1.Expression and clinical significance of Claudin-4 in human serous ovarian cancer

Objective To investigate the expression and clinical significance of Claudin-4 in human se-rous ovarian cancer.Methods 43 cases of serous ovarian cancer from Oct.2008 to May 2014 were studied.Re-al-time quantitative PCR( qRT-PCR) was applied to detect mRNA expression of Claudin-4 in serous ovarian cancer ( n=43 ) in comparison to the corresponding tumor-adjacent tissues.The protein expression of Claudin-4 was measured by immunohistochemistry( IHC) .Results mRNA and protein expression level of Claudin-4 was signif-icantly higher in serous ovarian cancer tissues than in adjacent normal tissues(P<0.05).The high expression of Claudin-4 protein was also associated with large tumor size, lymphatic metastasis, and advanced TNM stage( P<0.05 ) .Conclusions The expression of Claudin-4 is significantly higher in serous ovarian cancer tissues than in the adjacent normal tissues, and it is associated with clinicopathological features.Claudin-4 may become a new marker in early diagnosis and biological target therapy.
2.Analysis of colorectal cancer screening results in Shangcheng District

Objective:
To analyze the status of colorectal cancer screening results in Shangcheng District, Hangzhou City from 2020 to 2022, so as to provide the evidence for developing prevention and control strategies for colorectal cancer.
Methods:
According to Colorectal Cancer Screening Program for Zhejiang Key Populations, residents registered in Shangcheng District and at ages of 50 to 74 years were recruited and screened using Zhejiang Provincial Questionnaires for Assessment of Risk of Colorectal Cancer Screening among High-risk Populations and fecal immunochemical test (FIT). Residents positive for questionnaires or FIT, or both positive for questionnaires and FIT were served as a positive screening and colonoscopy should be underwent. The rates of positive screening, compliance of colonoscopy and different pathological diagnosis results were analyzed.
Results:
Totally 118 227 residents were screened in Shangcheng District from 2020 to 2022, with a positive rate of 16.00%. The positive rates of questionnaires, FIT and both questionnaires and FIT were 8.14%, 6.46% and 1.40%, respectively. Colonoscopy was performed among 6 501 cases, with a compliance rate of 34.37%. Colonoscopy detected 3 689 cases with colorectal lesions, with a detection rate of 56.75%. The detection rates of colorectal cancer, pericancer lesions and other benign lesions were 1.12%, 9.15% and 46.47%, respectively. The detection rates of positive screening, colorectal lesions and pericancer lesions were higher in men (22.16%, 61.25% and 12.45%) than those in women (11.62%, 51.98% and 5.67%; all P<0.05). The detection rates of positive screening, colorectal lesions, colorectal cancer and pericancer lesions appeared a tendency towards a rise with age (all P<0.05). The detection rates of positive screening and colorectal lesions appeared a tendency towards a decline, and the compliance rate of colonoscopy appeared a tendency towards a rise from 2020 to 2022 (all P<0.05). The detection rate of pericancer lesions was higher among questionnaire and FIT positive residents (65.37%, P<0.05).
Conclusions
The detection rates of colorectal cancer and pericancer lesions in Shangcheng District from 2020 to 2022 were 1.12% and 9.15%, respectively. Men, the elderly, questionnaire and FIT positive residents are key populations, and the compliance of colonoscopy for the populations should be improved.
3.Clinical significance of dynamic monitoring of plasma Pro-B-type Natriuretic Peptide in the evaluation of prognostic value and myocardial dysfunction in patients with septic shock

