1.Survey on awareness of hyperuricemia among patients and medical personnel in Yuetan community in Beijing

Objective To survey the awareness of hyperuricemia in patients and medical personnel in Yuetan community.Methods By using random sampling method, 492 patients with hyperuricemia and 84 medical personnel in Yuetan community were selected for questionnaire survey.Results The rate of awareness of hyperuricemia in patients was 33.2%(1 472/4 428), and 95.5%(470/492)of them had the requirement of health education on hyperuricemia.The awareness rate of hyperuricemia in medical personnel was 73.0%(736/1 008), and 46.4%(39/84) of them thought health education on hyperuricemia as important or very important.Conclusion The awareness of hyperuricemia is insufficient in patients and medical personnel.Health education should be reinforced for community management on hyperuricemia.
2.Community management of asymptomatic hyperuricemic patients

Objective To evaluate the efficacy of community management of asymptomatic hyperuricemic patients.Methods Two hundred and six patients with asymptomatic hyperuricemia in Yuetan community of Beijing were randomly and equally divided into study group and control group.Patients in study group received 12-month standardized management, while those in control group were given conventional management.After 12 months of intervention, the clinical data and related information were analyzed and the effect of intervention was evaluated by questionnaire survey.Results After 12-month intervention, the rates of unhealthy lifestyle in intervention group were significantly lower, and the exercise time was significantly more than those in control group [drinking:frequently 8.7%(9/103)vs.11.7%(12/103),seldom 83.5% (86/103) vs.68.9% (71/103),χ2=7.005, P=0.030;eating animal offal: seldom 88.4%(91/103) vs.76.7%(79/103),frequently 1.9%(2/103) vs.8.7%(9/103),χ2=6.302, P=0.043;eating seafood: seldom 74.8%(77/103) vs.52.4%(54/103),frequently 4.8%(5/103) vs.16.5%(17/103),χ2=12.867, P=0.002;eating thickened gravy: seldom 78.6%(81/103)vs.53.4%(55/103),frequently 2.9%(3/103) vs.13.6%(14/103),χ2=16.334, P=0.000;exercise time: seldom 17.5%(18/103)vs.24.3%(25/103),1-3 h 26.2%(27/103) vs.38.8%(40/103),4-6 h 32.0% (33/103) vs.20.4%(21/103), ≥7 h 24.3%(25/103) vs.16.5%(17/103),χ2=7.852, P=0.049].The blood uric [(403±54) vs.(474±71) μmol/L], cholesterol [(4.6±0.8) vs.(4.9±1.0)mmol/L], triglyceride[(1.65± 0.54) vs.(1.86±0.86) mmol/L] and systolic pressure [(128±10) vs.(131±14) mmHg(1 mmHg=0.133 kPa)] in intervention group significantly lower than those in control group (t=7.984,P=0.000;t=2.803,P=0.006;t=2.130,P=0.034;t=2.059,P=0.041).The awareness rate of hyperuricemia-related knowledge in intervention group was significantly higher than that in control group [85.1% (964/1 133) vs.45.5%(516/1 133), χ2=390.959,P=0.000].Conclusion Standardized community management is effective for patients with asymptomatic hyperuricemia.
3.Value of ultrasound elastography versus transrectal prostatic biopsy in prostatic cancer detection

Objective To detect the impact of ultrasound elastography in diagnosis of prostatic cancer,and to evaluate its capability in differentiating benign from malignant lesions.Methods From June 2013 to February 2015, we selected 50 patients with different prostatic lesions,and suspicious for malignancy were included.All patients had a conventional B-mode ultrasound examination and color Doppler imaging,and then real time ultrasound elastography was performed in the same session.Finally,the results were compared to the histopathological results of those lesions. Results The addition of Strain ratio parameter for evaluating the elastography images showed the highest sensitivity of 74.2%,specificity of 73.7% and accuracy of 74.0% at a best cut off point of 5.5 between benign and malignant lesions.Conclusion Prostate US combined with elastography can be a helpful tool for guiding malignant lesions.Also it can help in targeting the biopsy site.

