Objective To study clinical evaluation of xinqin granule combined with loratadine in treatment of allergic rhinitis and its effects on serum eosinophil(EOS),colony stimulating factor(CSF)and tumor necrosis factor(TNF)-α.Methods 90 patients of allergic rhinitis who received therapy from January 2014 to November 2016 in our hospital were selected.According to random number table,those patients were divided into the observation group(n=45)and the control group(n=45).The control group was treated with loratadine,while the observation group was combined with xinqin granule.Then the serum EOS,CSF and TNF-α,symptom score,clinical efficacy and adverse reactions were compared.Results After treatment,the serum levels of EOS,CSF and TNF-αin the observation group were significantly lower than that of the control group(P<0.05); the symptom score in the observation group was significantly lower than that of the control group(P<0.05); the total effective rate in the observation group was significantly higher than that of the control group[93.33％(42/45)vs.73.33％(33/45)](P<0.05); there was no significant difference in the incidence of adverse reactions between the two groups.Conclusion Xinqin granule combined with loratadine is well for allergic rhinitis,which can effectively reduce serum levels of EOS,CSF,TNF-α,relieve clinical symptoms,it's worthy of application and promotion.

OBJECTIVE To study the clinical features of OSAHS patients closely associated with supine position. METHODS The clinical data of 105 patients with OSAHS diagnosed by overnight polysomnography(PSG)were retrospectively studied. The patients were divided in two groups: the positional patients(PP)group, those patients who had a apnea-hypopnea index(AHI) that was at least two times higher than the lateral AHI, and the non-positional patients(NPP)group, those patients in whom the AHI in the supine position was two times less than that in the lateral position, and the two groups were compared with age, BMI and the severity of OSAHS. RESULTS The difference of age and BMI were not significant between the two groups[(40.4?10.5)year versus(37.0?10.6) year,(28.2?3.8)kg/m2 versus(29.1?3.0)kg/m2]. There was significantly different in AHI between two groups[(36.8?25.4)/h versus (51.2?29.0)/h ]. AHIsupine and AHInot-supine was significantly different in mild and moderate OSAHS patients [(23.9?13.8) /h versus(12.4?10.1)/h ]. CONCLUSION Mild and moderate OSAHS patients are mainly PP. Positional therapy should play an important role in treatment of mild and moderate patients.

Objective Our aim was to investigate the association between sudden sensorineural hearing loss (SSHL)and mean platelet volume (MPV)values,in order to evaluate the etiology of SSHL.Methods This study included 40 patients with SSHL in our ward between Jan.2012 and Jan.2014 and 30 age- and sex-matched healthy subjects were included as controls.The correlation among the levels of the number of platelets,mean plate-let volume(MPV),platelet distribution width (PDW)was evaluated in the two groups.ResuIts The platelet count was significantly lower in the SSHL than the control group(212.0±47.9 versus 237.8±46.2 ×109/L).MPV and PDW were significantly higher in SSHL(10.9±0.8 versus 10.3±0.8 fL;12.9%±2.0% versus 12.0%±1.3 %). The SSHL patients were divided two subgroups:the mild-moderate subgroup (hearing loss 25~70 dB)and the se-vere subgroup(hearing loss≥70 dB).The MPV values were insignificantly different between the two subgroups(11. 0±1.1 fL versus 10.8 ±1.0 fL).Compared to the control group,the two subgroups had both significant differ-ences in MPV.ConcIusion High MPV values are a risk factor for thrombotic vascular diseases.Considering the in-creased MPV levels,SSHL appears to be characterized by ischaemic or thrombotic events.

Objective To explore the clinical effect of the small incision with laser comprehensive treatment for patients with the great saphenous vein varices. Methods Small incision surgical ligation was done at the top and end of the great saphenous vein. Then took out the whole part or subsection of the great saphenous vein, with laser to treat the main part of the great saphenous vein and varicose vein group. Results 210 cases of 230 limbs all cured, cure rate was 100%. Conclusion The small incision with laser comprehensive treatment was an effective method for the great saphenous vein varices. It was worthy of clinical application and dissemination.

Objective To analyze postoperative complications and its prevention of small incision with laser comprehensive treatment the great saphenous vein varices. Methods Clinical data of 210 cases (total 230 limbs) in our hospital used small incision with laser comprehensive treatment the great saphenous vein varices were retrospectively analyze. Results 210 cases of 230 limbs,60 cases have concurrent subcutaneous congestion after surgery,6 cases have funicular induration,red and swallon,pain in the local crus,6 cases have skin paraesthesia due to saphenous nerve injure,4 cases have skin burns,2 cases have ulcer amalgamate infection. All of the cases have healed after the corresponding treatment. Conclusion This is a effective method for the great saphenous vein varices but may have complications. It should be taken seriously and processing.

