1.A new triterpene glycoside from the sea cucumber Colochirous anceps

Objective To search for triterpene glycoside with new structure. Methods We studied the sea cucumber Colochirous anceps belonging to the family cucumariidea with many chromatography methods and found a new sulfated triterpene glycoside, which structure was established by a combination of spectroscopic analysis (IR, UV, ESI-MS and 2D-NMR), chemical transformations and other chemical evidence. Results and Conclusion The new sulfated triterpene glycoside was isolated and named Colochiroside A (1) .
2.Percutaneous transluminal septal myocardial ablation versus septal myectomy for treatment of patients with hypertrophic obstructive cardiomyopathy: a Meta-analysis

Objective To evaluate the efficacy of percutaneous transluminal septal myocardial ablation (PTSMA) and septal myectomy (SM) for treatment of hypertrophic obstructive cardiomyopathy.Methods We searched all clinical controlled trials of PTSMA and SM in treatment of hypertrophic obstructive cardiomyopathy in Cochrane Library,PubMed,Web of Science,Wanfang Database and CNKI update to August,2012.The selected studies in Chinese and English were used for Meta-analysis with software RevMan 5.0.Results Five studies including 286 cases (PTSMA:149,SM:137) were included in the study.Left ventricular outflow tract pressure was significantly reduced by SM treatment compared with PTSMA (OR =9.07,95% CI:0.96-17.18,P =0.030).There were no significant differences in interventricular septum thickness (OR =1.15,95% CI:-0.79-3.10,P =0.240),left atrial diameter (OR =1.95,95% CI:-0.16-4.07,P =0.070),left ventricular end-diastolic (OR =0.70,95% CI:-0.79-2.19,P =0.360),end systolic diameter (OR =1.14,95 % CI:-1.65-3.93,P =0.420),degree of symptom improvement (chest pain:OR =1.07,95 % CI:0.35-3.23,P =0.910;syncope:OR =0.32,95% CI:0.08-1.23,P =0.100),and postoperative mortality (OR =1.63,95%CI:0.46-5.76,P =0.450) between PTSMA and SM treatment; but the rates of pacemaker implantation (OR =5.91,95% CI:2.04-17.11,P =0.001) and reintervention treatment (OR =6.62,95%CI:1.20-36.62,P=0.030) in patients with PTMSA were significantly higher than those with SM.Conclusions There is no evidence to indicate that PTSMA can completely replace SM,as an alternative choice the therapy can be used according to left ventricular out flow tract pressure,symptoms,anatomy of target vessel and other conditions.
3.Renal sympathetic denervation for treatment of resistant hypertension: a meta-analysis

Objective To systematically evaluate the effectiveness and safety of renal sympathetic denervation (RSD) for treatment of patients with resistant hypertension.Methods English and Chinese literatures of controlled clinical trials on RSD in treatment of resistant hypertension were searched from the Cochrane Library,PubMed,Web of Science,Wanfang Database and CNKI up to February 2013.Metaanalysis was performed with the selected studies by using software Rev.Man 5.0.Results Seven studies involving 354 cases with RSD and 146 controls were included for analysis.Compared with control group,RSD significantly lower both systolic and diastolic blood pressure in patients with resistant hypertension (systolic,1 month:MD =-18.04,95% CI:-19.94--16.14,P <0.01; 3 months:MD =0.01,95%CI:-28.10--17.98,P<0.01; 6 months:MD=-25.59,95%CI:-34.08--17.11,P<0.01; diastolic,1 month:MD=-7.53,95% CI:-8.60--6.45,P<0.01; 3 months:MD =0.01,95%CI:-12.5--4.47,P < 0.01; 6 months:MD =-10.54,95% CI:-16.44--4.63,P =0.000 5).In addition,there was no significant difference in adverse reactions between RSD treatment group and control group (OR =1.13,95 % CI:0.34-3.76,P =0.84).Conclusions Renal sympathetic denervation can effectively reduce blood pressure in patients with resistant hypertension 1-6 months postoperatively,without increase of adverse reactions.The long-term efficacy and safety need to be further observed.
4.Construction of retroviral vector inserted SV40 large T antigen gene and its expression in rat hepatocytes

