1.Application of medical elastic stockings in nursing of patients with deep venous thrombosis form-tion

ObjectiveTo discuss the application of medical elastic stockings in nursing of patients with deep venous thrombosis formation. Methods62 patients diagnosed as acute lower limbs deep ve-nous thrombosis from January, 2005 to January, 2008 were randomly divided into the experimental group and the control groups with 31 patients in each group, the control group took general nursing measures, the experimental group used the medical elastic stockings based on the general care measures, The inci-dence of post- thrombosis syndrome was observed in two groups and underwent χ2 test. ResultsAfter two years follow- up, 2 patients relapsed and 3 patients appeared post- thrombosis syndrome in the experimental group, 8 patients relapsed and 10 patients had post-thrombosis syndrome in the control group. The relapse rate and incidence rate of peat-thrombosis syndrome in the control group were higher than those of the experimental group. ConclusionsThe use of medical elastic stockings can prevent acute lower limbs deep venous throm-bosis, and can reduce the symptoms of post-thrombosis syndrome, which deserves to be spread widely.
2.Role of platelet endothelial cell adhesion molecule-1 in the lung injury caused by mechanical ventilation

ve To investigate the possible role of platelet endothelial cell adhesion molecule-1 (PECAM-1) in the lung injury induced by mechanical ventilation. Methods Twenty-four healthy small pigs of either sex, weighing (17.5?2.5) kg were randomly divided into four groups according to the tida volume of mechanical ventilation: control group without mechanical ventilation; low tidal volume group (A) VT=6 ml?kg-1; normal tidal volume group (B) VT=12ml?kg-1; high tidal volume group (C) VT =25ml?kg-1. The animals were anesthetized with intramuscular ketamine 15-20mg?kg-1 and diazepam 1.5-2.0mg?kg-1 and intubated and mechanically ventilated. Anesthesia was maintained with infusion of ketamine, procaine and succinylcholine. Respiratory rate was set at 18 bpm, I:E=1:2 and FiO2=35% , Femoral artery and vein were cannulated for intra-arterial pressure monitoring, blood sampling and infusion of fluid and anesthetics. ECG, rectal temperature and urine output were monitored. Venous blood samples and lung tissue from right lower lobe were obtained on 1st, 3rd and 7th day of mechanical ventilation for determination of serum and lung myeloperoxidase (MPO) activities, expression of PECAM-1 protein on pulmonary vascular endothelium cells and electron microscopic examination. 3.Measurement and correlation of electrolytes in EPS and urine of patients with chronic prostatitis

Objective To measure the concentration of electrolytes such as potassium,sodium,chloride,calcium in EPS and urine of patients with chronic prostatitis and to analyze the correlation of the electrolytes in different patient groups. Methods The concentrations of potassium,sodium,chloride and calcium in EPS and urine of 31 controls and 79 patients with prostatitis were measured and analyzed. Results There was no significant difference between the patients and normal controls in electrolytical concentration of potassium,sodium,calcium except chloride (P=0.001).The concentration of chloride in the patients was significantly higher than that in the normal controls.In the treatment effective group, potassium concentrations were decreased from 40.66?17.10 to 33.42?17.27 mmol/L after treatment.In the treatment ineffective group, potassium concentrations were (37.57?16.93)mmol/L before treatment and 50.66?18.77 mmol/L after treatment.Potassium concentrations were 36.02?12.36 and 48.90?16.93 mmol/L in the pain group and painless group,respectively.There were positive correlations between the concentrations of potassium and calcium of EPS,sodium and chloride of urine as well. Conclusions The concentrations of potassium in EPS are obviously different between the effective group and ineffective group or the pain group and painless group.There is a close relationship between the concentration of potassium and calcium of EPS,sodium and chloride of urine; and they are positively correlated.

