2.ULTRASTRUCTURAL INVESTIGATION OF LUMBAR INTERVERTEBRAL DISC

The normal lumbar intervertebral discs of rat, rabbit and human (fetal and adult) were studied under transmission electron microscope. In all the specimens the following findings could be observed. The annulus fibrosus was composed of fibrocartilage. However, the cells in the lacunae of the fibrocartilage might show changes in form with the location of the lacunae. The cells in those lacunae on the periphery of the annulus became elongated and lost their scalloped cytoplasmic membrane, thus resembled fibroblasts. On the contrary, the cells in those lacunae in the central portion of the annulus revealed characteristics of the notochordal cells, with pyknosis of nucleus and aggregation of glycogen and fat droplets in the cytoplasm. Consequently the annulus fibrosus exhibited a transition of their cellular components. Elastic fibers could be found in the annulus. Nucleus pulposus contained notochordal cells in a loose matrix. The notochordal cells might exist singly or in clusters and in their cytoplasm there were many vacuoles and aggregates of glycogen and microfilaments.
3.Regulative role of specific cytokine on expression of T-helper 17 cells and regulatory T cells

Objective To investigate the roles of a variety of cytokines including transforming growth factor beta (TGF-β),interleukin-6 (IL-6) and interleukin-2 (IL-2) in the differentiation of CD+4 Tlymphocyte cells.Methods T lymphocyte cells either in human peripheral blood or routine spleen were cultured in vitro under different stimulation conditions.Flow cytometry was used to analyze the percentages of CD+4IL-17+ T-helper 17(Th17) cells,CD+8 IL-17+ T cells,CD+4 CD+25 FOXP+3 T regulatory (Treg) cells among activated T cells.Results Differentiation of Treg cells,Th17 cells and CD+8 IL-17+ T lymphocyte cells was enhanced when murine splenic T cells were cultured with TGF-β.The levels of expression were (7.8±2.2)%,(12.6±3.1)%,(10.1±2.6)% ,respectively.Experimental control group was severally same type of T cells without cytokine treatment.The levels of expression were (4.8±0.6) %,(1.7±0.5) %,(1.0±0.4) %,respectively.There were statistically significant differences among them (q=4.09,8.80,9.61.P<0.05 or P<0.01).Under combination treatment with IL-6 and TGF-β,(17.8±5.3) % Th17 cells and (15.0±4.2)% CDCD+8 IL-17CD+ T cells were induced,whereas the levels of Treg cells whose differentiation were restrained were (4.1±1.2) %.The differences were statistically significant compared with the level of same type of T cells in TGF-β group (q=5.03,5.17,5.04,P<0.01).Moreover,combination treatment with IL-2 and TGF-β decreased percentages of Th17 and CDCD+8 IL-17CD+ T cells and increased percentages of Treg cells in T cell population.There was an opposite effect when anti-IL-2 was apphed.The percentages of Th17 and CD+8 IL-17+ T cells were increased and the percentages of Treg cells were reduced The regulation trend of T lymphocyte cells in human peripheral blood was similar with those in routine spleen.Conclusion Various cytokines are of great importance in the regulation of the balance between Th17 and Treg cell.
4.Application of hyperextension reduction with percutaneons kyphoplasty in treatment for osteoporotic vertebral compression fracture

ObjectiveTo discuss the clinical efficacy of hyperextension reduction with percutaneous kyphoplasty in treatment for osteoporotic vertebral compression fracture.MethodsTwenty-three patients with 23 vertebral compression fractures underwent hyperextension reduction with percutaneous kyphoplasty from September 2010 to October 2011.The vasual analogue scale(VAS) score and complications were recorded during followed up.Results All cases were successfully experienced the procedure using unilateral or bilateral percutaneous pedicle underwent hyperextension reduction.The operation time was (75 ± 45) minutes,the mean volume of cement injected into each vertebral body was(4.9 ± 1.9) ml.All patients had successful intervention without spinal cord injury,pulmonary embolism.Followed up(7.5 ± 6.5 ) months,the symptoms of the patients were improved,preoperative VAS score was (7.5 ± 0.7 ) scores,reduced to (3.4 ±0.4) scores at day 1 postoperatively,(2.7 ±0.5) scores at day 7 postoperatively and (2.3 ±0.5) scores at month 1 postoperatively,there was significant difference between preoperative and postoperative (P< 0.05).ConclusionsApplication of hyperextension reduction with percutaneous kyphoplasty can relieve pain and restore the vertebral body height,it may be an effective treatment method for patients with osteoporotic vertebral compression fracture.

