3.Research on early clinical teaching model with community practice in dental education

In order to solve the problem of the basic and clinical problems in medical education,Stomatological College of Chongqing Medical University has introduced early clinical education modes through the social practice of volunteer activities since 2009.The teaching modes of Come early into department come early into department and contact early with the patients and a combination of come early into department+contact early with the patients+community dental service were performed.A total of 525 students and 120 teachers in this program were investigated,more than 70% of the teachers believed the students were improved obviously in doctor-patient communication,clinical practice,and teamwork spirit.Most of teachers and students approved of a combination of come early into department+contact early with the patients+community dental service,which is worth popularizing.
4.Results of treatment of multimodality therapy following bladder-preserving surgery for muscle-invasive bladder cancer

Objective To evaluate the local control effect of radiotherapy and intravesical chemotherapy following bladder preserving surgery for musculoinvasive bladder cancer. Methods Twenty three patients with muscle invasive bladder cancer were treated by bladder preserving surgery followed by external radiation ( mean dose 52.5 Gy) combined with intravesical mitomycin infusion after bladder preserving operation (study group).For comparison, 29 similar patients received the same treatment without radiation served as control (control group).Results All patients in the study group completed their designed treatment,except two patients whose protocol was interrupted for 3 days and one week due to acute cystitis. The mean follow up duration was 41.6 months. The 3 year local recurrence rates were 18.8% in the study group and 44.8% in the control group(P=0.026).The 3 year distant metastasis rates were 18.2% and 24.1% (P=0.437),and the 3 year overall survival rates were 81.8% and 86.2% (P=0.670),respectively. Conclusion It is demonstrated that adjuvant radiotherapy and intravesical infusion chemotherapy following organ preserving surgical procedure are encouraging by giving favorable local control for musculoinvasive bladder cancer.
5.Protective effects of Chaiqin Chengqi Decoction on isolated pancreatic acinar cells in acute pancreatitis rats and the mechanisms

OBJECTIVE: To investigate the protective effects of Chaiqin Chengqi Decoction (CQCQD), a compound traditional Chinese herbal medicine, on acinar cells and the changes of intracellular calcium concentration ([Ca(2+)]i) in acute pancreatitis (AP) rats. METHODS: CQCQ serum (CQCQS) was prepared by intragastric administration of CQCQD in SD rats, and AP was induced in SD rats by biliary-pancreatic duct ligation method. Pancreatic acinar cells were isolated by collagenase digestion and co-incubated with the drug serum containing CQCQD in vitro, then cell viability was determined by methyl thiazolyl tetrazolium (MTT) method and intracellular fluorescent intensity (FI) was observed and analyzed to investigate the changes of intracellular calcium concentration ([Ca(2+)]i). RESULTS: Both 5% and 10% CQCQS could improve the viability of pancreatic acinar cells in AP rats (P<0.05), and 10% CQCQS was more effective than 5% CQCQS (P<0.05). [Ca(2+)]i was elevated in AP rats (P<0.05), while the level of [Ca(2+)]i elevation was reduced after CQCQS treatment (P<0.05). CONCLUSION: CQCQD has a protective effect on pancreatic acinar cells of AP rats, which may be associated with its inhibition effect on intracellular calcium overload.

7.Evaluation of the stress distribution in tooth implant-supported fixed partial prostheses on different bone types under dynamic loads.

OBJECTIVEThis study aims to evaluate the influence of different bone types on the stress distribution in tooth implant-supported fixed partial prostheses by using finite element (FE) analysis.
METHODSFour FE models of mandibular arch containing one implant splinted to the mandibular second premolar were built according to bone types I, II, III, and IV. Dynamic loads of 250 N were applied to the buccal and lingual cusps of the prostheses in different directions to simulate the masticatory cycle. The maximum Von Mises stresses were calculated using the FE analysis software.
RESULTSThe maximum Von Mises stresses of the cortical bones were 89.229, 91.860, 125.840, and 158.420 MPa, increasing from type I to type IV, respectively. The maximum Von Mises stresses of the trabecular bone were 58.584, 43.645, 21.688, and 18.249 MPa, decreasing from type I to type IV, respectively. During the process of dynamic loading, the maximum Von Mises stresses of the cortical and trabecular bones followed the order buccal to tongue loading>tongue to buccal loading>vertical loading.
CONCLUSIONThe results showed that bone type significantly influenced the stress distribution in bones, and that for tooth implant-supported fixed partial prostheses, bone types I and II were a better choice than bone types III and IV. More caution should be exercised when restoring missing teeth using tooth implant-supported fixed partial prostheses in softer bone regions.

