1.Clinical Observation of Alprostadil in the Treatment of Heart Failure with Normal Left Ventricular Ejection Fraction

OBJECTIVE:To investigate the clinical efficacy and safety of alprostadil in the treatment of heart failure in pa-tients with normal left ventricular ejection fraction (LVEF). METHODS:120 patients diagnosed as heart farlture with normal LVEF were divided into observation group and control group by radom number table method,with 60 cases in each group. Control group was given symptomatic treatment;observation group was additionally treated with Alprostadil dried emulsion for injection 10μg,bid,on the basis of control group. Both groups received treatment for 5 d. The cardiac function indexes,serum inflammatory factors and oxidative stress indexes were observed in 2 groups before and after treatment,and the occurrence of ADR was com-pared between 2 groups. RESULTS:Before treatment,there was no statistical significance in cardiac function indexes,serum in-flammatory factors and oxidative stress indexes between 2 groups (P>0.05). After treatment,above indexes of 2 groups were all improved significantly,and the observation group was significantly better than the control group,with statistical significance(P<0.05). No obvious ADR was found in 2 groups. CONCLUSIONS:Alprostadil can improve cardiac function of patients with heart failure of normal LVEF,improve serum inflammatory factors and oxidative stress level with good safety.
2.Effect of hongjintian decoction on serum MDA, SOD, NGF, MBP and BFGF levels of exercise-induced fatigue athletes

Objective To investigate the effect of Hongjintian decoction on the levels of serum MDA , SOD, NGF, MBP and bFGF of physical fatigue athletes.Methods 40 students of physical education college were randomly divided into experimental group and control group, 20 cases in each group.After the end of sport, the experimental group were given Hongjintian decoction, and control group were given sugar water, the levels of serum MDA, SOD, NGF, MBP and BFGF in two groups were detected.ResuIts Compared with control group, the level of MDA in experimental group was lower(P<0.05), and the level of SOD was higher (P<0.05); NGF and BFGF levels were higher in experimental group(P<0.05), MBP level was lower (P<0.05).ConcIusion Hongjintian decoction can significantly reduce serum MDA, MBP levels, increase serum SOD, NGF and BFGF levels of physical fatigue athletes.
3.Botulinum toxin-A on treatment of hemifacial spasm and the changes of blink reflex by double stimulation before and after treatment

Objective To further observe the effect of botulinum toxin A(BTXA) treatment on hemifacial spasm(HFS) patients and to investigate the value of double stimulation of blink reflex(BR) before and after BTXA injection. Methods Among 134 cases of HFS treated by local injection of BTXA, the changes of BR in 42 cases was recorded before and after the injection. Results Of totally 134 treated cases, 60 (44 8%) cases were completely relieved and the rest 74 (55 2%) cases were obviously improved after treatment, with an effective duration of 16 5?5 7 weeks.In 42 patients with determination of BR value,before injection the latency of R 1, R 2, R' 2 of the affected side showed within normal limit.The interval of R 2 was shorter and the amplitudes of R 1,R 2,R' 2 were higher .After injection , the R 1 disappeared in 7 patients,and R 1,R 2,R' 2 all disappeared in 5 patients,for the rest 30 patients,the latency of R 1,R 2,R' 2 was prolonged slightly,but showing no apparent differences The amplitudes of R 1,R 2 and R' 2 were lowered significantly ( P
4.Survey and analysis of health management of public health service cognition, utilization and satisfaction among rural immigrating pregnant women

Objective To analyze the cognition, utilization and satisfaction on pregnancy health management of basic public health services. and to find out potential problems and provide suggestions for promoting equality in rural basic public health. The Weishan Lake area was selected as the sample point and the left behind pregnant women in the countryside as control. Methods In the period from May 2016 to July, questionnaires were designed for the pregnancy health management of basic public health services, 330 left-behind pregnant women and 300 rural immigrating pregnant women were selected as our target interviewees. The average age of rural immigrating pregnant women and the left-behind pregnant women was (28.6 ± 3.7) years, and (28.3 ± 3.4) years old, respectively; the degree of education of rural immigrating pregnant women and the left-behind pregnant women was secondary specialized school or high school and junior middle school, respectively. The occupation of rural immigrating pregnant women was mainly workers;accounting for 31.4%, while the occupation of the left-behind pregnant women was farming, accounting for 36.7%. Results The awareness rate ofgeneral pregnancy examination and gynecological examinationin health management items of rural immigrating pregnant women and left-behind pregnant women was the highest, which were 77.7% and 78.3%, respectively, and the lowest awareness rate of the 2 prenatal follow-up was the lowest in the third trimester, which was 22.6% and 23.0%, respectively. The difference was not statistically significant. The maternal health management service understanding rate of rural immigrating pregnant women and left-behind pregnant women were 24.7% and 17.9% respectively , the difference was statistically significant (Z=-1.993, P=0.046);the awareness rate of delivery and breastfeeding in late pregnancy were 36.2% and 47.6% respectively, and the difference was statistically significant (χ2=7.933, P=0.005). The rate of the registering of the maternal health care handbook of rural immigrating pregnant women and left-behind pregnant women were 40.4%and 54%respectively, and the difference was statistically significant (χ2=11.065, P=0.001), Among them, the rate of registering in township hospitals was 46.6% and 73.3% respectively, and the difference was statistically significant (χ2=21.104, P=0.0000); the utilization rate of 3 or more antepartum examination and follow-up services was 8.4%and 23%respectively, and the difference was statistically significant (χ2=23.878, P=0.000). The growth and development of the health of pregnant women and fetal assessment and utilization rates were 9.8% and 23.6%, the difference was statistically significant (χ2=20.460, P=0.000); prenatal guidance (childbirth preparation, breastfeeding) utilization rate were 8%, 13.7%, the difference was statistically significant (χ2=5.011, P=0.027);abnormal or critical maternal referral utilization rate were 22%and 34.8%, the difference was statistically significant (χ2=12.133, P=0.001). Conclusion Imbalance exists in acquiring basic public health services of the two pregnant women cohort, the left-behind pregnant women are better than the rural immigrating pregnant women. Therefore, the government should propagate this service by multi-channel to improve their cognition and utilization of this policy implemented, and to promote the development of the services.
5.Cognitive impairment caused by cerebral small-vessel disease Magnetic resonance spectroscopy and diffusion tensor imaging

