1.The development of current strategies for the nursing and management of bloodstream infections from short-term placed central venous catheters

Center vein puncture is important to the clinical infusion technology, however, its correlation infection rates rising trend year by year, especially catheter-related bloodstream infections have become one of the main reasons for primary bacteremia, seriously affected the patients survival rate, and the processing method of catheter-related bloodstream infections are intricate. The treatment and nursing of adult bloodstream infections caused by short-term central venous catheter (including peripheral central venous catheter placement) according to the provisions of the Infectious Diseases Society of America to update the catheter related infections of diagnosis and treatment guidelines are reviewed in this paper, to provide reference for management of adult short-term central venous catheter.
3.Study on Extraction Technology of Hegan Lidan Granules

Objective:To optimize the extraction process of Hegan Lidan granules. Methods:In order to choose the optimal tech-nological parameters, the content of baicalin was determined by HPLC. An orthogonal method was utilized with solvent volume, extrac-tion time and extraction times as the impacting factors and the content of baicalin and extraction rate as the indices. Results:The opti-mal parameters were as follows:using 8-fold water as the solvent, the raw material was extracted three times with 2 h for each. Con-clusion:The process is steady and feasible, and can be used in the extraction of Hegan Lidan granules.
4.Decision-making under risk condition in benign epilepsy children with centrotemporal spikes

Objective To investigate the ability of decision-making under risk condition in benign epilepsy children with centrotemporal spikes (BECT).Method Seventy-eight BECT patients and seventyfive healthycontrols were studied by Iowa Gambling Task (IGT) with inexplicit probability and Game of Dice Test (GDT) with explicit probability.Results (1) There was a significant difference in IGT task scores between the two group (t=-2.56,P<0.05).The total number of advantageous choices in IGT were lower in BECT group (49.35±5.38) than that in healthy control group (52.24±6.23).BECT group (-1.41 ± 10.60) performed worse on task than healthy control group (4.43±11.88),and the difference was statistically significant (t=-3.21,P<0.05).The control group scored higher on Blockl,Block4 and Block5 than that of BECT group (P<0.05).(2) There was no significant difference between the BECT group (4.42±7.74) and the control group(5.49±6.81) in the GDT test scores (t=-0.91,P=0.37).(3) Pearson correlation analysis showed that there was no significant correlation between the age,age of onset,education,seizure frequency and onset duration and the score of IGT and GDT (r =-0.091-0.274,all P > 0.05).Conclusion There is no significant decision-making disability in children with BECT under the condition of certain risk probability,while there is significant impairment under the condition of uncertain risk probability.The decision-making disability may be associated with impaired frontal lobe function.
5.Changes of the peripheral Thelper cell cytokines of patients with chronic hepatitis B during antiviral treatment

Objective To determine the changes of peripheral levels of T helper cell cytokines of patients with chronic hepatitis B (CHB) during antiviral treatment,and to further explore its clinical significance.Methods The plasma levels of interleukin (IL)-2,IL-6,IL-10,interferon (IFN)-γ and tumor necrosis factor(TNF)-α of thirty-three CHB patients during antiviral treatment (entecavir) were measured using enzyme linked immunosorbont assay (ELISA).And their biochemical indicators of liver function were determined.The differences of cytokines levels before and after antiviral treatment were compared using ANOVA.The correlations between the changes of cytokines and alanine transaminase (ALT),hepatitis B virus (HBV) DNA levels were analyzed.Results Levels of IFNγ before and 12,24,48 weeks after treatment were (5.98±2.77),(5.95±3.37),(2.93±2.15) and (9.29±4.65) pg/mL,respectively (F=3.845,P<0.05),which were positively correlated with ALT levels (r =0.396,P<0.05).Both TNF-α and IL-10 levels declined after antiviral treatment,which were significantly different at different time points (F=20.156 and 16.695,respectively; both P<0.05),and both levels of TNF-α and IL-10 were positively correlated with ALT levels (r=0.354and 0.316,respectively; both P<0.05) and positively correlated with HBV DNA levels (r=0.382and 0.386,respectively; both P<0.05).While both IL-2 and IL-6 levels were not significantly different between before and after antiviral treatment (F=0.010 and 0.932,respectively; both P>0.05).The serum levels of ALT and HBV DNA before and after antiviral treatment were all significantly different (F=17.69 and 198.98,respectively; both P<0.05),which declined gradually during treatment and were positively correlated (r =0.581,P<0.05).Conclusions IL-10,IFNγand TNF-α may be involved in the pathologic process of CHB,and closely related to the deterioration of the disease.Monitoring plasma levels of these cytokines during antiviral treatment could be useful to understand the immune status and evaluate the efficacy of antiviral drugs.
6.Cosmetic efficacy of topical 5-aminolaevulinic acid-photodynamic therapy on cephalic and facial skin tumors

