1.0Influence of EmbryoGlue on the implantation of embryo and pregnancy outcome in vitro fertilization-embryo transfer

ObjectiveTo study the influence of EmbryoGlue on the implantation of embryo and pregnancy outcome in vitro fertilization (IVF)-embryo transfer (ET).Methods From August 2010 to January 2011,243 infertile patients in Reproductive Medical Center of Tianjin Medical University General Hospital who underwent IVF or intracytoplasmic sperm injection (ICSI) were divided into two groups,including 129 cases used EmbryoGlue as the embryo transfer medium in experimental group and 114 cases used G-2 as embryo transfer medium in control group.Pregnancy outcome were compared between two groups.Results (1) The female age,IVF/ICSI constituent ratio,previous failure cycles and infertile factors of patients did not show statistical difference between experimental group and control group (P >0.05 ).(2) The implantation rate of women in experimental group increased significantly compared with the control group [ 30.4% ( 85/280 ) vs.18.8% ( 48/255 ),P < 0.05 ] ; Clinical pregnancy rate increased significantly compared with the control group [48.8% (63/129) vs.34.2% (39/114),P <0.05] ; Multiple pregnancy rate increased significantly compared with the control group [ 34.9% (22/63) vs.20.5%(8/39),P < 0.05 ] ; Ectopic pregnancy rate decreased significantly compared with the control group [ 4.8%(3/63) vs.17.9% ( 7/39 ),P < 0.05 ].ConclusionEmbryoGlue can facilitate embryo implantation in IVF-ET and reduce the occurrence of ectopic pregnancy.
2.Clinical Observation of External Application of Wentong Paste Plus Local Block Therapy for Treatment of Stenosing Tendovaginitis at Radial Styloid

Objective To observe clinical curative effect of the external application of Wentong Paste plus local block therapy for the treatment of stenosing tendovaginitis at radial styloid. Methods A total of 108 cases of patients with stenosing tendovaginitis at the radial styloid were randomly divided into local block therapy group, combination group and Wentong Paste group, 36 cases in each group. The local block therapy group was given injection of blocking liquid into the tendon sheath, combination group was given external application of Wentong Paste(mainly composed of Radix Angelicae Sinensis, Fructus Psoraleae, Radix Angelicae Pubescentis, and Rhizoma et Radix Notopterygii) plus local block therapy, and Wentong Paste group was only given external application of Wentong Paste. Before treatment and after treatment for one and 2 week(s), visual analogue scale (VAS) scores of pain and range of motion (ROM) of wrist joint were observed for the evaluation of clinical effectiveness and safety. The patients were followed up for 3 months for the assay of recurrence rate. Results (1) After treatment for one week, the total effective rate of local block therapy group, combination group and Wentong Paste group was 77.8%, 100.0%, 72.2%respectively, and the total effective rate after treatment for 2weeks was 86.1%, 100.0%, 86.1% respectively. The combination group had better clinical efficacy than local block therapy group and Wentong Paste group (P < 0.05). (2) After treatment for one and 2 week (s), VAS scores of the 3 groups were obviously increased while ROM was much increased (P < 0.05 compared with those before treatment). The combination group had the lowest VAS scores and highest ROM(P < 0.05).(3) No adverse reaction was found in the 3 groups during the treatment. (4) The 3 month follow-up results showed that the recurrence rate of the three groups was 30.61%, 10.20%, 20.41% respectively, and the combination group had the lower rate than local block therapy group and Wentong Paste group(P<0.05). Conclusion The external application of Wentong Paste plus local block therapy is effective for the treatment of stenosing tendovaginitis at radial styloid by relieving pain, increasing the ROM, and reducing the recurrence rate. The one-week efficacy of Wentong Paste alone was inferior to that of local block therapy alone, but their two-week efficacy showed no obvious difference. Moreover, Wentong Paste has an obvious advantage in reducing the recurrence rate.
3.CT quantitative study of coal miner's pneumoconiosis.