The manifestation of clinical symptoms of cerebral small-vessel disease (SVD) is cognitive disorders. Its pathological changes are manifested as leukoaraiosis and/or lacunar infarction. However, it is not clear whether cognitive disorders caused by SVD is correlated with the severity of white matter damage. Magnetic resonance spectroscopy analysis can noninvasively detect metabolite concentrations of brain tissue in vivo and directly reflect cerebral metabolism; Diffusion-tensor imaging is a novel noninvasive technique for specific observation of white matter through different parameters such as mean diffusivity and anisotropy for quantitative evaluation of the abnormalities of fine structure of white matter. The combination of magnetic resonance spectroscopy analysis and diffusion-tensor imaging may more sensitively reveal the severity of white matter damage, and provide a research tool for cognitive disorders caused by SVD.resonance imaging
6.Influence of arsenic trioxide in vasculogenic mimicry of HepG2 cells and its mechanism

Objective To investigate the influence of arsenic trioxide (AS2 O3 )in the vasculogenic mimicry (VM ) of HepG2 cells, and to preliminary clarify the possible mechanism of inhibition of AS2 O3 on the VM. Methods Themean inhibitory concentration (IC50 )of AS2 O3 72 h after treatment of HepG2 cells was calculated by CCK-8 assay.The HepG2 cells were cultured on 3-D Matrigel and randomly divided into control group, 1/2 IC50 AS2 O3 group and IC50 AS2 O3 group.IPP software was used to calculate the number,length and area of VM,and the expression levels of VM-related proteins VE-cadherin and MMP-2, apoptotic-related protein caspase-3 and proliferation-related protein PCNA were detected by Western blotting method.Results The IC50 of AS2 O3 was 10μmol·L-1 72 h after treatment of HepG2 cells.The number,length and area of VM in 1/2 IC50 and IC50 AS2 O3 groups were significantly lower than those in control group (P<0.01);the number,length and area of VM in IC50 AS2 O3 group were also lower than those in 1/2 IC50 AS2 O3 group (P<0.05).Compared with control group,the expression levels of VE-cadherin and MMP-2 in 1/2 IC50 and IC50 AS2 O3 groups were decreased (P<0.05),and the expression levels of caspase-3 and PCNA had no significant change (P>0.05).Conclusion AS2 O3 can inhibit the forming of VM of HepG2 cells,which indicated that its mechanism may be related to inhibiting the expressions of VE-cadherin and MMP-2 .

Clopidogrel is an essential drug for the treatment of ischemic stroke.It has been recommended by the multinational stroke treatment guidelines and has been widely used in clinical practice.Many patients with ischemic stroke may still have stroke recurrence after the standard treatment of clopidogrel; therefore,we must pay attention to the phenomenon of clopidogrel resistance.This article reviews the biochemical mechanisms,gene polymorphism,laboratory testing,and response measures of clopidogrel resistance.

Objective To investigate the effect of medicinal charcoal tablets combined with blood purification therapy on on renal function, inflammatory mediators, oxygenation index and intestinal barrier function in patients postoperative severe abdominal infection.Methods 65 cases with severe abdominal infection from the hospital were selected and randomly divided into control group (32 cases) and experiment group (33 cases) by random digital table method.The control group were treated by clinical routine therapy and experiment group were treated on the basis of control group with medicinal charcoal tablets combined with blood purification therapy.The renal function, inflammatory mediators, oxygenation index, intestinal barrier function and efficacy were tested.ResuIts Compared with control group after treatment, the renal function of serum creatinine (Scr), blood urea nitrogen (BUM), uric acid (UA) levels were lower (P<0.05), the inflammatory mediators of tumor necrosis factor α(TNF-α), interleukin-6 (IL-6) and interleukin-8 (IL-8) levels were lower (P<0.05), the oxygenation index (OI) was higher(P<0.05), the intestinal barrier function indicators of plasma diamine oxidase (DAO) and endotoxin of lipopolysaccharide (LPS) were lower (P<0.05), the total efficiency were higher (χ2 =3.91, P<0.05) in experimental group.ConcIusion The medicinal charcoal tablets combined with blood purification could effectively improve renal function, reduce inflammatory mediators levels, improve intestinal barrier function, and increase oxygenation index in patients with severe abdominal infection, which has a good clinical curative effect for postoperative severe abdominal infection .
9.Palygorskite/hydroxyapatite/polycaprolactone/collagen composite scaffold for bone repair

