1.Clinical characteristic and therapy analysis of testicular germ cell tumour:45 cases report

Objective To investigate the diagnosis and therapy of testicular germ cell tumour.Methods The clinical data of 45 cases that were confirmed by pathology testicular germ cell tumour were retrospectively analyzed.Results All 45 cases were resected successfully.Among of them,28 cases seminoma,3 cases teratoma,2 cases embryoma,2 cases choriocarcinoma,4 cases mixed testicular germ cell tumour,5 cases yolk sac tumor,1 case spermatocytic seminoma.The treatment method was selected by pathology results.Eleven cases were lost,34 cases were followed up for 3 month to 5 years,25 cases seminoma,9 cases non-seminoma,5 cases were died.Conclusion Surgical resection and adjuvant chemotherapy are important factors for testicula disease.
2.Imaging Findings of Synovial Sarcoma

Objective To investigate the image characteristics of Synovial Sarcoma, and improve the diagnostic accuracy. Methods The clinical materials and image findings of synovial sarcomas proved by pathology and were retrospectively analyzed in 13 cases.Results ①Most synovial sarcomas located in close proximity to joints, especially large joints of lower limbs . ②The mass diameter was usually more than 5cm ,and most masses presented moderate density, and some with calcification inside it on X-ray films. ③Most masses density was not homogeneous and lower than that of the muscle on CT, the demarcation was clear or not. ④On T2WI /STIR of MRI, most tumours presented slightly hyperintense "cobble" nodules , with hypointense septa among the nodules. After contrast, the nodules were unenhanced or enhanced slightly,the septa were markedly enhanced. Conclusion Some image features of synovial sarcoma ,especially on MRI, are helpful for the diagnosis of it.
3.Experimental study of carotid sinus nerve block in prevention and treatment of hemodynamics instability caused by carotid sinus stimulation

4.MRI findings of spinal intramedullary tuberculoma and literature review

Objective To analyze the MRI findings of spinal intramedullary tuberculomas and the related literature was reviewed. Methods A retrospective study of 5 patients with intramedullary tuberculoma proved by clinical and radiological evidences was undertaken.Both T1-and T2-weighted images were obtained along with the postcontrast T1 WI.The locations,signal intensities,patterns of enhancement and morphology of the tuberculomas were observed.Results A tuberculoma in one patient was found in cervical spinal cord with slight hypointensity and nodular enhancement on T1 WI and hypointensity on T2 WI.Those in three patients were found in inferior thoracic spinal cord with typical “target sign”on T2 WI and rim-enhancement on postcontrast T1 WI,and the tuberculomas were ovoid along the long axis of spinal cord on sagittal images.A miliary tuberculoma in thoracic spinal cord in one of these three patients was found which could not be showed on plain MRI.The tuberculomas in last patient was located in conus terminalis with hypointensity and rim enhancement on T1 WI and hypointensity on T2 WI.Conclusion The MRI findings of spinal intramedullary tuberculoma are variable,and their characteristic manifestations include hypointensity or “target sign”on T2 WI,rim enhancement on postcontrast T1 WI along the long axis of spinal cord.
5.Effect of CDK14 on Proliferation of Human Esophageal Carcinoma Cells and its Possible Mechanism

Background:CDK14 is a novel cyclin-dependent kinase,which is overexpressed in a variety of cancer and related to their malignant behavior. Aims:To investigate the effect of CDK14 on proliferation of human esophageal carcinoma cells and its possible mechanism. Methods:Expressions of CDK14 and two cell proliferation markers,PCNA and Ki-67 were estimated in 8 fresh-frozen specimens of esophageal squamous cell carcinoma(ESCC),96 paraffin-embedded specimens of ESCC,and human ESCC cell line Eca-109 by Western blotting and immunohistochemistry. Correlations of CDK14 expression with the clinicopathological characteristics and prognosis of ESCC were analyzed. Serum starvation and release assay was performed to evaluate the relationship between CDK14 expression and cell cycle progression in Eca-109 cells. Furthermore,Eca-109 cells were transiently transfected with shRNA-CDK14 to reduce CDK14 protein level,and then the phosphorylation of tumor suppressor protein Rb,cell cycle progression and proliferation capability of Eca-109 cells were determined. Results:CDK14 was highly expressed in both ESCC tissue and cell line,which was paralleled with the expressions of PCNA and Ki-67 and correlated significantly with the tumor size,histological grade,invasiveness and metastasis of ESCC(P < 0. 05). The overall survival was poor in patients with high CDK14 expression than those with low CDK14 expression(P < 0. 05). Serum starvation and release assay showed that the expression of CDK14 was cell cycle-dependent. Knockdown of CDK14 reduced the expression level of phosphorylated Rb,induced significant G1 phase arrest and resulted in less colony formation in Eca-109 cells(P all < 0. 05). Conclusions:CDK14 is highly expressed in ESCC. It may promote cell cycle progression by phosphorylating downstream Rb protein,thus enhancing the proliferation of tumor cells,and ultimately participating in the occurrence and development of ESCC.
6.The Uses of Granulocyte-Macrophage Colony-Stimulating Factor(GM-CSF) in Leucocytopenia After Chemotherapy in Patients with Malignant Tumor

The present study was designed to confirm the use of GM-CSF, made by the Beijing United Biological Engineering Company, in leucocytopenia after chemotherapy in 12 patients with malignant tumor. In patients with malignant tumor the interval leucocytopenia needed shortened 5 days compared with that in control subjects. At the same time GM-CSF could be resistant to infection significantly, especially in patients with complicated infection, and the antibiotic applying time decreased. In addition, clinical experence had shown that there was no obvious side-effects in general dosage. Thus, these findings indicate that GM-CSF is a kind of essential drug in radical chemotherapy in patients with tumor.

