In the 46th annual meeting of the American Society of Hematology, the advance in the treatment of maligmant hematological disease concentrats on targeted therapies, including oblimersen,combination therapy rituximab with the chemotherapeutic agent, proteasome inhibitor bortezomid, flavorpiridol and radioimmunotherapy. Hematopoietic cell transplantation (HCT) is an effective therapy of maligmant hematological disease. The foremost advantage of the nonmyeloablative or reduced-intensity HCT is the reduction of treatment-related mortality .

Recenly,the incidence of posttransplatation lymphoproliferative disorder(PTLD) is rising,Immunodeficiency and EBV-infection are risk factors of pathogenesis related. The majority of PTLD originates from malignant proliferation of B-cell.Typical pathologic findings of PTLD are large number of plasmacytoid B cells in lymphotic tissue and often with necrosis in regional areas. Molecular biology findings are ras or p53 gene mutation and c-myc?bcl-6 gene rearrangement. Highly immunosuppressed patients may present with diffuse infiltration and functional failure of multiple organs,the prognosis is poor. Except for the decrease or discontinuation of the immunosuppressive therapy,antiviral agents and interferon- ?,anti B-cell monoclonal antibodies,autologous EBV-specific cytotoxic T cell and combination chemotherapy may achieve better responses.

Objective To investigate the association of the polymorphism of serotonin transporter gene linked polymorphic(5-HTTLPR) region with depression and its clinical symptoms in the Han Chinese.Methods PCR-based technique was used to genotype 5-HTTLPR in 64 patients with major depression and 67 unrelated controls,and the association between the polymorphism and clinical symptoms of depression was explored according to HAMD score.Results Fewer patients tended to be of L/L genotype(9.4%) than controls(19.4%),but there was no significant difference.Among women,fewer patients tended to be of L/L genotype(2.9%) than controls(13.9%),but there was no significant difference.The symptom analysis showed that depressive symptoms were significantly associated with 5-HTTLPR genotypes or alleles in the patients of L/L genotype,and that L allele had significantly lower depressive scores compared with those of other genotypes or alleles(P

Objective Developmental mandibular asymmetry is a common deformity in the human craniofacial skeleton and the treatment is not easy to get satisfactory results. A retrospective study was performed to investigate the clinical results and stability after treatment of developmental mandibular asymmetry by using combination of orthognathic surgery with orthodontics. Methods 58 cases of developmental mandibular asymmetry were treated by preoperative orthodontics-orthognathic surgery -postoperative orthodontics procedure. 82 times of operation, such as mandibular osteotomy, bimaxillary osteotomy, jaw plastic surgery, genioplasty, and distraction osteogenesis were performed according to different type of asymmetry. The facial appearance and occlusion relationship were evaluated with 1～4 years of follow-up. Results Satisfactory results were achieved in 53 cases (91.4 %). Primary satisfactory results were obtained in 4 cases (6.9 %). 1 case (1.7 %) with unsatisfactory result obtained aesthetic effect and ideal occlusion after operation again. Conclusions It is essential to use both of orthodontics and orthognathic surgery in order to ameliorate facial appearance, resume occlusal function, achieve good corrective result and prevent the recurrence of the developmental mandibular asymmetry.

Objective To study the gene mutation characteristics of patients with spinocerebellar ataxias(SCA) in Ningxia region.Methods The SCA3/Machado-Joseph disease(MJD)trinucleotide CAG repeat number was detected by polymerase chain reaction(PCR),capillary gel electrophoresis(CGE) and DNA sequencing in 6 patients with SCAs and 4 normal kindreds from a family in Ningxia region.Results All patients and 1 kindred were positive for SCA3/MJD;other 3 kindreds were negative for SCA3/MJD.The CAG repeat number was expanded to 66～81 in the patients with SCA3/MJD and a kindred,and 22～33 in other 3 kindreds.Conclusion There is SCA3/MJD in the patients with SCAs of Ningxia region.Gene examining has the great significance for definite diagnosis in the patients with SCAs.

Objective To evaluate the clinical efficacy in the treatment of unilateral micrognath ia by using EAM medical resin and hydroxyapatite (EH) composite material combined with the CAD/ CAM technique.Methods From July 2011 to October 2015,12 cases of unilateral micrognathia caused by different reasons were treated,based on the representative traits and requirements,refering to the unsymmetric counter part by right of CAD/CAM technique.By the 3-dimensional design and reconstruction,we reformed the EH composite material into purposed shape,and insert it in the operative area.Results All the materials were closely suitable to the mandible surfaces.1 case failed because the wound was torn apart;the other 11 patients recovered more than 6 months and were satisfied with the external appearance.Conclusions The EH composite materials combined with CAD/ CAM techniques could be a potential characterized remedy for the unilateral micrognathia.

