2.Comparative study on two types of digestive reconstruction after total gastrectomy in patients with gastric carcinoma

Objective To compare two types of digestive reconstruction and postoperative complications after total gastrectomy in patients with gastric carcinoma. Methods Roux-en-Y esophagojejunostomy and P-type Roux-en-Y esophagojejunostomy were performed in 50 patients with gastric carcinoma after total gastrectomy. The operating time and postoperative complications were observed. Results The gastrointestinal symptoms for Roux-en-Y esophagojeju-nostomy and P-type Roux-en-Y esophagojejunostomy serve as : diarrhea ( 11.8% and 12.1% ), dumping syndrome (3.9% and 3.3% ) and reflux esophagitis(5.9% and 6.0% ) ,the incidence of the difference was not significant ( than,P>0.05 ). The postoperative complications in P-type Roux-en-Y esophagojejunostomy( 13.1% ) slightly higher that Roux-en-Y esophagojejunostomy( 11.1% ) ,but the difference was not significant( P>0.05 ). Roux-en-Y esoph-agojejunostomy average operative time(3.1±0.3)h was significantly reduced than P-type Roux-en-Y esophagojeju-nostomy(3.7±0.2)h(t=3.35,P<0.05). Conclusion Roux-en-Y esephagojejunostomy and P-type Roux-en-Y esophagojejunostomy is a suitable type after total gastrectomy for patients with gastric carcinoma, but the former opera-tive time was shorter.
3.Relationship between operative procedures and complication of colorectal carcinoma with acute colonic obstruction in the elderly patients

Objective To study the relationship between operative procedures and complication of colorectal carcinoma with acute colonic obstruction in the elderly patients. Methods 65 elderly patients undergoing emergency operation for acute colonic obstruction were analyzed retrospectively. One stage tumor resection and anastomosis was performed in 25 cases with right-side colonic lesion and in 26 cases with left-side colonic lesion, Hartmann operation in 8 cases, Dixon operation in 4 cases and proximal colostomy in 2 cases. Results In 65 cases, 13 cases of postopera-tive complications,the rate was 20.0% (13/65) ,which infected with toxic shock two cases,four cases of pulmonary infection,anastomotic leakage two cases, wound infection, split 2 cases, death three cases, the mortality rate was 4.6%. The rates of postoperative complications in right-side colonic lesion, left-side colonic lesion, Hartmann opera-tion and Dixon operation were 69.2% ,0,30.7% and 0 respectively(P < 0.05 ~ 0.01). Conclusion Acute obstruc-tion of colon cancer,the right-side colonic lesion were safe,feasible and few complications in colorectal carcinoma with acute colonic obstruction.

Objective To investigate the degree of health knowledge and needs for health education of urban and rural inhabitants in Tianjin, and to study strategy of health education and health promotion in community. Methods Using a questionnaire survey, 900 inhabitants were investigated on their health knowledge, behavior, access to knowledge, and health education needs. Results The rate of health knowledge awareness was 65.63%, the rate of health promotion was 67.05%. Objective: To compare the incidences of root cracks after canal instrumentation with HyFlex CM system and the ProTaper Universal system .Methods:Sixty mandibular incisors were mounted in resin blocks with simulated periodontal ligaments , and the apex was exposed .The control group of 20 teeth was not prepared , and the other 40 teeth were divided into 2 experimental groups ( n=20 ) .The 40 root canals of the experimental groups were instrumented using HyFlex CM and ProTaper Universal to the major apical foramen ( AF).The horizontal sections 1 mm, 2 mm, and 3 mm from the apex were observed under an optical stereomicroscope at 25 ×magnification .The presence of cracks was noted . Results:No cracks were found in the control teeth .Cracks were found in 1 of 20 ( 5%) teeth in HyFlex CM group , and 17 of 20 ( 85%) teeth in ProTaper Universal group .The difference between the two experimental groups was statistically significant (P<0.01).Conclusion: The HyFlex CM files caused fewer root cracks than the ProTaper Universal files during the root canal instrumentation .
7.Detection and distribution characteristics of serological markers of hepatitis B in neonates, infants and preschool children in Zhejiang province

