OBJECTIVE: To probe into the application of antibiotics in orthopedic perioperative period. METHODS: The application of 591 patients with orthopedic disease in perioperative period from Jun. 2006 to Jun. 2008 were reviewed and analyzed. RESULTS: 591 patients were treated with antibiotics in perioperative period. 85 cases of preoperational medication accounted for 14% and 286 cases of drug combination 48%. Among 286 cases of drug combination, 263 cases of two-drug consisted of 92%, 23 cases of three-drug 8%; mean administration time was(9?2.3)d. CONCLUSION: Abuse, inappropriate use and inappropriate drug combination of antibiotics were existed, which need to be corrected.

ObjectiveTo explore the effective way of first aid skill standard training in regional central hospital.Methods60 residents from different regional central hospitals were received aid skills training based on two ways:namely OSCE ( multi-station structured skills test ) which lay particular em phasis on skills and traditional face to face way; and were assessed by uniform standards.ResultsThe scores of residents who received OSCE training were significantly better than those which received traditional face to face training ( P＜0.05 ),including.ECG,cardiopulmonary resuscitation,endotracheal intubation and doctor examination.ConclusionFirst aid skills standard training used by OSCE approach in regional central hospital can improve their first aid skills and should be promoted.

The aim of this study is to investigate the effect of fluoxetine (FLX) on the expressions of BDNF and Bcl-2 in the hippocampus, the amygdala and the prefrontal cortex of conditioned fear (CF) model mice. Forty eight mice were randomly divided into three groups, normal control group, CF stress group and FLX-pretreated CF group. The FLX-pretreated CF group was given FLX (10 mg x kg(-1) x d(-1)) for 7 days before CF stress. After CF stress model was established, all mice were given behavioral experiments to test whether FLX impaired or improved the auditory and contextual fear conditioning. Then mice were sacrificed. The expressions of BDNF and Bcl-2 were detected by Western blotting. The results showed that the freezing time of FLX-pretreated CF group was significantly lower than that of CF group; FLX pretreatment up-regulated the expression of Bcl-2 in the hippocampus at 1 d after CF stress (P < 0.001), but no significant differences was observed at 7 d; BDNF significantly increased in the hippocampus at 7 d (P < 0.001), but no differences at 1 d; the expressions of BDNF and Bcl-2 in the amygdala and the prefrontal cortex were of no obvious differences between CF group and FLX-pretreated CF group at 1 d or 7 d after CF stress. Parallel to these changes, pretreatment with FLX could affect histopathologic changes induced by CF stress. Furthermore, the results indicated that FLX pretreatment could protect against CF stress-induced neurological damage via the activation of BDNF and Bcl-2 in hippocampus.

Objective Taking methyl nonyl ketone as index to establish pharmacokinetic compartment model for study volatile oil from the whole herb of Houttuynia cordata by pharmacokinetic parameters in rabbits in vivo. Methods Using GC to determine the blood concentrations of methyl nonyl ketone in the volatile oil from whole herbs of H. cordata in rabbit plasma. The GC system used a capillary SPB-5 column and helium as carrier gas; the initial temperature was at 90 ℃, then raised to 190 ℃ by 10 ℃/min and kept for 5 min; the injector temperature was 250 ℃ and the FID detector temperature was 300 ℃; the injection volume was 5.0 ?L with splitless injection mode; n-hexane was used as solvent and n-pentadecane as interior standard substance. The blood concentration value of methyl nonyl ketone was fitted with 3P97 procedure. Results A capillary GC method was established for the detemination of methyl nonyl ketone in the volatile oil from whole herbs of H. cordata in rabbit plasma. Methyl nonyl ketone linearity was good (r=0.987 0) within 0.04—60.0 ?g/mL.The blood concentration-time course of precision in rabbit plasma conformed to two compartment model. Conclusion The experiment provides pharmacokinetic evidence for the rational administration and the further development of H. cordata and some methods for pharmacokinetic study of volatile oils from traditional Chinese medicinal materials.

