1.Curative Effect of Methylprednisolone Combined with Conventional Treatment on Severe Hand-foot-mouth Disease in Children

Objective:To explore the clinical effect of glucocorticoid methylprednisolone combined with the conventional treatment on severe hand-foot-mouth disease ( HFMD) in children. Methods:One hundred and twenty children with severe HFMD from our hos-pital were randomly divided into the hormone group and non-hormone group with 60 cases each. The non-hormone group was treated with western routine therapy, and the hormone group was treated with glucocorticoid methylprednisolone 2mg·kg-1 ·d-1 additionally. The changes in such symptoms as fever, rash, surprised and chatter, drowsiness and shock were recorded in all cases. The disappear-ance time of febrile, shock and herpes and hospital stay were compared between the two groups. Results:The clinical effect of the hor-mone group was better than the non-hormone group;it could shorten the disappearance time of febrile, shock, herpes and pharyngeal buccal redness (P<0. 05), while showed no effect on the hospital stay. Conclusion:Glucocorticoid combined with the routine thera-py in the treatment of severe HFMD in infants exhibits better clinical effect.
2.Establishment of a rat model of bone cancer pain induced by intra-tibia inoculation of Walker 256 mammary gland carcinoma cells

Objective To establish a rat model of bone cancer pain by intra-tibia inoculation of Walker 256 mammary gland carcinoma cells. Methods Sixty female Wistar rots weighing 180-200 g were randomly divided into 4 groups (a=15each):groupⅠ normal control; group Ⅱsham operation; group Ⅲtumor cell inoculation + normal saline (NS) and group Ⅳtumor cell inoculation + flurbiprofen. NS 0.2 nd and flurbiprofen 10 mg/kg in 0.2 ml were injected IV at 2 h before determination of pain threshold on 14 and 17 d after inoculation oftumor cells in groupⅢand Ⅳ respectively. On day 0, 4, 7, 10, 14, 17 and 21 after inoculation pain threshold was measured after determination of body weight. X-ray examination of the tibia was performed on day 14 after inoculation. The animals were killed on day 21 after inoculation for microscopic examination of the inoculated tibia. Results The animals started losing weight and the threshold to yon Frey hair stimulation was decreased from dhy 10 after inoculation in group Ⅲand Ⅳ. X-ray examination showed destruction of bone and microscopic examination showed tumor growing in tibia. Flurbiprofen significantly decreased mechanical hyperalgesia in group Ⅳ. There was no significant difference in paw withdrawal latoney to radiant heat among the 4 groups. Conclusion A model of bone cancer pain can be made by inoculation of Walker 256 mammary gland carcinoma cells into tibia characterized by mechanical hyperalgesia.
3.Exploration on the coaching of periodontal clinical practice for undergraduate students of stomatology

In order to improve the result of clinical practice for undergraduate students of sto-matology, the standardized training program was carried out before clinical practice by the teaching and researching Office of Periodontology of Affiliated Stomatological Hospital of Chongqing Medical University, in this program, based on our own realities, the structure and content of teaching was designed to guide the transformation of students to doctors, collective teaching and personalized teach-ing was combined to reform the conventional model of teaching and various student-oriented teaching methods were explored, including mimic forum of disciplines, intractable case discussion, clinical practice summary, course completion test and so on, thus promoting the result of clinical practice. Be-sides, the construction of teaching staff was strengthened, and the evaluation and management system was established to ensure the teaching quality, thus summarizing a set of efficient and effective clinical teaching methods presented in this paper.
4.Treatment and economic burden of urine incontinence in the rural elderly

ObjectiveTo explore the treatment and economic burden of urine incontinence (UI) in the rural elderly,and provide evidences for preventive intervention.MethodsA cluster sampling was carried out in 743 people (aged 60 years and over) from 5 villages dominated by Yanliu and Dafengshang health centers,Jixian County,Tianjin from 2007 to 2008.The information about general state of health,treatment and economic burden of disease were analyzed. Results The prevalence of UI was 33.4% (248 cases) among the 743 elderly people.Only 12 patients (4.8%) with UI ever visited community health-care service centers or hospitals for diagnosis and treatment,among whom only 1 case visited hospital within 1 year,8 cases during 1 to 5 years,and 3 cases over 5 years.And the method of treatment was all drug therapy.Among 248 cases with UI,there were 227 cases (91.5%) without any treatment,9 cases (3.6%) buying medicines by themselves.The direct cost for treatment of UI was 80-12000 yuan RMB,including 2 cases below 100 yuan RMB,2 cases 100-1000 yuan RMB,7 cases 1001-5000 yuan RMB and 1 case over 10000 yuan RMB. Conclusions Prevalence of UI is high while hospital-visiting rate is very low,and the financial burden of disease is serious,thus intervention measures for UI should be taken for the elderly people in rural areas of Jixian County,Tianjin.
5.Expression of pituitary glucocorticoid receptor and 11β-hydroxysteroid dehydrogenase 1 in obese diabetic rats

