Objective To probe into the health expenditure and medical expenditure of Chinese people in rural and urban areas as objective references to improve the funding strategy of China's health care system. Methods Statistics annuals from 1999 to 2007 were called into play to analyze the status quo and trends of both expenditures. Results Definite proportion of funds raised for rural and urban health expenditure has grown by 200%;a distinct gap exists between the growth of per-capita medical expenditure in rural and urban residents (152. 86% in urban and 172. 77% in rural areas);medical expenditure maintains a steady growth along with rising per-capita income, yet the health consumption remains at a low level in general, especially among rural residents;since 2003, the income elasticity of the medical expenditure among rural residents keeps rising, narrowing the relative gap with urban residents. Conclusion It is recommended to push up the income of rural residents with greater resources, and to encourage the trend of marginal social medical expenditure. In addition, it is necessary to upgrade medical services, expand medical service coverage, and encourage reasonable consumption of medical services by both rural and urban residents;and to focus the role of cultural factors for greater awareness of the people in health investment and self-healthcare.

Objective To assess the cost-effectiveness of different modes of the cataract operation. Methods Firstly, 291 cases of cataract inpatient were divided four groups according to operation modes (PHACO and ECCE) and the prices of intraocular lens (IOL) (high and low). Then the comparisons of inpatient cost, operation cost, postoperative VA, postoperative VF and QOL were made among four groups respectively. Finally, the cost-effectiveness of different cataract operation modes was analyzed by comparing the differences of their costs with the VA, VF and QOL improvement. Results There were significant differences in the above indicators among four groups in general (P 0.01). The VA improvement amount of PHACO was higher than that of ECCE whereas the VF and QOL improvement lower than those of ECCE. The price of IOL had no effect on the improvement quantity of VA, VF and QOL. When one unit amount of VA improvement wanted, the cost-effectiveness ratio of inpatient with PHACO and low-price IOL was the lowest. When one unit amount of VF and QOL improvement wanted, the cost-effectiveness ratio of patients with ECCE and high-price IOL was the lowest. When one unit amount of VA and QOL improvement wanted, the operation cost-effectiveness ratio of ECCE with low-price IOL was the lowest. And when one unit amount of VF improvement wanted, the operation cost-effectiveness ratio of ECCE with high-price IOL was the lowest. Conclusions There were cost differences among different cataract operation modes. The cost of PHACO was higher than that of ECCE. The ECCE mode and price of IOL were the main determinants of cost. The effects of improving VA and QOL by ECCE were better and the effect of improving VA by PHACO was better than by ECCE. The homemade or low price IOL had no influence on the cataractous operative effectiveness, but could decrease the operation cost. ECCE with low-price IOL was the optimal cost-effectiveness strategy and operation mode, especially for poor rural and remote areas.

With the development of mass spectrometer,protein and peptide separation technology,and data analysis tools,the study on protein expression profiling of various tissues,organs or cells has been developed rapidly.The most basic purpose of proteomics is qualitative and quantitative identification of the whole protein in a cell or tissue.Therefore,how to make full use of mass spectrometry data has an important significance in proteome.Recently years,proteomics has begun to be applied in allograft rejection and discovery of rejection related biomarkers in organ transplantation.Through the extensive application of the technology in organ transplantation,graft rejection can be earlier detection and treatment.Therefore,the proteomics research have potential application prospects in the diagnosis of the organ transplant rejection.