8.Empirical Study on Qi-yin Deficiency Syndrome of Type 2 Diabetes Based on Structural Equation Model

Objective To verify the relationship between qi-yin deficiency syndrome and the etiology and diagnostic information on type 2 diabetes mellitus.Methods Data of 185 cases of qi-yin deficiency syndrome of type 2 diabetes and 351 cases of other syndromes at TCM Hospital of Hongqiao District in Tianjin and Metabolic Disease Hospital of Tianjin Medical University were randomly collected. Etiology and diagnostic information of qi-yin deficiency syndrome were screened by the likelihood radio of forward step method by using Logistic regression analysis. On this basis, the relationship between qi-yin deficiency syndrome and the etiology and diagnostic information was verified by using maximum likelihood estimation method of the structural equation model.Results The goodness of fit index of the model (GFI) was 0.909; root mean square residual (RMR) of that was 0.071; comparative fit index (CFI) of that was 0.942, which suggested that the fitting effect of the model was good. The reasonable interpretation of the relationship between qi-yin deficiency syndrome and the etiology and diagnostic information was given. Conclusion From the aspect of single syndrome factors, the relationship between qi-yin deficiency syndrome and the etiology and diagnostic information on type 2 diabetes mellitus is verified.
9.Clinical and immunologic observation of Qingxianyin in the treatment of oral lichen planus

Objective:To evaluate of effects of Qingxianyin in the treatment of oral lichen planus(OLP),and to explore its regula-tion of T lymphocytes cellular immune mechanism.Methods:25 cases of OLP were treated by Qingxianyin for 8 weeks and 25 heathy subjects were served as the blank controls.Before and after treatment,peripheral blood CD4 +,CD8 + lymphocyte counts and percentages and CD4 +/CD8 + ratio were measured by flow cytometry.The clinical effects of Qingxinayin were evaluated.Results:After 8 weeks of treatment,significant effect,improvement and ineffecacy were observed in 1 8,5 and 2 cases respectively.The total effective ratio was 92.0%.The CD4 + lymphocyte counts and percentage increased(P <0.05),CD8 + counts and percentage de-creased(P <0.05),CD4 +/CD8 + ratio increased(P <0.05).After treatment the indexes changed towards those of the healthy con-trols.Conclusion:Qingxianyin is effective in the treatment of OLP by promoting mucosal healing,microcirculation and improving im-mune function.
10.Non-invasive visualization of tumors in the mouse liver using a novel nanoparticle contrast enhanced micro-CT imaging procedure

Objective To establish an in vivo imaging method of normal or tumorous liver in mice by using a new type nanoparticle contrast agent, ExiTron nano 12000, coupled with micro-CT imaging.Methods Six 6-8-week old male C57BL/6J mice were randomly divided into group A and group group B, by intravenous injection of 50μL and 100μL Ex-iTron nano 12000, respectively.In vivo Micro-CT scans were performed before contrast agent injection, 3 minutes, 24 hours, 7, 14, 28 and 56 days after injection.To determine which dose is suitable for long-term studies, gray scale value a-nalysis was performed on selected region of interest ( ROI) in the left lobe and right anterior lobe of the liver, and the chan-ges of liver tissue contrast was monitored after ExiTron nano 12000 injection.Three male HBV transgenic mice bearing liver tumors ( group C) were intravenously injected with the determined dose of ExiTron nano 12000 and were monitored by mi-cro-CT scans as above described.At 56 days after ExiTron nano 12000 injection, the mice were sacrificed and liver sam-ples were taken for histological analysis.Results Cross-sectional images taken at various time points and the average gray scale value ( AGSV) analysis in the mouse liver revealed that the AGSV peaked at 24 hours after injection of contrast rea-gent and good contrast still presented in the livers within 56 days of observation for both groups, though group B showed a significantly higher contrast than group A (P<0.01).Those data indicated that the dose of group B (100μL) was better to maintain ExiTron nano 12000 in the liver of mice for a long time.Contrast-enhanced by 100μL of ExiTron nano 12000, the liver tumor nodules in the mice of group C could be clearly delineated by Micro CT imaging during a 56 days observa-tion.Histological analysis revealed atypical hyperplasia, enlarged nuclei with hyperchromasia and cell necrosis in the tumors.Conclusions An in vivo imaging method was established to non-invasively visualize mouse liver using micro-CT combined with nanoparticle-based contrast agent and this technology may be applied to a live imaging of murine primary liv-er tumors.