Diabetic rat model was induced by high fat diet combined with streptozotocin (STZ). After the model was established, blood samples were taken from jugular veins to examine plasma adrenocorticotropic hormone (ACTH) and corticosterone, and hypothalamus and pituitary were removed for real-time PCR. There were no significant differences in basal plasma ACTH and corticosterone level among control, obese, and obese diabetic rats (P=0.07). The corticosterone rhythm in obese and obese diabetic rats was impaired. Hypothalamus glucocorticoid receptors (GR) mRNA expressions yielded similar results in the groups, but 11β-HSD1 mRNA expression in obese diabetic rats was up-regulated ( vs control rats, P<0.05 ). The expressions of GR and 11β-HSD1 in pituitary of obese diabetic and obese rats were significantly down-regulated (both P<0.05). In the obese diabetic rats, the impaired glucocorticoid negative feedback was partly due to down-regulation of 11 β-HSD1 and GR expressions in pituitary.
6.The effect of psychological intervention on level of hope and quality of life in the breast cancer patients

Objective To investigate the effect of psychological intervention on level of hope and quality of life in the breast cancer patients.Methods Ninety five breast cancer patients were randomized into intervention group and control group for an eight-week observation.Herth hope index(HHI)and Quality of life core questionnaire for European tumor patients(QLQ-C30)were used to observe the effect of psychological intervention on level of hope and quality of life in the breast cancer patients.Results Hope scores in total and in three subcategories were higher in intervention group((40.43±4.25),(13.28±1.61),(13.62±1.70),(13.53±1.52))than that in control group((37.82±4.49),(12.29±1.89),(12.90±1.67),(12.63±1.51))with statistically significant differences(P<0.05).Scores for general quality of life,body function and role function were higher in intervention group((82.27±15.11),(75.89±12.24),(72.34±22.86))than that in control group((65.82±14.16),(68.30±16.42),(53.06±27.36))with statistically significant differences(P<0.05).Scores for fatigue,pain,insomnia and loss of appetite were lower in intervention group((39.48±14.62),(25.18±16.98),(29.79±24.31),(18.44±21.77))than that in control group((49.43±17.58),(36.74±19.54),(46.26±28.72),(40.82±22.84))with statistically significant differences(P<0.01).Conclusion-Psychological intervention could improve the level of hope and quality of life in the breast cancer patients.
7.Relationship of chronic diseases and the history of gestation and delivery with urinary incontinence among elderly people of rural areas in Jixian county, Tianjin

Objective To explore the relationship of chronic diseases and the history of gestation and delivery with urinary incontinence (UI) among elderly people of rural areas, so as to lay a scientific foundation for formulating strategies of prevention and treatment for UI. Methods A cross-sectional study was carried out in two towns of Jixian county, Tianjin. A total of 743 people aged 60 years and over were selected with cluster sampling, and all information were collected with a standardized structured questionnaire by face-to-face interview. The relationships of chronic diseases and the history of gestation and delivery with UI were analyzed with univariate analysis and multivariate Logistic regression method. Results Prevalence of UI was 33. 4% among the people aged 60 years and over in two towns of Jixian, and it was higher in females than in males (43. 2% vs. 22.8%, χ~2= 34.70, P<0.0001). Logistic regression analysis revealed that the chronic respiratory diseases, prostate disease, neurological diseases or motor system diseases were all associated with UI in men, and the elderly men who suffered from prostate disease and neurological diseases had the highest risk of UI (OR=11. 47, OR=11. 76). Constipation, chronic respiratory diseases, diabetes mellitus and motor system diseases were all associated with UI in women, and the elderly women with chronic respiratory diseases had the highest risk of UI (OR = 4. 84). Elderly women who had more gravidity (OR = 1. 03), more parity (OR= 1.02), perineal laceration (OR = 1.72) and wound infection (OR= 1. 65) during delivery had higher prevalence of UI. Conclusions Prevalence of UI is higher among the elderly people in rural areas of Jixian county, and several chronic diseases and gestation and delivery history in women are all associated with UI in the elderly. For the prevention of UI, the elderly should actively look for treating their chronic diseases and the medical workers should strengthen health education or health care for pregnant women.

