Objective To explore the role of infection control in preventing infections in pregnancy-at-term(PAT)parturients with cervical incompetence. Method The strategies of infection control for 80 PAT parturients with cervical incompetence were enforced,including enhanced disinfection of hands,wards,cunnus,intensified care to skin,limited examinations of anus and pubes and prevention of complications. Results All the parturients and their babies were well and safe with one exception,who developed with infection but were cured and discharged by anti-infection managements,complement of energy and increase of nutrition intake. Conclusion Infection control may prevent the infections in PAT parturients with cervical incompetence,enhance the perinatal quality and ensure the safety of mothers and babies.

Objective To investigate the clinical manifestations and MR imaging features of olivopontocerebellar atrophy(OPCA).Methods The clinical data and MR findings of 8 patients with OPCA (OPCA group) were retrospectively analyzed,and 75 cases of healthy physical examination people were chosen as control group (they were divided into 21-30 years group,31-40 years group,41-50 years group,51-60 years group,61-70 years group,and 15 cases in each group),all subjects under went pons measurement.Results All 8 patients with OPCA were sporadic cases.The cerebellar symptom,autonomic symptoms and extrapyramidal symptoms were common manifestations.The head MR imaging of OPCA presented with cerebellum and brain stem atrophy,cross sign was the characteristic appearance.Vertical diameter and anteroposterior diameter of the pons in OPCA group patients was (20.63 ± 2.16) and (16.74 ±4.37) mm,compared with control group [(27.14 ±1.73) and (22.28 ± 1.49) mm],there was significant difference (P ＜ 0.01).In control group,vertical diameter and anteroposterior diameter of the pons maximum in 21-30 years group and 31-40 years group,increased with age,the value had been reduced,but there was no significant difference (P ＞ 0.05).Conclusion MR is the first choice for the diagnosis of OPCA,combine with clinical manifestations may improve the diagnostic accuracy.

AIM: To develop the methods for the quantitative analysis of gallic acid and total phenolic acid in Sedum Aizoon L. METHODS: Gallic acid was analyzed by HPLC on a Hypersil BDS C_(18) column and detected at 271 nm.The mobile phase was methol-water(adjusted to pH=3.0 with H_3PO_4)(90∶10)at a flow rate of 1.0 mL/min.Total phenolic acid was analyzed by spectrophotometry at 720 nm.The colour-developing agent was the mixture solution of 0.6%FeCl_3—0.9%K_3[Fe(CN)_6](1∶0.9). RESULTS: Calibration graphs were constructed in the range of 0.343 0-1.200 ?g(r=0.999 7) for gallic acid and 0.4640-2.320 ?g/mL(r=(0.999 3)) for total phenolic acid.The average recoveries were 97.91%(n=6) for gallic acid and 99.21%(n=6) for total phenolic acid.The RSD of the methods were 1.8% and 2.0%,respectively. CONCLUSION: The methods were fast,reliable,accurate and suitable for analysis of gallic acid and total phenolic acid and quality control in Sedum Aizoon L.

Objective To explore the effectiveness and safety of vaginal paravaginal repair(VPVR) plus vaginal bridge repair in the treatment of female pelvic organ prolapse (POP). Methods Sixty-five patients with different defects of pelvic floor underwent VPVR or plus vaginal bridge repair for posterior vaginal wall. Patients were followed up after operation. The cure rate was estimated subjectively and objectively. The patients' quality of life was evaluated by the pelvic floor distress inventory short form 20 (PFDI-20). Results All 65 cases were treated by vaginal hysterectomy and anterior vaginal repair, in which there were 33 cases underwent VPVR while 32 cases underwent VPVR plus middle area repair. Forty concomitant procedures for vaginal bridge repair were also performed. The average operative time was (110.00±20.12) min and blood loss was (119.52±45.33) ml. The symptom of stress urinary incontinence of 25 cases significantly released after operation. Four incision recovery delayed and there were no other complicatious occurred. Patients were followed up for 6-29 months,the objective cure rate was 100.00% (65/65) and subjective cure rate was 92.31%(60/65), and 58 cases (89.23%)improved significantly with the quality of life comparing with that of pre-operation by completing PFDI-20 (P<0.01). Conclusions It is an effective and safe procedure for VPVR plus vaginal bridge repair to correct median to severe anterior vaginal prolapse and posterior vaginal wall prolapse. More clinical trials are needed to evaluate their long-term outcome.

Objective To investigate the effect of the expression of Notch1 protein and the mutation of Notch1 gene in.T-cell lymphoma (TCL).Methods Immunohistochemistry was used to detect the expression of Notch1 protein,and PCR amplification and DNA sequencing were used to detect the mutation of Notch1 gene in the 26th and 27th HD domain and the 34th PEST domain in 30 cases.10 cases of reactive hyperplasia tissues of lymph node were as the control.Results The positive rates of Notch1 protein expression and Notch1 gene mutation were 70.0 ％ (21/30) and 56.7 ％ (17/30).8 cases of Notch1 mutations were detected in the HD domain,6 cases in the PEST domain,and 3 case in both HD and PEST domains.Inscrtion,deletion,nonsense mutation and missense mutation were included in Notch 1 mutations.Conclusion Notch1 gene mutation may play an important role in the expression of Notch1 protein.The occur of TCL is related to the expression of Notch1 protein and the mutation of Notch1 gene.