Objective To assess the effects of dynamic monitoring of plasma Pro-B-type Natriuretic Peptide(Pro-BNP) in the evaluation of prognostic value and myocardial dysfunction in patients with septic shock.Methods The levels of plasma Pro-BNP were tested in 102 patients with septic shock on the 1,3,and 5 days.Data on ICU mortality and 28-day mortality were collected.The patients were divided into non-dysfunction group (CI<3.0) and cardiac dysfunction group (CI≥3.0) according to levels of hemodynamic variables ( CI level).The prognostic value of plasma Pro-BNP was analyzed.Results Plasma Pro-BNP levels on the 1,3 and 5 days in 28-day survivors have a significant declining trend [( 1649.0 ± 257.5 ) ng/L,( 1867.6 ± 291.7 ) ng/L vs.(1229.9 ± 194.5) ng/L),while plasma Pro-BNP levels in non-survivors have an increasing trend [(4128.8 ± 1051.1),( 5315.9 ± 925.4 ) vs.( 6899.9 ± 1327.9 ) ng/L].There is statistical significance of plasma Pro-BNP levels at day 1,3 and 5 for the two groups ( t=3.057,5.083,and 6.290 respectively,P<0.01 ).The 28-day mortality and Plasma Pro-BNP levels in cardiac dysfunction group were higher than of non-dysfunction group ( 28-day mortality:62.1% vs.20.0%;Pro-BNP1:( 1592.8 ± 362.2 )ng/L vs.( 4556.6 ±732.7 ) ng/L;Pro-BNP3:( 2933.6 ± 421.7 ) vs.( 5768.5 ± 793.7 ) ng/L,Pro-BNP5:( 3014.5 ± 587.5 ) ng/L vs.(8873.9 ± 1670.1) ng/L ( x2=20.635,P=0.000,t=3.626,3.154,3.310,P<0.01].Pro-BNP level greater than 3053 ng/L on the 5-day was an independent prognostic indicator of 28-day mortality ( sensitivity:88.4%;specificity:91.5 % ).Pro-BNP level greater than 2378 ng/L on the 5-day was an independent prognostic indicator of hospital mortality ( sensitivity:84.9%;specificity:91.8% ).In multivariate logistic regression analyses,Pro-BNP level and APACHE score were independent predictors of 28-day mortality and hospital mortality.Conclusion Plasma Pro-BNP level shows obvious significance in evaluation of sepsis-induced myocardial depression severity in patients with septic shock.Pro-BNP level in ICU at day 5 after inclusion is an independent prognostic marker of mortality in septic shock.
4.Comparative study on the clinical efficacy of triple needling and conventional acupuncture in the treatment of cervical spondylosis of vertebral artery type

Objective To compare the clinical efficacy of triple needling and conventional acupuncture in the treatment of cervical spondylosis of vertebral artery type. Methods 49 cases with cervical spondylosis of vertebral artery type were randomly divided into two groups ,24 patients of control group treated with conventional acupuncture,25 patients of observation group treated with triple needling,the clinical results of two groups were compared. Results The obvious effective rate of the observation group was significantly higher than the control group, there were significant differences between two groups( x2 =4. 69 ,P < 0. 05). The improvement of average flow velocity(Vm) of the observation group was better than the control group, there was significant difference between two groups ( P < 0. 05 ).Conclusion Triple needling in the treatment of cervical spondylosis of vertebral artery type had better clinical results, and it was worthy of concern.
5.Evaluation of clinical efficacy and adverse effect of traditional Chinese medicine and acupuncture in treatment of ankylosing spondylitis

Objective To evaluate the clinical efficacy and adverse effect of Traditional Chinese Medicine and acupuncture in treatment of ankylosing spondylitis. Methods 46 patients of ankylosing spondylitis with informed consent were randomly divided into observation group and control group by number table,each of 23 cases.The control group received traditional Chinese medicine,the observation group was treated with acupuncture and traditional Chinese medicine,the two groups were treated for 1 month,the clinical effect were observed. Results In the observation group,excellent in 18 cases(78.3%),effective in 3 cases(13.0%),ineffective in 2 cases(8.7%),the total effective rate was 91.3%,it was significantly higher than that of control group the 11 cases(47.8%),8(34.8%),4 cases (17.4%)and 82.6%,the difference between the groups was statistically significant(x2=4.572,P<0.05).The patients of observation group had a rash in 1 case,1 case of pain;in the control group,1 case of rash,1 case of itching;all of the symptoms were relatively minor,did not affect the treatment.There were no significant adverse reactions in the patients of two groups. Conclusion Traditional Chinese medicine and acupuncture in treatment of ankylosing spondylitis had satisfactory effects and no significant adverse reactions,it was worthy to be recommended in the clinical application.
6.Serum protein biomarkers of postmenopausal osteoporosis based on TMT labeling and LC-MS/MS technology