ResultsPrevalence of UI was 33.4% among the people aged 60 years and over in two townships, and it was increased with ageing.The elderly women had a higher prevalence of UI than the elderly man (43.2% vs. 22.8%, P<0.0001). The prevalences of UI in groups of 60-, 65-, 70-, 75-, 80-, 85-95 years old were 28.6%,32.1%,34.1%,35.5%,47.8%and 30.0%, respectively, and increased with ageing (X2for trend=2.19,P=0.029). Mixed UI (MUI) was the most common in different types of UI, and the constitution ratios of stress UI (SUI), urge UI (UUI), MUI and other type of UI were 27.4%, 1.2%, 60.9%,10.5%, respectively. The prevalence of MUI was the highest among the four types of UI in each age group. In all age groups, the prevalences of SUI and UUI were the highest in 60~ age group, and the prevalence of MUI was the highest in 80 ~ 95 age group. ConclusionsPrevalence of UI is high among the elderly people in rural areas of Jixian County, Tianjin.
9.Primary Study of the Structure and Antigenic Epitopes of Antigen Pf332

Objective To understand the primary structure and potential antigenic epitopes of antigen Pf332(Ag332) of P.falciparum iso late FCC1/HN.Methods Based on the published Pf332 gene sequence , nine pairs of primers were designed for the PCR amplification of the Pf332 gen e fragments from genomic DNA of P.falciparum isolate FCC1/HN. The amplified gene fragments were subcloned into pMD-18T vectors and sequenced. The sequences were aligned using DNAstar software to obtain the full-length sequence of the gene Pf332. The primary structure and sequence homology of Ag332 were analyzed by SAPS, Tmpred, SingalP and Blastn programs. Three fragments, R0, R1 and R2, cor responding to nt#9595-10083, nt#10339-10767 and nt#10855-11247 of Pf332 gene were subcloned into the eukaryotic expression vector pcDNA3-S separately. The Balb/c mice were immunized with pcDNA3-S-R0, pcDNA3-S-R1 and pcDNA3- S-R2 separately, and the expressions of the recombinant proteins were detected by immunohistochemistry assay. The protective immune responses elicited by DNA I mmunization were analyzed by ELISA and parasite growth inhibition tests in vitro .Results Nine Pf332 gene fragments were specifically amplif ied, subcloned into pMD-18T vectors and sequenced. Pf332 gene of the P.falci parum isolate FCC1/HN was 16,377 bp in length, encoding a protein of 5,458 ami no acids, about 615.28kDa. The Ag332 contains 17 regions of highly degenerated Glu-rich repeats, with 30.18% Glu in total amino acids of Ag332. Ag332 of P.falciparum isolate FCC1/HN and 3D7 exhibited 94.55 % homology in amino acid residues. The results of immunohischemistry assay showed that R0, R1 and R2 were expressed in mice muscle tissue. The amount of IgG antibody of the groups immu nized with pcDNA3-S-R0, pcDNA3-S-R1 and pcDNA3-S-R2 were higher than those of blank and pcDNA3 groups (P<0.05). The result of parasite growth inhibition test showed that the immunized sera at 1∶5 dilution of groups of pcDNA3-S-R0, pcDNA3-S-R1 and pcDNA3-S-R2 had an incomplete inhibitor y effect on P.falciparum growth. Conclusion The antigen Pf332 is an large protein containing highly degenerated Glu-rich repeats. Pf332 gene fragments, R1 and R2 encoding potent antigenic epitope repeats.
10.Studv on the mental health status for 260 patients with breast cancer

ObjectiveTo explore the mental health status of patients with breast cancer and social support on their influence. Methods260 cases of patient with breast cancer were investigated by using the questionnaire of demographic characteristics and symptom check list 90(SCL-90)and social support rating scale(SSRS). ResultsBreast cancer patients ' mental health condition was worse than normal healthy people and their mood changes were obviously observed. Breast cancer patients achieved good social support.However, the urban patients with breast cancer got better social support than the rural patients and the difference were statistically significant(t =7.91, P =0.000). SCL-90 total score and its factors such as forced symptoms, sensitive interpersonal relationship, depression, anxiety, hostile, terror, paranoid,psychoticism,were negatively related with the social support (r =-0.278,-0.259,-0.165,-0.294,-0.215,-0.177,-0.175,-0.167,-0.219,all P < 0.05).ConclusionThe social support influences breast cancer patients'mental health,therefore,the patients with breast cancer should be given a better social support in order to improve the psychological health level and improve recovery.