Objective To study the change and the correlation of serum soluble vascular cell adhesion molecuh-1(sVCAM-1)level with diabetic retinopathy in type 2 diabetic patients.Methods 60 type 2 diabetic patients were selected for the study through the examination of ophthalmoscope and/or fundus fluorescence angiography by ophthalmologist.Diabetic patients were divided into three main groups:No signs of diabetic retinopathy(NDR)(n=20);Background DR(BDR)(n=20)Proliferative DR(PDR)(n=20).Healthy individuals matching sex and age of the patients were used as controls(n=20);Serum sVCAM-1 level was measured by ELISA,compared in diabetes without DR,with BDR,with PDR.These levels were compared with those of 20 controls.Results The serum level of sVCAM-1 in the DM patients with PDR or BDR and those without DR were significantly higher than those in healthy controls(all P＜0.001);Serum level of sVCAM-1 in PDR groups were higherthan those in DM patients with BDR or patients without DR(all P＜0.001);There was no difference between the patients with BDR and those without DR (P＞0.05).(4)In the DM patients,there was a positive correlation between serum sVCAM-1 and the course of diseases(r=0.338,P＜0.05),but no relationship with HbA1C,FBG,CHO,TG,LDL and INS.Conclusion Increased serum level of sVCAM-1 in different stage of DR patients suggested that they hagbe play an important role in the development of DR,and may assess the severity of diabetic retinopathy.The measuremem of serum sVCAM-1 levels in type 2 diabetic patients could be clinically useful for early diagnosis or treatment of diabetic retinopathy.

Objective To investigate the effect of expectant management on the maternal and/or infant out-comes of early onset severe preeclamp sia in different gestafional weeks. Methods A retrospective study was carried out on 158 patients with early onset severe preeclampsia. They were divided into three groups according to their onset gestation ago:group A( <28weeks,n=28) ,group B(≥28、<32weeks,n=51) and group C(≥32、< 34weeks,n =79). Results The rates of complications declined along with the postponement of the onset gestation age, but there was no statistical significant difference among these three groups. The neonatal asphyxia rate and perinatal infant mor-tality of these three groups declined along with the postponement of gestation age, and there were statistical significant differences among these three groups ( P<0.05 ). Expectant treatment time of group B was significantly longer than that of the other two groups ( P<0.05 ), and cesarean section was a main method of pregnancy termination for the groups B and C. Conclusion The smaller the gestational ages in the early onset severe preeelampsia,the higher the maternal and/ or infant complication rates, neonatal morbidity and mortality.

OBJECTIVE: This study aimed to inspect the hypothesis that obstructive sleep apnea and hypopnea syndrome (OSAHS) was commonly worse in REM sleep than in NREM sleep stage, and also to analyze the relevance with the risk factors of OSAHS. METHOD: One hundred eighteen patients diagnosed as OSAHS by overnight polysomnography (PSG) were studied retrospectively. The severity of OSAHS in REM versus NREM sleep was compared. Then the patients were divided into two groups: AHI(REM) > or = AHI(NREM) group, and AHI(REM) < AHI(NREM) group, and the two groups were compared in age, sex, BMI, AHI, AHI supine, oxygen desaturation and duration of apnea. RESULT: There was no significant difference between AHI(REM) and AHI(NREM). AHI(REM)/AHI(NREM) was 1.0 +/- 0.5. A higher AHI NREM than AHI REM group was found in 55.9% of the 118 patients with OSAHS. There were no differences between the clinical and PSG founding of two groups. CONCLUSION The severity of OSAHS is not greatly influenced by sleep stage.

Objective:To evaluate the optimized effect of erector spinae plane block (ESPB) combined with general anesthesia when used for the patients undergoing laparoscopic pancreaticoduodenectomy.Methods:Sixty-eight American Society of Anesthesiologists physical status Ⅱ orⅢ patients of both sexes, aged 18-64 yr, with body mass index of 18-24 kg/m 2, scheduled for elective laparoscopic pancreaticoduodenectomy, were divided into general anesthesia group (group G, n=34) and ESPB combined with general anesthesia group (group EG, n=34) using a random number table method.In group E, ultrasound-guided ESPB was performed before induction with general anesthesia, and 0.375% ropivacaine 20 ml was injected into both sides.Total intravenous anesthesia was applied in both groups.Patient-controlled intravenous analgesia (PCIA) with sufentanil 1.5 g/kg in 100 ml of normal saline was performed after surgery.The PCIA pump was set up to deliver a 2 ml bolus dose with a 5-min lockout interval and background infusion at 3 ml/h.Analgesia was performed until 24 h after operation, and the visual analogue scale score at rest was maintained at ≤4.Sufentanil 0.1 g/kg was intravenously injected as rescue analgesic when visual analogue scale score >4.The extubation time and occurrence of intraoperative cardiovascular events were recorded.The amount of sufentanil used during operation and within 24 h after operation was recorded.The time to first pressing the analgesia pump after operation and effective pressing times of PCA within 24 h after operation were recorded.Time to first flatus, first ambulation time and length of postoperative hospital stay were recorded.The development of postoperative adverse reactions such as nausea and vomiting, irritability and respiratory depression within 24 h after operation was recorded. Results:Compared with group G, the incidence of intraoperative hypertension and tachycardia was significantly decreased, the extubation time was shortened, the consumption of sufentanil during operation and within 24 h after operation was reduced, the time to first pressing the analgesia pump was prolonged, the effective pressing times of PCA within 24 h after operation were reduced, time to first flatus, first ambulation time and length of postoperative hospital stay were shortened, and the incidence of nausea and vomiting, irritability and respiratory depression within 24 h after operation was decreased in group EG ( P<0.05). Conclusion:The combination of ESPB and general anesthesia is helpful in achieving an anesthesia mode of lower opioid consumption and more helpful for inhibition of postoperative pain responses and for early postoperative recovery than general anesthesia alone when used for the patients undergoing laparoscopic pancreaticoduodenectomy.
