Objective To summarize the experiences of tension-free method in inguinal hernia recurrence.Methods Using the PHS Ethicon Ine,42 patients with recurrent hernia were performed tension-free hemioptasty.Results The operation time was from 40 to 150 minutes.The patients could walk up within 1 d after the procedure.No incision infection was found.The eomplicatiom included 4 retention of urine.3 large serotal hematoma requiring surgical drainage.The follow-up period ranged from 6 months to 3 years,no recurrence and foreign body sensation in inguinal region were observed.Conclusion PHS call be anatomically adapted to repairing and augmenting the myopectineal orifice.It is suitable to the patients with recurrent inguinal hernia.

Objective To investigate the killing effect by berberine and cytosine deaminase enzyme suicide gene on rectum cancer cells in vitro.Methods Human rectum cancer HR-8348 cells were cultured,then transfected by recombinant adenovirus suicide gene the survivial percentage of which was examined by MTT method.Results Recon of pcDNA3.1(+)-CD was constructed successfully.The CD gene sequence analysis indicated th a+ recon pcDNA3.1(+)-CD contain intact gene.RT-PCR showed CD gene expressed stably in the HR-8348 cells,which is sensitive to 5-FC above the density of 100?g/ml.Elevated along with the medicine density with response time lengthening,its damaging effect enhanced.However for the density above 5-FC 250g/ml,the change of cell killing rate was not apparent.when by 0.1,0.3,3.0,30.0?m density berberine unites the 5-FC on rectum cancer of the intestines Respectively,the cell inhibitory action elevates along with density rises,but 3.0?m,30?m the inhibition change was not apparent.Under 250?g/ml of 5-FC,0.1,0.3,3.0,30.0?m of berberine united the 5-FC The HR-8348 cells growth suppression rate were:27.7%,42.4%,52.3%,56.3% respectively.Conclusion Inhibition function to human rectum cancer HR-8348 cells can be strengthened by berberine combined 5-FC.

Objective To compare the activation of microglia in vitro induced by oligomeric and fibrillar Aβ,and research the effects of Piper Kadsura Ohwi extracts on the microglial activation.Methods Microglia were divided randomly into 4 test groups,intervened by fibrillar Aβ25-35,fibrillar Aβ25-35 + Piper Kadsura Ohwi extracts,oligomeric Aβ25-35 and oligomeric Aβ25-35 + Piper Kadsura Ohwi extracts respectively.Also a blank contrast group was set without any intervention.48 hours later,activation of microglia was tested by immunocytochemistry,using the CD68 molecule as a specific marker of microglial activation and the incidences of active microglia in different groups were compared.Results Each of the 4 test groups had a higher positive incidence of CD68 expression than that of the blank contrast (5.1％ ) (P ＜ 0.05 ) ; positive incidence of fibrillar Aβ + Piper Kadsura Ohwi extracts group (52.1％) was lower than that of the fibrillar Aβ group (60.8％) (P＜0.05) ; positive incidence of oligomeric Aβ + Piper Kadsura Ohwi extracts group (67.0％) was not significantly different with that of the oligomeric Aβ group (71.2％),P=0.101.Conclusion Both fibrillar and oligomeric Aβ has the ability to activate the silent microglia.Piper Kadsura Ohwi extracts could inhibit the activation of microglia induced by fibrillar Aβ25-35,but didn't show significant effects on the activation induced by oligomeric Aβ25-35.

Dihydroartemisinin is an important derivative of artemisinin. We used dihydroartemisinin as the starting material, through esterification, amination and acylation, a series of novel piperazine-sulfonamide contained dihydroartemisinin derivatives were firstly synthesized and their chemical structures were confirmed by IR, 1H NMR, 13C NMR and HR-MS. X-diffraction was used to determine the final configuration of the compound 3c. And the in vitro anti-HeLa activities of compounds 3 were analyzed with CCK-8 method. The preliminary bioassay test shows that compound 3 showed the best inhibition activities against HeLa with IC50 values of 0.14 micromol x L(-1).

