OBJECTIVE:To investigate the effects of Maiqi jiangtang pill combined with insulin on related indicators in mater-nal gestational diabetes. METHODS:135 GDM women were randomly divided into control group (69 cases) and observation group(66 cases). Control group was given diet controlling and exercise therapy,Novolin R insulin biosynthesis and protamine syn-thetic human insulin were injected before meals and bedtime from 4-6 units with the ratio of 4∶2∶3∶1,and decided whether to ad-just the insulin dose based on the blood glucose monitoring results(increase or decrease 2-4 units each times);observation group was additionally given 6 g Maiqi jiangtang pill,4 times a day. The treatment course for both groups was 15 d. Fasting plasma glu-cose(FPG),2 h postprandial glucose(2 h PG),glycated hemoglobin(HbA1c),body mass index(BMI),maternal pregnancy out-comes(preeclampsia,preterm labor, puerperal infection, postpartum hemorrhage, neonatal asphyxia,low-weight children and noc-turnal hypoglycemia)and incidence of adverse reactions in 2 groups were observed. RESULTS:FPG,2 h PG,HbA1c,BMI,pre-eclampsia,preterm labor,low-weight children, neonatal asphyxia and nocturnal hypoglycemia in observation group were signifi-cantly lower than control group, the differences were statistically significant(P<0.05);there were no significant differences in the puerperal infection and postpartum hemorrhage between 2 groups (P>0.05). And there were obvious adverse reactions between 2 groups during treatment. CONCLUSIONS:Maiqi jiangtang pill combined with insulin can effectively reduce the blood glucose of GDM women, with good safety.

Objective To evaluate the effectiveness of immediate quantitative coronary angiography (QCA) analysis in percutaneous coronary intervention (PCI). Methods The parameters of QCA and conventional methods before and after PCI were compared and statistics was performed by using t test or ANOVA methods. Results One hundred and two patients were enrolled in our study. Significant differences between QCA and conventional methods were found in evaluation of lesion length [ ( 22.9 ± 8.9 ) mm vs (24. 8 ± 10. 6) mm,t = 9. 63, P < 0. 05 ], stenosis diameter [ (3.0 ± 0.4 ) mm vs (2. 9 ± 0. 7) mm, t = 6. 31, P < 0. 05 ] and stenosis area [ ( 87. 8 ± 10. 7 ) mm2 vs ( 85.0 ± 12.9 ) mm2, t = 2. 54, P < 0.05 ], and also in different vessels. Stenosis diameter and stenosis area after stenting in target lesion were lower than the international standards. Conclusion Immediate QCA analysis can be effective in directing stent implantation.

BACKGROUND:At present, there is lack of relevant biomechanical model for the T6-T7-T8 rib-vertebral fix unit.In addition, there is no support of parameters of basic studies on reasons and reasonable explanation of screw breakage, poor quality of bone fusion and adjacent segment degeneration.OBJECTIVE:To develop a three-dimensional finite element model of bone graft with vertebral tuberculosis debridement and posterior rib-vertebral unit fixed system through tuberculosis of thoracic spine (T6-8),and to analyze the stress so as to improve it.METHODS:Spiral CT data of one male patient (172 cm,71 kg,39-year-old) with T7 vertebral tuberculosis were imported into computer to develop a three-dimensional finite element model of bone graft with vertebral debridement and posterior vertebral unit fixed system through tuberculosis of T6-8 by Mimics 13.0 and Ansys 11.0 finite element software.500 N pressure and 10 N?m torque were loaded to the vertebral body by 3 kinds of physiological loads which simulated flexion, extension and lateral bending. The stress distribution of fixation devices under different loads was compared.RESULTS AND CONCLUSION: At the positions of anteflexion and extension, the stress mainly concentrated to screw tail, and the stress of upper screw was greater than the middle and lower screws. For connecting rods, the stress of the middle was always less than the lower middle and the middle stress was zero.At lateral bending position, the stresses of upper and middle screw tail were quite, and the unilateral stress of connecting rod was also equivalent. For three different dynamics at the same point, the stress of middle connecting rod increased in the lateral bending motion, and the stress of lower screw tail was equivalent. These data suggested that it is prone to fatigue fracture at upper screw tail by bone graft with vertebral tuberculosis debridement and posterior rib-vertebral unit fixed system through tuberculosis of thoracic spine (T6-8) at the three positions of anteflexion, extension and lateral bending. The lower connecting rod at the positions of anteflexion and extension and the middle connecting rod at the position of lateral bending easily cause fatigue fracture.

Objective:Based on the artificial intelligence (AI) technology in endoscopy and the internet platform, to explore and construct a safe, standardized, scientific and rigorous database for digestive endoscopy, and to provide reference and evidence for the data quality control of AI in digestive endoscopy in China. Methods:After referring to relevant guidelines and standards, data collection and labelling standards of digestive endoscopy of 12 common gastrointestinal diseases were determined. The software of online collection and labelling of multi-center digestive endoscopy data in Shandong Province was developed. Endoscopic equipment with a domestic market share of >5% was used and dozens of experienced endoscopists from 9 medical centers in Shandong Province were uniformly trained for data labelling. From July 2019 to July 2020, the endoscopic examination data from 9 medical centers including Qilu Hospital of Shandong University, Shandong Provincial Hospital , Liaocheng People′s Hospital, Linyi People′s Hospital, Weihai Municipal Hospital, Taian City Central Hospital, Binzhou Medical University Hospital, Yantai Yuhuangding Hospital and Qilu Hospital of Shandong University (Qingdao) were prospectively and continuously collected and labeled. The optimized, desensitized, and generalized data were uploaded to the server. After the file synchronization, data processing, and expert review, a multi-center digestive endoscopy AI database with standard data collection and labelling in Shandong Province was constructed, namely cloud platform. Descriptive methods were used for statistical analysis. Results:The collection and labelling standards for multi-center digestive endoscopy AI data in Shandong province was established. The software of online collection and labelling of multi-center digestive endoscopy AI data in Shandong province was developed. The database in Shandong province was successfully constructed. In the database, 43 010 lesions, 40 353 images, and 11 289 examinations were labeled. Among them, there were 2 906 cases of early esophageal cancer, 2 912 cases of early gastric cancer, 2 397 cases of early colorectal cancer, and 9 773 cases of colorectal polyps (5 539 cases of adenomatous polyps, 1 161 cases of non-adenomatous polyps and 3 073 case of undetermined polyps). Conclusions:The multi-center AI cloud platform for digestive endoscopy in Shandong Province adopts unified standards and collection and labeling software, which ensures the safety and standardization of endoscopy data. It provides a reference and basis for the construction of a quality control system for standardized data collection and labelling of digestive endoscopy AI data in our country and for the third-party data supervision.