OBJECTIVETo study the value of CT quantitativeness in the diagnosis of coal miner's pneumoconiosis.
METHODS104 cases were examined by HRCT scan at top of aortic arc, carina of trachea, 3 cm below the bifurcation of bronchi, among them there were 87 patients with different stages of coal miner's pneumoconiosis, 17 cases of normal males as the control group. All images were determined by CT density histogram at specific region (- 1,024-0 HU). Calculated the percentage of each pixel included a varying number of CT value, and the ratio of density values in the specific region.
RESULTSThe ratio of density values in the region of -983 (-) -778 HU was 87.31% in normal control group, and 80.51%, 75.27% and 72.99% respectively in the I, II, III stages of coal miner's pneumoconiosis. There were statistically significant differences among the groups (P < 0.01).
CONCLUSIONCT quantitative histogram information was able to observe the fibrosis and its degree of coal miner's pneumoconiosis. It has a good diagnostic value for its reliability and objectiveness.

4.The research status and progress of ocular manifestations related to acute coronary syndromes

Acute coronary syndromes (ACS) are clinical syndromes caused by the instability or rupture of coronary atheromatous plaques. The development and treatment of ACS are closely related to some ocular manifestations. Hypertensive retinopathy, retinal arteriovenous occlusion, diabetes retinopathy, and age-related macular degeneration are associated with a high risk of ACS. Retinal vascular occlusion may be a potential postoperative complication of the percutaneous coronary intervention for ACS. Ocular parameters such as the retinal vascular diameter and density, the blood flow velocity of the ophthalmic artery, and the choroidal thickness are expected to be markers for assessing or predicting the risk of ACS, which are of important value for the prevention of cardiovascular events such as ACS.
5.Role of different nicotinic acetylcholine receptor subtypes in skeletal muscles in sepsis-related nerve-muscle dysfunction in septic rats: neuroelectrophysiology

Objective To evaluate the role of different nicotinic acetylcholine receptor ( nAChR) subtypes in skeletal muscles in sepsis-related nerve-muscle dysfunction in septic rats by neuroelectrophysiol-ogy. Methods SPF adult male Sprague-Dawley rats, weighing 200-220 g, aged 2-3 months, were stud-ied. Sepsis was induced by cecal ligation and puncture in chloral hydrate-anesthetized rats. Twelve portions of sciatic nerve-tibialis anterior muscles taken at day 3 after successful establishment of the sepsis modelserved as group S. Another 12 portions of sciatic nerve-tibialis anterior muscles of normal rats served as group C. The endplate potential ( EPP ) was monitored by intracellular microelectrode recording. Six por-tions of nerve-muscle samples were incubated with the adult nAChR (ε-nAChR ) specific antagonist wa-glerin-1 at a concentration of 1 μmol∕L, the fetal nAChR (γ-nAChR) specific antagonist αA-OIVA at a concentration of 250 nmol∕L and neuronal nAChR (α7-nAChR) antagonist methyllycaconitine ( MLA) at a final concentration of 6 nmol∕L. The amplitude of EPP was recorded at 15 min of incubation, and the differ-ence in the amplitude of EPP (ΔEPP) and changing rate of EPP (ΔEPP%) were calculated. Another 6 portions of samples were incubated for 15 min with the Kreb mixture of waglerin-1 and 250 nmol∕LαA-OIVA at a final concentration of 1 μmol∕L, the Kreb mixture of waglerin-1 and 6 nmol∕L MLA at a final concen-tration of 1 μmol∕L, and the Kreb mixture of αA-OIVA and 6 nmol∕L MLA at a final concentration of 250 nmol∕L, and then the amplitude of EPP was recorded. Washout was performed with antagonist free Kreb so-lution for 15 min between treatments with different blockers. Results Compared with group C, ΔEPP andΔEPP% were significantly decreased after antagonizing ε-nAChR, ΔEPP% was decreased after antagoni-zing γ-nAChR, ΔEPP and ΔEPP% were increased after antagonizing α7-nAChR, the amplitude of EPP was increased after antagonizingε-nAChR+γ-nAChR andε-nAChR+α7-nAChR, and the amplitude of EPP was decreased after antagonizing γ-nAChR+α7-nAChR (P<0. 05). ΔEPP and ΔEPP% were significantly higher after antagonizing α7-nAChR than after antagonizingγ-nAChR in group S ( P<0. 05) . The EPP was significantly lower after antagonizingε-nAChR+α7-nAChR than after antagonizingε-nAChR+γ-nAChR ( P<0. 05) . Conclusion Weakened function of ε-nAChR and enhanced function of α7-nAChR in the mem-brane of the end-plate in skeletal muscle are the main reasons for sepsis-related nerve-muscle dysfunction in septic rats, and the role is comparable between them.
6.iPLA2 β prevents high glucose -induced injury of human renal tubular epithelial cells via regulating ferroptosis