Objective To observe the effect of comprehensive community intervention on cardiovascular risk factors in metabolic syndrome (MS) high risk group.Methods One hundred MS high risk patients were divided into intervention group (50 patients) and control group (50 patients) by random digits table.The intervention group accepted intervention on health education,health action,health habit and individual intervention on drug.The control group accepted natural intervention.The changes of waistcircumference,body mass index (BMI),blood pressure,fasting plasma glucose (FPG),lipids and serum uric acid (UA) were observed.Results In intervention group,the number of physical exercise,alimentary control and medication compliance increased 12％,16％ and 18％,and the number of smoking and alcohol drinking decreased 8％ and 6％.But in control group,the number of physical exercise,alimentary control,medication compliance increased 2％,0 and 4％,and the number of smoking and alcohol drinking decreased 4％ and 2％.There was significant difference (P＜ 0.05).Before treatment,the level of waist circumference,BMI,systolic pressure,diastolic pressure,FPG,total cholesterol,triacylglycerol,high density lipoprotein cholesterol (HDL-C),low density lipoprotein cholesterol (LDL-C) and UA between two groups had no significant difference (P ＞ 0.05).After 1 year's intervention,except HDL-C,the level of other index had significant difference between two groups (P ＜ 0.05 or ＜ 0.01).Conclusion Comprehensive community intervention can improve the status of obesity and dyslipidemia,and reduce blood pressure,plasma glucose and UA.

Underlying both the angles of norm-referenced test and criterion-referenced test, the qualitative or(and)quantitative analysis and evaluation were made on the tests of 10 medical academic courses in recent 4 years in reliabil-ity and item quality. From the angle of norm-referenced test, 67.4 percent of the items had good quality, 60 percent ofthe tests could differentiate the classes and 12.5 percent of the papers could differentiate the individual person accord-ing to their reliability. However, from the angle of criterion-referenced test, approximately 90 percent of the items werequalified; 87.5% had high reliability to determine whether an examinee passes the test or not.[

Objective To induce the immune tolerance of heart grafts with infusion of isogeneic bone marrow mesenchymal stem cells (BMSCs) in heart transplant rats.Method Donor Wistar rats and recipient F344 rats were randomly divided into 4 groups:acute rejection group (group A),Wistar rats as the donors and F344 rats as the recipients for heart transplantation; low dose cyclosporin A(CsA) group (group B),recipient F344 rats given low dose CsA; BMSCs group (group C),recipient F344 rats given isogeneic BMSCs; BMSC and low dose CsA group (group D),the recipient F344 rats given isogeneic BMSCs and low dose CsA.The serum cytokine levels were determined,and the donor heart pathological changes and survival were observed postoperatively.The relative level of Foxp3 mRNA expression in the spleen of the recipient F344 rats was also observed.Result The blood levels of interleukin-2 (IL-2) and interferon-γ(INF-γ) were significantly reduced,but IL-4 and IL-10 levels were increased (P＜0.05),and the survival time of donor heart was significantly prolonged in group D as compared with groups A,B and C (P＜0.05 for all).Heart pathological examination revealed a mild acute rejection in group D,moderate acute rejection in groups B and C group,and severe acute rejection in group A respectively.The expression of Foxp3 mRNA was significantly lower in group A than in groups B,C and D (P＜0.05 for all),and that in group D was significantly higher than in groups B and C (P＜0.05 for both),but there was no significant difference between between groups B and C (P＞0.05).Conclusion Intravenous administration of BMSCs can alleviate immunorejection in heterotopic rat heart transplantation.Low-dose CsA acts synergistically with BMSCs to significantly inhibit acute rejection after heart transplantation.The partial mechanisms involve the suppressive effect of BMSCs on the expression of Foxp3 mRNA and modulation on cytokine.

Viral hepatitis is a major liver disease caused by virus infection .Viral hepatitis is popular in China , mainly caused by hepatitis B and hepatitis C viruses .Experimental animal model is a necessary platform for the research on mechanism of viral infection and pathogenicity , for treatment and vaccine development .Up to date, a great progress in the development of viral hepatitis animal models has been achieved in spite of the most of findings are limited to hepatitis B and C.Here, we summarize the recent findings of viral hepatitis animal models , focusing on the tree shrew animal model and its modeling strategy .