OBJECTIVE:To investigate the influences of Schwann cells-alginic acid sodium hydrogel transplantation on cellular apoptosis,Bcl-2 expression,and lower limb locomotor function in a rat model of spinal cord injury.METHODS:SD rats of clean grade were randomly divided into 4 groups:normal control,simple injury,Schwann cells,and Schwann cells-alginic acid sodium hydrogel.Spinal cord transaction model was established in the latter 3 groups.Gelatin sponge blocks containing Schwann cells suspension were transplanted into the Schwann cells group.Schwann cells-alginic acid sodium hydrogel was transplanted into Schwann cells-alginic acid sodium hydrogel group.No treatments were performed in the normal control and simple injury groups.At 12 hours,1,3,7,and 21 days after surgery,animals were assessed using Basso,Beattie and Bresnahan (BBB) locomotor rating scale and were sacrificed.The spinal cord-transected segments were taken to prepare paraffin sections for TUNEL and Bcl-2 staining to quantitate apoptotic cells and Bcl-2 cells in the injured spinal cord and to investigate their distributions.RESULTS:A small number of slightly stained Bcl-2 positive cells were observed in the normal control group.In the simple injury group,Bcl-2 immunoreactive cells peaked at 3 days after surgery,and the expression level was close to normal level at 14 days.Following Schwann cells-alginic acid sodium hydrogel transplantation,Bcl-2 immunoreactive cells in the spinal cord-transected segments were significantly increased till 7 days (P＜0.05) and remained this level for more than 14 days.In the simple injury group,apoptotic cells were most as compared with the remaining 3 groups,and peaked at 1 and 7 days following spinal cord injury,and they were mostly distributed in the white matter.BBB scores were significantly higher in the Schwann cells-alginic acid sodium hydrogel transplantation group than in the simple injury and Schwann cells groups (P＜0.05).CONCLUSION:Schwann cells-alginic acid sodium hydrogel transplantation could inhibit cellular apoptosis and enhance Bcl-2 expression in the spinal cord-transected segments,and thereby promote the recovery of locomotor function after spinal cord injury but did not reach normal levels.

Objectives To explore the impact of Aβ oligomer(Aβo) and fibrillar Aβ( Aβf) on the ethology of model rat and to explore whether or not piper kadsura ohwi(PKO) can ameliorate the ethological changes.Methods AD animal model was made by continuous intracerebroventricular infusion of Aβ through mini-osmotic pump.After surgery,rats of Aβo + PKO group and Aβf + PKO group received intraperitoneal injection of 10％PKO everyday,rats from Aβo + DMSO group and Aβf + DMSO group received intraperitoneal injection of 2.5％ DMSO.The treatment lasted 5 weeks.Morris water maze experiment began on the 31st day after surgery.Results ( 1 ) Acquisition trials:the escape latency was longer in Aβo group ( ( 89.40 ± 7.20 ) s ) than that in Aβf group ( (65.00 ± 10.89 ) s ) ; escape latency was shorter in Aβf + PKO group ( ( 34.00 ± 11.26 ) s) than that in Aβf group and Aβf + DMSO group( (60.6 ± 6.95 ) s) ; escape latency was shorter in Aβ3o + PKO group ( ( 65.33 ±8.89) s) than that in Aβo group and Aβo + DMSO group ( ( 85.60 ± 6.02) s).( 2 ) Robe trial:the times ( 3.00 ±0.71 ) and the proportion of time (0.23 ± 0.02 ) and path(0.23 ± 0.04) spent in the target quadrant in Aβo group was less than that in Aβf group(6.00 ± 1.58,0.25 ± 0.01,0.26 ± 0.03 ) ; the three parameters in Aβf + PKOgroup were more than those in Aβ3f group and Aβf+ DMSO group; the three parameters in Aβo + PKO group were more than those in Aβo group and Aβo + DMSO group.Conclusions Aβ oligomer had a more severe impact on the ethology of AD model rats than fibrillar Aβ did; piper kadsura ohwi may ameliorate the ethological changes of AD model rats.

