[Objective] To sum up Mr. Li Xueming's clinical experience, avoiding later learners from losing their way, not only richening the science, but also benefiting the people. [Method] By sorting out his treatment experiences, such as applying Pathogenesis No.19 of Internal Classics,make correspond-ing summary on widely col ecting others good points, smartly using the ancient formulae with clear differentiation of signs, as wel as analysing the medica-tion and pharmacy.[Result] Mr. Li Xueming's clinical treatment originates fromInternal Classics, uses the method of Zhongjing, refers to Jingyue, inher-its from Yegui, differentiates Yin ang Yang, cold and hot, deficiency and excess, organs and viscera; his formulae are like formations, curing diseases with drugs used. [Conclusion] Doctor Li has deep TCM theory,rich experience, outstanding clinical effect, worthy inheritance and promotion.
2.Clinical Experience to Treat Threatened Abortion with Chen ’s Miscarriage Prevention Method

Objective] To summarize the clinical experience to treat threatened abortion with Zhejiang Chen Mushan Gynecology calming fetus. [Method] By analyzing the understanding of Chen’s Gynecology to etiology and pathogenesis of threaten abortion, to review typical cases and summarize its characteristic of treatment, to sum up the clinical experience of threatened abortion. [Result]The etiology of threaten abortion of Chen ’s Gynecology is due to fetus fire flaring up, qi and blood deficiency, spleen and kidney insufficiency, therefor causing fetus yuan qi not solid, fetus unease and/or threatened abortion. The paper introduces Chen’s miscarriage prevention and proposes to treat threatened abortion by focusing on tonifying qi and blood, strengthening kidney and spleen, clearing heat and cooling blood, integrating prevention with treatment, emphasizing in harmonizing in the treatment. [Conclusion] Chen’s treating method for threatened abortion by calming fetus has accumulated rich clinical experiences with strong characteristics, it is worth to inheritance and popularizing.

4.Influence of Simiao Pill Intervention on the Life Quality of Patients with Damp Heat and Blood Stasis Type of Chronic Pelvic Inflammatory Disease

Objective To investigate the influence of Simiao pill intervention on the life quality of patients with chronic pelvic inflammatory disease( CPID) . Methods Eighty-five cases of damp heat and blood stasis type of CPID were randomly divided into two groups:the treatment group( n=43) was treated with Simiao pill decoction combined with other Chinese herbs;the control group(n=42)was treated with Fuke Qianjin capsules.Three months after the treatment, clinical efficacy was evaluated and life quality was assessed by WHOQOL-BREF. Results The total effective rate was 90. 70% in the treatment group and 71.43% in the control group(P<0.05).Before the treatment, score of each dimension of WHOQOL-BREF in both groups was not significantly different(P>0.05).After the treatment, score of each dimension of WHOQOL-BREF in both groups were increased, and the total score was(104.21±6.67)in the treatment group and(89.60±7.31)in the control group(P<0.05). Conclusion Simiao pill decoction combined with other Chinese herbs can effectively treat damp heat and blood stasis type of CPID and improve the life quality of the patients.
5.Analysis on prenatal gene diagnosis in 49 cases carrying same type thalassemia

Objective To explore the effective means and important significance for preventing the born of neonatal patients with severe thalassemia.Methods Among the pregnant women and spouses receiving prenatal examination in our hospital from January 2013 to December 2015 were performed the thalassemia screening and gene diagnosis,49 couples carrying the same type thalassemia were conducted the prenatal amniotic fluid thalassemia gene diagnosis and follow up after prenatal diagnosis.Results In 49 couples carrying the same type thalassemia,the main gene mutation types of α-thalassemia detected by the gene diagnosis were --SEA/aα(50.0%),-α3.7/αa (36.5%) and-α4.2/αa (11.5%),which of β-thalassemia were CD17/N(42.0%),CD41-42/N (26.0%) and IVS-Ⅱ-654/N(22.0%).The results of prenatal diagnosis showed that there were 4 cases of HbH disease,2 cases of Bart's hydrops fetus,10 cases of severe β-thalassemia,19 a-thalassemia carriers,10 β-thalassemia carriers,1 case of co-inheritance of a-and β-thalassemia,and 3 health fetuses.The follow up results were consistent with those of prenatal diagnosis.Conclusion Conducting prenatal screening and diagnosis of thalassemia in pregnant women can effectively prevent the birth of neonatal patients with severe thalassemia.
6.Effect of Dachengqi Decoction with ultrasonic conductance on postoperative recovery of laparoscopic gastrectomy under general anesthesia