Objective To establish an enterovirus 71(EV71) infection model of tree shrew primary renal cells.Methods Tree shrew primary renal cells were obtained by trypsin digestion.After subculture and purification,EV71 virus was used to infect these primary cells.The culture supernatant of these EV71-infected cells was collected for virus titer detection at 1,2,4,6 and 8 days post-infection.The cells were collected for detection of EV71 VP1 protein by Western blot assay.Furthermore,the expression and location of VP1 protein in the infected cells were detected by indirect immunofluorescence assay.Vero cells were taken as positive control to evaluate the infectivity of EV71 virus to tree shrew primary renal cells.Results Morphologically,the cultured cells were proved to be majorly consisted of the primary renal cells after subculture and purification.The obtained primary cells were infected by EV71 virus.The virus titer was up to 1.3×106 TCID 50/mL during 48-96 h post-infection,proving that EV71 virus infected and proliferated in the tree shrew primary renal cells.Western blot showed that the viral VP1 protein was detected from infected primary cells at 2 to 8 d post infection.VP1 protein was also observed in the cytoplasm at 2 to 6 d post infection by indirect immunofluorescence.Compared with Vero cells,the infectivity of EV71 virus to tree shrew primary renal cells and its proliferation were confirmed.Conclusions Based on the successful establishment of cell culture of tree shrew primary renal cells,the infectivity to the obtained cells and proliferation of EV71 virus in the cells are confirmed.The model of EV71 virus-infected tree shrew primary renal cells is initially established.

Objective To explore the proliferation characteristics of primary small intestinal epithelial cells of tree shrews and the characteristics of human rotavirus(RV) G1P[8] infection to these cells,and establish a model of tree shrew primary small intestinal epithelial cells infected with human rotavirus G1P[8].Methods The primary small intestinal epithelial cells were obtained by collagenase Ⅺ and dispase I digestion from tree shrew.After purification and identification,the obtained primary small intestinal epithelial cells were infected with RV.Then,culture supernatants of infected cells were collected every 12 hours after infection.Viral titer and viral load were subsequently determined.Western blot and indirect immunofluorescence observation were used to detect the expression of RV protein VP6 in the primary cells.The infectivity of RV to the tree shrew primary cells was finally evaluated.Results After purification and identification of primary epithelial cells from the tree shrew,high purity above 90% primary tree shrew small intestinal epithelial cells was obtained.These primary small intestinal epithelial cells could be infected with RV virus by comparing the virus infectivity to primary renal cells,HCT116 cells and MA104 cells.The virus titer reached to 2.0×105TCID 50/mL at 72 h after infection.Using Western blot and indirect immunofluorescence observation,the specific viral protein of VP6 was determined to be expressed in the tree shrew primary small intestinal epithelial cells,and were located in the cytoplasm from days 1 to 5.Conclusions The separation,purification and cultivation methods of tree shrew primary small intestinal epithelial cells are successful,and the tree shrew model of RV-infected the tree shrew primary small intestinal epithelial cells is successfully established.

OBJECTIVETo screen the hepatocyte proteins that interact with hepatitis B virus X protein (HBx).

METHODSThe recombinant plasmid pSos-HBx was constructed by inserting Sos-HBx fragment into the bait vector, and after sequence verification the plasmid was transformed into competent yeast cells. The expression and self-activation of Sos-HBx protein was detected in the yeast cells. The hepatocyte proteins interacting with the bait protein was screened with CytoTrap yeast two-hybrid technique.

RESULTSThe reconstructed plasmid harboring HBx gene expressed Sos-HBx protein in the yeast cells without self-activation of the protein. CytoTrap yeast two-hybrid system identified 6 hepatocyte proteins that interacted with HBx, including fibronectin 1, translationally controlled tumor protein, IQ motif and WD repeats 1, follistatin, orosomucoid 1, and disulfide isomerase family A member 3.

CONCLUSIONSix HBx-binding hepatocyte proteins have been identified using the CytoTrap yeast two-hybrid system, which provides clues for further investigation of the role of HBx protein in hepatitis and liver cancer.
