnaire filled by parents.The matching sample t analyses were used,then the differences of rating scale between pre and post treatment were compared.The potential influencing factors information of therapeutic effects including symptom severity degree when visiting,IQ,sensory integration capability and achievement of school etc.were collected.Finally,the correlation analysis was used for continuous variables andvariance analysis for dichotomization variables to analyze and check the factors' influence of declining scores of behavior scale.Results:The final prescribed daily dose for patients who completed dose titration was(17.3?7.7) mg on average,computed by kilo bodyweight was(0.45?0.11) mg/(kg?d).All the scale scores between pre and post treatment were significantly different(the pre-treatment score of ADHD rating scale completed by teachers was 30.25?9.16,and post-treatment score were 19.87?10.03,P
5.Effects of Nourishing Yin and Clearing Heat Decoction on Level of Glucocorticoid and the Glucocorticoid Receptor mRNA Expression in MRL/lpr Mice

Objective To observe the change of glucocxnticoid(GC) and the messenger RNA expression of its receptor(GRmRNA) in MRL/lpr mice with the intervention of nourishing yin and clearing heat decoction.Methods Forty MRL/lpr mice were randomly divided into four groups:model control group,TCM group,west medicine group,combined treatment of traditional chinese medicine and west medicine group,10 mice in each group.The group was intragastrically administrated normal sodium,nourishing yin and clearing heat decoction,prednisone suspension,nourishing yin and clearing heat decoction plus prednisone suspension,0.5 mL every time,once daily for 10 weeks respectively.Meanwhile,10 Kungming mice were selected as normal group,administrated normal sodium,like the model group.The expression of glucocxnticoid receptor-alpha and beta(GR?mRNA,GR?mRNA) were detected by reverse transcription polymerase chain reaction(RT-PCR).Results Compared with normal,the expression of GRmRNA was increased in model groups,but the level of GC was not different.Compared with model group,GR?mRNA and GR?mRNA of MRL/lpr mice were improved in all treatment groups,especially in combined treatment of traditional Chinese medicine and western medicine group.Conclusion Nourishing yin and clearing heat decoction has effect of upregulating the expression of GRmRNA in MRL/lpr mice,so as to enhance the effect of glucocorticoid and reduce its side effects.
6.Investigation into physical activity and eating behaviours of obese children aged 1~6 years

Objective To analyze the risk factors related to obesity of preschool children. Methods The physical activity and eating behavior of 120 obese children aged 1~6 years were investigated with questionnaire by case control 1:1 matched. Results Obesity group spent significantly longer time to watch TV than control group, but the time for physical activity, feeding pattern during the first 4 months, the time of initial supplementary foods and the sleeping duration were not different between both groups. Conditioned logistic regression of multi-variables showed that good appetite, fast eating, overeating of carbohydrate and meat , and snacks before breakfast were the risk factors of child obesity. Conclusion The bad eating behaviors and the lack of physical activity were related to obesity in preschool children.
7.DSM-IV and ICD-10: A Comparison of Clinical Features of Attention Deficit-Hyperactivity Disorder and Hyperkinetic Disorder

Objective: To examine the differences on clinical features between DSM-Ⅳ ADHD and ICD-10 HKD patients. Methods:516 children referred to psychiatric clinic because of overactivity, inattention, and impulsivity symptoms were divided into two groups: Children meeting both criteria ( HKD-ADHD group, n=199), and children meeting only DSM-Ⅳ ADHD criteria (ADHD group, n=317). Diagnoses were confirmed by structured interview with parents using Clinical Diagnostic Interview Scale. Results: Whereas the combined type is predominant in HKD-ADHD group (75.4%), predominately inattentive type take account of the majority in ADHD group (67.2%). Compared to ADHD group, HKD-ADHD group includes the patients with younger referral age(9.6?2.2 vs 10.7?2.6) and earlier onset of age(4.6?1.9 vs 5.9?2.5), more opposite defiant disorder comorbidity (41.2% vs 26.2%), more disturbed behaviors, and higher sensory integrative dysfunction prevalence (72.6% vs 56.9% ). However, among the patients not older than 9-year-old there is higher prevalence of learning disability in ADHD group (26.7% vs 14.8%).Conclusion: ICD-10 criteria identified a severe group of patients than those identified by DSM-Ⅳ.However, the patients satisfied only DSM-Ⅳcriteria has exhibited more academic difficulty and behavioral disturbance compared with normal children, so we recommend DSM-Ⅳ as clinical diagnostic tool so that earlier interventions could be possible.
8.Treatment of the congenital hip dislocation of the high age children by Pemberton operation and accessory measures

Objective A review of cases of congenital hip dislocation in high age children by Pemberton operation and accessory measures and investgate if the hip joint stiffness rate and ischemic necrosis rate of femoral head after operation are reduced. Methods From February 1987 to February 2000, 76 hip congenital dislocation of 58 high age children (7 years old to 14 years old) were treated by Pemberton operation and accessory measures, and were followed up from 2 years to 11 years(average, 5 years and 2 months). The accessory measures included: 1)traction of the leg for enough time with enough weight before operation, 2) immobilizing the leg by skin traction in neutral position and plank shoes instead of traditional plaster cast after operation, 3)early movement of the hip joint after operation and was allowed ambulatory late, 4) periodically the hip joint was mobilized passively. Results Six cases can only bend their hip joint below 90 degrees, accounting for 7.9% of all. 29 ischemic necrosis of femoral head occurred which accounted for 38.2%. The rate of ischemic necrosis of the femoral head and hip joint stiffness of high age children after operation were reduced obviously according to the standard by Salter. Conclusion Pemberton operation was preferred in treating the congenital hip dislocation of high age children. The accessory measures reduced the rate of ischemic necrosis of femoral head and hip joint stiffness.
9.Differentiation of adipose-derived mesenchymal stem cells to renal tubular epithelial cells

Objective To detect whether adipose-derived mesenchymal stem cells (ADMCSs)can differentiate into renal tubular epithelial cells and be used for treatment of acute kidney injury (AKI).Methods ADMSCs were separated and cultured in vitro,and were transfected with lentivirus carrying reporter genes of firefly luciferase and monomeric red fluorescent protein (fluc-mrfp).Transfected ADMCSs (2 × l06) were directly injected into renal parenchyma after establishment of AKI models induced by ischemic reperfusion injury.Distribution and differentiation of ADMSCs in rats were detected by bioluminescence imaging (BLI) and immunofluorescent staining,respectively.Results ADMSCs had positive expressions of CD29 and CD90,rather than expressions of CD34,CD45 and CD31.Lentivirus-transduced ADMSCs had stable expressions of mrfp and fluc reporter genes.BLI showed persistent fluorescence signal in vivo even at day 14 after ADMSCs transplantation.Immunofluorescence staining further indicated that the transplanted ADMSCs could differentiate into tubular epithelial cells.Conclusion ADMSCs in vivo can differentiate into renal tubular epithelial cells,which provides an experimental basis for more researches on ADMSCs transplantation for treatment of AKI.
