Objective We aimed to provide references for nurses allocation by selecting the nursing items and measuring nursing time. Methods We recorded the nursing workload of 130 nurses in 10 wards within one week by literature review, Delphi method and questionnaire investigation. Results Totally 125 items of nursing items were included,101 of which were direct nursing items and 24 of which were indirect ones.The average time per day per patient was 185.1 minutes for level one nursing,95.4 minutes for level two nursing, 58.5 minutes for level three nursing.Average nursing time per day,working time per day per nurse and nursing time per day for per patient in each unit had statistical difference,P<0.05.The time for direct nursing items was 64.13% and for indirect nursing items was 35.87%. Conclusions Allocation of nurses according to measurement of nursing time could reach the aim of reasonable allocation and effective utilization of nursing human resource.

Objective To use the genetic diagnosis technique to perform the gene detection in the patients with thalassemia for understanding the main gene mutation types and their gene frequencies .Methods Totally 5 500 outpatients and individuals un‐dergoing physical examination in the hematology department of the Huizhou Municipal Central People′s Hospital from January 2010 to October 2014 were taken as the research subjects .The GAP‐PCR and membrane reverse hypridition technology were adopted to conduct the gene analysis of α‐andβ‐thalassemia .Results A total of 1 604 cases ofα‐globin gene change were detected ,accounting for 29 .16% of the total detected subjects ;1 096 cases ofβ‐globin gene change were detected ,accounting for 19 .93% of the total de‐tected subjects ;119 cases of αβ‐complex thalassemia were detected .Conclusion The gene screening of α‐ and β‐ thalassemia pro‐vides the valuable basic data for conducting the marital and fertility instructions .

The killing effects of five chemical disinfectants on hepatitis A virus (HAV) were determined by ELISA The results showed that 2% glutaraldehyde completely killed HAV within 10 min and 025% peracetic acid killed 74.5% HAV within 5 min and 40% iodophors killed 60.3% HAV within 5 min. It is suggested that glutaraldehyde, peracetic acid and iodophors are more effective agents to kill HAV.

Objective:To investigate the impact on the level of serum IL-8 and IL-18 by Helicobacter pylori eradication therapy in patients with rheumatoid arthritis.Methods:Helicobacter pylori were assessed by 14-Curea breath test in patients with rheumatoid arthritis.All patients were divided into Hp positive group and Hp negative group on the basis of 14-C urea breath test results.The Hp positive group were divided into anti-helicobacter pylori group and control group.The level of ESR,serum C reactive protein (CRP), RF,IL-8 and IL-18 were measured in all patients before and after 12 weeks of treament.And the number of joint swelling,joint pain/tenderness,morning stiffness time, hands grip strength were recorded before and after 12 weeks treatment.Results: 12 weeks after treatment,the effective rate in the Hp negative group and the anti-helicobacter pylori group was higher than that in the control group (84.09%vs 62.50%,χ2=5.41,81.25% vs 62.50%,χ2=4.17,P<0.05).The clinical symptoms significantly improved and the levels of ESR, C-reactive protein, RF, IL-8 and IL-18 significantly reduced in the three groups ( P<0.05 ) .The clinical symptoms improved more obviously in the Hp negative group and the anti-helicobacter pylori group than that in the control group.The levels of ESR,C-reactive protein,IL-8 and IL-18 in the Hp negative group and the anti-helicobacter pylori group was lower than that in the control group.While there was no significantly difference in the level of RF in the three groups.Conclusion:From a certain extent ,Hp eradication therapy can improve the clinical curative effect of rheumatoid arthritis.

Objective To evaluate the clinical effect of trans reformed catheter infusion for treatment of fallopian tube obstrution with Yuebei infertility.Methods 86 cases of fallopian tube obstrution were recanalized by trans reformed catheter infusion. Results 86 cases with 169 obstructive fallopian tube altogether, among them 119 were recanalized by once of this procedure, 33 by twice. The total effective rate was 89.9%. 86 cases were all followed up including 39 cases got pregnant after once of this procedure, 18 were pregnant after twice of the performance. The total pregnancy rate reached 66.3%. Conclusions Trans reformed catheter infusion therapy is a safe and effective method to treat infertility resulted from fallopian tube obstruction.

