1.Prevention of acute exacerbations of chronic obstructive pulmonary disease

Objective Acute exacerbations of chronic obstructive pulmonary disease (AECOPDs) are significant events which are associated with worsened quality of life and more rapid decline in lung function.And they also can increase mortality and health care costs.Pharmacologic interventions can alter the frequency of AECOPDs and COPD-related hospitalizations, mainly by focusing on three ways: antiinfective agents, antiinflammatory agents and antioxidants which also become the chief clue to the current researches for new drugs and therapies.The combination therapy of inhaled corticosteroids and long-acting β2-adrenoceptor agonists is the most widely recommended strategy for acute exacerbation prevention.But the inhaled corticosteroids possibly increase the risk for pneumonia.Further studies are needed to understand the mechanism of benefit in patient subpopulations, and to compare between efficacy of combination therapy and side-effect with inhaled corticosteroids, bronchodilators, or their combination in different patient phenotypes.We must have a better understanding of the heterogeneity implicit for COPD and the types of COPD exacerbations, and individualized treatments need to be investigated.
2.Detection of HPV DNA in Condylomata Acuminata by Direct in Situ PCR

The purpose of this study is to ascertain the significance of diagnosis of human papillomavirus (HPV) infection and the histological pattern of distriution of HPV in condylomata acuminata(CA) by direct in situ PCR (ISPCR) technique. of the 25 patients with BA, all 10 cases with typical histopathological features of CA gave a positive reaction in tile upper layers of the epithelium. HPV was also detected in the granular ladyer and whole prickle cell layer of 11 cases of CA with no typical koilocytes. In the direct ISPCR oligonucleotides labelled with bio tin were incorporated in situ during amplification, then immunohistochemical method was used for detection. In this technique the viral existence in paraffin sections can be directly compared with the histologic changes, and can help pathologist to analyze the possibility that HPV is present in the suspected lesions.
3.Predicting the potential of myocardial recovery after left ventricular assist

Objective: This study was undertaken to explore the effect of left ventricular assist device (LVAD) on failing heart after myocardial ischemia. By detecting the changes of plasma atrial natriuretic peptide (ANP), brain natriuretic peptide (BNP) and cardiac troponin I (cTnI) levels within 6 hours after the implantation of LVAD, we review the basis on which neurohormones may be used to determine prognoses of failing heart and choose an optimal predictor. Methods: 15 adult healthy dogs were divided into 3 groups randomly. The LVAD was implanted in LA-AO bypass in all three groups. Myocardial ischemia was induced by ligating the main left anterior descending coronary artery (LAD). In group A, after a ligation of 10 minutes, the myocardium was reperfused for 6 hours. In group B, after a ligation of 40 minutes, the myocardium was reperfused for 6 hours. In group C, after a ligation of 40 minutes, the LVAD was used for LV support for 6 hours. Results: After 6 hours reperfusion,in group C, the hemodynamics was significantly improved, the ANP, BNP and cTnI were return to normal level, and myocardial ultrastructure was recovered significantly. While in group B, the hemodynamic, the neurohormones, and myocardial ultrastructure were worse. Relational analysis demonstrated that ANP and cTnI levels were influenced by hemodynamics obviously, but there was a weak relationship between circulating BNP and hemodynamics. Plasma BNP level was able to identify the cardiac function status. Conclusion: LVAD can be beneficial to improve cardiac function and can reduce the plasma levels of ANP, BNP and cTnI. Plasma BNP level can identify the cardiac function status. Those findings indicated that plasma BNP determination provides important prognostic information about cardiac function and may be a better prognostic indicator.
4.The analysis of the effect of the joint method containing salvia tetramethylpyrazine,trimetazidine, alprostadil in the treatment of unstable angina

