1.Effects and safety of PercuSurge distal protection device in coronary intervention in acute myocardial infarction

Objective To evaluate the effects and safety of PercuSurge distal protection device(DPD) in coronary intervention in patients with acute myocardial infarction. Methods From December 2003 to December 2005, 174 acute myocardial infarction patients who received primary coronary intervention were included into this study. Patients were divided into the DPD group (n=78) and the control group (n=96) according to whether Percusurge DPD was attempted during emergency PCI. The basic clinical characteristics, angiographic results, and follow up data before discharge were compared. TIMI flow grades and myocardial blush grades were performed in all cases after emergency PCI. Results Success application was achieved in 72 out of 78 patients with PercuSurge DPD with varies extent of material collected from the basket. There was no significant difference between the two groups in basic clinical characteristics and angiogram before PCI. Post-PCI TIMI flow grades (94.9% vs 79.2%) and myocardial blush grades (2.65?0.68 vs 2.22?0.94) were significantly higher in the DPD guoup than in the control group(P
2.Clinical Observation of Rosuvastatin Calcium Tablets Combined with Qishen Yiqi Dripping Pills in the Treatment of Chronic Heart Failure

OBJECTIVE:To investigate clinical efficacy and safety of Rosuvastatin calcium tablets combined with Qishen yiqi dripping pills in the treatment of chronic heart failure (CHF) and it effects on inflammatory level,oxidative stress injury and cardiac function of patients.METHODS:Ninety CHF patients in our hospital during Aug.2014-Apr.2016 were divided into observation group and control group according to random number table,with 45 cases in each group.Control group received routine anti-heart failure treatment as cardiac,diuretic,dilating vessel,and Qishen yiqi dripping pills orally 0.5 g,half an hour after meal,tid;observation group was additionally given Rosuvastatin calcium tablet orally 20 mg,at bedtime,qd,on the basis of control group.Cardiac function,serum inflammatory factor,BNP and oxidative stress levels were compared between 2 groups before and after treatment.Clinical efficacies and the occurrence of ADR were observed in 2 groups.RESULTS:Before treatment,there was no statistical significance in LVEF,LVESD,LVEDD,TNF-α,IL-6,BNP,SOD,MPO and MMP-9 levels between 2 groups (P>0.05).After treatment,LVEF of 2 groups were increased significantly,while LVEDD,LVESD,TNF-α and IL-6,BNP levels were decreased significantly;the above indexes of observation group was significantly better than those of control group,with statistical significance (P<0.05).MPO and MMP-9 of observation group were significantly decreased,while SOD level was significantly increased and better than that of control group,with statistical significance (P<0.05),but there was no statistical significance in SOD and MPO,MMP-9 levels of control group before and after treatment (P>0.05).The clinical response rate of observation group was 97.8%,which was significantly higher than 82.2% of control group,with statistical significance (P<0.05).There was no statistical significance in the incidence of ADR between 2 groups (P>0.05).CONCLUSIONS:Rosuvastatin calcium tablets combined with Qishen yiqi dripping pills show significant therapeutic efficacy for CHF,can effectively reduce inflammatory level,relieve oxidant stress injury,delay the process of ventricular remodeling,and improve cardiac function with good safety.
3.Using predictive care in stage divided treatment and salvage of child persistent epileptic status

Objective To discuss the application and effect of predictive care in stage divided treatment and salvage of children's persistent status of epilepsy (SE).Methods Sixty-four children with SE were admitted to the First Peoples Hospital of Wenling in Zhejiang Province from December 2014 to December 2016 as the research objects, a controlled study was adopted, in which the stage divided therapeutic method was compared with the conventional method for treatment of SE, and the children were divided into an observation group and a control group, 32 cases in each group. Foresight treatment method was used in the observation group: the first stage of treatment (early treatment or pre-hospital treatment) was intravenous injection of the first-line drugs (benzodiazepines); the second stage of treatment (progressive stage treatment or initial treatment) mainly used epileptic drugs to prevent recurrence; the third stage treatment (referred to after the first-line drug in combination with one second line antiepileptic drug treatment, the disease was still unable to be controlled) adopted the mild hypothermia therapy to reduce the SE attack amplitude and decrease recurrence; in terms of aspects of predictive nursing measures, the respiratory tract nursing, basic nursing, the prediction of complications and their nursing, etc. were implemented. The routine nursing without care about the stages was carried out in the control group. The SE control time, skin lesions, false aspiration, lung infection, tongue bite, cerebral edema, etc. the incidenceof complications were observed in two groups of children.Results The SE control time was significantly shorter in observation group than that in the control group (minutes: 18.13±3.15 vs. 25.19±2.69,P < 0.05), and the incidence of complications was obviously lower in observation group than that in the control group [6.25% (2/32) vs. 28.13% (9/32),P < 0.05]. Conclusion Using different therapy at different stages of children SE combined with predictive care can effectively shorten the SE control time, decrease the incidence of complications and elevate the therapeutic and nursing effects.
4.Oral activated charcoal decreases serum phosphate level and calcium phosphorus products in dialysis patients with refractory hyperphosphataemia