Objective To establish a diagnosis method for fungal infection using two-round PCR,and to evaluate its sensitivity in the detection of clinical specimens suspected to be infected with fungi.Methods A total of 29 specimens of clinical sputum and alveolar wash solution were collected from patients with suspicious fungal infection.All specimens uaderwent direct microscopy with 10%KOH,fungal culture,one-round PCR and two-round PCR.The fungal universal primer targeting ITS regions of rDNA was used in PCR.The detection rate for fungi was compared between these methods.Results The detection rate for fungi was 20.69%by direct microscopy,37.9%by fungal culture,17.2%by one-round PCR,and 48.3%by two-round PCR.More than one species of fungus were detected in 6.9%(2/29),3.4%(1/29)and 24.1%(7/29)of these specimens by fungal culture.one-round PCR and two-round PCR, respectively.There was a significant difference in the detection rate between two-and one-round PCR(x2=6.34,P＜0.05).With regard to the detection rate for more than one species of fungus,two-round PCR was significantly higher than one-round PCR and fungal culture(x2=4.09,6.30.bom P＜0.05).Conclusion Two-round PCR may help to improve the sensitivity of molecular diagnosis of fungus-infected specimens.

Objective To investigate the changes of inositol 1,4,5-triphosphate receptor typeⅠ(IP3RⅠ)expression in the myocardial tissue of rats with endotoxic shock and probe the possible mechanisms of myocardial depression.Methods Thirty-three male Wistar rats were divided into four groups:control group (n=6) were injected normal saline(4ml/kg),and the other 3 groups (n=9) were injected endotoxin via femoral vein with a different dose of 10mg/kg,5mg/kg,and the artificial sacrificed group(10mg/kg,the rats were sacrificed when the mean arterial pressure was first rapidly decreased,then gradually increased to the normal level).IP3RⅠexpression in the myocardial tissue of rats was detected by immunohistochemistry,Western blot and colloidal gold immunoelectron microscopy technique.Results IP3RⅠexpressions were increased significantly in each group of rats with endotoxic shock than in the control group (P<0.01),and most obviously enhanced in the the artificial sacrificed group.The expression of IP3RⅠwas related to the level of mean arterial pressure.Ectopic expression of IP3RⅠwas observed around the cell membrane of the cardiac myocytes.Conclusion (1)The expression of IP3RⅠenhances in the myocardial tissue of rats with endotoxic shock,and exists ectopic expression.(2) The expression of IP3RⅠwas related to the level of mean arterial pressure.(3) IP3RⅠ may be related to the cardiac contractility and involved in the regulation of mean arterial pressure.

OBJECTIVE: To investigate the clinical characteristics of ocular symptoms in patients with allergic rhinitis (AR) and the impact of the ocular symptoms on patients' quality of life. METHOD: AR patients' history and clinical data were collected and analyzed. One hundred cases were extracted from adult patients with and without ocular symptoms in each group and their quality of life were evaluated using rhino-conjunctivitis quality of life questionnaire (RQLQ). RESULT: Totally 1119 cases were collected and 859 cases had ocular symptoms. Of the patients with ocular symptoms, 582 cases were mild, 234 cases were moderate, 43 cases were severe. Eye itching was the most common symptom, followed by tears, hyperemia and swelling. Patients with ocular symptoms had longer disease history than non ocular symptom group (P < 0.05). There was a positive correlation between ocular and nasal symptoms (P < 0.01); patients with ocular symptoms had more severe nasal symptoms than patients without ocular problems; while patients with moderate to severe AR had more severe ocular symptoms than mild AR patients (P < 0.01). Female patients had higher incidence (P < 0.05) and ocular symptoms score (P < 0.05) than male. Children less than 10 years old had a relative lower incidence and score of ocular symptoms. While the incidence and score had an increasing trend for patients older than fifty. There was no difference regarding the type and number of allergen in the patients with and without ocular symptom. Moreover, patients with ocular symptoms had higher scores in the domains of non-nose/eye symptoms, practical problems, ocular symptoms, emotional function and total score in RQLQ than patients without ocular symptom. CONCLUSION It was common for the AR patients to have ocular symptoms, and ocular symptoms had obvious influence on the patients' quality of life. So controlling of the ocular symptoms should not be ignored during the diagnosis and treatment of AR.