Objective To evaluate the feasibility of the joint method containing salvia tetramethylpyrazine,trimetazidine,alprostadil in the treatment of unstable angina.Methods 70 unstable angina patients met the inclusion criteria were randomly divided into the observation group of 35 cases and the control group of 35 cases.The control group was given trimetazidine 20mg,tid,and alprostadil 100mg,1 qd.The observation group were given salvia tetramethylpyrazine 10ml on the basis of the control group,qd.Two groups were treated for 14d.The frequency and duration of angina attack was observed.The ECG was monitored.Clinical efficacy differences was compared before and after treatment.Results The total effective rate of the observation group was 100%.The effective rate were 86%.The control group were respectively 91% and 60%.The total effective rate of observation group was significantly higher than the control group(x2 =6.838,P < 0.05).The effective rate of observation group was significantly higher than the control group (x2 =5.851,P < 0.0 5).The angina frequency,angina duration,stroke output quantity (SV),eiectionfraction(EF) of the observation group and the control group after treatment improved significantly than before treatment,after the treatment the angina frequency,angina duration,stroke output quantity (SV),ejection fraction (EF) of the observation group and the control group were respectively (2.69 ± 0.46) time/min vs (6.46 ± 0.62) time/min,(3.26 ± 0.32) min vs (5.54 ± 0.42) min,(49.7 ± 3.4) ml vs (43.2 ± 2.8) ml,(69.2 ± 5.9) % vs (55.3 ±4.8) %,the angina frequency,angina duration of the observation group was significantly lower than the control group,and the stroke output quantity(SV),ejection fraction(EF) of the observation group were significantly higher than the control group(all P < 0.05).Condusion The joint method containing salvia tetramethylpyrazine,trimetazidine,alprostadil have exact clinical efficacy,reduce the time of angina attack frequency and duration,effectively improve myocardial blood supply,improve heart function,which is worthy of clinical use.
5.Study on bone metobolism change in thirty hyperthyroidism patients

Objective:To study the effect of thyroid function on bone metabolism. Methods:Serum FT3,FT4 were investigated by radioimmunoassay (RIA) and bone mineral density of spine (L2~4) weremeasured by dual energy x-ray absorptinmetry and other markers related to bone metabolism were alsomonitored in 30 patients with hyperthyroidism and 30 healthy volunteers. Results :The levels of FT3,FT4,ALP were significantly higher than those of the normal controls. BMD of spine decreased significantly incomparison with the controls ,and the degree of severity and incidence increased with age. Conclusion:Thy-roid hormone might speed up bone turnover directly with increased bone resorption to induce bone massloss.
6.Effects of hydrogen-rich saline given during reperfusion on global cerebral ischemia-reperfusion injury in rats

ObjectiveTo evaluate the effect of hydrogen-rich saline given during reperfusion on global cerebral ischeraia-reperfusion (I/R) injury in rats.Methods Seventy-two adult male SD rats,aged 2.0-2.5 months,weighing 260-300 g,were randomly divided into 3 groups (n = 24 each):sham operation group (group S),group I/R and hydrogen-rich saline group (group H).In groups I/R and H cerebral I/R was induced by occlusion of 4 vessels( cauterization of bilateral carotid arteries and 15 min occlusion of bilateral common carotid arteries).In group H intraperitoneal 0.6 mmol/L hydrogen-rich saline 5 ml/kg was injected at 6 h of reperfusion,while equal volume of normal saline was injected instead of hydrogen-rich saline.Eighteen rats of each group were sacririced at 24 h of reperfusion,and then the hippocampi were removed for determination of malondialdehyde (MDA),tumor necrosis factor-α (TNF-α),interleukin-6 (IL-6)contents,and nuclear factor-κB (NF-κB) activity and activated caspase-3 expression.Another six rats of each group were sacrificed at 72 h of reperfusion,and then brain tissues were removed for microscopic examination and counting the number of uninjured pyramidal cells in hippocampal CA1 region.ResultsCompared with S group,the contents of MDA,TNF-α,IL-6 and NF-κB activity were significantly increased,activated caspase-3 expression was significantly up-regulated,uninjured pyramidal cells in hippocampal CA1 region were significantly decreased in I/R group( P < 0.05).Hydrogen-rich saline given during reperfusion attenuated the above-mentioned I/R-induced changes( P < 0.05 ).The histologic damage of the hippocampal CA1 region was significantly slighter in group H than group I/R.ConclusionHydrogen-rich saline given during reperfusion can reduce global cerebral I/R injury in rats through inhibition of lipid peroxidation,inflammatory response and apoptosis.
7.Common deafness gene mutations of non-syndromic hearing loss in Liaoning.