Objective To investigate the clinical efficacy of treatment for chronic pancreatitis and mass in the head of the pancreas.Methods The clinical data of 19 patients with chronic pancreatitis with mass in the head of the pancreas who were admitted to the China-Japan Union Hospital of Jilin University from May 2008 to May 2013 were retrospectively analyzed,including 2 patients with gallstone.Methods of treatment were selected by in traoperative pathological results.All the patients were followed up by outpatient examination and telephone interview till January 2014.Results The operations of the 19 patients were successfully done,including 11 with pancreaticoduodenectomy,5 with exploratory laparotomy and 3 with the laparoscopic cholecystectomy and choledochojejunostomy.The operation time and intraoperative blood loss were (249± 140)minutes (50-390 minutes) and (320 ± 260)mL (50-900 mL).The postoperative mean time of recovery of gastrointestinal function and duration of postoperative hospital stay were 3.0 days (2.0-6.0 days) and 22.6 days (8.0-30.0 days) after the operation.The mean casein-IgG4 of 12 patients was higher (2.64 g/L) after operation.Five patients with operation-related complication were cured after symptomatic treatment,among 2 patients with leakage of the pancreatic fluid,2 with operative incision fat necrosis and liquefaction and 1 with bile leakage.No patients died perioperatively.Nineteen patients were followed up for 11.0 months to 36.0 months with the median time of 28.2 months.The pancreatic head mass of 8 patients with non-pancreaticoduodenectomy were reduced or disappeared by a computed tomography (CT) or magnetic resonance imaging (MRI) rescan,who were treated by the oral prednisone.No complications were detected in the other 11 patients with pancreaticoduodenectomy.Conclusion Method of surgery or medicine treatment for atypical mass in the head of the pancreas is selected according to the pathology results by effective communication with patients and family members,exploratory laparotomy and interpretative aspiration biopsy.

Objective To study the life quality and influencing factors for empty nester elderly in rural area in Hunan,and to provide the scientific basis for the effective interventions.Methods The two villages of Yiyang and Huaihua were randomly selected by cluster sampling from June 2011 to December 2011 in Hunan.Data of the world health organizations quality of life instrument-short version (WHOQOL-BREF) was collected by face to face interview with the empty nesters.The risk factors for the quality of life in empty nester elderly in rural area were analyzed by the multiple linear regression analysis.Results A total of 785 valid questionnaires were acquired,among which 472 cases were from empty nester elderly (60.1％,472/785),the average age of them was (69.3±7.3) years,and the male:female ratio was 1.1:1 (250:222).The literacy of 81.8％ of empty nester elderly were under junior middle school,69.7％ of them married with consorts who were still living,74.4％ of them were farmer,75.2％ of them had monthly income under 500 Yuan,56.4％ of them got the income from their own labor,69.9％ of them considered that their relationship with children were harmony.64.2 ％ of empty nester elderly in village suffered from chronic diseases and 16.7 ％ of them suffered from accidental injury within the last one year.The scores in physiological health,psychological health and quality of life were lower in empty nester elderly than in the non-empty nester elderly [(60.8±15.7) vs.(63.1±13.5),(59.0±12.1) w (64.6±17.4),(59.5±9.8) vs.(63.0 ± 10.1),t=3.18,4.43,4.91,all P＜0.05].The multiple linear regression analysis showed that the influencing factors for quality of life in empty nester elderly were education background,marital status,source of finance,relationship with children,chronic disease and accidental injury within the last one year (β=1.693,2.426,0.779,1.060,-7.007,-3.784,all P＜0.05).Conclusions The empty-nest elderly accounts for more than half of the elderly in rural area,who have the lower levels in physio-psychological health and quality of life.We should improve their awareness and knowledge of health,build their harmonious relationship with spouse and children,improve the social security and medical insurance system,actively promote the knowledge of chronic disease and accidental injury,in order to improve the quality of life in empty nester elderly and achieve the healthy aging.

Objective To deal with measurements of the relative convexity, to observe the morphological characteristics of zygoma and zygomatic arch, and to provide references for anthropology and medical science. Methods 108 dry skulls (62 male and 46 female) from Wuhan University Medical School were used, the zygomatic relative convexity of both sides were measured, and the morphological characteristics were observed in this study. Results Zygomatic process point lied in the middle of zygoma in horizontal direction, and situated 15-20 mm below the external middle 1/3 of infra-orbital margin. The zygomatic arch was anterolateral convexity, and face width in male was bigger than that in female. Objective To investigate the changing of endothelial progenitor cells and nitric oxide in patients with pulmonary arterial hypertension secondary to chronic obstructive pulmonary disease.Method Mononuclear cells from 20 healthy persons (the control group, n = 20) , COPD patients without PAH (COPD non-PAH group, n =30) and patients with PAH secondary to COPD (COPD and PAH group, n=30) were investigated. Total mononuclear cells were isolated from peripheral blood by Ficoll density gradient centrifugation. EPC number and migration were assayed by colony forming unit-EPCs (CFU-EPCs) assay and modified Boyden chamber assay , respectively. EPC adhesion assay was performed by replanting those on fibronectin-coated dishes , then adherent cells were counted. The concentration of NO was measured with method of nitrate reductase.Result The numbers of CFU and migration, adhesion activity of circulating EPCs in COPD[ (21.9±3. 9)CFU-EPC] and PAH group[ (14. 2 ±3. 5)CFU-EPC] were significantly lower than that in non-COPD group and COPD non-PAH group [ ( 24.9 ±4.1) CFU-EPC ]. It was also observed that a strong negative correlation between the levels of PAH and the numbers of CFU and adhesion, migration activity of circulating EPCs. The NO level in the PAH group [ (43. 6 ±4. 8)ng/ml] was significantly lower than that in control group [ ( 67. 17±4.9 ) ng/ml ] ( P < 0.01). The NO level was positively correlated with number and migration ability of EPCs( r =0. 77,0.71, P <0.01) , but not correlated with adhesion ability.Conclusion The number of CFU and migratory, adhesive activity of EPC in patients withPAH secondary to COPD was significantly decreased. These changes may be associated with low level of plasma NO.