BACKGROUND:Bone transplantation is the only method for the repair of bone defects. However, traditional bone transplantation has some disadvantages. Bone tissue engineering, as a new treatment strategy, can achieve the desire therapeutic outcomes. OBJECTIVE:To fabricate a new tissue-engineered scaffold for improving bone repair effectively. METHODS:Hydroxyapatites (HA) with different Ca/P (1.50/1.67) ratios were synthesized by chemical precipitation method and microwave radiation method. Composite scaffolds of palygorskite (APC)/HA/polycaprolactone (PCL)/col agen (COL), APC/calcium deficiency HA (CDHA)/PCL/COL, and APC/PCL/COL (control group) were prepared by solution perfusion-solvent evaporation and ion leaching method. The material characterization, active ingredients, hydrophilic property, and mechanical properties were evaluated by scanning electron microscope, infrared spectrometer, surface contact measuring instrument and universal mechanics, respectively. The histocompatibility of the implant with the host was assessed through animal experiments. RESULTS AND CONCLUSION:By precise control of pH range, HA with different Ca/P ratios could be synthesized. The mechanical properties, air permeability, hydrophilic property of the APC/HA/PCL/COL and APC/CDHA/PCL/COL composite materials were significantly increased compared with the APC/PCL/COL composite material (P<0.05), while the porosity, water absorption expansion rate were significantly decreased (P<0.05). Results from our animal experiments showed that no immune inflammatory reaction was observed suggesting that the composite materials hold good histocompatibility. To conclude, the APC/HA (1.50/1.67)/PCL/COL composite materials are promising bone substitutes in bone tissue repair.
10.Clinical features and outcomes of 78 children with non-Hodgkin lymphoma

ObjectiveTo explore the clinical features and factors inlfuencing the prognosis of childhood non-Hodgkin's lymphoma (NHL).MethodsPathologically diagnosed 78 pediatric patients with NHL and treated in the Afifliated Hospital of Qingdao University from January 2004 to August 2013 were collected and analyzed. Patients were grouped according to age, sex, tumor size, immunologic classiifcation, B-symptoms, LDH, hemoglobin and clinical staging. The 5-years event-free survival rate (EFS) were calculated and analyzed by Kaplan-Meier method, and the difference of the survival rate between groups were com-pared. Using Cox proportional hazards model, we analyzed the possible factors that might inlfuence 5-years event-free survival rate EFS , such as age and clinical staging. TheOR value and the 95%CI were calculated.ResultsAmong the 78 cases, median age of onset is 7 years old, male to female ratio is 2.90:1, there are 25 cases of T-cell type and 53 cases of B-cell type. According to pathological types,Burkitt lymphoma is the most common (34.6%), followed by T-lymphoblastic lymphoma (20.5%), diffuse large B-cell lymphoma (11.5%). According to the St. Jude malignant lymphoma staging system, there are 2 cases in stage I, 9 in stageⅡ, 35 in stageⅢ and 32 in stageⅣ. Swelling of periphery lymph node (80.7%) was observed as initial symptom in 26 cases of lymphoblastic lymphoma. Among 45 cases of mature B-cell tumor, the main clinical feature including abdominal cavity and gingival were observed in 27 cases of Burkitt lymphoma. Among the 73 cases received treatments, 66 cases (90.5%) attained CR (complete remission) and 4 cases (5.5%) attained PR (partial remission) by cytology and radiographic assessment after two course of combined chemotherapy, 2 cases (2.7%) rapidly relapsed after the remisson of one course treatment, 1 case (1.3%) appeared the central nervous system inifltration in the chemotherapy. With median follow-up time of 42 months, the 5-year EFS of the 73 cases was (67.0+5.5)%. Single factor analysis showed that B-symptom, LDH, and clinical staging were signiifcantly correlated with prognosis (P<0.05), while age, sex, tumor size, hemoglobin and immune classiifcation was independent of prog-nosis (P>0.05). Multiple factor analysis showed that LDH and clinical staging inlfuenced the prognosis (OR=3.34,95%CI 2.275?10.683,P<0.01;OR=4.354,95%CI 1.519?12.475,P<0.01) .Conclusionclinical features of childhood NHL are variable. LDH and clinical staging at primary diagnosis are important factors affecting the prognosis.