OBJECTIVE:
Investigate common deafness gene mutations in patients with severe and profound non-syndromic hearing loss in Liaoning in order to understand their hereditary etiologies and characteristics at the molecular level.
METHOD:
Peripheral blood samples were obtained and the DNA templates were extracted from 128 non-syndromic hearing loss patients who are sporadic in clinics. The deafness gene chip was applied to detect hot-spot deafness gene mutations including GJB2, GJB3, SLC26A4 and mitochondrial 12S rRNA. Deafness etiology questionnaires, pure tone audiometry, auditory brainstem response, tympanometry and temporal bone CT were also applied.
RESULT:
Various types of gene locus mutations were seen in 52 of the 128 patients (40.6%); (1) GJB2 gene mutations (n=22) included c. 235 del C homozygous mutation (n=10), c. 235 del C heterozygous mutation (n=5); c. 176_191 del 16 heterozygous mutation (n=l); c 35 del G heterozygous mutation (n=l); c. 235 del C/c. 299_300 del AT mutation (n=l), c. 235 del C/c. 176_191 del 16 mutation (n=l), c. 35 del G/c. 176_191 del 16 mutation (n=l); c. 299_300 del AT/c. 919-2 A>G mutation (n=l), c. 235 del C/c. 919-2 A>G mutation (n=l). (2) SLC26A4 gene mutations (n=30) included c. 919-2 A>G homozygous mutation (n=6), c. 919-2 A>G heterozygous mutation (n=17), c. 2168 A>G homozygous mutation (n=l), c. 2168 A>G heterozygous mutation (n=2), c. 2168 A>G/c. 919-2 A>G mutation (n=2), c. 919-2 A>G/GJB2 c. 235 del C mutation (n=2); (3) No GJB3 and mitochondrial 12S rRNA mutation. Genetic deafness was confirmed at the gene level in 24 cases (18.8%) and 28 patients (21.9%) were diagnosed as carriers of genetic deafness gene mutations.
CONCLUSION
Genetic deafness occupies a large population in deaf community in Liaoning. Molecular genetic screening for these mutations and genetic counseling are effective methods to prevent the occurrence of hereditary hearing loss and provide theoretical guidance.

8.Enhanced chemosensitivity of Hep-2 through down-regulating expression of SOX2 by RNAi.

OBJECTIVE:
To investigate the effect of SOX2 on chemotherapy sensitivity of human laryngeal epithelial cells Hep-2.
METHOD:
We designed and synthesized RNAis for silencing the expression of SOX2 in Hep-2 cells and selected the most effective RNAi by Western blot analysis. Then the recombinant plasmids of pGCsi-H1-SOX2 and pGCsi-H1-NC were constructed and transfected into Hep-2 cells to build cell lines of psiSOX2-Hep-2 and psiNC-Hep-2. CCK-8 assay had been used to test the sensitivity of Hep-2 cells to 5-FU and PTX after silencing SOX2 expression. Hoechst staining had been used to exam the changes of Hep-2 cells apoptosis treatment by 5-FU and PTX after silencing SOX2 expression. Furthermore, the changes of apoptosis-related genes expressions were detected by Western blotting.
RESULT:
The cell lines of psiSOX2-Hep-2 and psiNC-Hep-2 were successfully established, and the expression of SOX2 protein was decreased 78% in psiSOX2-Hep-2 cells compared with psiNC-Hep-2 cells. After reducing SOX2 expression, the sensitivity of Hep-2 cells to 5-FU and PTX were increased and the IC50 values for 48 h were decreased to 8.12 μg/ml and 5.16 μg/ml. Meanwhile, the apoptosis rate and the expression of apoptotic gene Bax and cleaved caspase-3 expression were dramatically increased and anti-apoptotic genes survivin and Bcl-2 were significantly decreased in psiSOX2-Hep-2 cells compared with psiNC-Hep-2 cells.
CONCLUSION
Down-regulating the protein expression of SOX2 by RNAi will significantly enhance the sensitivity of human laryngeal epithelial cells Hep-2 to 5-FU and PTX.