Objective To screen the serum protein molecular markers of postmenopausal osteoporosis by the proteomicsanalysis using Tandem Mass Tag (TMT) combined with liquid chromatography-tandem mass spectrometry (LC-ESI-MS/MS).Methods 20 serum protein samples were recruited (10 cases of postmenopausal patients with osteoporosis and 10 cases of postmenopausal women without osteoporosis)and the high abundance ratios protein was removed,differentiation protein was extracted and labeled with TMT reagent.Then,mass spectrometric detection,data analysis of differentially expressed proteins,and analysis of biological information were carried out.Results 87 significantly differentially expressed proteins were screened from the differentiated protein expression profile by LC-ESI-MS/MS combined with TMT labeling.While 50 proteins were up-regulated,and 37 proteins were down-regulated.Differentially expressed proteins were analyzed by GO annotation,these proteins are mainly involved in 15 kinds of biological processes,7 kinds of cellular component,6 kinds of molecular function.RAB7A,TSP1,GAS6,SPP24 were screenedas candidate proteins which were related to mechanism of bone remodeling of osteoporosis.By STRING 10.0 protein interaction network analysis tools,RAB7A,TSP1,GAS6 were located in the center of the interaction network.SPP24 was located at edge of the network,but it is directly related to the protein BMP-2 of bone remodeling.RAB7A,TSP1,GAS6,SPP24 may be associated with the pathogenesis of postmenopausal osteoporosis.Conclusion These results provide that the proteomics analysis by using TMT coupled with LC-ES1-MS/MS was a feasible method for screening the molecular biomarkers.It suggests that RAB7A,TSP1,GAS6 and SPP24 may be useful biomarkers which can be used both in diagnosis and treatment of postmenopausal osteoporosis.
7.The association between plasma N-terminal pro-B-type natriuretic peptide and extravascular lung water index in patients with septic shock

Objective To study the dynamic change in plasma N-terminal pro-B-type natriuretic peptide (NT-proBNP) levels and its correlation with extravascular lung water index (EVLWI) in patients with septic shock. Methods Sixty-two patients with septic shock admitted to Department of Critical Care Medicine of Beijing Shijitan Hospital were enrolled. The patients were divided into survival group(39 cases)and non-survivors group (23 cases)according to 28-day prognosis. Venous blood was collected after intensive care unit(ICU)admission. The changes in plasma NT-proBNP and hemodynamics indexes levels were analyzed to evaluate their predictive value for clinical outcomes. Results Acute physiology and chronic health evaluation Ⅱ(APACHEⅡ)score(23.2±2.5 vs. 28.1±2.6),sequential organ failure assessment(SOFA score:7.74±2.80 vs. 12.43±3.00)and hemodynamics indexes including EVLWI〔ml/kg:7.0(6.0,8.0)vs. 9.0(7.0,12.0)〕,blood lactate(mmol/L):3.60±2.30 vs. 10.40±2.70)and NT-proBNP〔ng/L:945.0(228.0,1 246.0)vs. 5 471.0(3 308.0,11 174.0)〕in survivors were significantly lower than those in non-survivors,and cardiac index〔CI(L?min-1?m-2):4.23±0.85 vs. 3.31±0.74〕, global ejection fraction(GEF:0.205±0.054 vs. 0.149±0.054)were significantly higher than those in non-survivors (P<0.05 or P<0.01). Correlation analysis showed a positive correlation was found between NT-proBNP and EVLWI (r=0.277,P=0.010),and negative correlations were found between NT-proBNP and CI(r=-0.367,P=0.001), GEF(r=-0.259,P=0.017). No correlation was found between NT-proBNP and GEDVI,SVRI. Receiver operating characteristic curve(ROC curve)analysis showed that the area under the ROC curve(AUC)for plasma NT-proBNP predicting the outcome of septic shock patients was 0.869±0.042,95% confidence interval(95%CI)was 0.786-0.952,with the maximum sum of sensitivity and specificity 1.695 to determine NT-proBNP predicting septic shock patient's death cut-off value was 2 071 ng/L,under this cut-off value,the sensitivity was 81.4% and specificity,88.1%. The maximum AUC for EVLWI predicting the outcome of patients with septic shock was 0.690,cut-off value was 7.5 mL/kg,under this cut-off value,the sensitivity was 69.8% and specificity,66.7%. Maximum AUC for CI predicting the outcome of patients with septic shock was 0.785,cut-off value was 3.48 L?min-1?m-2,under this cut-off value,the sensitivity was 69.8%and specificity,66.7%. Maximum AUC for GEF predicting the outcome of septic shock patients was 0.794,cut-off value 0.175,under this cut-off value,the sensitivity was 76.2% and specificity, 81.4%. Multivariate analyses showed CI and NT-proBNP levels were independent predictors of the prognosis〔CI:P=0.001,odds ratio(OR)=9.183,95%CI 2.362-35.694;NT-proBNP:P=0.024,OR=1.000,95%CI 0.999-1.000〕. Conclusion The plasma NT-proBNP level which is correlated significantly to EVLWI can evaluate the severity of septic shock and can predict the prognosis of such patients.
8.Clinical-grade gene curation strategy in the development of short stature related gene panel by next generation sequencing