We have trcated 40 cases of active peptic ulcer with Omeprazolc from Jun. 1990 to Mar. 1991. Compared with it, We have treated 40 cases of active peptic ulcer with Ranitidine during the same time. 80 patients with active peptic ulcer were divided into two groups. The age, scx and size of ulcer between the two groups have been compared. The ulcer diameter was from 3 to 20mm. The number of the ulcer was from 1 to 5.40 patients were treated with Omeprazole 20～40mg p. o. everyday. Shu-Xue-Ning (SXN) injection was used as study subject in order to explore the feasibility of pharmacodynamics experiment of Phase 0 clinical trial in Chinese materia medica (CMM).This paper tried to establish key-technique standards which fit to the evaluation on CMM phase 0 (early) clinical evaluations.According to the established research methods,this research selected volunteers and assured low dose SXN injection inside their body.And then,serum before and after medication were collected for the HAEC in vitro experiment.Blood sample of the clinical dosage was used as control to verify the tendency consistency of pharmacodynamics in low dose and clinical dose.The results showed that in the H2O2 inducing HAEC damage experiment,low dose SXN injection can affect the cell proliferation,protect the shape of cells,reduce the release of ion owing to cell damages and influence the secretion in cells.Complete-sequence RNA gave differential multiterm expression before and after the low dose.It was concluded that compared with the clinical group,low dose medication had similar metabolic tendency.The pharmacodynamics experiment of phase 0 CMM clinical trial is feasible.

Objective To research the feasibility and superiority of video-assisted thoracoscopic surgery(VATS) in the diagnosis and treatment of thoracic traumas.Methods Video-assisted thoracoscopic surgery was used in 40 patients with thoracic traumas,including thoracic exploration,repair of pulmonary laceration,and evacuation of clotted hemothorax.[WTHZ]Results All the 40 patients were cured,including simple VATS in 31 patients,thoracoscopy-assisted mini-incision surgery in 8 patients,and conversion to open surgery in 1 patient.The time of operation was 79.9?33.1 min.The amount of blood clots and noncondensing blood cleared was 567.5?177.8 ml.The closed thoracic drainage tube was removed at 24~48 h postoperatively,with a drainage volume of 220?45.6 ml.The length of hospital stay was 4~13 d(mean,8.7 d).The sutured wound healed by first intention in all the patients.No postoperative complications were observed.Follow-up reviews in 35 patients for 6~12 months(mean,8.6?2.6 months) revealed uneventful recovery and no trauma-related complications.Conclusions As compared with conventional open surgery,VATS has shown advantages of exact diagnosis,timely management,little invasion,and quick recovery for patients with thoracic traumas.

Objective To explore the effects and its possible mechanism of Fluvastatin on carotid artery atherosclerotic plaque(CAAP) in elderly patients.Methods 92 patients with CAAP detected by Color Doppler Ultrasonography were divided randomly into group A(48 cases) which treated with Fluvastatin 40 mg/d and group B(44 cases) which treated with 80 mg/d respectively for 40 weeks after a washout period(Fluvastatin 40 mg/d for 8 weeks).The changes of CAAP areas,the levels of serum total cholesterol(TC),low-density lipoprotein(LDL),high sensitivity C-reactive protein(hs-CRP) were observed between the before and after treatment.Results There were 40 cases in each group completed the Fluvastatin treatment for 48 weeks.After treatment,the CAAP areas in group A and B were significantliy decreased from(0.20?0.18)cm2 and(0.20?0.19)cm2 to(0.12?0.15)cm2 and(0.11?0.12)]cm2(allP

Objective To investigate the mechanisms of fluconazole resistance in clinical and experimental induced isolates of C.glabrata.Methods Efflux of rhodamine 6G was performed to evaluate the effects of efflux pumps.The expression levels of transporter genes CDR1,CDR2,SNQ2 and ERG11 were examined by real-time RT-PCR.Meanwhile,sequence of PDR1 was determined by PCR based DNA sequencing.Results Efflux pumps of all fluconazole-resistant isolates had stronger effects than that of susceptible isolates,consistently with significant upregulation of CDR1,but no obvious difference was found in CDR2 or SNQ2.Also,no notable change in the expression level of ERG11 between susceptible and resistant isolates.PDR1 mutations existed in both clinical and experimental induced isolates of C.glabrata,among which P927S,L543P and S947L haven't been reported previously.Conclusion Mutations of PDR1 were induced by fluconazole both in vivo andin vitro,which will result in overexpression of CDR1 and strengthen the effect of efflux pump.