Objective To investigate the biological characteristics of human umbilical cord derived mesenchymal stem cells(UCMSCs) and inducing them to differentiate into neurons in vitro,in order to provide stem cell resource for the tissue engineering artificial nerve. Methods UCMSCs were cultured from Wharton jelly of human umbilical cord in the condition of sterilitas,surface antigens of UCMSCs were detected by immunohistochemistry.The frist group of cells were added by virgin nutrient,the second group of cells were induced by merely growth factors,the third of cells were induced by merely astragalus mongholicus and the fourth by the astragalus mongholicus with growth factor,observed the morphology of the cells of the two groups under inverted microscope,and determine the positive expression rate of nerve cells' markers,such as NSE,NF and GFAP.The results were statistically analyzed.Results UCMSCs were strongly positive for CD29,CD44,CD105,weakly positive for CD105 and negative for CD34,CD45.After induced by Astragalus mongholicus,UCMSCs were strongly positive for GFAP,NSE. Conclusion UCMSCs can be successfully cultured from the adherent tissue pieces,Astragalus mongholicus can successfully induce them to differentiate into neurons in vitro,to provide a new method of making the tissue engineering artificial nerve and treat theperipheraly nervus defect.
8.An exploration of the application of dataflow in picture archiving and communication systems

Objective To enhance the function of picture archiving and communication systems (PACS) and the efficiency of medical imaging by adjusting dataflow. Methods About 9500 examinations consecutive data in July of 2003 were analysed and and followed. The dataflow was adjusted by adding digital radiology servers and temporal servers. Results The network bottleneck was resolved and the system load balanced, then the function of the system was enhanced. The secure running of the system was ensured in condition of urgency or increasing of patients and image data. Conclusion Adjustment of dataflow in PACS makes full use of all resources, reduces unnecessary time-wasting , further improves the working efficiency in our department.
9.Improvement in the solubility of mangiferin by HP-?-CD inclusion

AIM:To study the optimum condition of hydroxypropyl-?-cyclodextrin inclusion for mangiferin in order to improve the solubility of mangiferin. METHODS: By comparing the condition of inclusion of mangiferin in hydroxypropyl-?-cyclodextrin obtained,including its drying method,inclusion compound stood the test of infrared spectrophotometry,UV spectrophotometry,differential scanning calorimetry analysis,phase solubility diagran and solubility determination. RESULTS: The paddling process and freeze drying could put in HP-?CD inclusion compound use the ratio of host/guest molecules was 1∶1,the solubility of mangiferin of included complex was 35.99 mg/mL,about 300 times as large as uninclused mangiferin(0.111 mg/mL). CONCLUSION: Hydroxypropyl-?-cyclodextrin inclused mangiferin compound could be prepared by paddling process and freeze drying.Its solubility of the compound improves significantly and it is stable.
10.Effect of mild hypothermia induced by endovascular cooling with heating exchange catheters on severe traumatic brain injury in patients

Objective To investigate the effects of mild hypothermia induced by endovascular cooling with heating exchange catheters on severe traumatic brain injury in patients. Methods Twenty patients with severe traumatic brain injury aged 18-60 yr were randomly divided into 2 groups (n=10 each):ice bay cooling group (group IBC) and endovascular cooling group (group EVC).The state of consciousness was scored on a Glasgow coma scale (GCS).The patients had GCS scores of 3-8. The patients underwent emergency surgery.A probe of intracranial pressure monitor was placed during operation.In group EVC intravascular heat exchange catheters were inserted via femoral vein and connected to intravascular heat exchange system (CoolGard 3000, Alsius, USA). In group EVC body temperature was reduced to 34℃ and maintained at this level for 48 h. MAP, HR, body temperature and intracranial pressure (ICP) were continuously monitored and cerebral perfusion pressure (CPP) was calculated. Blood samples were taken from peripheral vein for determination of serum concentrations of neuron specific enolase (NSE), myelin basic protein (MBP) and S-100B (by enzyme linked immunosorbent assay) and GCS scores were assessed at 10 min before (baseline) and 12, 24, 48 and 72 h after operation. The state of consciousness was again assessed 3 months after operation and scored on Glasgrow outcome scale(GOS). Results ICP was significantly lower and CPP was higher after operation in group EVC than in group IBC. Serum concentrations of NSE, MBP and S-100B were significantly lower after operation in group EVC than in group IBC. Conclusion Mild hypothermia induced by endovascular cooling with heating exchange catheters can effectively reduce severe traumatic brain injury in patients.