Objective To investigate the serological patterns of hepatitis B, understand the prevalence of hepatitis B infection in neonates, infants and preschool children based on hospital data in Zhejiang province and provide a basis for prevention and control of local pediatric hepatitis B infection. Methods Five serological markers were detected on electrochemiluminescence analyzer Roche E601. The results of all serological markers in children with hepatitis B from 2006 to 2010 were exported from laboratory information system and 24 892 (15 810 boys and 9 082 girls) cases were selected for further analysis according to the exclusion criterion including duplicate analysis, children older than 7 years and children living out of Zhejiang province. SPSS13.0 software and EXCEL 2003 were employed for statistical analysis in this study. Results Average HBV vaccination coverage rates among 15 413 boys and 8 840 girls were 99. 16%(98. 87% -99. 45%) and 99. 17% (98. 29% -99. 53%), respectively. Seventeen serological patterns of hepatitis B were observed, and "anti-HBs(+)alone"(64. 997%) was the dominant patterns followed by "negative" (18. 231%), "anti-HBc(+)anti-HBs(+)"(9.911%) and "anti-HBe(+)antiHBc(+) anti-HBs(+)"(4.395%), and the positive rates for other patters were 0. 004% -0. 992%. There were significant difference of detection rates for "anti-HBc(+)alone" and "anti-HBe(+)anti-HBs(+)"patterns between male and female (χ2=7.143,8.123, all P<0.01), and there were no significant difference of detection rates for the other 15 patterns between male and female (all P >0.05). Ninety-four HBsAg positive cases were detected, accounting for 0.38%. Totally 19 899 anti-HBs positive cases were detected, accounting for 79.94%. Totally 16 187 cases were "anti-HBs(+) alone", accounting for 81.3%of the total anti-HBs(+) cases. The group of 1-2 years and 2 day-2 week had the highest and lowest rate for anti-HBs(+)alone respectively. The ratios in three groups with different anti-HBs levels (10-100 mIU/ml,100-1000 mIU/ml and > 1000 mIU/ml) in 16 178 anti-HBs(+) children were 23. 64%, 27.66% and 13. 70%, respectively. Totally 3 904 cases with anti-HBc(+) were detected, accounting for 15.68%. The highest positivity rate and lowest positivity rate were in the group of 2-4 week and 3-4 week respectively. In total 1 565 cases with negative results, the lowest negativity rate and highest negativity rate were in the group of 9-12 month (3. 25%) and 6-7 age (36. 21%) respectively. Conclusions The dominant serological patterns of hepatitis B in local young children is "anti-HBs (+) alone" and most of the anti-HBs concentrations ranged from 100 to 1000 mIU/ml, followed by "negative". The total positive rate of HBsAg is mild. The rate of hepatitis B serological patterns in children is associated with ages but independent of gender. For interpretation of serological patterns of HBV in children below 12 months (especially in 3 months) the status of their mothers' HBV infection should also be taken into consideration.
8.Aquaporins in lung

Aquaporins (AQPs) is a novel family of membrane protein which is in charge of the transportation of water across the membranes. It has been testified that AQPs may be related to many physiological functions and pathophysiological disorders. In lung, 6 subtypes of AQPs were found, 4 of which have been located in certain tissue. AQP1, AQP3, AQP4 and AQP5 are located in upper respiratory tract and may contribute to the mucus production, bronchial moisturizing and gland secretion. AQP1 and AQP5 are located in lower respiratory tract and may participate in directing the water movement across the blood-gas barrier. AQPs in natal lung may be an important factor to accelerate the reabsorption of the lung liquid. In adult, some diseases such as asthma, pulmonary edema, adult respiratory distress syndrome, which have been considered unbalanced water movement, may be also concerned with AQPs. The relationship between lung edema and AQPs have not been elucidated in details. It should be verified how AQPs work in more organs and tissues as well as in some diseases.
9.Analysis correlating clinical features and prognosis in the resected N2 non-small cell lung cancer

Objective Non-small-cell lung cancer with ipsilateral mediastinal lymph node metastasis (N2) belongs to heterogeneous subgroup. We analyzed the prognosis of patients with resected N2 NSCLC to determine the clinical significance. Methods The present study comprised 146 consecutive patients whom underwent surgical resection of N2 NSCLC between January 1997 and January 2000. Histological type, location, T primary tumor status, operation mode, clinical N2 (cN2) factor, N2 level(single or multiple), number of positive nodes(N2-num), and the cycles of adjuvant chemotherapy were estimated from the date of operation using the Kaplan-Meier and Log Rank analysis. The Cox regression model evaluated the influence of factors on the survival. Results The 3-and 5-year survival rate of these N2 NSCLC patients were 19.86% and 14.56%, respectively. The patients with tumor in the right lower lobe showed a significantly longer survival than left lobes. The histological type, tumor location, cN2 factor, N2 level and N2-number were associated with survival. A multivariate analysis using Cox regression identified 4 factors of prognosis: tumor site, T status, N2 level and clinical N2 status. Conclusion This article has identified N2 NSCLC subgroups and found that patients with mN2, N2L1 and single N2 have better prognosis.
10.Comparison of local and systemic application of ibandronate on fracture healing in rabbits

BACKGROUND:Ibandronate is the typical representative of bisphosphonates drugs,which prevent bone absorption. OBJECTIVE:To verify the effect of Ibandronate on fracture healing after local application. DESIGN,TIME AND SETTING:A randomized control experiment was accomplished in the Medicine Center Laboratory of Southeast University in October,2008. MATERIALS:Thirty-six healthy New Zealand rabbits,aged 6-8 month,weighting 2.1-2.6 kg. Ibandronate injection(1 mL:1 mg) was supplied by Biomedical Engineering Centre of Hebei Medical University. METHODS:Thirty-six healthy New Zealand rabbits were randomly divided into the control,systemic application and the local application groups,with 12 rabbits in each group. All the rabbits were prepared fracture healing models. Rabbits in control group were given no more treatment after model preparation. Animals in the systemic application control group was given 0.1 mg Ibandronate injection through vein or locally. The state of fracture healing were examined and then scored by X-ray at week 2,4,6 and 8 after model preparation. At weeks 1,2,3 and 4,the level of serum tartrate-resistant acid phosphatase 5b(TRAP-5b) were assayed with ELISA. At weeks 2,4,6 and 8,three rabbits were selected from each group randomly and were killed by aeroembolism respectively. Their calluses were intercepted from fracture sites as the research objects. Pathological changes were observed in callus samples and immunohistochemistry stain was performed to examine type I collagen expression in tissues of callus. MAIN OUTCOME MEASURES:①Fracture healing scores. ②The level changes of TRAP. ③The percentage of bone trabecula area. ④Osteoblast index. ⑤Absorbance value.
