1.INQUIRY ON THE APPRAISABLE STANDARD OF THE FAT OF CHILDREN AND JUVENILE

Based on the state of the development of the body and the subcutaneous fat of 4314 students (includidg boys and girls) at four middle schools in Zibo City, the fat was appraised and analyzed by the body-block indexand the thickness of the subcutaneous fat. The stand-ard-proposal value was also put forward by using y+1.645 Sy.x as the dividing-line value of the groups of different ages and the fat of both sexes. The comparison with foreign standards concerned showed that tally rate of the musculus triceps brachii and the shoulder-blade region of the fat sufferers was 76.87% and 51.7%. The tally rate of the sum of the above two terms reached 100% among the male and the female groups, from 15 to 18 years old. According to the current appraisable standard, the rate of fat children and juvenile for male was 2.86%, while for female, 4.0%. 2.Research on remote access system of digital medical library

Objective To study and analyze technology of proxy server,VPN and specific remote access system.Methods The application of digital medical library was studied in such four aspects as adaptability,security,accessibility and extensibility.Result Many approaches were put forward to break the limits of IP address and realize remote access to digital resources.Conclusion In order to meet the actual requirements in the remote access of digital medical library,the following rules should be observed: the technical scheme with obvious functional limitations should be avoided;importance should be laid on mutual sharing and remote access between chief library and branch library;the maximum concurrent access should be born;multiple certification methods should be utilized as well as multiple techniques such as information distributing.
3.The Affect of Chronic Stress to Ultrastructure and Expression of Vascular Cell Adherence Molecules-1 in Thoracic Aorta in Rats

Objective: Studying the affect of chronic stress to ultrastructure and the expression of vascular cell adherence molecules-1 in thoracic aorta in rats. Methods: 24 male Wistar rats are divided into two groups randomly, including the test group and control group. The number of each group is 12. The test group are compelled to receive a series of stimulation after they are fed 1 week in quiet surrounding, but the control group are not. All of rats are executed at the same time. After have drawn the materials from the thoracic aorta, we observe the ultrastructure by electronic microscope and the expression of vascular cell adherence molecules-1 by the method immunohistochemistry in thoracic aorta. Results: ①The ultrastructures in test group are changed, but the control group are normal; the modification is concentrated on the obvious swelling, vacuolicitical transformation in mitochondria, but the control are not. ②The VCAM-1 is highly expressed in thoracic aorta of the test group, there is not in control group(P

5.Investigation and analysis of quality of life in children and adolescents with type 1 diabetes

Objective The purpose of the study was to assess the quality of life in children and adolescents with type 1 diabetes.Methods 89 patients with type 1 diabetes admitted by Beijing children's hospital and 196 matched healthy children and adolescents were involved.QOL was assessed using the Pediatric Quality of Life Inventory (PedsQL) 4.0 Generic Core Scale.Results Children and adolescents with type 1 diabetes had lower general HRQOL compared with the healthy matched children and adolescents,especially in emotional functioning and school functioning,HRQOL in girls was significantly lower than HRQOL in boys regarding social functioning.Conclusions The QOL of children and adolescents with type 1 diabetes was impaired.
6.Application of Braden evaluation scale in pressure ulcer prevention of bedridden patients in neurological department

Objective To evaluate the predictive effect of Braden scale for the risk of development of pressure ulcers (PU) in the department of neurology bedridden patients and to explore subgroup preventive measures. Methods 400 cases newly hospitalized bedridden patients in the department of neurology were collected with no pressure ulcers at the first evaluation and pressure ulcer risk was continuously predicted by a Braden scale skin assessment. The high-risk, middle-risk and low-risk groups were randomized into the experimental group and the control group respectively. Routine preventive measures were taken for the control group while the air fluidized bed for the high-risk group, the sponge mattress for the middle-riskgroup, and turning the body over every 4 hours for low-risk group. Other preventive procedures were undertaken simultaneously in beth the experimental and the control groups. Results The area under the ROC curve (AUC)was 0.771 and 0.828 at the first and last time Braden scale scores respectively. Such vMues as sensitiveness, specificity, positive predictive value, negative predictive value were found in higher level,when the diagnosis value was 17. There was no significant difference of incidence rate of the subgroup pressure ulcers between the high-risk, middle-risk, low-risk groups compared to the control group. Conclusions The effect of predicting pressure ulcer risk for bedridden patients in the department of neurology with Braden scale was fairly good, while the score 17 as the diagnosis value was ideal. The air fluidized bed for the high-risk group and the sponge mattress for the middle-risk group resulted in no significant decrease of incidence rate of the pressure ulcer, while taming over the patients' body every 4 hours for low-risk groups showed acceptable and therefore saving medical resources.
7.Phased nursing care of children with congenital hyperinsulinism

Objective To explore the nursing measures of patients with congenital hyperinsulinism during hospitalization and to develop nursing strategies. Methods A total of 131 patients with congenital hyperinsulinism were enrolled in the Department of Endocrinology and Metabolism, Beijing Children′s Hospital. The patients were divided into four nursing stages for the treatment of children. The first stage: diagnosed as soon as possible and minimized hypoglycemia on children caused by injury; the second stage: patient feeding, monitoring diazoxide caused by gastrointestinal tract, circulatory system adverse reactions; the third stage: monitoring blood glucose changes, timely adjustment of octreotide dosage; the fourth stage: perioperative period of care, the prognosis of the full mission;throughout the focus of care was to keep the vein pass patency to maintain normal blood sugar. Results The number of patients receiving one to four stages of nursing were 131, 104, 18, 5 cases, respectively. The number of patients with satisfactory results after treatment was 27, 71, 15, 1, respectively. For patients who did not receive satisfactory glycemic control of the 15 patients were followed up, of which 5 cases were lost, 6 cases with improvement of hypoglycemia symptoms 2 years later, 4 cases were dead due to multiple organ damage. Conclusions The treatment of congenital hyperinsulinism patients is responsible for the long process, targeted care measures should be carried ou aiming at different treatment methods in order to obtain a satisfactory care effect.
8.Meta-analysis of efficiency and safety of WBRT±temozolomide in treatment of brain metastases from NSCLC

