1.Long-term Effects of Tangshan Earthquake on Psychosomatic Health of Paraplegic Suffers

Objective: To study the long-term effect of Tangshan earthquake on psychosomatic health of paraplegic suffers. Method: 64 paraplegic suffers of Tangshan earthquake and 64 normal controls were interviewed and assessed with self made questionnaire for psychosomatic health, SCL-90, SAS, SDS, CMI (Cornell Medical Index) and SSRS (Social Support Rating Scale). Results: 6 patients (9%) were diagnosed as PTSD according to CCMD-2-R, this rate was higher than that of normal citizen experienced the earthquake. 32 patients had Acute Stress Reaction. At present, patients' group had poorer mental health than control reflected by SCL-90, SAS, SDS and CMI. Conclusion: The severity of trauma both mentally and physically has great influence on mental health of suffers even after 25 years.
2.Factors Related to Hospitalized Schizophrenics with Metabolic Syndrome

Objective:To explore the relative factors of hospitalized schizophrenics with metabolic syndrome. Methods: Patients data were collected by the way of face-to-face questionnaire to patients or their guardians, consulting patients medical record, checking physical or laboratory examination. Information include age, duration of the disease, the classification of antipsychotics and dosage, fast plasma glucose, plasma lipids, blood pressure, etc of hospitalized schizophrenics. The relative factors of metabolic syndrome were analyzed. Results; Among 797 schizophrenics, there were 153 cases (19. 2% ) suffered from metabolic syndrome. Single-factor analysis showed that the patients with longer duration of the disease, taking clozapine, etc, were at high risk of metabolic syndrome, and sex didnt relate to metabolic syndrome; Logistic regression analysis appeared that metabolic syndrome correlated with age, clozapine, CRP concentration, etc. Conclusion: The possible risk factors causing metabolic syndrome in schizophrenia include, duration of the disease, etc.
3.Treatment of intertrochanteric and subtrochanteric fractuers of the femur with close reduction and the improved proximal femoral nail

[Objective]To investigate the clinical outcomes of treatment of intertrochanteric and subtrochanteric fractures of the femur with the improved proximal femoral nail(PFN).[Method]Between April,2002 to Mach 2005,a total of 81 cases of intertrochanteric and subtrocha nteric femoral fractures were treated with close reduction and the improved PFN.There are 45 males and 36 females,with an average age of 63.6(35 to 91).According to the Evans classification: there are 5 cases of type Ⅰ,10 cases of type Ⅱ,12 cases of type ⅢA,21 cases of type ⅢB,28 cases oftypel Ⅴ,5 cases of type against femoral peritrochanteric.[Result]Sixty-two cases were followed up for 12 to 47 months,averaged 28.6 months.Fracture union was obtained in all the patients.Of 62 cases,one case had according to Harris criterion 56 cases were excellent,(rate 90.3%) and 2 cases good(3.2%) excellent and good rate of the function of the hip joint was 93.5%.[Conclusion]The improved PFN is a useful device in the treatment of perit rochanteric fractures of the femur.It is relatively easy with less surgical trauma,it and also has the advantages of anti-rotation,stress-dispersing,rigid fixation and allowing early functional exercises.The complications can be avoided with improved operative technique.
4.The acquisition of alcohol-conditioned place preference in post-traumatic stress disorder-like rats and its relationship with the dopamine D1 receptor

ObjectiveTo evaluate whether post-traumatic stress disorder(PTSD)-like rats more easily to acquire the alcohol induced conditioned place preference(CPP),and its relationship with dopamine D1 receptor.Methods40 SD rats were randomly divided into four groups:PTSD + ethanol group ( PE.),control + ethanol group (CE),PTSD + saline group(PS),and control + saline group(CS).All rats of each group were trained for CPP with alternate injections of alcohol ( 2 mg/kg,i.p.) and saline ( 10 ml/kg,i.p.).PE and PS group were subjected to single prolonged stress (SPS) for PTSD model.Four groups were evaluated the performances of freezing behavior and plus maze test after SPS 24 hours and 7 days.And on SPS 7 days four groups were respectively detected D1 dopamine receptor-positive cells number in amygdale by immunohistochemistry.ResultsCompared with control group,freezing time of PTSD group was remarkable longer on 7 days after SPS not 24 hours( (89.13 ±8.60) s vs(22.25± 5.85) s,q =8.77,P < 0.01 ),and number of entry into the open arms and time spent in the open arms of PTSD group were both less than control group on 7 days after SPS not 24 hours( (4.25 ± 1.26) vs ( 14.38 ± 2.18),( 12.38 ± 3.30) s vs (40.38 ± 7.29 ) s,q =4.74 and 4.08,P < 0.01 ).In CPP,the CPP value of post-conditioning only in PE group was obviously higher than that of pre-conditioning ( q=31.81,P< 0.01 ).The CPP value of postconditioning in PE group was higher than that of CS group,CE group and PS group( q=-38.32,-22.21,-33.38,P<0.05).There were no significant differences in the number of D1 dopamine receptor-positive cells in the amygdale region among four groups (F =0.07,P >0.05 ).ConclusionThe PTSD-like rats are easier to acquire the alcohol CPP,which maybe not relate to the changes of the number of D1 dopamine receptor-positive cells in amygdala.
5.The clinical application of moderate hypothermia

