ObjectiveTo evaluate the effect of the comprehensive surgical management of primary venous reflux disease in lower extremity. MethodsComprehensive surgical management was performed on 108 patients( 126 limbs) with primary venous reflux disease between February, 1996 and Sept, 2000. All limbs were performed with superficial venous operation. External valvuloplasty of femoral vein valve was given to 83 cases(97 limbs) simultaneously, and to 22 cases (26 limbs) with concomitant subfascial endoscopic perforator surgery(SEPS). 3 cases (3 limbs) were operated with concomitant SEPS only. ResultsVenous claudication, swelling, and aching disappeared in 80.2% of limbs( 101/126) and apparent improvement in 19.8 % of limbs(25/126). All varicose veins were gone. Ulcer healing pre sented in 80 % of the limbs(8/10) with ulcers and ulcer decreasing in 20% of the limbs(2/10) in 3~6 months after superficial and deep venous operations. Ulcer healing presented in 23 cases(26 limbs) with ulcers and ulcer decreasing in remaining 2 limbs in 14 to 32 days after additional SEPS besides superficial and or deep venous operations. Deep venous valve sufficiency was demonstrated in 89.7% (113/126) of the limbs by Color Duplex. ConclusionIn the patients with severe primary venous reflux disease, comprehensive surgical management with concomitant superficial, deep and perforating veins might greatly improve the clinical effect.

Objective To investigate the influence of proinflammatory factor interleukin-18(IL-18) on vein endothelial cell function by activating NF-κB mediated cell signal pathway and its association with deep vein thrombosis(DVT).Methods Recombinant human IL-18 was used to act on in vitro cultured human umbilical vein endothelial cell(HUVECs).The NF-κB activation inhibitor was used to conduct interference. The detection measures of real time fluorescence quantitative PCR,Western blot, immunofluorescence and flow cytometry were used to verify whether IL-18 affect the expression of endothelial cellular function markers such as HUVECs normal statusand vWF,P-selectin and tissue plasminogen activator(t-PA) by activating NF-κB mediated cell signal pathway. Moreover the mechanism of IL-18 participating in the DVT was performed the comprehensive analysis by combining with previous study. Results IL-18 could activate NF-κB in endothelial cell, increased the p65 expression in nucleus, decreased the intracellular IκBα expression and significantly increased early apoptosis cells in HUVECs;adding QNZ(EVP4593) could significantly inhibit the activation effect of IL-18 on NF-κB,the occurrence of cellular injury and apoptosis was significantly reduced;IL-18 could promote the abnormal expression of DVT related endothelial cell markers vWF,P-selectin and t-PA (P<0.05).But various markers could recover conventional expression after inhibiting NF-κB activation. Conclusion The interaction between Il-18 and NF-κB causes the abnormality of HUVECs growth status and function, which may be the DVT onset related pathogenic mechanism.

Objective To evaluate the effectiveness of loop electrosurgical excision procedure(LEEP) for the management of cervical intraepithelial neoplasia(CIN) grade Ⅱ. Methods A total of 28 patients were pathologically diagnosed as having CIN grade Ⅱ by colposcopic cervical biopsy.High-risk human papillomavirus(HPV) DNA testing showed positive results in 96.4% of the patients(27/28).The procedure was conducted under surface anesthesia.A loop electrode was used to cut through the cervical tissues, and then a square-shaped electrode or a small-sized loop electrode was utilized to complete the resection of lesions, including parts of the cervical canal. All the patients were followed postoperatively. Results The intraoperative blood loss was 0~20 ml,and the operation time was 5~10 min.Out of the 28 patients, postoperative abnormal vaginal bleeding was found in 3 patients. No other complications occurred. The resected tissues showed no obvious charring changes. The lesions of CIN completely disappeared in 15 patients(53.6%),subsided to grade Ⅰ in 5 patients, remained in grade Ⅱ in 5 patients, and progressed to grade Ⅲ in 3 patients.Follow-up examinations in the 28 patients for 6~24 months(mean,16 months) found no residual lesions or recurrence. The high-risk HPV DNA findings turned negative in 23 patients. Conclusions LEEP is a safe and effective procedure for the treatment of CIN grade Ⅱ,with advantages of little invasion, simplicity of performance, and no need of hospitalization.

BACKGROUND:During the restoration of residual molar crown, a little part of tooth is still remained commonly. After the restoration, with various forces, stress distribution affects directly the results after restoration. Finite element method is gradually applied in stress analysis on artificial tooth.OBJECTIVE: To establish the three-dimensional (3-D) finite element model of post-inlay restoration of the first residual mandibular molar crown so as to provide experimental data for improving model establishment of complicated teeth and analysis on the property of stress distribution of restoring methods.DESIGN: Repeated observation and measurement were given.SETTING: Department of Stomatology and Department of Radiology of First Hospital affiliated to Sun Yat-sen University;Department of Solid Mechanics,College of Traffics and Communications, South China University of Technology; Department of Restoration of Guanghua College of Stomatology.MATERIALS: The experiment was performed in Department of Solid Mechanics, College of Traffics and Communications of South China University of Technology from November 2003 to December 2004. Six first mandibular molars on the right side with normal morphology in vitro were collected, and Toshiba Xpress/SX spiral CT machine, image photo synthesis software and finite element analysis software ANSYS were applied in the experiment.METHODS: 1 of the 6 first mandibular molars on the right side with normal morphology in vitro was selected for pulpectomy, which was the best in density and near to clinical requirement in morphology. With pulpectomy, the prosthesis of braking-lock post-inlay restoration was prepared. Spiral CT-cross scanning was performed in premolar crown before the restoration, the residual crown with post-inlay in main root canal after restoration and the residual crown with braking-lock second post-inlay restoration. With image photosynthesis software, 3-D digital model of residual tooth and metal part was established and the entire tooth model was prepared after adhesion of two parts. In order to provide better boundary conditions of simulated natural tooth in practice, alveolar bone was considered. Under Mesh order in ANSYS software, automatic mesh generation was performed in the model directly.MAIN OUTCOME MEASURES: Establishment of 3-D finite element models of residual tooth before restoration, post inlay, and alveolar bone and tooth after restoration and the results of mesh generation.RESULTS: By establishing 3-D finite element models of residual tooth before restoration, post inlay, alveolar bone and tooth after restoration and automatic mesh generation, there were altogether 117720 units and 20988nodes. Good geometric similarity presents between the construction model of 3-D finite element model and solid tissue.CONCLUSION: Combination of 3-D finite-element model with spiral Ctcross technology establishes complex dental models, simulates practical conditions authentically and is good in operation.
