1.UV Second Derivative Spectrophotometric Determination of Synthetic Decanoyl Acetaldehyde in Compounded Chinese Medicinal Prescriptions

UV second derivative spectrophotometry was used for the analysis of compounded Chinese medicinal prescription in order to eliminate interference from prescription base. Amplitude D and △A were taken as the quantitative informatious. Synthetic decanoyl acetaldehyde in compounded Chinese medicinal prescription were determined by two methods. The Correlation coefficicnts of the standard curve were found to be 0.9994 and 0.9996,the mean recovery to be 98.10% and 96 31%.
2.Root causes analysis of medical safety (adverse) events and counter measures research

Objective To analyze characters and causes of medical safety (adverse) events and to propose corresponding countermeasures.Methods 322 medical safety events in some tertiary hospitals were analyzed by using the root causes analysis method.Results Identifying errors,medical defects,medicine mistakes were the top 3 events possessed 16.77% (54/322),16.15% (52/322),13.98% (45/322);The top 3 departments were Neurology,General surgery,Oncology which possessed 9.63% (31/322),7.45% (24/322),7.14% (23/322).Medical staff with a title of middle and highly were involved in possessed 33.23% (107/322),25.78% (83/322),and nursing staff was the top one possessed 56.83% (183/322).Night working shift was the maximum occurrence time possessed 39.75% (128/322).The top 3 medical acts which more easier to trigger medical adverse events were incomprehensive nursing care,irregular operation of diagnosis and treatment,clinical supervision defect which possessed 17.39% (56/322),15.84% (51/322),15.22% (49/322).The slight consequence case was the top one possessed 47.20% (152/322).Resolving the dispute by consultation between hospital and patient was the main way possessed 82.26% (51/62).Conclusions More attentions should be paied to supervision of some certain department management and key sections,and improve the professional technical level,do preventive construction systems about medical safety (adverse) as well as emergency event handling construction mechanism by consummate supporting system and effective staff arrangement.
3.Clinical analysis of 78 cases of critical asphyxia neonatorum

Objective To investigate measures of preventing asphyxia neonatorum in order to reduce the rate of asphyxia neonatorum.Methods Clinical datas on a total of 78 cases of critical asphyxia neonatorum from the year 2003~2004 in Longgang Central Hospital were collected and analyzed.Related high risk factors and effective precaution measures of reducing the rate of asphyxia neonatorum were studied.Results The rate of asphyxia neonatorum from January 2003 to December 2004 was 4.22%,of which it was 1.70% of critical asphyxia neonatorum and 8% of treated deaths.The first seven related factors to critical asphyxia neonatorum of this group were:fetal distress(47%),premature delivery(49%),umbilical factors-torsion,prolapse,shortening(32%),breech presentation(26%),high risk PIH(14%),monstrosity(6%),placental abruption and placenta praevia(6%).Conclusion Effective measures of reducing critical asphyxia neonatorum are to strengthen the monitoring of pregnant women and their antenatal care so as to find and treat PIH early and correct breech presentation in time,try to reduce premature delivery,find early and properly treat fetal distress and oafs and prohibit illegal accouchement.
4.Effect of nateglinide or sequential treatment with metformin on glycemic stability in newly diagnosed type 2 diabetic patients

The effect of nateglinide or sequential treatment with metformin on glycemic stability in newly diagnosed type 2 diabetes was investigated. Thirty-four cases of newly diagnosed type 2 diabetes received nateglinide therapy, or sequential treatment with metformin according to fasting and postprandial blood glucose, and were classified into isolated nateglinide therapy group(n=14) and sequentially treated with metformin group(n=20). Glycemic stability, reflected by mean amplitude of glycemic excursions(MAGE) and HbA1C, was determined in all patients before and after therapy for three months. HbA1C and MAGE in two groups were all improved after treatment(P<0.05). The therapy of nateglinide alone or combined with metformin can significantly improve glycemic stability in newly diagnosed type 2 diabetes.
5.Combination of fetal cord blood coagulation factor Ⅷ activity assessment with gene sequencing in prenatal diagnosis of hemophilia A

Objective To evaluate the assessment of fetal cord blood coagulation factor Ⅷ activity ( Ⅷ: C) combined with gene sequencing in prenatal diagnosis of high-risk fetuses of hemophilia A(HA). Methods Percutaneous umbilical blood sampling was performed in 79 pregnant woman with high risk of HA and male fetuses at 20-35 weeks of gestation and maternal peripheral blood was also taken. The plasma F Ⅷ: C was detected by one-stage method, and the concentration of von Willebrand factor antigen(vWF: Ag) was determined by ELISA. Long distance polymerase chain reaction(LD-PCR) was used in 6 fetuses to detect the intron 22 inversion in F8 gene, and LD-PCR was also applied for mothers with fetal carriers of intron 22 inversion and gene sequencing for those mother with fetuses without intron 22 inversion. Results The maternal concentration of FⅧ: C of these 79 women was 52%-139% with an average of (99.60±28. 10)%. For the fetuses, 23 were below 10%with an average of (2. 64±1.92)%, eight of which were <1% and 15 between 1% and 10%. Among the 12 fetuses with the concentration of FⅧ :C at 10%-30%, averagely (19. 78±6.71)%, five were induced and the rest seven pregnancies and the other 44 pregnancies with fetal FⅧ :C over 30%,average (58. 60± 12.12)%, were continued. All of the above 51 fetuses were followed up after birth without any abnormalities till present. Genetic diagnosis was offered for 6 fetuses, whose F Ⅷ: C concentration < 10 % (three > 1% and the other three < 1% ), and intron 22 inversion was detected in two and no mutation was found among the rest four. Conclusions Fetal blood FⅧ: C and vWF: Ag concentrations determination may offer prenatal diagnosis of HA, and is accurate in detecting HA intron 22 inversion and its carriers when combined with LD-PCR.
6.The time of getting out of bed after interventional therapy of hepatocellular carcinoma:Meta analysis