Objective Design short stature panel with gene curration strategy.Methods The gene curation process was introduced in detail.The strength of a gene-disease relationship was evaluated based on publicly available genetic and experimental evidence.This process in short stature panel design and its effect on gene selection was further demonstrated.Results After gene curation, the number of gene in list was effectively decreased from 1 276 to 705.The panel sequencing reached a diagnosis rate of 19.7% among a cohort of 371 nation-wide ascertained short stature patients.The gene curation process reduced the risk of false positive findings and decreased diagnostic cost and working hours without affecting the diagnosis rate.Conclusion Gene curation is an important step for NGS-based test and should be widely exercised.
9.Genome-wide copy number variations analysis in 64 patients with unexplained intellectual disability

Objective To investigate the genetic basis of patients with intellectual disability,and to assess the application of single nucleotide polymorphisms (SNP)-array in the molecular diagnosis of intellectual disability.Methods Sixty-four patients with intellectual disability who were identified in Maternal and Child Health Hospital of Guangxi Zhuang Autonomous Region from January 2013 to June of 2015 were enrolled.Genomic DNA was extracted from peripheral blood and was analyzed with Illumina Humancyto SNP-12 300K gene array chip.All identified copy number variants (CNVs) were analyzed with references from databases such as ClinVar,DECIPHER,OMIM and DGV(Database of Genomic Variants),as well as comprehensive literature review from PubMed database to determine the pathogenicity of CNVs.Results Sixteen cases of the above 64 patients were found to have CNVs with genomic alterations,including 6 cases microdeletions/microduplications associated with known syndromes,3 cases microdeletions and microduplications with clear clinical relevance (non-syndrome),1 case numerical chromosome aberration,1 case unbalanced translocation and 5 cases CNVs of unknown clinical significance.The detection rate was 25% (16/64 cases).Among these 16 abnormalities,6 cases of them could not be detected by using karyotyping analysis because their sizes were less than 5 Mb,and the smallest detected missing fragment was 0.53 Mb.Conclusion SNP-array gene chip technique with the advantages of higher efficiency,high-resolution and good accuracy,which can be applied to the genetic diagnosis of intellectual disability.
10.Protective effects of carnosine against closed head injury in mice.

OBJECTIVETo investigate the protective effects of carnosine against experimental closed head injury (CHI) in mice.
METHODSThe CHI model was established by free-falling weight-drop. Carnosine (250 mg/kg or 500 mg/kg) was administered intraperitoneally 30 min before brain trauma, then q.d for 7 d; while normal saline was administrated for control group. The neurological defect was evaluated by neurological severity score (NSS) within 7 d; the survival rate and the histological alternations were observed.
RESULTSCarnosine prevented the body weight loss of mice at dose of 500 mg/kg; significantly increased the survival rate, and reduced the neurological defect and histological damage at dose of 250 and 500 mg/kg.
CONCLUSIONCarnosine can attenuate closed head injury in mice.