5.The prevalence rate of mild cognitive impairment and its related factors in community elderly in Hangzhou City

Objective To understand the prevalence rate of mild cognitive impairment (MCI) and its related factors in community elderly so as to provide evidence for the community intervention. Methods A cross-sectional study was performed in a random sampling of 925 elderly people. Mini-mental state examination (MMSE), activities of daily living (ADL) scale and questionnaire were used for face-to-face interview. Chi-square test and logistic regression analysis were applied in this study. Results The prevalence rate of MCI was 21.1% in elderly people. Female, high age, illiteracy, loss of spouse, lack of exercise, no professional activitives and not doing some work for the descendants were the risk factors for MCI. Conclusions The aging and the low educational level are the risk factors for MCI in the elderly. High self-care ability scores are the protective factors for MCI.
6.Clinical effect of Wenxin granule combined with metoprolol on arrhythmia in elderly patients with coronary heart disease

Objective To explore the clinical effect and safety of Wenxin granule combined with metoprolol on arrhythmia in elderly patients with coronary heart disease (CHD).Methods 186 elderly CHD patients with arrhythmia were divided into 3 groups according to therapy:group A (n=62,treated with Wenxin granule),group B (n=60,treated with metoprolol) and group C (n=64,treated with Wenxin granule plus metoprolol).Clinical effect and adverse effect were retrospectively analyzed.Results The excellence rate and total efficiency rate were higher in group C than group A andB [53.1% (34 cases) vs.32.3% (20 cases),30.0% (18 cases); 93.8% (60 cases) vs.40.3% (25 cases),40.0% (24 cases) ; respectively,all P<0.05].There were no significant differences in the incidence of adverse effect in the 3 groups [4.8% (3/62),8.3% (5/60) vs.8.3% (5/64),all P >0.05].Conclusions Wenxin granule combined with metoprolol improves clinical effect,decreases premature beat and has a better safety for elderly CHD patients with arrhythmia,which is worthy of clinical application.
7.Analysis of factors affecting postoperative telephone follow-up response rate of lung cancer patients and countermeasures

Objective To analyze the telephone follow-up response rate and related factors in lung cancer patients thus to explore the countermeasures to increase the telephone response rate.Methods One thousand six hundred and eight post-surgery lung cancer patients were followed up by telephone interview,calculate response rates and analyze the impact factors on response rates of surgical patients. Results The lung cancer patients' overall response rate of one thousand six hundred and eight cases was 83.1%.Reservation phone type, phone number,family history of cancer are major factors affecting lung cancer patients' telephone followed up response rate(P<0.05). Conclusions Telephone follow-up of patients with lung cancer response rates is at a low level.To improve tumor response rates of follow-up and follow-up quality is a long and arduous task,requiring medical personnel continue to learn professional knowledge,summing up a reasonable follow-up methods,but also needing patients and their families to understand and cooperate.
8.Application of tracer methodology in management of high-risk medicine

ObjectiveTo explore the effect of tracer methodology on management of high-risk medicine.Methods The management of high-risk medicine was investigated with tracer methodology to find out the problems in the medicine management, construct effective management system and establish tracer programme and pyramid management mode for high-risk medicine management.The rate of adverse events in high-risk medicine management before the use of tracer methodology was compared with that after the use.Result The incidence of adverse events was significantly lower than that before the use of tracer methodology (χ2=5.379,P<0.05).Conclusion The tracer methodology in the management of high-risk medicine can be effective in the reduction of incidence of adverse events in high-risk medicine management and promotion of safety management.

Objective To evaluate the prevalence of obstructive sleep apnea syndrome (OSAS) and its characteristics in elderly patients with cardiovascular diseases, and provide reference for the clinical decisions. Methods All patients who were hospitalized in department of cardiovascular medicine from January to June in 2007 were invited to participate in the current study. A total of 317 hospitalized elderly patients were recruited into this study. All participants were assessed by portable bedside nocturnal polysomnograph and Epworth sleepiness scale (ESS). Results Among 317 patients, 281 cases (88.6%) met the criterion of obstructive sleep apnea (OSA) [apnea and hypopnea index (AHI)≥5] and 47 cases (14.8%) met the criteria of obstructive sleep apnea syndrome (OSAS) (AHI≥5 and ESS≥9). When the severity of OSA (as indicated by AHI) was considered as a dependent variable, multiple regression analysis indicated that it was significantly associated with minimal SaO2 and the oxygen desaturation index, while age, habitual snoring, ESS, BMI, mean SaO2 and the duration of SaO2≤ 90% did not show significant effects on the severity of OSA. Conclusions High prevalence of obstructive sleep apnea syndrome (with daytime sleepiness) is found in elderly hospitalized patients and the rate of obstructive sleep apnea is much higher in patients without daytime sleepiness symptoms. Minimal SaO2 and the oxygen desaturation index are the important predicting factors for the severity of OSA, while age, BMI, habitual snoring, sleepiness are not correlated with the severity of OSA after adjusting minimal SaO2 and oxygen desaturation index.