The knock out vector pHK was constructed with E coli vector pET 28a and shuttle vector pHY300PL, by using denatured DNA and homologous recombination technique, the kanamycin resistance gene ( Kan r) from integrated alkaline protease gene engineering strain BP071 was knocked out successfully, and the 11 positive clones were obtained The yield of the best positive clone BP0715 was stable as same as BP071 The methods provided the good experience for the industrial microbiology research, and it was foundation for studying on the safety of genetically modified organisms
6.ROLE OF LYSOSOMES IN THE DEVELOPMENT OF EXPERIMENTAL CARDIAC HYPERTROPHY

In present study, animal model of cardiac hypertrophy induced by abdominal aortic stenosis was utilized, and the lysosomes were investigated by electron microscopy and ultrastructural cytochemistry. The results showed that while cardiac hypertrophy developed, lysosomes participate in apparent remolding of cardiac muscle cells; in the late stage of cardiac hypertrophy, a great deal of autophagocytoses were viewed in some muscle cells which may result in irreversible cell damage.
7.Clinical analysis of acute cervical spinal cord injury with stress ulcer

ObjectiveTo study the mechanisms and related factors and preventive managements of the stress ulcer complicating with acute cervical spinal cord injury. MethodsThe clinical findings of eight cases of the acute cervical spinal cord injury with stress ulcer were reviewed retrospectively.ResultsFive cases were cured by conservative treatment, one case was cured after operations, two cases died of breath and circulation failure.ConclusionStress ulcer is a critical complication of acute cervical spinal cord injury. It should be prevented actively.
8.Ultrastructural evidence for vulnerability of hip region to fracture in the aged

Objective To probe ultrastructurally into the mechanism why hip region is the predilected site of fracture in aged people, yet with femoral head exempted.Methods Proximal femoral ends were collected from 14 aged people, who had undergone endoprosthetic replacement for intra-capsular fracture of the femoral neck, and from 7 young men died of brain injury. These specimens were processed and subjected to scanning electron microscopic and transmission electron microscopic observation for bone resorption.Results As compared with the specimens from young men, there were three types of bone resorption in the proximal femoral ends of aged people, i.e. osteoclastic resorption and osteocytic resorption of the trabeculae of cancellous bone, and osteoclastic resorption of Haversian systems of the cortical bone. In the proximal femoral ends of the young men, the arch structures composed of trabeculae in the femoral head showed a round or roundish configuration, while those at the femoral head-neck junction revealed an oval profile and those in the femoral neck exhibited a narrow oval contour. In the proximal femoral ends of the aged people, the morphological characteristics of the trabecular arch remained unchanged despite overt bone resorption.Conclusions Vulnerability of hip region to fracture in aged people is due to a decline in the structural strength of bone tissues of the proximal femoral ends resulting from osteoclastic and osteocytic resorption. The round and roundish trabecular arch of the femoral head can bear external forces from different directions, so that no fracture occurs here, while the oval and narrow oval arches at the head-neck junction and in the neck region fail to stand the horizontal external forces (abduction and adduction injuries), thereby fracture easily happens.
9.Association of serum C peptide level with the severity of diabetic foot ulcers and its healing rate

Objective To investigate the association of serum C peptide concentration with the severity and the outcome of diabetic foot ulcer (DFU). Methods The clinical data of 257 inpatients with DFU were collected, including fasting and postprandial 2h C peptide levels and C peptide area under curve (AUCCP ). The patients were followed up on the outcomes of ulcers and death. The associations of serum C peptide concentration with the Wagner degree, infection severity, and healing rate were analyzed. Results The medians of fasting and 2h postprandial serum C peptide as well as AUCCP were 1. 37(0. 02 ~ 9. 00) nmol/ L, 3. 22(0. 02 ~ 29. 61) nmol/ L, and 511. 65 (3. 60 ~ 2 691. 30)nmol·min-1 ·L-1 respectively, which were lower than general levels. The time of follow-up in our study was 2. 8 (1. 0 ~ 5. 1) years. By the end of study, the wound of 75. 88% patients was healed, 3. 5%undergone major amputation, and 23. 74% died. After adjusting for relative factors, there were no significant associations of serum fasting and postprandial C peptide levels and AUCCP with Wagner degree and infection severity (P>0. 05). Cox regression analysis showed that the fasting plasma C peptide and hemoglobin were the independent protective factors for the healing of ulcers; old age, male, higher infection degree, and diabetes family history were their independent risk factors ( all P < 0. 05). Conclusions The lower plasma fasting C peptide concentration in patients with DFU is not correlated with Wagner degree and infection severity, but closely related with healing rate.
10.Ultrastructural investigation of traumatic avascular necrosis of femoral-head.

OBJECTIVE: To investigate ultrastructurally into traumatic avascular necrosis of femoral head. METHODS: Femoral heads were procured during endoprosthetic replacement for 8 cases of intracapsular fracture of femoral neck, which had been subjected to internal fixation and developed Stage II avascular necrosis. The specimens were processed and studied under transmission electron microscope. RESULTS: In the normal bone tissues of the avascular necrotic femoral heads, the collagen fibrils constituting the collagen bundles of the bone matrix were of uniform diameter, and were regularly arrayed with orientation. In the necrotic bone tissues of the avascular femoral heads, the collagen fibrils were of unequal diameter, and were haphazardly arrayed without orientation. From the trabeculae of the avascular femoral heads emerged three different conditions of osteocytes: (1) In the trabeculae of normal bone tissues, the osteocytes belonged to both degenerative phase and formative phase osteocytes with the latter showing signs of degeneration; (2) In the necrotic trabeculae, the osteocytes perished leaving behind empty lacunae; (3) Over the trabeculae of the interface region could be discerned many active osteogenic cells, which were on their way of transformation into osteoblasts. In the trabeculae here, the osteocytes belonged to both formative phase and degenerative phase osteocytes. CONCLUSIONS: These results suggest that in the femoral heads with traumatic avascular necrosis, there is active osteogenetic activity around the necrotic bone tissues, amply implying a local reparative response following partial bone necrosis.