5.Microsurgery for acoustic neuroma and the facial-acoustic nerve protection

Objective To explore the protection of the facial-acoustic nerve during microsurgery for acoustic neuroma via the suboccipital retrosigmoid approach. Methods The clinical data of 25 cases with acoustic neurinoma treated by the suboccipital retrosigmoid approach were retrospectively analyzed. Results Total removal was achieved in 22 patients and subtotal in 3 patients. 20 with Cochlear nerve anatomic preservation at 2 weeks after surgery and 3 patients with effective hearing and 13 patients lost effective hearing but retained measurable hearing and 9 patients completely lost hearing( the New Hannover Classification V grade). The facial nerve was preserved anatomically in 18,H-B Grade I ~ IV in 15,Grade V ~ VI in 10. There were not cerebrospinal fluid leakage, intracranial infection and death in all cases. Conclusion When operating,the relation between the tumor and the important structure surrounding could should be understood fully. Then removed the tumor piece by piece with microsurgical techniques. That the effective way to keep the facial nerve and its function being protected,and could improve the prognosis of the operation.
6.The treatment of patients with ruptured intracranial aneurysms of hunt-hess Ⅲ-Ⅳ grade in the cerebral vasospasm stage

Objective To study the reasonable treatment to the patients with raptured intracranial aneurysms of hunt-hess Ⅲ-IV grade in the cerebral vasospasm stage. Methods The clinical data of 80 patients with ruptured intracranial aneurysms of hunt-hess Ⅲ-IV grade were analyzed retrospectively. All of the patients were clipped closed by microsurgery (36) or embolized intervention(44) in 3~18 days after disease. According to the time of surgical intervention after disease,they were divided into 42 patients with early(3~10 days) surgical group and 38 patients with advanced(11~18 days) surgical group. The different operative methods and different treatment periods were compared in order to evaluate the effect to the prognosis, the DIND incidence and the ruptured again aneurysms during operation incidence. Results There was no statistical significance in the effect about early or advanced surgical intervention to the prognosis, the DIND incidence and the ruptured again aneurysms during operation incidence (P > 0.05). There was no statistical significance in the effect about advanced interventional embolization or clipping closed by microsurgery to the prognosis,the DIND incidence and the ruptured again aneurysms during operation incidence(P > 0. 05). There was statistical significance in the effect about early interventional embolization or clipping closed by microsurgery to the prognosis,the DIND incidence and the ruptured again aneurysms during operation incidence(P< 0.05). The interventional embolization had better prognosis,more lower DIND incidence and more lower ruptured a-gain aneurysms during operation incidence than the clipping closed by microsurgery. Conclusion The patients with ruptured intracranial aneurysms of hunt-hess Ⅲ-IV grade in the cerebral vasospasm stage should be earlier intervened surgically,then it seems that interventional embolization is more reasonable which is worth referencing in clinic. The good operation habits during operation and the effectively strengthening management after operation had positive significance to ensure operative effect and to improve prognosis.
7.Cause analysis of intracranial hemorrhage during operation on patients with brain injury

Objective To study the possible reasons and the processing strategy of brain injury operation complicated with intracranial hemorrhage.Methods The clinical data about 22 patients who were digged skull operation because of brain injury and complicated with intracranial hemorrhage in operation in neurosurgery from 2007 June to 2008 February were retrospectively analyzed,and different types intracranial hemorrhage were classified.Results In 22 patients,12 patients were complicated with epidural hematoma,in which the patients (9/12,75.0%) complicated with opposite hematoma were common;5 patients were complicated with intracerebral hematoma,in which the patients (3/5,60%) complicated with ipsilateral hematoma were common;4 patients were complicated with acute subdural hematoma,and all the patients complicated with opposite hematoma;1 patient was complicated with sub-flap hematoma.Conclusions In the patients who are complicated with intracranial hemorrhage because of digging skull operation to brain injury,epidural hematoma are most,then intracerebral hematoma,subdural hematoma,sub-flap hematoma.The important measures of preventing secondary intracranial hemorrhage are definiting force mechanism and distinguishing fracture line before operation,gradiently decompressing,completely removing crushing brain tissue,protecting reflux veins,rigorously stopping bleeding during operation.
8.Comparison of the incidences of apical root cracks after canal preparation with two nickel-titanium rotary systems:an in vitro study