Objective To evaluate the efficacy and safety of Shenqi Fuzheng injection in the treatment of patients with laparoscopic gastric cancer. Methods A total of 94 cases with laparoscopic gastric cancer included in this study were divided into control group and experimental group with 47 cases in each group.The patients in both groups received laparoscopic gastric carcinoma.Patients in control group received fluorouracil, leucovorin and oxaliplatin infusion( FOLFOX4 ) chemotherapy and patients in the experimental group revieved Shenqi Fuzheng injection.Both groups received two courses treatment of 14 days for one course.The clinical efficacy, inflammatory levels, tumor indicators and adverse drug reactions were compared between the two groups.Results The clinical efficacy in experimental group was 95.74%, which was significantly higher than that in control group 82.98%(P<0.05 ) .The serum level of serum serum albumin (ALB), prealbumin (PA), glutathione-S-transferase (GST), carcinoembryonic antigen (CEA) and carbohydrate antigen 199 (CA199) after treatment in experimental group were lower than the control group (P<0.05) .The adverse drug reactions was not statistically different between the two groups.Conclusion Shenqi Fuzheng injection has significant efficacy in the treatment of laparoscopic gastric cancer with high safety.

Objective To investigate the efficacy and safety of percutaneous microwave ablation in the treatment of hypersplenism.Methods From March 2007 to May 2011,38 patients with hepatitis B virus caused liver cirrhosis and complicated with hypersplenism received percutaneous microwave ablation treatment for several times.Before percutaneous microwave ablation treatment,the volume of spleen was calculated according to 3D computed tomography (CT) scan.Ultrasound,blood routine,urine routine,liver function test,kidney function test,serum amylase and lipase were also tested.For the first time,1/3 volume of spleen was ablated.If no complication were observed in one week after ablation,then another 1/3 volume of spleen was ablated.Blood routine and liver function test were checked on the 1st,3rd and 5th day after microwave ablation.Blood routine,liver function test and ultrasound were examined on 7th and 14th day after microwave ablation.On the 30th day CT examination was conducted.Ultrasound,blood routine,urine routine,liver function test,kidney function test,serum amylase and lipase were detected at one month,three month and every three month after microwave ablation.The follow up duration was over two years.The t-test was performed for clinical data comparison.Results The mean ratio of ablated spleen was (47 ±5)％ (range 41％ to 57％).Preoperative white blood cell count was (2.46±0.78)× 109/L,which gradually increased after operation and peaked on the 3rd day after operation ((5.34 ± 2.10) × 109/L).Then gradually decreased,which was (3.16 ± 1.02) × 109/L at 24 month and the difference was statistically significant compared with that of preoperation (t=-3.349,P＜0.01).Preoperative platelet count was (46.58 ± 17.30) × 109/L,which gradually decreased after operation and was lowest on the 3rd day after operation.Then gradually increased,which peaked at 30 days after operation ((101.79 ± 25.80) × 109/L) and then gradually decreased,which was (61.97 ± 15.09) × 109/L at 24 month and the difference was statistically significant compared with that of preoperation (t＝-4.135,P＜0.01).The inner diameter of portal vein was (14.66±0.88) mm preoperation,which was (13.22±0.64) mm at three month after operation and the difference was statistically significant compared with that of preoperation (t=8.145,P＜0.01).It was (14.64±0.81) mm at six month after operation and the difference was not statistically significant compared with that of preoperation (P＞ 0.05).The major adverse effects were fever,left upper abdominal pain,left shoulder pain,pleural effusion,intraperitoneal hemorrhage and temporary hemoglobinuria which all recovered after symptomatic treatment.No severe complication such as uncontrollable bleeding,splenic abscess,spleen rupture and the surrounding organ injury and treatment related death were observed.Conclusion Percutaneous microwave ablation for several times could safely destroy suitable volume of spleen,increase platelet and white blood cell count,improve portal hypertension and with rare complications,which might be a minimally invasive techniques with clinical application value in the treatment of the hypersplenism.
