1.Current diagnostic status,clinical and pathologic manifestation for Crohn's disease

Objective: To evaluate the contemporary diagnostic and therapeutic status for Crohn's disease(CD),and analyze its clinical and pathologic manifestation.Methods: Retrospectively we reviewed 220 hospitalized inflammatory bowel disease(IBD) cases in which 48 were diagnosed as CD.Data of diagnostic and therapeutic details were recorded.Results: In the past 10 years 44 of the 48 CD cases were diagnosed in recent and 75.0% of the cases were in the onset age range from 17 to 40 years.The most common symptoms were abdominal pain,diarrhea and bloody stool.16(33.3%) of the cases were accompanied with extra-intestinal manifestations,(3(6.3%)) with perianal abscess, and 2((4.2%)) with intestinal fistulation.The main findings through colonoscopy were ulceration,obstruction and cobble stone sign,with a diagnostic correspondence of 85.7%(36/42).Non-caseous granulomas were totally identified in 43.2%(19/44) of the histology.Thirty cases were administrated with Sulfasalazine/Mesalazine(SASP/5-ASA) or corticosteroids/immuno-suppressor in which 27 got clinically improved.Thirteen patients underwent surgery.Conclusion: The diagnostic incidence of Crohn's disease has been increased in recent years.The combination of endoscopy,radiography and histology is the best way for the diagnosis of CD.Small intestinal endoscopy and capsule endoscopy with repeated histopathology and follow-up are helpful for the diagnosis.
2.The application of PDCA model in management of hospital disinfection supply

Objective To explore a management strategy for disinfection supply in hospital. Methods A PDCA (plan, do, check, and action) model was applied to establish a manngement strategy for the disinfection supply, strengthen the education among all staff, promote the competence level of disin-fection supply center, organize joint inspection group for regular union supervision, give feedback promptly and implement strictly. Results By applying the PDCA model, the staff in the disinfection supply center strengthened the consciousness in standard cleanness, disinfection and sterilization, the number of pack-ages prepared by clinic room was reduced, and the supply of sterilized materials were more centralized. Conclusion The management strategy based on PDCA model of disinfection supply in hospital is effec-tive.
3.Whether Guangzhou Reach of Zhujiang River Will be Taken as a Swimming Area:an Investigation of Water Quality of Urban River Surge and a Certain Section

Objective To know the sanitary situation of urban river surge and a certain section of Zhujiang River and whether Guangzhou reach of Zhujiang River will be taken as a swimming region. Methods From July 1 to July 15,2007,the period of swim,the spring-tide water and ebb-tide water at urban river surge and a certain section were sampled and tested. 54 water sample at urban river surge and 40 at a certain section,the items included pH value,turbidity,dissolved oxygen,chemical oxygen demand, amino-nitrogen and heat-resistant coliform. Results The results indicated that pH value and chemical oxygen demand of the water at urban river surge and a certain section were eligible,but amino-nitrogen,dissolved oxygen and heat-resistant coliform were not eligible,the turbidity was high,the water quality did not meet the Hygienic Standard for Swimming Place (GB 9667-1996). Objective To evaluate the efficacy and safety of vasopressin(Pitr- essin)on esophageal variceal bleeding in patients with cirrhosis.Methods 42 cirrhotic patients with acute esophageal variceal bleeding confirmed by endoscopy were randomly divided into 2 groups and administered with Pitressin and Pituitrin for 48 hrs respectively.The balloon tubes were placed in the stomach before the treatment,and the duration of hemostatsis,volume of transfusion,rebleeding and short-term survival period were recorded.Results After 12 hrs treatment,the hemostatic rate in Pitressin group was 72.7%,significantly higher than that in Pituitrin group(40.0%,P
5.Clinical features, endoscopic findings and outcome of ischemic colitis

Objective To overview the clinical and endoscopic features of the ischemic colitis. Methods Retrospective analysis was conducted. Results Of the 60 cases, 87% were over 50 years, and females were more than males. 76.7% of the cases were associated with the basal diseases such as cardio cerebrovascular disorders, diabetes and hematological diseases as well as the history of abdominal operation. Colonoscopy showed mucosal edema, congestion, erosion and ulceration. 87.1% of the cases were cured after treatment and 16.7 % were improved. The outcome may be associated with the age, severity of the lesion, course, basal diseases and the complications. Conclusions Endoscopy plays a vital role in the early diagnosis of ischemic colitis.
6.Retrospective clinical analysis of 155 cases of hospitalized ulcerative colitis.

