1.A discussion on accounting method for usage efficiency of no charge medical consumables

Objective:To discuss the accounting method for clinical usage efficiency of free medical consumable under information management system, and analyze these usage efficiency data.Methods: To adopt the medical material management system to collect usage quantity and amount of free medical consumables and chargeable medical consumable, respectively, from different department in every month, and then generalize usage quantity of different consumable; to apply hospital information system (HIS) to collect monthly patient data of different disease area or department; to gather statistics of monthly data proportion of usage ratio of free medical consumable as classified statistic principle by applying the consumption of them, patient quantity and usage proportion depend on data from two systems; and then got the line analysis charts of monthly usage efficiency and average usage efficiency of free medical consumable of each department in one year.Results: The data were evaluated through usage rate of free medical consumable of each department; the reference data from direct data and graph analysis depended on different classification and time interval, and they were provided for clinic practice in order to the free medical consumable can be more reasonable used in guiding clinical department.Conclusion: The purchase and used quantity of medical consumable can be grasped through information management system, which is scientific in management and usage for them, can avoid excessive waste and reduction of medical consumable, ensure them can be safety and efficiency used in clinical diagnosis and treatment. It make medical materials to be used in clinical diagnosis safely and effectively.
2.Gravitational erythema:a case report

A 15-year-old female patient presented with orthostatic erythema in the limbs for half a year. Skin examination revealed that diffuse reticular erythema appeared in the limbs after standing with her arms hanging down for 30 seconds, while the erythema disappeared and skin color returned to normal after raising her arms or lying flat for 30 seconds. The rash in the limbs was reproduced by inflating a sphygmomanometer cuff to 60 mmHg for 2 minutes with the limbs in the horizontal position. Antinuclear antibodies were positive at a titer of 1 ∶1 000. Histopathological examination showed dilated small vessels in the superficial dermis without obvious inflammatory cell infiltration. The patient was diagnosed with gravitational erythema.
3.Effects and related mechanisms of preconditioning with puerarin on hippocampus CA1 region neuronal injury after focal cerebral ischemia-reperfusion

Objective To observe the effect of puerarin preconditioning on hippocampal CA1 region neuronal injury and further explore its mechanisms. Methods 30 male Sprague-Dawley rats were randomly assigned into sham, model,and puerarin preconditioning ischemia-reperfusion groups. The middle cerebral artery was occluded for 60 min by an intraluminal filament before reperfusion to induce transient focal cerebral ischemia reperfusion rats. Puer-arin preconditioning groups rats received puerarin(100 mg / kg,i. p)1 h before ischemia. The histological changes of hippocampal CA1 region neurons were measured by HE staining on the 5th day after reperfusion. The expressions of Caspase-3 and Caspase-9 proteins in the CA1 region were examined by immunohistochemistry staining. Results The results of HE staining showed that,compared with the model group,puerarin preconditioning significantly re-duced the number of cerebral ischemia induced injury of hippocampal CA1 region neurons(P < 0. 05). Immunohis-tochemistry data showed that the expressions of Caspase-3 and Caspase-9 in the hippocampal CA1 region of puerarin preconditioning group were significantly lower than that of model group(P < 0. 05). Conclusion The results indi-cate that puerarin preconditioning 1 h before ischemia exerts neuroprotective effects on focal cerebral ischemia-reperfusion induced neuronal injury. The neuroprotective mechanisms of puerarin may attribute to inhibiting the ex-pressions of Caspase-3 and Caspase-9 protein.
4.Effect of patient's preoperative visit to operating room on the preoperative apprehension