Objective: To compare the incidences of root cracks after canal instrumentation with HyFlex CM system and the ProTaper Universal system .Methods:Sixty mandibular incisors were mounted in resin blocks with simulated periodontal ligaments , and the apex was exposed .The control group of 20 teeth was not prepared , and the other 40 teeth were divided into 2 experimental groups ( n=20 ) .The 40 root canals of the experimental groups were instrumented using HyFlex CM and ProTaper Universal to the major apical foramen ( AF).The horizontal sections 1 mm, 2 mm, and 3 mm from the apex were observed under an optical stereomicroscope at 25 ×magnification .The presence of cracks was noted . Results:No cracks were found in the control teeth .Cracks were found in 1 of 20 ( 5%) teeth in HyFlex CM group , and 17 of 20 ( 85%) teeth in ProTaper Universal group .The difference between the two experimental groups was statistically significant (P<0.01).Conclusion: The HyFlex CM files caused fewer root cracks than the ProTaper Universal files during the root canal instrumentation .
9.Comparison of periapical radiography and cone-beam computed tomography in en-dodontic treated teeth for assessment of periapical lesions

Objective:To compare the assessments of the presence or absence and the changes of post-treatment periapical lesions using periapical radiographs versus cone-beam computed tomography (CBCT).Methods:Imaging records were selected from a sample of 233 radiographic examinations (in-cluding periapical radiographs and CBCT)of patients with chronic apical periododontitis who received treatment and review in Department of Cariology and Endodontology of Peking University School of Stoma-tology.After testing the inter-observer and intra-observer agreement,two observers determined the pre-sence or absence of a periapical lesion and classified the changes of lesions in 4 categories:absence,re-duction or enlargement of lesion,or uncertain.The differences between CBCT and periapical radiographs in detecting the presence or absence and the changes of periapical lesions were compared using McNemar test and Fisher’s exact test respectively.Results:In the study,233 teeth were assessed with both periapi-cal radiographs and CBCT images.Periapical lesions were detected in 178 teeth on CBCT scans and in 146 teeth on periapical radiographs.CBCT images revealed a significantly larger number of lesions than were revealed by periapical radiographs (P <0.01 ).There was disagreement on the changes of lesions between the two radiological technologies in 25% teeth,and the difference was statistically significant (P <0.01 ). Conclusion:Significant differences were observed between periapical radiographs and CBCT in detection of post-treatment periapical lesions and assessment of the changes of periapical lesions.
10.The research of methotrexate in treating systemic lupus erythematosus

Objective To investigate the curative effect of methotrexate in treating systemic lupus erythema-tosus(SLE). Methods The chosen outpatients were divided into 2 groups,group A(28 cases) :MTX combined with small dosage of prednisone (prednlsone 10mg/d) ; group B (20 cuses) : medium dosage of prednisone (prednisone 30rag/d). Observation includes SLE disease activity index(SLEDAI) ,relapse while dosage of prednisone was reduced to small amount and drug side effects. Results The SLEDAI score of group A was reduced from (9.4±2.1) to (4.1±0.9) ,and group B from (8.6±2.5) to (4.0±1.0). Compared the result of before and after treatment be-twcen the two groups, the curative effect is obvious (P<0.01). However, there was no significant difference of efficacy between the two groups(P>0.25). While dosage of prednisone was reduced to 10mg/d ,there were 8 cases of relapse in group B,and only 3 cases in group A during the entire observation period. The difference is significant (P<0.025). The side effect of group A is gastrointestinal reaction and for B is mainly the cushing syndrome. Conclusion MTX is an effective treatment fw the light and medium activity SLE,and can reduce the dose of prednisone, prevent long-term using large dose of prednisone which will cause side effect.