9.Study on correlation between intestinal flora characteristic and insulin resistance

Objective To explore the correlation between the change of intestinal flora and insulin resistance in pre-diabetes population,and the role of intestinal flora in the occurrence and development in the pre-diabetes population.Methods two hundreds and fifty cases of pre-diabetes in our hospital were selected and divided into the group A(pre-diabetes intervention group)and B(prediabetes non-treatment group).Fifty cases of diabetes(positive control group)served as the group C.The cohort study was adopted.The follow-up intervention lasted for two years.The intestinal flora and blood biochemical indicators were detected in different groups and at different time periods.The insulin resistance index was calculated.Results The total bacterial count before intervention in the group A and B was significantly higher than that in the group C;Enterococcus genus in the group C was significantly higher than that in the group A and B;the proteus and lactobacillus content in the group A and B was higher than that in the group C,but the difference was not significant;Bifidobacterium genus in the group C was higher than that in the group A and B.After intervention,the total bacterial amount in the group Awas significantly higher than the group B and C,but there was no statistical difference between the group B and C;enterococcus genus in the group C was significantly higher than that in the group A and B,enterococcus genus after intervention in the group A was reduced and which in the group B was increased,the difference had statistical significance;the proteus and lactobacillus content in the group A and B was higher than that in the group C,but the difference was not significant;bifidobacterium genus in the group C was higher than that in the group A and B,moreover which after intervention in the group B was increased,the difference was statistically significant.The insulin resistance before processing had no statistical difference between the groups A and B,which in the group C was higher than the group A and B,the insulin resistance after intervention in the group A was decreased,the difference was statistically significant.The total bacterial count,enterococcus and bifidobacterium were positively correlated with insulin resistance,and bacteroides,proteus and lactobacillus had no significant correlation with insulin resistance.Conclusion Intestinal flora has correlation with insulin resistance,and serves as a marker of pre-diabetes development,implementing the probiotics adjustment combined the quantization aerobic muscular movement can realize the adjustment of insulin sensitivity and improves the diabetic development in the prediabetic population.
10.Four bacterial studies on children with chronic otitis media with effusion.

OBJECTIVE:
To research the prevalences of four kinds of bacteria including Alloiococcus otitidis, Streptococcus pneumonia, Haemophilus influenzae, and Moraxella catarrhalis in children with chronic otitis media with effusion (SOM) of the middle ear effusion, and the reproduction of the nasopharynx, so as to explore their meaning for the children with SOM.
METHOD:
Alloiococcus otitidis, Streptococcus pneumonia, Haemophilus influenza, and Moraxella catarrhal were investigated in the samples obtained from middle ear effusion and nasopharyn- geal swabs, using PCR and conventional bacterial culture methods.
RESULT:
By bacterial culture, the pathogen detection rate from middle ear effusion was 3.6%,while the nasopharynx was 54.0%, the detection rate of Streptococcus pneumonia, Haemophilus influenza, Moraxella catarrhalis was 10.8%, 27.0%, 4.5%, respectively, the drug susceptibility results for 51 samples of bacterial culture positive showed that 39 cases was sensitivite to the β-lactam antibiotic; By PCR, the number of detecting various kinds of bacteria simultaneously in middle ear effusion or in the nasopharynx were 6 and 34. The bacteria prevalences of S. pneumoniae, H. influenzae, M. catarrhalis, and A. otitidis are 5.4%, 5.4%, 3.6%, and 42.3% in the middle ear effusion, are 25.2%, 27.0%,13.5% and 34.2% in nasopharyngeal, respectively.
CONCLUSION
(1) PCR method is more sensitively detecting the bacteria than conventional bacterial culture methods. (2) The chronic SOM of children may be a combination of mixed bacterial infection, A. otitidis may be the most common pathogen of children SOM. (3) For children of SOM, if antibiotics are chosen to be used early in the disease, we suggest using the β-lactam antibiotics.