Objective To investigate and summarize the clinieal effects and cosmetic results of 5-aminolaevulinic acid-photodynamic therapy (ALA-PDT) on cephalic and facial skin tumors.Methods Patientswith skin basal eell carcinoma(BCC),squamous cell carcinoma (SCC),keratosis seborrheica (KS),and solar keratosis (SK) were included in this study.Inoperable cases were given topical ALAPDT,and received clinical response evaluation and satisfying questionnaire after 3 months follow-up.Resalts28 patients including 16 BCCs,8 SCCs,2 KSs and 2 SKs received ALA-PDT.100% BCC had responseto PDT,including 15 cases with complete response (CR);only one recurred.Overall response rate was 67% for SCC,2 of 8 cases failed to continue the treatment,3 eases CR,1 case partial response (PR) and 1 patient with no response.Response rate was 0% in KS.100% of SK had response to PDT,and 2 cases showed CR and PR,respectively.64.2% patients (18/28) showed extreme satisfaction for cosmetic outcome,11% (3 cases) satisfaction,7.1% (2 cases) little satisfaction,and 10.7% (3 cases) no satisfaction.Conclusion Topical ALA-PDT is an effective and satisfied treatment with lower recurrent rates, especially for cephalic and facial skin tumors including BCC,SCC and SK,but no response for KS.
7.The effect of infrared radiation on the expression of epidermal growth factor and its receptor in atrophic gastritis

Objective To study the effect of infrared irradiation on the expression of epidermal growth factor EGF) and epidermal growth factor receptor (EGFR) in the gastric mucosa of rats with chronic atrophic gastritis CAG),and to explore its mechanism.Methods Forty healthy adult and clean Wistar male rats were randomized into a normal control group (N group),a model control group (M group) and an infrared group (I group) using a random number table.CAG was induced in the rats of the M and I groups by intra-gastric infusion of 2 ml of a mixture of 30% alcohol and 2% sodium salicylate once every day for 8 weeks.This was combined with compulsory running and irregular fasting as pathogenic factors.After the CAG model had been established,the rats in the I group were given infrared irradiation for 10 rain once every day for 20 days.Then all of the rats were sacrificed and the expression of EGF and EGFR was observed using immunohistochemistry.Results The positive staining scores for EGF and EGFR of the M group were 4.91±2.58 and 6.75±2.63,significantly higher than those of the N group (2.10±0.65 and 4.20±2.24 respectively).Those of the I group were 2.75±1.01 and 4.75± 1.66,significantly lower than those of the M group,though not significantly different from the N group's averages.Conclusion Infrared irradiation can inhibit over-expression of EGF and EGFR in the gastric mucosa of rats with chronic atrophic gastritis.This should protect the gastric mucosa and promote tissue repair and healing.
8.Efficacy of enternal nutrition support with high-energy density nutrition emulsion in severely burned patients

