1.Influence of enteral and parenteral nutrition support on gastric juice pH in patients with severe brain injury

Objective To evaluate the influence of early entemal nutrition and parenteral nutrition support on gastric juice pH in patients with severe brain injury. Methods Totally 168 patients with severe brain injury ( Glasgow Coma Scale ≤ 8 ) admitted to our hospital from January 2007 to May 2009 were equally and randomly divided into early enternal nutrition (EEN) group and parenteral nutrition (PN) group. Gastric juice pH and upper gastrointestinal hemorrhage were monitored on admission and on the 3rd, 5th, and 7th day after admission. Results The gastric juice pH was significantly decreased in patients with severe brain injury. In addition, it was significantly higher in EEN group than in PN group on the 3rd, 5th, and 7th day after admission ( P < 0. 001 ). The alimentary tract hemorrhage incidence showed no difference on admission between two groups but was significantly lower in the EEN group on the 3rd, 5th, and 7th day ( P < 0. 05 ). Conclusions Hypersecretion of gastric acid may occur in patients with severe brain injury. EEN can neutralize gastric acid and increase gastric juice pH,which may lead to the decrease of upper gastriontestiual hemorrhage. Monitoring of the gastric juice pH also can provide a warning message of stress ulcer bleeding.

4.Clinical Observation on 68 Cases of Pollinosis Treated by Chinese Herbs Combined with Acupuncture and Moxibustion

Objective To find out an effective therapy to treat pollinosis in Switzerland by Chinese herbs and acupuncture & moxibustion.Methods Gu Beng Tuo Min Fang and Acupuncture & moxibustion were used on 68 patients with pollinosis,and compared with clinical efficacy before and after treatment.One course was 20 times,and Continued about 3 months.Results Among the treated 68 cases,cured in 47,improved in 12,and total effective rate was 86.8%.Conclusion A combination of Chinese herbs and Acupuncture & Moxibustion has an exact therapeutic effect on pollinosis in Switzerland.

Objective:To explore the role of CD28 in the activation of human peripheral blood ?? +T cells by Mycobacterium tuberculosis (Mtb) low molecular peptide antigen in vitro Methods:Mtb antigen and anti CD28 monoclonal antibody (mAb) were used as signal 2 to stimulate the r? + T cell from PBMC the expression of CD28 molecule on ?? +T cells, proliferation rate of ?? +T cells and expression of CD69 molecule on activated ?? +T cells were analyzed by using flow cytometry Results:CD28 molecule was expressed on 50% of ?? +T cells Neither Anti CD28 nor Mtb antigen alone, but both presence, could stimulate ?? +T cells activation and proliferation CD69 molecule was expressed on activated ?? + T cells.Conclusion:The CD28 molecule could provide the costimulatory signal in the full activation of ?? +T cells by Mtb low molecular peptide antigen CD69 molecule was also an early activation marker of ?? +T cells
6.Identification of the functional phenotype of transgene in kidneys of human tissue inhibitor of metalloproteinase-1 (TIMP-1) transgenic mice

Objective To thoroughly explore the pathophysiological roles of TIMP-1 during the progressive course of renal diseases, the study was aimed at identifying the functional phenotype of endogenous and exogenous genes in kidneys of human TIMP-1 transgenic mice. Methods Renal histological changes between 3-month-old wild type mice (n=8) and 3-month-old transgenic mice (n=8) were analyzed through PAS staining of paraffin sections. The mRNA and protein expressions of h/mTIMP-1, mTIMP-1, TIMP-2, TIMP-3, MMP-9, MMP-2, and COLⅣ?5 mRNA were detected by Northern blot and Western blot. The activities of gelatinases and TIMP-1 were examined by gelatin zymography and reverse zymography, respectively. Results No difference in histological picture in kidneys was found between wild type and transgenic mice. In contrast with wild type mice, it was found that in kidneys of transgenic mice, the mRNA and protein expressions of h/mTIMP-1 and its activity were up-regulated (P0.05). Conclusion The transgene was expressed steadily in kidneys of human TIMP-1 transgenic mice, and it induced the compensation of MMPs/TIMPs.
7.Intervention of siRNA to the effect of gelatinase on the expression of ICAM-1 in human renal tubular epithelial cells

Objective To inhibit the expression of matrix metalloproteinase-2(MMP-2) and matrix metalloproteinase-9(MMP-9) in human kidney tubular epithelial cell(HKC) with specific small interference RNA(siRNA),then to observe whether the expression of intercellular adhesion molecule-1(ICAM-1) on HKC could be influenced.Methods HKCs were transfected with MMP-2,MMP-9 siRNA or irrespective siRNA respectively,untransfected HKC was considered as blank control.Four groups of HKCs were stimulated with 100nmol/L phorbol myristate acetate(PMA) for 18h.Total RNA or protein of cells were extracted with Trizol and RIPA cell lysate respectively,the mRNA expression levels of MMP-2,MMP-9 and ICAM-1 were detected by real time PCR,the protein expression levels of above indexes were detected by western blotting and immunofluorescence combined with confocal laser microscopy.Results The specific siRNA could inhibit the expression of MMP-2 and MMP-9 in HKCs effectively.Immunofluorescence combined with confocal assay demonstrated that while HKCs was stimulated with PMA,the expression of ICAM-1 was up-regulated,which was significantly up-regulated in the group transfected with MMP-9 siRNA(P0.05).Conclusions The inhibitory expression of MMP-9 of HKC may up-regulate the expression of ICAM-1,implying that,besides taking part in the degradation of extracellular matrix,MMP-9 may throw an influence on the proceeding of renal fibrosis by inflammatory pathway.

IgA nephropathy ( IgAN) , which is characterized by the presence of IgA deposits in the glomerular mesangi-um, is the most common form of primary glomerulonephritis in the world .Some studies have revealed that the expression and changes of certain factors involved in IgAN pathology lesion in urine can provide facilitate the diagnosis , assessment and prognosis of IgAN .Studies on urine biomarkers in recent years were reviewed in this paper .

Objective To explored the role of CD40/CD40L and B7-1/CD28 interaction in the immunopathologic process of folic acid-induced nephropathy(FAN) and investigate the intervention effect of the anti-B7-1 monoclone antibody(B7-1mAb). Methods GD1 mice were administrated by i. p with FA only or co-treated with B7-1 mAb for the intervention study. Kidneys were harvested for immunohistochemical, Western blot assays and serum for renal function evaluation at day 1, 3,5, 7, 14 and 21. Results Continuous expression of CD40, B7-1 from day 1 to day 21 on the renal tubular epithelial cells was detected in this model. 10.Relationship between leptin level and insulin resistance in patients with hypertensive cerebral hemorrhage

Objective To investigate the changes of leptin level in patients with intracerebral hemorrhage and the relationship between leptin level and insulin resistance(IR). Methods 57 patients with hypertensive cerebral hemorrhage (group A ), 50 patients with hypertension (group B), 41 healthy people (group C) were selected to determine the value of fasting blood-glucose ( FBG), fasting blood-insulin( FBI), leptin levels as well as insulin sensitivity index (ISI). Results The level of FNS and leptin was significantly higher (P < 0.05 ) while the level of ISI was lower (P < 0.05 ) in group A than those in group B and C. Correlation analysis showed that leptin level in HCH patients was negatively correlated with ISI, and positively correlated with FNS. Conclusion The results indicate that higher level of leptin and IR may play an important role in the occurrence of hypertension complicated cerebral hemorrhage.
