Objective To probe into the treatment effect and the influencing factors of early goal directed therapy(ECDT)in patients with septic shock.Method One hundred and twenty-six patients with septic shock were assigned into the treatment group(n=62)and the control group(n=64).The treatment group was administrated with EGDT,while the control group used conventional therapy for cycle and capacity support. Then sequential organ failure assessment(SOFA)score,APACHE Ⅱ score before treatment and 28-dav-mortality were respectively recorded before the above-mentioned treatment and at 24 hours after the therapy in the two groups. The treatment group was again subdivided into two groups after 6 hours recovery according to the resuscitation results:the sufficiently resuscitated group(n=40)and insufficiently resuscitated group(n=22).Age,genders,correlated haemodynamic parameter,SOFA score,APACHE Ⅱ score and serum lactic acid concentration before resuscitation and 28 d mortality were compared between the two groups. Results There was no significant difference in SOFA score and APACHE Ⅱ score before resuscitation between the treatment group and the control group[SOFA:(12.26±4.37)vs.(12.54±5.21);APACHE Ⅱ:(21.26±6.03)vs.(21.64±6.80)].The two scores at 24 hours after resuscitation[SOFA:(9.18±3.63)vs.(10.62±4.27);APACHEⅡ:(14.92±3.81)vs.(18.21±4.25);P<0.05]and 28-day-mortality rate(48.39%vs.76.56%,P<0.05)of the treatment group were significantly lower than those in the control group. There was significant difference in the age, mean arterial pressure (MAP),APACHEⅡ score between the sufficiently restoring group and the insufficiental resuscitated group. The 28-day-mortality of the sufficiently restoring group was significantly lower than that in the insufficiently restoring group. Conclusions EGDT can improve the prognosis in patients with septic shock.Ages,MAP and APPCHE Ⅱ score before resuscitation can be the important factors of EGDT.

Objective The concentration of NE and TNF-α was detected for elucidating the change of them in sera and bronchoalveolar lavage fluid (BALF) of rabbits with acute lung injury interposed by XueBiJing therapy in underwater explosion. Methods Underwater explosion decive was applied to cause acute lung injury of rabbits. The 20 rabbits were randomly divided into two groups which were injury group and XueBiJing therapy group, respectively. The concentration of NE and TNF-α in sera and BALF were detected by ELISA method in 24 hours after bursting. Results The concentration of TNF-α in sera (353.30±166.86 ng/L) of rabbits in therapy group were significantly lower than those in injury group (552.30± 169.64 ng/L;t=2.645,P =0.016).The concentration of NE in sera (63.40 ± 36.09 ng/ml) were lower than that of rabbits in injury group (97.60 ± 36.20 ng/ml;t=2.116,P=0.049).At the same time the concentration of NE in BALF (102.10± 9.50 ng/ml) of rabbits in therapy group were significantly lower than those in injury group (136.70± 13.60 ng/ml;t=6.593,P=0.000).Conclusion The expression of TNF-α in sera and NE in sera and BALF in rabbits with acute lung injury interposed by XueBiJing in underwater explosion were lower than those of injury group.

Objective To investigate the awareness of osteoporosis and to evaluate the effect of related health education among Beijing community residents. Methods Total 1 000 patients, who visited in 4 community health service centers in Beijing from June to December 2011,were selected as study participants. The questionnaire survey on the awareness of osteoporosis was conducted in all participants, and the items of questionnaire included the risk factors, symptoms, prevalence of osteoporosis, the recommended daily calcium intake and risk of falls.Five hundred participants in 2 health service centers were designated as intervention group and 500 in another health service centers as control group. In intervention group the health education of osteoporosis was conducted every 2 to 3 months and the outpatient follow-up was provided every 2 months;the contents of health education included the clinical manifestation, prevention and control of osteoporosis, and the preveution of fall.In control group only the general outpatient consultation and referral were provided. The second questionnaire survey was conducted 2 years after intervention, the results were compared with baseline and the intervention effect was evaluated. Results Before intervention,58.0% (580/1 000) participants had exercise,43.1% (431/1 000) participants took calcium supplement and vitamin D,and 12.1% (121/1 000)took medication following doctor's advance. The rates of fall prevention and administration of calcium vitamin D in intervention group before intervention [27.8% (139/500) and 38.7% (194/500)] were lower than those in control group [33.8% (308/500) and 47.5% (237/500)].After two-years of intervention the adoption rate of health behavior in intervention group was improved markedly, the highest one was diet control [increased by 38.4% (192/500)] and the lowest one was medication [increased by 17.3% (87/500)].Compared to control group the diet control rate in intervention group was significantly higher [74.0% (370/500) vs.23.6% (108/457),x2 =261.887,P=0.000].In addition, unhealthy behaviors were also reduced:55.0% participants reduced caffeine consumption and 72.2% participants reduced sodium intake in intervention group. Conclusion The awareness of osteoporosis among Beijing community residents is not satisfactory, but intervention focusing on osteoporosis prevention education is effective to change their behavior.

OBJECTIVE:
To detect mutation of NDP gene in a pedigree affected with Norrie disease.
METHODS:
Sanger sequencing was used to analyze the NDP gene at Xp11.3. Prenatal diagnosis was performed on amniotic fluid sample after the causative gene was detected.
RESULTS:
Sanger sequencing has revealed a c.2T>C (p.M1T) missense mutation of the NDP gene in the proband and the fetus. The same variation was not found in ClinVar and HGMD database.
CONCLUSION
The c.2T>C mutation of the NDP gene probably underlies the Norrie disease in this pedigree.