Objective To study the effect of medically activated charcoal on serum phosphorus level and calcium-phosphorus products in dialysis patients with poorly controlled hyperphosphatemia. Methods A single-center,prospective,self-controlled study was performed.Medically activated charcoal was administered 4.5-7.2 g per day with meals for three months to hemodialysis or peritoneal dialysis patients with hyperphosphatemia after taking calcium-based phosphate binders.The levels of blood phosphorus,calcium,calcium-phosphorus products,intact parathyroid hormone (iPTH),albumin and hemoglobin were detected before and after the treatment.The results were analyzed using paired t-test. Results After 3 months of treatment,the patients' serum phosphorus level was significantly reduced from (2.16 ±0.34) mmol/L (pretreatment) to (1.85±0.30) mmol/L (post-treatment) (P<0.01).Similarly,the serum calciumphosphorus products were lowered from pre-treatment level of (63.93 ±8.83) mg2/dl2 to posttreatment of (54.12±8.37) mg2/dl2 (P<0.01).Serum albumin level was slightly reduced from (41.7±2.9) g/L to (40.1±2.2) g/L (P=0.001).In contrast,there were no significant changes in serum calcium and iPTH levels when compared pre- to post-treatment values (P=0.734 and P=0.665,repectively). Conclusion In combination with calcium-based phosphate binder therapy,oral medically activated charcoal can effectively reduce the levels of blood phosphorus and calciumphosphorus products in dialysis patients with refractory hyperphosphatemia.
5.Clinical features and prognostic factors of chronic glomerulonephritis with malignant hypertension

Objective To find out the clinical characteristics and prognostic risk factors of malignant hypertension (MHT) with pre existed chronic glomerulonephritis.Method The clinical and renal pathological data of 38 chronic glomerulonephritis patients with MHT hospitalized in our department from 1990 to 2002,and the associated factors for renal prognosis were analyzed. Results All the patients suffered from chronic glomerulonephritis.The ratio of male to female was 3 75∶1.The average age was (29 5?7 74)years old. The rates of recognition, therapy and control of hypertension before MHT onset were 75 7%, 13 5%and 5 4%respectively. IgA nephropathy was the most common pathological type.Of 38 cases,3 maintained normal renal function, 6 presented acute renal failure,13 presented chronic renal failure, and 16 had acute attack based on chronic renal failure. Twenty four patients were followed up for 1~6 months after antihypertensive therapy and renal function of 10(41 7%) was improved. 1 year renal survival rate of 20 patients was 55%. The analysis of risk factors showed that those patients who were possessed of positive hypertension family history(P=0 03), high SBP/DBP levels(P=0 023,0 047), and high scores of chronic index of renal pathology(P=0 032) had worse prognosis. The level of highest serum creatinine and the scores of chronic index of renal pathology were associated with 1 year renal survival(P=0 031,0 037).Conclusions MHT occuring in young patients with chronic glomerulonephritis is not rare, which becomes an important reason inducing rapidly decrease of renal function, especially in patients with IgA nephropathy. Actively antihypertensive therapy is helpful for renal function recovery. The low rates of recognition, therapy and control of pre existed renal hypertension may play key roles in the development of MHT. It is important to recognize and treat the hypertension at early stage of renal disease to improve the renal survival.
6.Application of venous flow-through flap in complex finger replantation

Objective To analyze the outcome of applying venous flow-through flap in replantation of complex severed finger.Methods From March,2011 to August,2012,15 cases of complex severed fingers were repaired by flow-through flap with two sets of venous system of forearm vein and one stage repair of wound.The time from injury to operation was 1.5-5.5 h (mean 2.5 h).Vascular defect length ranged from 1.5 to 11.0 cm (mean 3.6 cm);and soft tissue defect of 1.5 cm × 3.0 cm to 11.0 cm × 11.0 cm.All digits had severe soft tissue defect and segmental defect of blood vessels.All the finger blood circulation was disorder.Results All flaps and replanted fingers survived completely,except 1 case of postoperative venous crisis occurred which was remission after the vascular transplantation,and 1 case of skin flap necrosis at the distal part which was healed after skin grafting; Fourteen cases were followed-up from 7 to 20 months.At the final followed-up the flaps were of good consistency and appearance.Function of the finger was graded excellent in 7 cases,good in 5 cases,and poor in 2 cases.All flaps and replanted fingers survived completely over a period of 12 to 30 months follow-up.The flaps were of good consistency and appearance.Function of the finger was graded excellent in 7 cases and good in 5 cases.Conclusion With less injury at donor site,and good repair results,venous flow-through flap is well indicated in complex finger replantation with soft tissue defect and vascule defect.
7.Influence of parental compliance on therapeutic effect of children with epilepsy