Objective To study the moderate hypothermia with iced saline given intravenously with ice cap and ice sheet used in patients with severe stroke or craniocerebral trauma in order to clarify its clinical therapeutic effect and recovery of neurologic function for providing reliable clinical data.Methods A total of 84 patients suffered severe stroke (cerebral hemorrhage or cerebral infarction) or traumatic brain injury with GCS (Glascow Coma Scale) <8 admitted within 24 hours after onset were enrolled for study.The exclusion criteria were functional decompemsation of heart,liver,kidney and lung as cardiovascular complications of hypertension,hematological and other diseases,and craniocerebral trauma with multiple injuries of thorax,abdomen and extremities.Patients were divided into observation group and control group (n =42 in each group).The patients of two groups were managed to maintain respiratory tract patent,prevention of infection,dehydration,reduction of intracranial pressure and treated with Cytidine Diphosphate Choline,piracetam,ganglioside sodium and other conventional therapy.In control group,patients treated with ice cap and ice sheet,whereas in observation group,patients were treated with iced saline injected intravenously in addition to ice cap and sheet.At admission,the GCS score of patients was assessed and 6 months later,GCS score and Glascow Outcome Scale,(GOS) score of patients were determined,and 12 months after the treatment,quality of life score were evaluated by KPS.Results The time from onset of stroke or from trauma accident occurred to initiation of hypothermia treatment and time from initiation of hypothermia therapy to the target temperature reached in observation group were (8 ±4) hours and (3± 1 )hours respectively,and in control group,they were ( 16± 5) hours and (6 ± 2) hours,and there were significant differences between two groups ( P < 0.05 ).The mean GCS score of patients at 6 months in the observation group and in the control group were (7.01 ± 1.12) and (4.02 ± 1.11 ) respectively (P <0.05 ) ; whereas the GOS score at 6 mouths of patients was categorized into good outcome,disabled outcome and poor outcome,and the good outcome in the observation group was significantly better than that in the control group (P < 0.05 ),and no statistically significant difference in disabled outcome was found between two groups (P > 0.05),and the poor outcome of GOS score was significantly lower in observation group than that in control group ( P <0.05 ).Twelve months after the treatment,the long-term quality of patients'life assessed by KPS score in observation group was significantly better than that in control group ( P < 0.05).Conclusions The moderate hypothermia therapy by using iced saline administered intravenously combined with ice cap and ice sheet for severe stroke and head trauma patients conferred better therapeutic effects than the treatment with ice cap and ice sheet.
6.Antibiotic prophylaxis for transrectal prostate biopsy:the value of rectal swab culture and targeted antimicrobial treatment

Objective To evaluate the targeted antimicrobial prophylaxis in men undergoing trans-rectal ultrasound guided prostate biopsy based on rectal swab culture results.Methods From July 2008 to April 2012 we studied differences in infectious complications in men who received targeted vs standard empirical ciprofloxacin prophylaxis before transrectal ultrasound guided prostate biopsy.All 344 patients were divided into 3 groups according to their hospital records,inculding group A:without antimicrobial prophylaxis(105 eases) ;group B:antimicrobial prophylaxis with fluoroquinolone(117 cases) ;group C:targeted prophylaxis used rectal swab cultures results(122eases).All cases received enema with diluted iodophors before biopsy.We identified men with infectious complications within 7 days after transrectal ultrasound guided prostate biopsy using the electronic medical record,following 3 conditions.Results In group A of 105 case,17 cases of fever were recorded,including prostatic abscess of 3 cases and septicemia of 1 case.Three cases of fever were occurred in group B,including 1 case of bacteremia and 1 case of prostatitis.No infectious complications were recorded in group C.Conclusions Targeted antimicrobial prophylaxis was associated with a notable decrease in the incidence of infections complications after transrectal ultrasound guided prostate biopsy,although fiuoroquinolone can provide good protective effets.
7.A randomize controlled trial of early response between repetitive transcranial magnetic stimulation and modified electroconvulsive therapy in patients with first-episode depression