Objective To evaluate the safety and comfort of postoperative patients with hepatocellular carcinoma after interventional therapy in the early stage and late stage, so as to determine the time to get out of bed after operation. Methods We searched PubMed, EMbase, Cochrane Library (2016 second), Web of Science, CBM, Wanfang Data and CNKI database, collect test/ambulation time of hepatocellular carcinoma after interventional therapy were searched from inception to September 10, 2016. Meta analysis was performed by 2 reviewers independently by screening literature, extracting data and evaluating the risk of bias in the study. RevMan 5.0 software was used to analyze the data. Results A total of 9 studies were included in the study, and 1164 patients were included. The Meta analysis results showed that:compared with 24 hours in bed after operation, 12 hours early ambulation method to reduce postoperative pain, postoperative [odds radio (OR)=0.07, 95% confidence interval (CI) 0.03-0.15, P <0.05] and dysuria (OR =0.18, 95% CI 0.10-0.33, P < 0.05), abdominal distension (OR =0.14, 95% CI 0.07-0.29, P<0.05), insomnia (OR=0.15, 95% CI 0.05-0.43, P<0.05). But in the local punctura, such as congestion (OR=0.99, 95%CI 0.45-2.18, P=0.98), hematoma (OR=0.90, 95% CI 0.38-2.13, P=0.80), the difference was not statistically significant. Conclusions The available evidence indicates that the early postoperative 12 hours after interventional therapy can effectively reduce the incidence of postoperative complications. To be included in the quantity and quality of the research, the conclusion still need to carry out more high-quality research to be verified.

Objective To investigate the clinical value of radiopaque markers method on gastrointestinal motility.Methods Using self-made markers,2 mm?2 mm,20 markers were put into a capsule.42 patients with functicnal dypepsia(FD),50 patients with chronic functional constipation(CFC)and 20 healthy controls were included in the study.Each subjects ingested a capsule together with standard food and plain abdominal films were made after 4 h,48 h and 72 h.Gastric emptying rate at the fourth hour,gastrointestinal emptying rate at 48 h and 72 h and emptying index at 72 h were calculated.Results 66.6%(28/42)FD patients had delayed gastric emptying rate.Gastrointestinal emptying rate at 48 h and 72 h were lower in CFC patients than in controls (?
8.Effect of Qizhu Oral Liquid on radiation injury in mice

AIM: To observe the protective effect of Qizhu Oral Liquid on radiation injury in mice. METHODS: The model was made in mice by X-ray radiation. The white blood cells(WBC), bone marrow nucleated cells (BMNC) were measured. The weight of thymus and spleen were measured. RESULTS: Qizhu Oral Liquid could increase WBC and BMNC remarkably. It could increase the weight of thymus and spleen of mice signifcantly. CONCLUSION: Qizhu Oral Liquid is effective in protecting the X-radiation injury mice.
9.Low-density lipoprotein immune complexes in patients with type 2 diabetes

This study was designed to observe the clinical value of low-density lipoprotein immune complexes (LDL-ICs) in patients with type 2 diabetes mellitus (T2DM). Fifty-four patients with type 2 diabetes + macroangiopathy (DM-MA group), 48 patients with type 2 diabetes (DM group), 45 patients with macroangiopathy ( MA group), and 30 normal controls ( NC group) were enrolled. LDL-ICs levels in the DM-MA group were significantly higher than those in the other three groups. The levels of LDL-ICs in the MA group were higher than those in the NC and DM groups. There was a significant difference in LDL-ICs between the DM and NC groups. LDL-ICs levels were positively related with body mass index (BMI), glycesylated hemoglobin Alc(HbAlc), blood pressure, total cholesterol, and triglyceride. There was no significant relationship between low-density lipsprotcin cholesterol and LDL-ICs. A significantly negative correlation was recorded between high-density lipoprotein cholesterol and LDL-ICs. Our study suggests that LDL-ICs, blood pressure, and HbAlc may be risk factors of macroangiopathy in type 2 diabetic patients.

Objective To explore the clinical signiifcance of combined detection of thyroid stimulating hormone (TSH) and free thyroxin (FT4) in dried blood spots in screening for congenital hypothyroidism (CH) in neonates. Methods The TSH and FT4 levels in dried blood spot were measured by time-resolved lfuorescence immunity in live born neonates from June to December 2013. If the screening was positive, the blood was drawn and the serum TSH and FT4 were measured and compared with the results from dried blood spots. Results In a total of 31 199 neonates screened, 12 cases were diagnosed with CH and the prevalence rate of CH was 1/2 600;4 cases were hyperthyropinemia and no pituitary CH was detected. There was no signiifcant difference between TSH or FT4 levels in dried blood spot and those in serum in neonates diagnosed with CH (P>0.05). Conclusions Combined detection of TSH and FT4 in dried blood spot can be used for neonatal screening of CH. It can be applied for early distinguishing CH from hyperthyropinemia, and also helpful for early diagnosis and treatment of central CH.