Objective To investigate the influence of parental compliance on the therapeutic effect of children with epilepsy.Methods Two hundred and sixty children with epilepsy and their parents admitted to the First People's Hospital of Wenling from December 2013 to June 2016 were enrolled,and the classical Morisky medication adherence questionnaire (MMAS-8) was applied to evaluate the compliance of parents for treatment of their children with epilepsy;after the patient taking drug for 3 days,fasting venous blood was collected in the morning,the concentration of the blood drug was tested and the influence of parent compliance on the blood drug concentration of the child with epilepsy was evaluated.Results In 260 patients,122 cases took karma form,and 138 cases took valproate orally.There were parents with good medication compliance in 130 cases (50%),medium medication compliance 80 cases (30.76%) and poor adherence to the doctor order in 50 cases (19.23%).In cases using medication irregularly,there were 26 cases sometimes without taking any drug (10.0%),17 patients' medication being interrupted (6.54%) and 10 cases having excessive medication (3.85%);no relationships were found between parental compliance and each of the following items,family role,occupation and age (all P > 0.05);and the compliance was related to gender,indicating that women's good compliance level was higher than that of males';the education level was positively proportional to the compliance,and the compliance of parents with senior high school or above degree was higher than those with primary school and junior secondary school levels (83 cases vs.9 cases,38 cases,both P < 0.01).Under situation of parents with poor compliance,their children had blood drug concentration higher or lower than proper range of drug level (high in 22 cases,low in 41 cases,higher than the result in good compliance 0 cases and 17 cases respectively),thus seriously affected the safety and efficacy of the treatment;the patients' frequency of irregular medication in parents' good compliance group was significantly lower than that in parents' poor compliance group [3.08% (4/130) vs.72.0% (36/50),P < 0.05].Conclusion To improve the therapeutic effect of epileptic children,their parental good cooperation is necessary.
8.Design, synthesis and biological evaluation of novel 4-substituted-3-nitrobenzamide derivatives.

A series of novel 4-substituted-3-nitrobenzamide derivatives were designed and synthesized. The structures of the target compounds were confirmed with 1H NMR, 13C NMR, MS and element analysis. Anti-tumor activities against HCT-116, MDA-MB435 and HL-60 cell lines in vitro were evaluated by SRB assay. The results indicated most of the target compounds exhibited potent anti-tumor activity. Compound 4a showed the most potent inhibitory activities against three cancer cell lines with the GI50 values of 1.904-2.111 micromol x L(-1). Compounds 4g, 41-4n exhibited more potent inhibitory activities against MDA-MB435 and HL-60 cell lines with the GI50 values of 1.008-3.586 micromol x L(-1) and 1.993-3.778 micromol x L(-1), respectively. The structure-activity relationship of these compounds is discussed preliminarily.
9.Roles and expressions of the NMDA receptor subunits (NR2A and NR2B) in visual cortex area of kittens with the normal visual development and anisometropic amblyopia.

In order to understand the roles of the other subunits, we investigated expression of the NMDA receptor subunits (NR2A and NR2B) in visual cortex of normal and anisometropic amblyopia kittens with different ages in the present study. We examined the expressions of NR2A and NR2B in the visual cortex of the kittens by immunohistochemistry with polyclonal anti-NR2A antibody and anti-NR2B antibody, respectively. Using immunohisto-chemical Streptavidin Perosidase (SP) method, we observed the dynamic changes of NR2A and NR2B with microscope and computer-assisted image analyses. We found that NR2A and NR2B remained low expression after the peak of the critical period of kitten visual development; compared with normal group of the same age, NR2A expresses low. However, the difference is not significant for NR2B before maturation period of visual development. NR2B rises after the maturation period of visual development. According to this, the component of NR2A and NR2B can be affected by anisometropia. This research suggests that the difference of NR2A and NR2B expressions may affect the formation of amblyopia.

10.Study on Volatile Ingredient and GC-MS Fingerprint of Bu-yang Huan-wu Decoction

This study was aimed to establish GC-MS fingerprint of Bu-yang Huan-wu (BYHW) decoction. The 5HP-MS quartz capillary column was used. The temperature program carrier gas was He. The velocity was 1.0 mL·min-1. The injection volume was 1 μL. The split ratio was 1:20. The MS conditions were electron impact (EI) ion source with the transmission line temperature at 280oC, ion source temperature at 230oC, and quadrupole temperature at 150oC. The mass scan range was m/z 50~550. And the results were contrasted with ChemStation and NIST 05a. The results showed that nine common peaks were obtained in the fingerprint of eleven batches of BYHW decoction by peaks matched with the Similarity Evaluation System for Chromatographic Fingerprint (2004 A Version). The similari-ty of fingerprints was calculated more than 0.93. It was concluded that fingerprinting can be used as a method of quality control and provide evidence for the research of volatile ingredient of BYHW decoction.
