1.Effect of daptomycin on serum copeptin and procalcitonin in septicopyemia induced by MRSA

Objective To investigate the effect of daptomycin on the serum levels of proclacitonin (PCT) and copeptin in septicopyemia patients induced by methicillin-resistant Staphylococcus aureus (MRSA). Methods 54 cases septicopyemia patients induced by MRSA were selected and divided into two groups, 27 cases in each group. The two groups received fluid replacement therapy and nutrition support, the control group received vancomycin (0.5g per times, three times daily) with intravenous drip, and the study group received daptomycin (6mg/kg, once daily) with intravenous drip. The serum PCT, proclacitonin levels pre-and post-treatment in two groups were detected, the acute physiology and chronic health evaluationⅡ (APACHEⅡ) and sepsis-related organ failure assessment (SOFA) were used to evaluate the patients' condition, the clearance time of pathogenic bacteria was recorded and the clinical efficacy was compared between two groups. Results Compared with before treatment, serum PCT, C-reactive protein (CRP), interleukin-1β(IL-1β) and IL-6 in two groups decreased(P<0.01), the count of WBC, NE% and copeptin decreased (P<0.01), the APACHEⅡ and SOFA score were lower(P<0.05); compared with the control group, the PCT, CRP, IL-1 beta and IL-6 in study group were lower(P<0.05), the count of WBC, NE% and copeptin level were lower(P<0.05), APACHEⅡ and SOFA score were lower(P<0.05), the pathogen clearance rate was higher(P<0.05), clearance time was shorter(P<0.05), the total efficiency was higher(P<0.05). Conclusion Daptomycin can reduce serum PCT and copeptin in patients with septicopyemia induced by MRSA, and remove pathogenic bacteria rapidly, inhibit the inflammatory reaction, safe and reliable.
2.Application of Milk Protein Challenge Test and Serum IgE Antibody Detection in Children with Milk Protein Allergic Diseases

Objective To investigate the application effects of milk protein challenge test and serum specific IgE antibody de-tection in children with milk protein allergic diseases.Methods 76 cases of children with milk protein allergic diseases of North Hospital of the People’s Hospital in Urumqi City from January,2013 to December,2015 were treated with milk pro-tein challenge test.The clinical characteristics,results of milk protein challenge test and serum IgE antibody detection,and the value change of routine blood indexes were analysed and compared.Results Diarrhea and hematochezia were the most common clinical features,vomiting,hematemesis and abdominal distention were rare.47 cases (61.8%)showed positive re-sults in milk protein challenge test,and 29 cases (38.2%)showed negative results.42 cases of positive children had delayed allergy,and 5 cases showed rapid allergy.Compared with the negative group,the positive rate of serum specific IgE antibody in positive group didn’t change obviously,the difference was not statistically significant (14.9% vs 6.9%,χ2=0.466,P>0.05).The neutrophil count in positive group was higher after challenge than that of before challenge (t=2.480,P<0.05). The number change of blood cell,neutrophil and platelet in positive group were no statistical differences compared with those in negative group (t=0.500,1.395,0.296,all P>0.05).Conclusion Diarrhea and hematochezia were the most commom disease of digestive tract in children with milk protein allergic diseases.It is necessary to perform milk protein challenge test in clinical practice,however,serum specific IgE antibody is not considered as its diangnosis basis.
3.Research progress of the protective effect of flavanoids on experimental liver injury of animals

Various kinds of flavanoids such as flavone,flavonol,flavanone,isoflavone,flavanol and so on,have protective effects on the liver injuries,which are induced by chemicals,drugs,immunostimulation,alcohol and hepatic ischemia reperfusion in some degree.These kinds of protection are relevant to the function of flavanoids such as elimination of free radical,resistance of oxidation,resistance of lipid peroxidation,adjustment of immune function and so on.The study of the effect of flavanoids on animal experimental liver injury plays an important role in the development of drugs for prevention and treatment of liver diseases.This article is to summarize the research progress of the effect of flavanoids on experimental liver injury of animals.
4.Application and exploration of the pervasive teaching method in medical humanity education

ObjectiveTo explore an effective educational method of medical humanity to improve the medical humanistic quality for nursing students and to lay a basis for the culture of medical personnel with high-quality.MethodsTwo classes of nursing students in grade 2010 were selected randomly and divided into the control group ( CG,n=65 ) and the experimental group ( EG,n=62 ).Personnel training project of our school was applied in the control group including the teaching of compulsory basis medical humanity courses:medical psychology,interpersonal communication,medical ethics and laws from the first semester to the third semester after entering the school.The pervasive teaching method was used in the EG.The basic content of humanistic quality,education was integrated systematically into the teaching of every course,class and extra-curricular activity in a planned way.The check on the topic of etiquette,verbal expression and communication and psychological quality ( the full mark of each topic was 10 points ) was conducted in the third semester in the two groups.The average points of the three topics were graded as the students' final mark of this course.T-test analysis on students' mark of each subject was used.Results The result of t-test showed that the marks of etiquette and psychological quality in EG were obviously higher than those in CG ( P<0.05 ),however,no statistic difference in speech communication and expression was observed.ConclusionThe pervasive teaching method is beneficial to students concerning the accepting,understanding,digesting and absorbing of medical humanity knowledge can it can effectively enhance the humanistic quality of medical students.
5.Etiology of Headache after Aneurismal Subarachnoid Hemorrhage