Objective To study the mechanism of glutamine dipeptide on wound healing after operations on bums. Methods Totally, 30 burned patients were randomly divided into study group and control group (15 cases in each group). All patients received parental nutrition support after operation. Study group patients were supplemented with glutamine dipeptide at a dose of 0. 5 g · kg±1 · d±1. The plasma free hydroxyproline level was measured by a standard amino acid analyzer 1 day before operation and 7 days after operation and the wound healing time was recor± ded. Results The plasma free hydroxyproline levels of both groups were higher than the normal values before opera± tion but without significant difference [control group: (2. 24 ±0. 84) fig/ml, study group: (2. 32 ±0.92) μg/ml, normal value: (1.27 ±0.44) μg/ml]. On the post±operative 7 day, the plasma free hydroxyproline level of study group [ (4. 31 ±1. 05) μg/ml] was significantly higher than that of control group [ (3. 04± 1. 01) μ/ml] (P = 0.002). The wound healing time of study group [ (29. 7±5.3) d] was shorter than that of control group but with± out significant difference [ (33. 3 ±7.5) d, P = 0. 14 ]. Conclusion The intravenous supplementation of glutamine dipeptide may increase the plasma hydroxyproline level after operation and thus promote wound healing.
9.Expression of ASBT and ASGPR mediated receptors for oral liver-targeting preparations in a rat model of hepatic alveolar echinococcosis

ObjectiveTo investigate the feasibility of apical sodium-dependent bile salt transporter (ASBT) and asialoglycoprotein receptor (ASGPR) in the design of oral liver-targeting preparations for the treatment of hepatic alveolar echinococcosis (HAE) by measuring the expression of ASBT and ASGPR. MethodsA total of 18 male Sprague-Dawley rats were selected, among which 10 were used to establish a model of HAE (HAE group) and 8 were used as controls (normal group). Immunofluorescence assay, Western blotting, and quantitative real-time PCR were used to measure the expression distribution, protein expression level, and mRNA expression level of ASBT in the ileal tissue of HAE model rats and normal rats; the same methods were used to measure the expression level of ASGPR in the non-diseased liver tissue and the marginal zone of liver tissue lesion of HAE model rats and the liver tissue of normal rats. The t-test was used for comparison of normally distributed continuous data between two groups; a one-way analysis of variance was used for comparison between three groups, and the least significant difference t-test was used for comparison between two groups. ResultsThe results of immunofluorescence assay, Western blotting, and quantitative real-time PCR showed that compared with the normal group, the HAE group had significantly upregulated expression of ASBT in the ileal tissue (t=5309, 4.110, and 28.060, all P<0.05) and a significantly higher expression level of ASGPR (the closer to the lesion, the higher the expression) (F=110666, 128.201, and 143.879, all P<0.001). ConclusionASBT and ASGPR can be used as potential mediated receptors for oral liver-targeting preparations for HAE, which provides a theoretical basis for the design of oral liver-targeting preparations for the treatment of HAE.
10.Theory of mind in benign epilepsy children with centrotemporal spikes

Objective To investigate the theory of mind ( TOM) and eye basic emotion recognition in benign epilepsy children with centrotemporal spikes( BECT) . Methods Totally 51 BECT patients( BECT group) and 51 healthy controls( control group) were studied by Yoni task and Eye Basic Emotion Discrimi-nation Task (EBEDT). Results ①Compared with healthy controls,BECT got significantly lower score ((54. 02±6. 03) vs (58. 04±5. 41),F(1,100)=10. 34,P<0. 05))in Yoni task,especially in affective(hot) TOM .②There was no significant difference between the BECT group(M(P25,P75):103(96,108)) and the control group(M(P25,P75):105(96,110))in the total score of Eye Basic Emotion Discrimination Task(Z=-1. 37,P>0. 05),but got significantly lower score in recognizing sadness(M(P25,P75):16(14,17) vs 18 (15,19),Z=-3. 05,P<0. 05)and fear(M(P25,P75):15(14,17) vs 16(16,18),Z=-2. 21,P<0. 05).③Pearson or Spearman correlation analysis showed that there was no significant correlation between the total score of TOM and the age,age of onset, education,seizure total number,MMSE,the Digital Span test, the verbal fluency test and eye basic emotion recognition ( r=-0. 257-0. 908,all P>0. 05) . Conclusion BECT children have impairments in sadness and fear emotion recognition and TOM aspects.