Objective:To explore the effect of Dachengqi Decoction on the postoperative recovery of laparoscopic gastrectomy under general anesthesia.Methods:From March 2015 to May 2018, 96 patients with gastric cancer undergoing laparoscopic gastrectomy under general anesthesia in our hospital were selected as the study objects and divided into two groups according to the random number table method. The control group received routine nursing. The observation group received the intervention of Dachengqi Decoction with ultrasonic conductivity on the basis of the control group. The recovery of gastrointestinal function, the levels of gas (serum gastrin), MTL (plasma motilin) and Quality of Recovery 40 (QoR40) were compared between the two groups.Results:The gastrointestinal function indexes of the observation group were better than those of the control group ( P<0.05). Compared with one day before operation, the levels of gas and MTL in the two groups were lower 24 hours after operation, but the levels of GAS and MTL in the observation group were (92.56±5.12), (258.58±5.59) ng/L, which were higher than (52.12±3.15), (185.63±4.12) ng/L in the control group ( t values were 46.608, 72.782, P<0.01). Compared with the pre intervention group, QoR40 scores of the two groups were improved after intervention, and the observation group was (198.56±5.58) points, which was higher than (152.12±2.63) points of the control group ( t value was 52.157, P<0.01). Conclusion:Dachengqi Decoction can promote the recovery of patients after laparoscopic gastrectomy under general anesthesia.
7.Protection of vagus nerve during the cervical vagal schwannoma resection and functional rehabilitation.

OBJECTIVE:
To explore the clinical anatomy of vagal schwannoma and the methods to preserve the continuity of vagus nerve during tumor resection and postoperative functional rehabilitation methods of the nerve.
METHOD:
To review 15 cases of vagal schwannoma from 2002.07 to 2011.08 treated in the Department of Otolaryngology, West China Hospital. The operative modality was to strip the tumor while keeping the Schwann membrane almost intact so that to protect the nerve fibers as much as possible. postoperative treatment included glucocorticoids, neurotrophic medication, as well as voice and swallowing rehabilitation.
RESULT:
Among 15 cases of schwannoma patients, 3 cases (3/15) experienced hoarseness of voice with the ipsilateral vocal cord located at the median position 2 years after operation. 2 cases(2/15) experienced no obvious hoarseness of voice with vocal cords slight vibration when pronouncing and reached normal vocal cord movement 3 months after operation, 10 cases were healed without vocal cord complications, 7 cases (7/15) experienced choking during drinking and the symptom was gradually eliminated 1 3 months thereafter.
CONCLUSION
To carefully discern and preserve the vagal fibers during the operation of vagal schwannoma could eliminate postoperative hoarseness and choking. Protection of superior laryngeal nerve should be also brought to the forefront because it could affect the quality of swallowing and speaking.

8.Salvage surgery of thyroid carcinoma in advanced stage with common carotid artery involvement.

OBJECTIVE:
To investigate the feasibility of salvage surgery of thyroid carcinoma in advanced stage with common carotid artery involvement.
METHODS:
Reviewed 4 cases of advanced-stage thyroid carcinoma treated in the Department of Otolaryngology, West China hospital from July 2006 to September 2009. CT revealed that the common carotid artery entrapped in the tumor without obvious anatomical space. 2 cases were proved to be low-differentiated squamous carcinoma pathologically and the rest 2 cases were papillary carcinoma. Total thyroidectomy, total laryngectomy and bilateral neck dissection were performed in 3 cases while in one case, total thyroidectomy, bilateral neck dissection and partial tracheal resection being performed. Adjacent prepared cervico-pectoral flap was used to reconstruct the defect of cervico-thoracic trachea. Rupture of common carotid artery in one case took place and was repaired by mere suture. One case of low differentiated squamous carcinoma was given postoperative chemotherapy and radiotherapy while the rest 3 cases given no adjuvant chemotherapy and radiotherapy. The follow-up time lasted from half a year to 2 years.
RESULTS:
Half a year postoperatively, the low-differentiated squamous carcinoma patient not receiving postoperative chemoradiotherapy suffered from lumber spinal metastasis, one year after operation he was still alive and half a year later lost follow-up, the other 3 patients were alive from half a year to 2 years postoperatively without any obvious clue of recurrence or remote metastasis.
CONCLUSION
Salvage surgery is still feasible in thyroid carcinoma in advanced stage with common carotid artery involvement. The larynx and trachea should be kept intact as possible but the reconstruction of the trachea defect is recommended to be less prior in the situation when carotid artery is involved in.