Objective :
To investigate the expression of Ca2 + -independent phospholipase A2 β (iPLA2 β) in human renal tubular epithelial cells (HK-2) induced by high glucose(HG) ,the relationship between iPLA2 β and ferroptosis and the protective mechanism of HG treated HK-2 cells.
Methods:
The HK-2 cells were treated with 30 mmol /L glucose,the overexpression model was constructed by transfection of iPLA2 β plasmid.Ferrostatin-1 ( Fer1) (an inhibitor of ferroptosis) and erastin (an activator of ferroptosis) were used as controls.After 36 hours of intervention,the kit detected the levels of superoxide (SOD) ,malonaldehyde(MDA) and iron in HK-2 cells.DCF immunofluorescence was used to detect intracellular reactive oxygen species ( ROS) .The expression of ACSL4, GPX4,LPCAT3,TFR1 in HK-2 cells were measured by Western blot.
Results :
The expression of iPLA2 β downregulated in HG-induced injury of HK-2 cells.The levels of ROS and MDA in HK-2 cells increased,while the levels of GSH and SOD decreased.The expression of ACSL4,LPCAT3 and TFR1 decreased,and the expression of GPX4 increased in HK-2 cells.However,these indexes were improved after Fer-1 intervention.iPLA2 β overexpression could reduce the injury of HK-2 cells via attenuation of KIM-1. Further research revealed that iPLA2 β overexpression inhibited oxidative stress and ferroptosis in HK-2 cells injury induced by high glucose.Meanwhile,
the improvement effect of iPLA2 β on HG-induced HK-2 cells damage could be eliminated by erastin.
Conclusion
iPLA2 β prevents HG-induced injury of HK-2 cells via regulating ferroptosis.
7.Value of SpyGlass single-operator choledochoscopy system in the diagnosis and treatment of patients with biliary tract diseases

Objective To investigate the value of SpyGlass single-operator choledochoscopy system in the diagnosis and treatment of patients with unexplained biliary stricture, complex bile duct stones, or other biliary tract diseases. Methods A retrospective analysis was performed for the clinical data of the patients with biliary tract diseases who were diagnosed and treated with SpyGlass in The Second Affiliated Hospital of Nanjing Medical University from December 2017 to June 2020. For the patients with biliary stricture, the biliary lesions were fully visualized under the guidance of SpyGlass, and SpyBite biopsy was performed if necessary; the patients with bile duct stones were treated with SpyGlass-guided direct-view laser lithotripsy; for the patients with gallbladder disease, the cystic duct was superselected with the assistance of SpyGlass. The SpyGlass system was analyzed in terms of its sensitivity, specificity, and accuracy rate in diagnosis and treatment, lithotripsy success rate, stone clearance rate, procedure success rate, and incidence rate of complications. Results A total of 58 patients underwent SpyGlass procedure. SpyGlass was used to evaluate biliary stricture of unknown nature in 44 (76%) patients; SpyGlass visual impression had a diagnostic sensitivity of 92% (24/26), a specificity of 94% (17/18), and an accuracy of 93% (41/44), and SpyBite biopsy had a diagnostic sensitivity of 71% (15/21), a specificity of 92% (11/12), and an accuracy of 79% (26/33). SpyGlass was used for the treatment of bile duct stones in 8 patients (14%), with a lithotripsy success rate of 83% (5/6) and a stone clearance rate of 88% (7/8). A guide wire under the SpyGlass system was to superselect the cystic duct in 5 patients (9%), with a procedure success rate of 80% (4/5). In one patient (1%), SpyGlass was used to assist the removal of common bile duct stones after liver transplantation and the treatment of bile duct anastomotic stricture. A total of 5 patients (9%) experienced complications after surgery. Conclusion The SpyGlass choledochoscopy system is accurate, safe, and effective in the diagnosis and treatment of unexplained biliary stricture, complex bile duct stones, and other biliary tract diseases.