Objective To evaluate the effect of the patient's preoperative visit to the operating room on the patient's apprehension before operation.Methods One hundred and Sixty patients aged 16-64 yr with fear visual analog scale (FVAS) score≥4 scheduled for elective surgery were randomly divided into 2 groups(n=80 each):control group and study group.In control group the anesthesiologists visited their patients the day before operation ns usual;while in study group the anesthesiologists brought their patients to the operating room,showed them the environment and anesthesia equipment and assured of the safety of the operation and anesthesia.The degree of fear was scored according to FVAS (O=no fear,1-3 mild,4-6 moderate,7-10 severe) and was evaluated on the 1st morning after admission(T1),the night before operation(T2)and before induction of anesthesia(T3).MAP and HR at T1-3 and blood glucose level at T1,3 were mensured and recorded.Results The patient's preoperative apprehension was significantly allayed by patient's preoperative visit to the operating room.The MAP,HR,incidence of moderate and severe fear at T2,3 and blood glucose level at T3 were significantly lower in study group than in control group.Conclusion Patient's preoperative visit to the operating room can allay precperative apprehension.
5.Studies on the risk factors associated with macula choroidal neovascularization in high myopia

Objective To investigate the prevalence of macula choroidal neovascularization(CNV) in high myopia patients and the associated risk factors.Methods Fifty-six outpatients(112 eyes) with high myopia were medically examined from Mar.2008 to Jun.2009.The inspection items,including A scan,B scan,diopter and optical coherence tomography(OCT),were carried out to check whether CNV existed and its growth fashion.The patients' basic status,main symptoms and the pathologic change in macula were recorded.Results The prevalence of CNV in the 56 outpatients(112 eyes) was 14.3%.Gender showed no correlation with CNV(r=-0.015,P=0.879),while age(r=0.629,P=0.000),diopter(r=0.725,P=0.000),axial length(r=0.236,P=0.013),posterior scleral staphyloma(r=0.344,P=0.000),atrophy of choroid and retina in posterior pole(r=0.421,P=0.000),lacquer crack(r=0.421,P=0.000) and Fuchs spot(r=0.519,P=0.000) were significantly correlated with the prevalence of CNV.Conclusions CNV may highly occurred in the patients with high myopia.Posterior scleral staphyloma,atrophy of choroid and retina in posterior pole,lacquer crack,Fuchs spot,age,axial length and diopter seem to be the risk factors for the occurence of CNV.
6.Microstructure and modification of Nd and Zn trace elements in a Mg-Zn-Y-Nd vascular alloy stent

BACKGROUND: With the continuous development of materials science, magnesium alloy vascular stent materials have become a hot research. Because of the mechanical properties and biocompatibility of commercial magnesium alloy, it is difficult to meet the requirements of vascular stents. Therefore, effective measures to improve the sten's surface properties and comprehensive performance become the focus of research.OBJECTIVE: To study the histology and surface modification of vascular stents in rapidly solidified Mg-Zn-Y-Nd alloy.Methods: The low-zinc Mg-2Zn-0.2Y alloy with good mechanical properties and corrosion resistance was selected as the basic material, and Nd and Zn elements were added to refine the alloy stents. After the microstructure of the stent was extruded, the surface modification of the stent was completed and the comprehensive properties of the alloy were improved. The new magnesium alloy for the stent was obtained and the stent surface was modified. The metallographic microstructure, scanning electron microscopy and radiological analysis were used to study the microstructure and mechanical properties of the prepared stents. The mechanical properties of the stents were investigated by hardness and tensile tests.RESULTS AND CONCLUSION: (1) Metallographic microstructure results showed that: when Y elements were not added, the second phase of the magnesium alloy was rod-shaped, and there were a few granules embedded in the matrix. After addition of 0.5% Y elements, in the second phase of the magnesium alloy stent, the shafts were significantly reduced in number, and granules were increased in number and evenly distributed in the body. After the addition of 1% Y,the second phase number increased, a large number of dendrites were visible in the grains, and discontinuous rods existed in the second phase. After the addition of 1.5% Y, the second phase was rod-shaped, with mixture of large and local dendrites in the alloy. (2) X-ray diffraction test results: Mn-Zn-0.5Nd alloy and Mn-Zn-1.0Nd alloy contained the same phases (Mg4Zn7 and (Nd, Y) 2Zn17 phase). When the concentration of Nd increased to 1%, the new MgZn2 phase appeared in the alloy. (3) SEM & EDS test results of modified magnesium alloy showed that after magnesium alloy modification, the second phase contained Zn, Nd and Y elements, and their contents were very close. EDS analysis showed that after the addition of Zr elements, the level of Zn elements in the lamellar second phase decreased significantly, and the level of Nd and Y elements increased, indicating a more stable performance. (4) Micro-hardness test results showed that with the increasing of the content of magnesium alloy, the alloy microhardness increased. (5)Tensile test results showed that the tensile strength and yield strength of the Mg-Zn-Y-0.5Nd-Zr stent were significantly higher than those of Mg-Zn-Y-0.5Nd, Mg-Zn-Y-1.0Nd,Mg-Zn-Y-1.0Nd-Zr stents (P < 0.05); and the elongation at break of Mg-Zn-Y-0.5Nd-Zr and Mg-Zn-Y-1.0Nd-Zr stents was significantly higher than that of Mg-Zn-Y-1.0Nd and Mg-Zn-Y-0.5Nd stents (P < 0.05). To conclude, with Mg-2Zn-0.2Y as core materials, the material modification could be completed by the addition of Nd and Zn elements, and the surface modification could be implemented by extruding and refining the stent microstructure. The modified material has excellent properties.