9.Determination of 1,8-TMP rhein and its pharmacokinetics in rat plasma by HPLC

Aim To develop a HPLC method for the determination of the concentration of 1,8-TMP rhein in rat plasma and study the pharmacokinetics of 1,8-TMP rhein in rat plasma after single dose i. v. administration of 1,8-TMP rhein (2, 4, 8 mg·kg - 1 ). Methods Emodin was used as an internal standard. Plasma sam-ples were extracted with methanol and analyzed by HPLC. The mobile phase was methanol - 0. 1% for-mic acid water (78 ∶ 22, V/ V), with a flow rate of 1. 0 mL·min - 1 and UV 275 nm as the detection wave-length. The plasma concentration of 1,8-TMP rhein in rats was determined by HPLC after single-dose intrave-nous injection in rats with 2,4 and 8 mg·kg - 1 of 1,8-TMP rhein, and the pharmacokinetic parameters were caclulated by DAS 2. 1. Results The result of cali-bration curve was linear over the range of 0. 05 ~ 10. 00 mg·L - 1 (r = 0. 996 2). The lower limit of quantifica-tion was 0. 05 mg · L - 1 . The intra-day and inter-day precision (RSD% ) were both lower than 6% , and the extraction recoveries were higher than 88% , respec-tively. The validated method was successfully applied to a pharmacokinetic study after i. v. administration of 1,8-TMP rhein in rats with a dose of 2,4 and 8 mg· kg - 1 . The T1 / 2 was (68. 35 ± 1. 36), (69. 32 ± 2. 1) and (69. 32 ± 2. 03) min, respectively. The AUC0 - t was ( 101. 03 ± 24. 90 ), ( 144. 79 ± 3. 29 ) and (231. 92 ± 19. 30 ) min · mg · L - 1 , respectively. Conclusion A simple and specific HPLC method for the analysis of 1,8-TMP rhein is successfully developed and applied to a pharmacokinetic study in rat plasma.
10.Chlorfenapyr poisoning:mechanisms,clinical presentations,and treatment strategies

BACKGROUND:Chlorfenapyr is used to kill insects that are resistant to organophosphorus insecticides.Chlorfenapyr poisoning has a high mortality rate and is difficult to treat.This article aims to review the mechanisms,clinical presentations,and treatment strategies for chlorfenapyr poisoning. DATA RESOURCES:We conducted a review of the literature using PubMed,Web of Science,and SpringerLink from their beginnings to the end of October 2023.The inclusion criteria were systematic reviews,clinical guidelines,retrospective studies,and case reports on chlorfenapyr poisoning that focused on its mechanisms,clinical presentations,and treatment strategies.The references in the included studies were also examined to identify additional sources. RESULTS:We included 57 studies in this review.Chlorfenapyr can be degraded into tralopyril,which is more toxic and reduces energy production by inhibiting the conversion of adenosine diphosphate to adenosine triphosphate.High fever and altered mental status are characteristic clinical presentations of chlorfenapyr poisoning.Once it occurs,respiratory failure occurs immediately,ultimately leading to cardiac arrest and death.Chlorfenapyr poisoning is difficult to treat,and there is no specific antidote. CONCLUSION:Chlorfenapyr is a new pyrrole pesticide.Although it has been identified as a moderately toxic pesticide by the World Health Organization(WHO),the mortality rate of poisoned patients is extremely high.There is no specific antidote for chlorfenapyr poisoning.Therefore,based on the literature review,future efforts to explore rapid and effective detoxification methods,reconstitute intracellular oxidative phosphorylation couplings,identify early biomarkers of chlorfenapyr poisoning,and block the conversion of chlorfenapyr to tralopyril may be helpful for emergency physicians in the diagnosis and treatment of this disease.