Objective To construct the retroviral vector inserted SV40 large T antigen gene and transfect it into rat hepatocytes, analyze the status of SV40 large T antigen gene expression in rat hepatocytes, and to establish important basis for clinical hepatocytes transplanation. Methods Retroviral vector inserted SV40 large T antigen gene was(constructed) by DNA recombinant techniques in vitro, then the combinant vector was determined with enzyme(digestion) and sequencing and was transfected into the PA317 cell lines by liposome mediation and screened(anti-G4)18 positive clones. The viral titer was determined with the NIH3T3. After transfected into separated and purified rat primary hepatocytes, the SV40 large T antigen gene expression was detected by PCR and immunohistochemical (methods). Objective To investigate Ureaplasma urealyticum in cervical specimens of gynecologic check-up population and female sex workers(FSW)and its biovars and serotypes.Methods The gynecologic check-up patients and female sex workers were screened using Uu liquid selective medium.The positive cases were then divided into biovars and serotypes by the means of PCR.Results Ninety-eight FSW and164gyne-cologic check-up patients were investigated.In the FSW group,89cases(90.8%)were positive in culture,in-cluding biovar164cases(71.9%),biovar1+27cases(7.9%),biovar218cases(20.2%).In71biovar1positive cases,biovar1+2and co-infection of serotypes of biovar1were defined as mixed infection.Infection with single serotype of biovar1was defined as simple infection.So in71biovar1positive cases of FSW group,there were17mixed infections(23.9%)and54simple infections(76.1%).Simple infections were then subdivided into serotypes,including serotype117cases(31.5%),serotype322cases(40.7%),and serotype615cases(27.8%).In gynecologic check-up group,104cases(63.4%)were culture positive,in-cluding biovar197cases(93.3%),biovar27cases(6.7%).In97biovar1positive cases,there were11mixed infections(11.3%)and86simple infections(88.7%).In86simple infections,there were serotype122cases(25.6%),serotype337cases(43.0%),and serotype627cases(31.4%).Conclusions The posi-tive rate of Uu in FSW is higher than that of gynecologic check-up population(P
6.Changes of Electroencephalogram in Post-stroke Hemiplegia Patients Treated with Electroacupuncture at the Nerve Trunk

Objective: To observe the short-term effects of electroacupuncture at different acupoints on patients with hemiplegia after cerebral infarction. Methods: Patients with cerebral infarction were randomly allocated into a group of electroacupuncture nerve trunk and a group nerve trunk or routine acupoints, respectively, once a day, for 14 d. Results: Electroacupuncture at nerve trunk had better effects than at routine acupoints. The short-term improvement of electroencephalogram had significant differences (P<0.05). Conclusion: Electroacupuncture at the nerve trunk could improve the neurologic impairment and brain function, and it has a better result than routine electroacupuncture.

A 57-year-old man was admitted to the hospital for a 7-year progressively spreading plaques involving the entire body surface, and multiple irregularly sized red nodules and infiltrated patches on the face, trunk and limbs. Histopathological examination showed pleomorphic tumor cells diffusely dis-persed throughout the dennis, giving an appearance of low proliferation. Some cells with cytoplasmic pro-cesses appeared multiangular in shape, lmmunohistochemically, tumor cells were negative for CDla or S-100, but positive for CD45, FXIIIa, CDl4, MHC- Ⅱ, CD68 and lysozyme with extracellular interstitial expression. Ultrastructurally, the cells exhibited cytoplasmic processes and irregularly sized nuclei; no Birbeck granules were observed. Vesicules of low electron-density were seen diffusely in cytoplasm and extracellular matrix. The case is herein diagnosed as cutaneous non-Langerhans cell histiocytosis, which presents with a chronically invasive clinical course. These cells may develop from immature dermal dendritic cells.
9.Safety of a novel biodegradable stent implanted in the coronary artery in a porcine model