Objective To study the changes of neutrophil surface molecule CD64 and inflammatory factor levels in patients with burn infection. Methods 46 cases of patients with burn infection who were treated in the department of burn in our hospital between March 2010 and October 2015 were selected as the observation group. Meanwhile, 46 cases of healthy people who underwent physical examination in our hospital during the same period were included in the control group. The levels of neutrophil surface molecule CD64, C-reactive protein (CRP), white blood cell count (WBC) and inflammatory factor [interleukin IL-6 (IL-6), interleukin (IL-8), tumor necro-sis factor (TNF-α) in the two groups of subjects were determined by flow cytometry and were compared. Results In the observation group, the levels of CD64, CRP, WBC and inflammatory factors were significantly higher than those in the control group (P < 0.05). Conclusion The levels of CD64, CRP, WBC and inflammatory factors in patients with burn infection are significantly higher than those in healthy people , which indicates that neutrophil surface molecule CD64, inflammatory factors and burn infection are closely correlated.

Objective To study the clinicopathological and immunohistochemical features, histogenesis and biological behavior of solid pseudopapillary tumor of the pancreas ( SPT ). Methods Routine HE and immunohistochemical ( SP) methods were used in 20 cases of SPT. Results There were 18 females and 2 males, age ranging from 13 to 48 years with mean age of 25. 3 years. Abdominal pain and palpable mass were among the main complains. Seventeen cases were followed-up from 9 to 120 monthes. Fourteen cases were alive. Tumors were encapsulated, mixed with solid and cystic tissues. Histological features were psudopapillary structure with a fibrovascular core. Immunohistogically, the tumors were positive for a-1-AT ( 17 cases) , vimentin ( 14 cases) , synaptophysin ( 10 cases) , CgA (5 cases) ,CK and insulin (2 cases) ,glucagon and S-100 (1 case) ,PR (14 cases) , ER (1 case) ,pS2 (6 cases) , but all were negative for CEA and gastrin. Conclusion SPT is of low-graded malignancy and a distinct clinicopathologic entity in young female patients with both exocrine as well as endocrine differentiation. The tumor is closely related with sex hormone receptors.

Objective: To investigate the refractive status of children and adolescents aged 4-18 years in Chengdu, and to analyze the epidemiological characteristics of refractive parameters. Methods: A cross sectional survey was conducted among 82 024 children and adolescents aged 4-18 years in Gaoxin District of Chengdu from August to September 2021. The prevalence of screening myopia, low vision rate, high myopia rate, refractive status and axial development were analyzed. Results: The prevalence of screening myopia in students aged 4-18 years was 40.42%(33 158/82 024). Low myopia was 24.51%(20 108/82 024), moderate myopia was 13.05%(10 703/82 024) and high myopia was 2.86%(2 347/82 024). The prevalence of screening myopia was 1.81% (233/12 848) in kindergarten, 34.44%(17 095/49 644) in primary school, 79.73%(9 738/12 214) in junior high school and 83.25% (6 092/7 318) in senior high school. The rate of visual impairment increased by year from the age of 4, and the rate of myopia increased most rapidly from 6 to 15 years old, the prevalenct of high myopia was compared between adjacent age groups: there were statistical differences between 9-15 years old were more likely to be nearsighted than boys( P <0.035 7). Significant differences in screening myopia between 5- and 7-17 year old groups,and in boys and girls.The median total diopter was 0.40 D at the age of 4 and developed to -2.90 D at the age of 18 . The diopter of girls aged 8-15 years was higher than that of boys, and there was statistical significance( Z=-2.53, -4.09, -5.67, -8.64, -5.56, -4.97, -2.52, -2.14, P <0.05). The axial length gradually increased with age, with the mean value of (22.31±0.59) mm at 4 years old and (24.91±1.00) mm at 18 years old. The mean corneal curvature did not change with age (43.19±1.47)D. Conclusion 6-15 years old is a especially critical period for myopia development. More efforts need to be taken to decrease the prevalence of myopia before 6 years old. Prevention of the development of high myopia should start before the age of 10. The prevalence of myopia in girls is higher than that in boys, more prevention and control of myopia should focus on girls.

ObjectiveTo explore the effects of continuous nursing care based on theory of goal attainment on self-health management ability and quality of life in patients with stroke. MethodsTotally 126 patients with stroke treated in a ClassⅢ Grade A hospital in Anhui Province from December 2017 to December 2018 were selected using convenient sampling and stratified into treatment (n=64) and control (n=62) groups. Patients in the control group received conventional nursing care, while patients in the treatment group received continuous care based on the theory of goal attainment. Self-Health Management Scale for Patients with Chronic Diseases (SHMSPCD) and Quality of Life Inventory (QLI) were used to assess the effects before and after intervention. ResultsBoth the total self-health management ability and quality of life scores of the treatment group were higher than those of the control group 1, 3 and 6 month after intervention, and there were statistically significant differences between the two groups (P＜0.05). ConclusionsContinuous nursing care based on the theory of goal attainment can improve the self-health management ability and quality of life of the patients with stroke.

The human Gadd45 protein family plays critical roles in DNA repair, negative growth control, genomic stability, cell cycle checkpoints and apoptosis. Here we report the crystal structure of human Gadd45γ [corrected], revealing a unique dimer formed via a bundle of four parallel helices, involving the most conserved residues among the Gadd45 isoforms. Mutational analysis of human Gadd45γ [corrected] identified a conserved, highly acidic patch in the central region of the dimer for interaction with the proliferating cell nuclear antigen (PCNA), p21 and cdc2, suggesting that the parallel dimer is the active form for the interaction. Cellular assays indicate that: (1) dimerization of Gadd45γ [corrected] is necessary for apoptosis as well as growth inhibition, and that cell growth inhibition is caused by both cell cycle arrest and apoptosis; (2) a conserved and highly acidic patch on the dimer surface, including the important residues Glu87 and Asp89, is a putative interface for binding proteins related to the cell cycle, DNA repair and apoptosis. These results reveal the mechanism of self-association by Gadd45 proteins and the importance of this self-association for their biological function.