Yin-Yang1 (YY1),as a ubiquitous nuclear transcription factor with double transcriptional activity in eukaryotic cells,regulates many genes transcription and plays an important role in the cellule biological processes.Overexpression of YY1 is found in several tumor types recently,and may play a role in tumor development and progression by regulating oncogenes,tumor suppressor genes,angiogenesis related factors,as well as apoptosis.YY1 may become a new kind of tumor markers,is conducive to estimate the prognosis of patients with tumor and gain important breakthrough in cancer targeted therapy.

Objective To investigate the expression of apoptosis-related neuclosome assembly protein-1(NAP-1) gene in related brain region of 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine(MPTP) treated mice and beta-amyloid treated rats. Methods The animal models with central nervous system degeneration were established by consecutive administration of MPTP to C57BL mice and injection of beta-amyloid to brain ventricular of SD rats. The RNA in striatum and substantia nigra of mice and the RNA in cortex and hippocampus of rats were extracted. The levels of NAP-1 mRNA in these samples were estimated by RT-PCR. Results For mice, the expression of NAP-1 were decreased after MPTP treatment in substantia nigra, while its expression showed no significant change in striatum. In contrast, no effect on NAP-1 gene expression by beta-amyloid injection can be detected in cortex and hippocampus of rats. Conclusions The apoptosis of dopaminergic neurons in nigrostriatal pathway after MPTP treatment may be related to the expression of NAP-1 gene. But the mechanism of neurodegeneration after beta-amyloid injection in rats may be different.
10.Epidemiology investigation of nonalcoholic fatty liver disease in Yichang professional crowd and its correlation with the metabolic syndrome

Objective To study the prevalence rate of nonalcoholic fatty liver disease (NAFLD)and its correlation with the metabolic syndrome among professional crowd in Yichang city ,and analyze the characteristic of prevalence of NAFLD .Methods Physical check‐up and liver ultrasonography were done and fasting blood GLU ,TG ,HDL‐C ,UA ,CRP were measured for sampling survey professional people in Yichang city .We sampled 6 450 people in 15 company ,including 3 284 men ,3 166 women(20 to 70 years old) ,the results were analyzed .Results The NAFLD prevalence rate of Yichang professional crowd was 21 .71% ,28 .68% in male and 14 .47% in female respectively(P<0 .01) .The prevalence rate in male was higher than that of female before 60 years old . NAFLD prevalence rate in women showed a trend of increasing along with the age growth ,the incidence rate come up to 31 .31%when women were over 60 years old .The highest prevalence rate of MS related components in NAFLD group were obesity (69 .98% )、high blood TG level(61 .10% ) .Conclusion Male before 60 years old and female over 60 years old of Yichang profes‐sional crowd belong to NAFLD high‐risk groups ,the group should be focused on as regular monitoring ,prevention and interven‐tion .NAFLD prevalence rate significantly increased in people with MS .The most important factors of suffering from NAFLD are o‐besity ,high blood TG level .