BACKGROUND: Alveolar bone grafting is known to reduce nasal asymmetry by supporting a defective alar baseand a sunken nostril. However, there are no studies on details of changes in the upper lip with appropriatemeasurements.OBJECTIVE: To measure the change in the upper lip height and nostril sill after alveolar bone grafting.METHODS: Forty-six congenital unilateral cleft lip and alveolus (UCLA) patients (mean age 9.85 years). The patients underwent alveolar bone grafting with autogenous iliac bone grafts. The average bone graft volume was 2.98 cm3.Photographs of the frontal, lateral and basal views were obtained using standardized photographic techniques. The fixedposition points on the patients' faces were measured. Preoperative and postoperative upper lip height and projection, theproportion indexes of the nostril sill were compared.RESULTS AND CONCLUSION: The 6-month follow-up results showed that the height of the upper lip was increased,but no significant difference was found. The projection of the upper lip was more prominent, but there was not statisticallysignificant difference bbetween pre- and post-operation. The nostril sill was significantly elevated in three of four distanceitems. In conclusion, the height of the upper lip is elongated and the nostril sill is elevated after alveolar bone grafting inUCLA patients.

Objective To evaluate the short -term effect of tegafur combined with oxaliplatin in the treatment of advanced colorectal cancer and its impact on long -term prognosis .Methods 70 patients with advanced colorectal cancer were selected .The patients were randomly divided into tegafur group ( oxaliplatin plus tegafur ) and capecitabine group ( oxaliplatin combined with capecitabine ) according to the digital table ,35 cases in each group , The short-term efficacy,side effects,2-year survival rate and median survival time were compared between the two groups.Results The total effective rate of the tegafur group was 85.71％, which of the capecitabine group was 77.14％,but the difference was not statistically significant between the two groups (χ2 =0.850,P=0.356).After treatment,the levels of VEGF in the two groups were significantly lower than those before treatment ( t =21.694, 20.558,P=0.305,0.249).There was no statistically significant difference between the two groups (t =0.998, 1.242,P=0.281,0.307).The 1-year survival rate was 51.43％and the 2-year survival rate was 25.71％of the tegafur group,compared with 45.71％ and 17.14％ in the capecitabine group ,the differences were not statistically significant (χ2 =0.229,0.764,P=0.632,0.382).The median survival time of the tegafur group was 13.5 months, which of the capecitabine group was 13.0 months,there was no statistically significant difference ( Z=1.304,P=0.752).The incidence rate of hand -foot syndrome of the tegafur group was 5.71％,which was lower than 22.86％of the capecitabine group (χ2 =4.163,P=0.027).Conclusion The efficacy of tegafur combined with oxaliplatin in the treatment of advanced colorectal cancer is similar to capecitabine plus oxaliplatin , with a slightly lower complication rate.

Objective To evaluate diagnostic method and treatment strategy for free floating thrombus in carotid artery.Methods From Ju12016 to Oct 2017,7 patients with free floating thrombus in carotid artery was diagnosed at our department.The medical history,symptoms,diagnosis,treatment strategy and prognosis of those patients were analyzed retrospectively.Results Among 7 patients,4 were symptomatic;4 patients were concomitant with severe carotid artery stenosis and 3 with mild to moderate stenosis.3 received carotid endarterectomy and patch angioplasty.4 received carotid artery stenting with distal cerebral protection divice.There was no perioperative and 30-day stroke,myocardial infarction,death or hyperperfusion syndrome occurred.The 12-month follow up showed no restenosis,no free floating thrombus recurrence and no ischemic cerebrovascular event.Conclusion Free floating filling defect in carotid artery is a typical sign for unstable plaque.Both carotid endarterectomy and carotid artery stenting can be used for the treatment of free floating thrombus.

Objective To recognize the relationship between high cutantous artery branches and descending branch of lateral femoral circumflex artery artery,and to investigate the blood supply of anterolateral thigh flap in clinical.Methods Retrospective analysis 152 cases of anterolateral thigh flap from November,2003 to December,2016.It contains cutting with descending branchs in 99 cases,cutting with lateral branchs in 43 cases,the union of them in 8 cases and high cutantous artery branches in 2 cases.Results The flaps survival in 147 cases,cutting necrosis in 3 cases and partly necrosis in 2 cases.Descending and lateral branchs were both dominance neurovascular bundle of vastus lateralis muscle,independently or commonly dominate the skin of anterolateral thigh,occurrence rate was 33.5％.Conclusion The descending branch of lateral femoral circumflex artery include lateral branch,all of which are the neurovascular bundles,supply the anterolateral thigh muscle,and divid into some perforator branches crossing the musle to the anterolateral thigh skin subsequently,which constitute "the anatomical functional unit".High cutantous artery branche is the one of all of the branches,its origin lies hight and arise from lateral branch.