BACKGROUND:At present, the main research fields about coronary stents are the whole degradation biological materials with high biocompatibility and drug control ed release systems.
OBJECTIVE: To evaluate the safety after the two novel biodegradable stents implanted in coronary arteries in porcine models.
METHODS:The normal ful y biodegradable stents were made up of the poly-L-lactide and the antiproliferative drugs paclitaxel, and the novel biodegradable stents were added in amorphic calcium phosphate at the basis of normal biodegradable stents. (1) Five normal ful y biodegradable stents were randomly implanted into the coronary arteries of five porcines, and five novel biodegradable stents were randomly implanted into the coronary arteries of the remaining five porcines by coronary angiography. The blood biochemistry and C-reactive protein levels were measured pre-operation and at 28 days after operation. Coronary angiography was utilized to observe the lumen unobstructed at 28 days after surgery. (2) Under a microscope, seven normal ful y biodegradable stents and seven novel biodegradable stents were implanted into right external iliac arteries of 14 rabbits. Blood urea nitrogen and creatinine levels were measured before surgery and at 28 days after operation.
RESULTS AND CONCLUSION:At 28 days after operation, there were no significant changes in porcine glutamic-pyruvic transaminase, aspartate aminotransferase, triacylglycerol, total cholesterol, low density lipoprotein and C-reactive protein levels compared with that before operation, but urea nitrogen and creatinine levels were significantly higher than that before operation (P<0.05). The result of coronary angiography showed that no in-stent thrombosis or stenosis was detected in either group. There was no significant difference in urea nitrogen and creatinine levels in both groups. These results suggested that it is safe and compatible after the two novel biodegradable stents implanted in coronary arteries of porcine models, and the stents had good histocompatibility.
10.The clinical effect of traditional Chinese medicine dialectical therapy for treatment of gastrointestinal dysfunction in patients with acute lung injury/acute respiratory distress syndrome undergoing mechanical ventilation

Objective To explore the clinical effect of traditional Chinese medicine (TCM) dialectical therapy for treatment of gastrointestinal dysfunction in patients with acute lung injury / acute respiratory distress syndrome(ALI/ARDS)undergoing mechanical ventilation. Methods A prospective,randomized controlled trial was conducted. Ninety-six ALI/ARDS patients admitted in intensive care unit(ICU)and treated with mechanical ventilation in Tianjin First Central Hospital were chosen and randomly divided into traditional Chinese medicine(TCM) group and conventional therapy group using a random number table,48 patients in each group. Conventional therapy alone was used in conventional therapy group,and TCM therapy of primarily using Dachengqi decoction combined with conventional therapy was applied in TCM group〔Dachengqi decoction was composed of mongolian milkvetch root 15 g, pilose asiabell toot 15 g,Chinese angelica 10 g,officinal magnolia bark 10 g,tangerine peel 10 g,immature tangerine fruit 10 g,peach seed 10 g,white peony root 12 g,red peony root 12 g,immature bitter orange 6 g,mongolian dandelion herb 30 g,radish seed(stir-fried)30 g,foxtail millet sprout 20 g,barley sprout 20 g,glauber salt 9 g (with water),rhubarb 10 g(added in water at last)〕,one dose orally taken daily for 28 days. The intra-abdominal pressure(IAP),gastrointestinal diseases in TCM symptom score and the incidence of gastrointestinal dysfunction were compared between the two groups before treatment and on the 3rd,6th and 8th day after treatment. Results There were no statistical significant differences in IAP and TCM symptom scores between the two groups before treatment (both P>0.05),but after treatment with the prolongation of therapeutic time the IAP and TCM symptom scores were decreased gradually compared with those before treatment,having reached the valley value on the 18th day and the changes in TCM group were more remarkable〔IAP(mmHg,1 mmHg=0.133 kPa):0.91±0.69 vs. 2.08±0.92, TCM symptom score:48.33±10.41 vs. 88.33±20.21,both P<0.05〕. In the TCM group,the incidences of the untoward symptoms and signs of gastrointestinal dysfunction such as bleeding of stress ulcer,toxic intestinal paralysis, abdominal distension,diarrhea and so on were lower than those in conventional therapy group〔stress ulcer bleeding:16.7%(8/48)vs. 39.6%(19/48),toxic intestinal paralysis:16.7%(8/48)vs. 43.8%(21/48),abdominal distension:10.4%(5/48)vs. 37.5%(18/48),diarrhea:6.3%(3/48)vs. 33.3%(16/48),all P<0.05〕. Conclusion Addition of TCM Dachengqi decoction on conventional treatment can effectively lower the incidence of gastrointestinal dysfunction in treatment of patients with ALI/ARDS undergoing mechanical ventilation.
