To explore how to assess objectively the effect of swallowing treatment by a case study of one female patient with nasopharynx cancer after radiotherapy; the swallowing treatment include: dilatation of cricopharyngeal muscle (total 44 times), larynx raising method, tongue base exercises and acupuncture [main aperture:Jin-jin, Yuye(EX-HN12), Lianquan(CV23), Fengchi(GB20), Yamen(GV15), Baihui(GV20)], mainly using the videofluoroscopy to testify curative effect; the results demonstrated the duration of swallowing thick barium (270% w/v) including oral transit time plus pharyngeal transit time shortened 14.3%, distance of larynx raising increased 36.7%, oral and pharyngeal residue reduced 10% compared with that before treatment, all these data totally explained the swallowing function improved a little, but not significantly. The research testify the measuring parameter with videofluoroscopy can be used to assess the swallowing function and curative effect objectively.

Journal of Clinical Otorhinolaryngology Head and Neck Surgery 2016;30(1):84-88
Nowadays, various voice disorders are sprung out towards teachers of primary and secondary school, affecting their personal life and social work. Through literature review, the author aims at clearing up series of assessments and methods of voice training which are suitable for the teachers of primary and secondary school, such as establishing a right way of respiratory, phonation and resonance, avoiding occupational risk factors such as vocal abuse or misuse to regain healthy voice. Review of the several relevant literature and commentary about voice disorders of the teachers in primary and secondary school. Although various methods of voice training were reported from time to time to guide us SLP to carry out voice therapy, however, deviations of the clinical data from scholars' different points of view are waiting for our further support through case-control study.

Objective To evaluate the advantages and disadvantages of different puncture positions in percutaneous nephrolithotomy. Methods Three hundred fifty-six patients who underwent PCNL were analyzed from March 2012 to October 2015. The passage caliber was 16F-20F. There were 217 cases in prone position and 139 cases in supine position. Results The successful operation in PCNL was 354 cases , while the remaining 2 cases were performed by open surgery. The primary stone clearance rate was 75.5%. The additional PCNLs were performed in 23 cases, and 63 cases of residual calculi were treated by ESWL. 11 patients were treated due to infection or bleeding by the additional PCNLs. There were 3 cases with massive hemorrhage which were treated by Interventional embolization therapy , 12 cases in postoperative fever , no renal resection , no intestinal injury, no deaths. There was no significant difference in stone clearance rate and complication rate between the two groups. Conclusion The puncture position of PCNL can be optional based on the stone size , stone location, degree of hydronephrosis ,and patient′s cardiopulmonary condition individually.

ObjectiveTo investigate the clinical therapeutic effect and safety of solifenacin suceinate in the treatment of overactive bladder symptom after benign prostatic hyperplasia (BPH)operation.MethodsAmong 115 patients receiving BPH surgery,58 cases were given solifenacin at bedtime 4 d after surgery for 20 d (treatment group),and 57 cases were given anisodamine after surgery (control group). The urination of patients before and after pulling out catheter were observed, and urodynamic examination, international prostatic symptoms score (IPSS) and overactive bladder symptom score (OABSS) were used to evaluate urination status.ResultsIn treatment group,IPSS and OABSS decreased from 28.3 to 11.3 score and from (14.2±1.2) to (2.9±0.7) score before and after treatment, respectively (P<0.01).In control group,IPSS and OABSS decreased from 27.3 to 11.8 score (P<0.01) and from (14.2±1.6) to (11.3±1.1) score before and after treatment, respectively (P>0.05).ConclusionsThe proper use of solifenacin after prostate operation may release bladder distress and facilitate rehabilitation in patients with overactive bladder symptom.

Objective To assess the feasibility of low-dose MDCT pulmonary angiography (MDCTPA) by using low kVp combined with individual-adapted low tube current protocol and noise reduction filter algorithms. Methods Fifty-six consecutively patients with MDCTPA were enrolled in the study. The patients were randomly divided into group A (120 kVp,100 mAs) and group B (100 kVp,individual-adapted tube current about 1.0 mAs/kg).Two series of images were reconstructed in group B:B1 with standard filter algorithms (FC13,B1) and B2 with noise reduction filter algorithms (FC11,B2).The image noise, quality and radiation dose in two groups were compared. Results The CTDIvol,DLP and E of group B were 5.1 mGy,83.3 mGy· cm and 1.4 mSv respectively, which were reduced by 64.3%,66.4% and 66.7%,respectively (F =32.57,12.32,11.98,P < 0.05) when compared with those in group A.Compared with group A,the image noise in B1 was increased by 118.2% (t =10.05 P <0.05),the central and peripheral SNR and CNR and scores were increased by 51.5%,46.6%,50.8%,45.3%and 20.4%,respectively(t =7.20,6.30,6.58,5.54,8.35,P < 0.05).Compared with B1,the image noise in B2 was reduced by 32.5% (t =6.12,P <0.05),the central and peripheral SNR and CNR and scores were increased by 46.2%,46.2%,45.9%,46.9% and 18.4% (t =3.19,3.55,2.95,3.37,5.42,P <0.05).Compared with group A,the image noise in B2 group was increased by 47.3% (t =4.03,P <0.05),the central and peripheral SNR and CNR and scores were reduced by 29.1%,21.8%,28.2%,19.6% and 8.2%(t =4.06,2.82,3.68,2.22,3.02,P<0.05).Conclusions Keeping effective diagnostic image quality, the protocol with low kVp and individual-adapted low tube current for low-dose MDCT pulmonary angiography would be effective and feasible, and the radiation dose would be significantly reduced.

Objective To report the clinical, myopathological and genetic features of a patient with mitochondrial encephalomyopathy, lactic acidosis and stroke-like episodes (MELAS)/Leigh syndrome (LS) overlap syndrome who carried m.10158 T>C mutation. Methods The patient′s clinical and imaging materials were collected. An open biopsy of right biceps brachii was performed. DNA samples were prepared from the patient and her mother′s blood. Direct sequencing of the complete mitochondrial genome was performed to detect the mtDNA mutation.Western blotting was used to estimate the content of respiratory complexes in the patient′s muscle. Results The patient was a 40-year-old female. She had seizures and lost consciousness for 9 months. Brain MRI findings consisted of asymmetrical lesions in the cerebral cortex of the frontal and temporal lobes, as well as symmetrical lesions bilaterally in the basal ganglia. Muscle biopsy showed typical ragged red fibers. Direct sequencing of the complete mitochondrial genome from blood and muscle of the patient revealed the T-to-C transition at nucleotide position 10158 in the MT-ND3 gene. The mutation rate was 9.31% and 70.0%, respectively.Western blotting demonstrated that the contents of complexes Ⅰ and Ⅳ were significantly lower in the patient′s muscle mitochondria compared with the normal controls (53.1%±1.2% vs 88.6%±1.7%, t=4.08, P<0.05;57.3%±2.4% vs 80.1%±2.1%, t=3.39,P<0.05).Conclusion We reported a case of MELAS/LS overlap syndrome who carried m.10158 T>C mutation in MT-ND3 gene and DNA test is very important for the diagnosis of the disease.