Objective:To explore the differences of the early response time between transcranial magnetic stimulation (rTMS ) and modified electroconvulsive therapy (MECT )in patients with first-episode depression.Methods:Totally 65 first-episode patients aged 18 -45 year were enrolled in the study.The diagnoses were made according tothe Diagnostic and Statistical Manual of Mental Disorders,Fourth Edition (DSM-Ⅳ)criteria for depression.The patients randomly received rTMS(n =33)or MECT (n =32).All patients received co-therapy of escitalopram (10 -20 mg /day).Stimulation (20 Hz)was performed five times a week for four weeks and,a total of 20 consecutive working days over the left dorsolateral prefrontal cortex(DLPFC).MECT group received MECT treatment 3 times a week for 3 weeks,a total of eight times.The Hamilton Depression Rating Scale-24 items (HAMD)was used to assess the severity of clinical symptoms and reduction rates.Results:Compared with the pa-tients in MECT group,the patients in rTMS group showed a less reduction rates [(25.0.±3.1),(34.4 ±7.7), (52.2 ±7.8),(69.1 ±8.3),vs(28.2 ±5.4),(38.8 ±5.1 ),(57.7 ±6.8),(75.4 ±8.6),Ps <0.05]in the first four weeks and the suicidal behavior scores reduced less in rTMS group in the first two weeks (P <0.05).Conclu-sion:It suggests that both rTMS at 20 Hz and MECT combined with escitalopram could improve the first-episode depression within a week.While the efficacy of rTMS combined with escitaloprm slightly less than MECT com-bined with essitaloprm in rapid response and reducing the suicide risk of acut period.
8.Clinical Efficacy of Vitrectomy Combined with Cataract Surgery in Treatment of Proliferative Diabetic Retinopathy

Objective:To study the clinical efficacy of vitrectomy combined with cataract surgery enin the treatmt of proliferative diabetic retinopathy.Methods:90 patients of proliferative diabetic retinopathy who were treated from January 2013 to June 2016 in our hospital were selected.According to random number table,those patients were divided into the observation group (n=45) and the control group (n=45).The control group was treated with vitrectomy+lentectomy,while the observation group was treated with vitrectomy+ phacoemulsification.Then the best corrected visual acuity,tear interleukin (IL)-2,fluorescein (FL),breakup time of tear film (BUT) and Schirmer test(SIt) and incidence of complications were compared.Results:After operation,the best corrected visual acuity of observation group was better than that of the control group(P<0.05);the tear IL-2 level in the observation group was significantly higher than that of the control group (P<0.05);the FL in the observation group was significantly lower than that of the control group,the BUT and SIt were significantly higher than those of the control group (P < 0.05);the incidence rate of capsular opacity,iris neovascularization,corneal edema,xerophthalmia in the observation group was significantly lower than that of the control group (P <0.05).Conclusion:Vitrectomy combined with phacoemulsification was effective for proliferative diabetic retinopathy,which could promote the recovery of visual acuity after surgery,improvement of tear secretio and decrease the complications.
9.Anti-cytomegalovirus (CMV) effect of antisense oligodeoxynucleotides phosphorothioates (AS S OND)

AIM To study anti CMV effect of S ODNs complementary to the initial code region of major immediate early (MIE) mRNA, and to the intron1/exon2 boundary of MIE mRNA precursor. METHODS To evaluate the anti CMV effect by determining viral antigen on infected 2BS cells by in situ ELISA. RESULTS Both of 2 AS S ODNs and their sense sequence control showed anti CMV effect. The medium effective concentration (EC 50 ) were 4 53, 10 2, 26 2 and 30 1 ?mol?L -1 . The secretion of CMV antigens were delayed by 3 d~4 d by 8 0 ?mol?L -1 AS 1. It showed increased effect by administrating earlier postinfection, by supplementing the S ODN at intervals, by conbinating with ganciclovir, and by packaging with liposome. A slight cytotoxicity was observed at the concentration of 64 0 ?mol?L -1 . Northern blot analysis indicated that the MIE mRNA decreased after the treatment of AS 1. It suggests that disintegration of the mRNA in the hybridized duplex by activated RNase H played an important role as the mechanism of specific action, and "time and effect" analysis suggested that interference of viral adsorbtion and penetration may be the important mechanism of nonspecificity. CONCLUSION AS S ODNs targeted to MIE gene are effective anti CMV agents which can be developed as a new type of chemotherapy drugs against CMV infection.
10.Effects of activities of daily living and life care on depressive mood for elderly in community

Objective To investigate the prevalence of the depression and the activities of daily living in the elderly population in Hebei province.Methods In this cross-sectional study,3 176 subjects aged over 60 were selected from 4 urban communities in Hebei Province.The demographic data was collected,and the Center for Epidemiological Survey,Depression Scale,(CES-D) and the activities of daily living Scale (ADL) were used to assess their depression status and activities of daily living.Results Among the 3 176 subjects,268 cases (8.4%) got the CES-D scores of over 15 and 232 cases (7.3%) got the ADL scores of over 26.14.2% of depression group needed life care,27.9% of depression group had absence of daily living activities.Multivariate logistic results showed that gender (OR=2.142),diseases situation (OR=2.214),marriage (OR=0.484),and life abilities like shower (OR =1.333),walk around home (OR=1.275),wear clothes(OR=2.129),moving on a wheelchair(OR=1.118) had significant effect on depression.Conclusion Elder people population who have characteristics like female,single,illness,can't take shower by themselves,life care depended,more likely get higher CES-D scores and depression.