Objective To compare the efficacy and safety between whole brain radiotherapy (WBRT) and WBRT combined with temozolomide (TMZ) in the treatment of non-small cell lung cancer with brain metastases.Methods According to the retrieval strategy,the Chinese and English literatures before February 2018 were retrieved from EMbase,Cochrane,PubMed,Wanfang database,Chongqing VIP and CNKI,The target literatures were selected according to the inclusion and exclusion criteria,The quality of the included studies and extracted data was independently assessed by 3 researchers,The RevMan 5,3 and STATA 12,0 software was used for statistical analysis,The objective remission rate (ORR),the total survival period (OS),the progression-free survival (PFS),and the side effects of chemotherapy were evaluated.Results In total,17 trials consisting of 1128 patients were included,The results of Meta-analysis demonstrated that compared with the WBRT group,the ORR was significantly higher (OR=2.54;95%CI:1.93-3.36;P<0.001),the PFS was significantly longer (MS R=1.329;95%CI:1.143-1.545;P<0,001),and the incidence of hematological toxicity (OR=3.44;95%CI:1.63-7.26;P< 0.05) and the gastric intestinal reaction (OR=1.69;95%CI:1.24-2.31;P<0.05) was significantly higher in the WBRT+TMZ group,The heterogeneities were within the acceptable range with statistical significance,The results of OS were invalid due to relatively large heterogeneity,The incidence of headache did not significantly differ between two groups (OR=1.05;95%CI:0.72-1.55;P=0,79).Conclusions Compared with WBRT alone,WBRT combined with TMZ is beneficial to improve the short-term efficacy,whereas the incidence of hematological toxicity and gastrointestinal reaction is higher,The occurrence of headache does not significantly differ between two groups,The benefit of long-term survival remains uncertain.
9.Role of three-dimensional speckle tracking imaging in predicting the prognosis of light-chain cardiac amyloidosis with normal left ventricular ejection fraction

Objective:To evaluate the left ventricular myocardial strain in patients with light chain cardiac amyloidosis (AL-CA) with normal left ventricular ejection fraction (LVEF) by three-dimensional speckle tracking imaging(3D-STI), and to explore the clinical value of 3D-STI in predicting the prognosis of AL-CA patients with normal LVEF.Methods:A total of 80 patients with AL-CA and LVEF≥50% were retrospectively analyzed in the Xijing Hospital of Air Force Military Medical University from October 2014 to May 2020.According to whether the patients had endpoint events, they were divided into endpoint event group and non-endpoint event group. The clinical data, conventional echocardiographic parameters, 3D-STI related parameters and follow-up results were collected. Cox regression proportional hazards model was used to analyze the survival status of AL-CA patients with univariate and multivariate regression analyses, in order to find the relevant indicators of conventional echocardiography and 3D-STI to predict adverse events.Results:All patients were followed up for 20(7.3, 40.8) months. At the end of follow-up, 25 patients had all-cause deaths. Compared with the non-endpoint group, the endpoint event group had significantly increased left ventricular end diastolic maximum wall thickness (MLVWT), peak early diastolic flow velocity/peak early diastolic velocity at mitral annulus(E/e′) (all P<0.05), and decreased LVEF, left ventricular global longitudinal strain (GLS) and basal segment longitudinal strain (LS) (all P<0.05). Multivariate cox regression analysis after adjusting for age and gender showed that basal segment LS ( HR=0.812, 95% CI=0.675-0.976, P=0.026) was an independent predictor of end-point events in patients with AL-CA. Kaplan-Meier survival curve showed that AL-CA patients with basal segment LS≤13.07% were more likely to have endpoint events. Conclusions:Basal segment LS can be used as a predictor of endpoint events in patients with AL-CA.
10.Practice of informationized closed-loop management in the consultation of over-dose narcotics

OBJECTIVE To explore the effect of informationized closed-loop management in the consultation of over-dose narcotics. METHODS Since November 2021, the informationized consultation mode of over-dose narcotics had been established innovatively in our hospital on the basis of the previous process of over-dose narcotics use; the consultation mode of over-dose narcotics transformed from paper to information. The standardized rate of consultation for over-dose narcotics, the waiting time of consultation for patients using over-dose narcotics were compared before (from January to April 2021) and after the improvement (from November 2021 to February 2022). RESULTS The informationized closed-loop management of over-dose narcotics consultation in our hospital was realized by improving hospital information system (maintaining basic information, embedding relevant reminder and consultation modules, and establishing query and statistics functions) and introducing mobile terminal. After establishing the mode, the standardized rate of the consultation increased from 55.56% to 93.75%; the waiting time for consultation was shortened from (20.61±9.77) min to (5.87±2.45) min. CONCLUSIONS The informationized closed-loop management can effectively improve the standardization rate of consultations for over-dose narcotics and reduce the waiting time for consultation for patients using over-dose narcotics.