Objective To investigate retrospectively the clinical characteristics of hospitalized ulcerative colitis(UC)patients of our department,and evaluate the diagnosis and treatment.Methods The cases of UC patients from 1989 to Jun 2005 were retrospectively reviewed. We analyzed the patients' symptoms,diagnosis,and treatment.Results There were 155 cases reviewed. The sex ratio(M/F)was 1.54∶ 1. The average age of patients was 41.8. The peak age of onset was 20~30. 32.3% of our cases were mild type,and 41.3% were moderate type,severe type were 26.5%. Of these pateints,52.9% were pancolitis,and 39.4% were chronic recurrent type,which was very common. Most cases(72.9%)were diagnosed by clinic combined with endoscope and pathology. Most of the severe UC responded well to the drug therapy. Very less(4.5%)had to underwent surgery. No patient died.Conclusion Most of our patients are mild or moderate type,and they respond well to drug some severe and moderate types need steroids and immuno suppressor. The extracolonic manifestation rate(12.9%)and operation rate(4.5%)are low.
7.Application of urodynamic study in 452 patients with benign prostatic hyperplasia

Objective To analyze the urodynamic data of the patients with benign prostatic hyperplasia(BPH)and provide evidence for the therapy.Methods 452 patients with BPH were selected.The mean age was(65.6 ±5.65)years(51 to 89 years).All patients had intact history documents which included the results of cystoscopy,urodynamics,doppler,DRE and PSA.Results 15 cases were not diagnosed with BOO.430 cases were diagnosed with BOO,among them,65 cases of bladder detrusor muscle weakness,bladder detrusor instability(detrusor,DI,instability)in 365 cases,3 cases were normal,and the other 4 cases had detrusor atony.In 445 patients the mean maximum urinary flow rate(8.32±3.15)ml/s,the average residual urine volume(87.68±79.46)ml,the average maximum flow rate when the detrusor pressure(62.32±7.54)cm H2O,maximum urethral closure pressure was(86.43±18.35)cmH2O,among which 19 cases were not suitable for surgery.At the end of the 426 patients underwent surgical treatment,postoperative follow-up of 3 to 18 months,the International Prostate Symptom Score(IPSS)from 0 to 5,the quality of life index(QOL)score of 0 to 1,Qmax 11 to 21 ml /s,residual urine(45.6±36.2)ml.There were significant differences between the preoperative and postoperative indexes(P<0.05).Conclusion Urodynamic tests can avoid the blindness in the surgery treatment of BPH.
8.Early expression of tumor necrosis factor alpha in blood after gunshot wounds in dogs

Objective: To study the pathophysiological role of tumor necrosis factor ? (TNF ?) and interleukin6 (IL 6) in trauma by assaying concentration of TNF ? in blood after maxillofacial gunshot wounds in dogs. Methods: Before and after gunshot wounds, concentration of TNF ? and IL 6 in the plasma of dogs were assessed by ELISA method at different times. Results: Although the systemic injuries in our gunshot wound model were no very severe and the dogs had no severe complications, the TNF ? concentration in plasma became significantly higher in the first 2 6 h after the gunshot wounds. They varied greatly with each dog and became normal after 10 h without treatment. IL 6 was not detected in blood. Conclusion: TNF ? expression in blood after gunshot wounds is the normal host immune response to the trauma, its circulating concentrations might be associated with the injury severity. But their transient expressions do not significantly increase the severity of the trauma. [
9.Analysis of Non-rational Prescriptions in Pharmacy Intravenous Admixture Service

OBJECTIVE:To analyze the non-irrational prescriptions in pharmacy intravenous admixture services to ensure patient's medication safety.METHODS:We examined the prescriptions in pharmacy intravenous admixture services from Sep.2006 to Feb.2007 and analyzed the non-irrational ones regarding incompatibility,improper choice of infusion carriers,improper medication frequency,and the incompatibility of Chinese medicines etc.RESULTS:The non-irrational prescriptions totaled 126 cases,of which,46 associated with incompatibility,38 with improper choice of infusion carriers,25 with improper medication frequency and 17 with incompatibility of Chinese medicines.Most of the irrational drug use behaviors were corrected by informing the clinic these medication errors.CONCLUSION:Examining prescriptions in pharmacy intravenous admixture services can improve the safety and effectiveness of patient's use of intravenous drugs.

Tumor necrosis factor (TNF)-related apoptosis-inducing ligand (TRAIL) can be induced in ischemia and other pathological states and mediate neuronal apoptosis after cerebral ischemia.Understanding the upregulation mechanism of TRAIL in ischemic brain tissue is promising to develop new treatment for stroke.