Objective To explore the etiology of headache after aneurismal subarachnoid hemorrhage (SAH) and find clinical features of various kinds of headache.Methods All of the 107 patients with SAH and intracranial aneurysm diagnosed upon CT scan and angiographic demonstration within 24 hours after onset were evaluated by numeric rating scales (NRS) 1,2,3,5,7,10 and 14 days after SAH. Patients suffered from moderate-severe headache were examined by cranial CT scan and transcranial Doppler (TCD) to find the reason, and their diversities were analyzed.Results 86.9% (93/107) patients with SAH suffered from moderate-severe headache. 9.7% (9/93) of them were caused by rebleeding of untreated aneurysm and the pain always occurred abruptly, always followed by conscious disturbance/new neurological signs. CT scan found hydrocephalus in 16.1% (15/93) patients with headache, they could aggravated with time but sometimes released spontaneously; TCD found intracranial vasospasm in 12.9% (12/93) of patients, their clinical features were no more than other 61.3% (57/93) patients with negative results, but patient's condition could exacerbate caused by later brain ischemia or even infarction.Conclusion Majority of SAH patients suffer from headache.
6.The relationship of serum vitamin D3 level and inflammatory factor in primary hypertensive patients

Objective To study the relationship between serum 1,25 -dihydroxy vitamin D3 level and inflammatory factor in elderly primary hypertension patients.Methods Sixty -eight primary hypertension patients were selected as hypertension group,and 60 healthy subjects were selected as control group in this study.The serum 1,25 -dihydroxy vitamin D3 level and plasma inflammatory factor were measured and compared.Results The serum 1,25 -dihydroxy vitamin D3 level in the hypertension group was significantly lower,while plasma inflammatory factor in the hypertension group was significantly higher than in the control group(t =6.445,7.326,8.174,8.556,all P <0.01).Pearson correlation analysis showed that the serum 1,25 -dihydroxy vitamin D3 level was negatively related with systolic blood pressure,plasma diastolic blood pressure and inflammatory factor (r =-0.425, -0.367, -0.254, -0.263, -0.296,all P <0.05).Logistic repression analysis revealed that the inflammatory factor was a risk factor for hypertension,while the 1,25 -dihydroxy vitamin D3 level was a protecting factor for primary hypertension. Conclusion The serum 1,25 -dihydroxy vitamin D3 level is significantly lower in elderly primary hypertension patients than in healthy subjects.The serum 1,25 -dihydroxy vitamin D3 level and the inflammatory factor are predic-tive factors for hypertension.

Objective To evaluate correlation of CT features of tuberculous adrenal glands with duration of adrenal insufficiency and propose a new staging criteria for adrenal TB Methods CT features of 28 cases of documented adrenal tuberculosis were analysed retrospectively The patients were divided into 3 groups according to the duration of adrenal insufficiency: group A (10 cases , less than 1 year), group B (13 cases, 1 year to 4 years), group C (5 cases, longer than 4 years) The main CT features analyzed included size and shape of the adrenal gland, calcification ,and low density foci Results Enlargement of the adrenal gland: group A(10) , group B (13), group C (1) Basic configuration of adrenal gland were identifiable in group A ,whereas others had more irregular shape Calcification: group A (2) , group B (11),group C (5) Low density foci: group A (3), group B(0), group C (0) Conclusion CT features of adrenal glands correlated with duration of adrenal insufficiency in adrenal tuberculosis It is more reasonable to use tri phase criteria according to CT appearance in adrenal tuberculosis
8.Management of Research Instrument in Neurosurgical Institute

9.Therapeutic effect observation of lymphoplasmapheresis in treatment of Guillain-Barre syndrome

Objective To observe the therapeutic effect of lymphoplasmapheresis (LPE) in treatment of Guillain-Barre syndrome (GBS). Methods The initial recovery time of muscle strength, muscle strength score difference, clinical therapeutic effect and the security after treatment with LPE were observed in 34 GBS patients. Meanwhile, 17 GBS patients applied with therapeutic plasma exchange (TPE) named TPE group were served as control group. Results LPE group was treated with LPE 51 times in all, and the mean times was 1.5. TPE group received TPE 33 times in all, and the mean times were 1.9. In LPE group, the average initial recovery time [(12.74?7.18)d] was significantly shorter than that in TPE group [(24.35?14.22)d] (P0.05).Conclusions The therapeutic effect of LPE is distinguished for treatment with GBS and the side effect is rare. It is worth to apply in clinic.
