1.Effect of nordihydroguaiaretic acid on chronic atrophic gastritis in rats

Objective To study the preventive effect of lipoxygenase inhibitor nordihydroguaiaretic acid (NDGA) on chronic atrophic gastritis (CAG). Methods To construct CAG rat model, 72 male rats were randomly divided into 6 groups, including normal group, CAG model group, NDGA group with different dose and positive groups (folacin). After feeding for 24 weeks, all rats were executed, their stomach mucous membrane was picked out and stained with H.E. The expression of 5-LOX and P16 protein in mucous membrane epithelia cells were detected by immunohistochemistry. Results The CAG incidence rate in model group was significant higher than normal group (77.8% vs 0% , P <0. 05), which indicated that C AG model rat was successfully established. The CAG incidence rate of model group was significantly higher than NDGA groups (77.8% vs 25% ,27.3% ,25%, P<0.05), while no significant difference was found between positive group and NDGA groups (30% vs 25% ,27.3% ,25% , P>0.05) .The expression of 5-lox in model group was higher than NDGA groups (44% vs 25% ,27% ,25%, P<0.05). The expression of P16 protein in model group was lower than NDGA groups (66.7% vs 83.3% ,81.8% ,83.3%,P<0.05) , and there were no significant differences between NDGA groups and positive group (83. 3%,81.8% ,83.3% vs 80%, P>0.05). Conclusions NDGA could prevent the occurrence of N-ethyl-N-nitro-N nitrosoguanidine-induced chronic atrophic gastritis in rat. NDGA could down-regulate the expression of 5-LOX and up-regulate the expression of P16 in stomach mucous membrane of N-ethyl-N-nitro -N-nitrosoguanidine-induced chronic atrophic gastritis in rat.
2.Application of run-through wire stretch technique in the procedure of endovascular therapy for complex subclavian artery occlusive diseases

Objective To discuss the application of run?through wire stretch technique in the procedure of endovascular therapy for complex subclavian artery occlusive diseases. Methods The clinical data of 22 patients who were diagnosed complex subclavian artery occlusive diseases were retrospectively analyzed from June 2013 to January 2016 in vascular surgery department of Wangjing Hospital .The patients were treated endovascular therapy using run?through wire stretch technique in all procedures successfully. Results The complex subclavian artery occlusive diseases were including long segment occlusion of the left subclavian artery and ostial stenosis or occlusion of the right subclavian artery. The left subclavian artery occlusion patients was 16 and right subclavian artery ostial stenosis or occlusion was 6. The technical success rate was 100%. All patients were using combination of femoral artery and brachial artery approach. Run?through wire stretch technique was used in the procedure of endovascular therapy, which had improved the technical success rate. After the treatment the symptom improvement rate was 88%. The symptoms of upper limb and posterior circulation ischemia were improved. Conlusion Upper limb artery combined femoral artery approachand run?through wire stretch technique can improve the recanalization in the procedure of endovascular therapy for complex subclavian artery occlusive diseases.
3.Normal epithelial cell specific-1 gene hypermethylation in the carcinogenesis of hepatocellular carci-noma

Objective To probe the gene expression of normal epithelial cell specific-1 (NES1) in normal liver cells and liver cancer cell lines , and investigate the gene methylation status and its impacts on gene expression and cell biology .Methods The expression level of NES1 mRNA was detected in HepG2 and L02 cells by RT-PCR and RT-QPCR, and the level of gene methylation was examined by MSP .We de-tected cell viability by MTT , NES1 mRNA by RT-QPCR and cyclinD1, P21 and P53 level by Western blot after treating cells with 5-Aza-CdR.Results Compared with L02, NES1 mRNA expression in HepG2 was significantly reduced, and the level of NES1 exon 3 CpG island methylation in HepG2 cells was much higher than that in L02 cells.After demethylation , NES1 mRNA expression and protein level of p 21 and p53 in HepG2 cells were up-regulated , while the cell viability and the level of CyclinD 1 were decreased .Conclu-sions In hepatocellular carcinoma , low expression of NES1 mRNA is related to the gene exon 3 CpG island methylation .NES1 exon 3 methylation may be one of the molecular mechanisms for reducing NES 1 mRNA level, and 5-aza-dC could inhibit cell proliferation by inducing cell cycle arrest in HepG 2 cells.
4.Misdiagnoses and management of the vascular vagovagla reflexes induced by intervention therapy for pe-ripheral arterial diseases

Objective To investigate the misdiagnosed vascular vagovagla reflexes (VVR) and the treat-ments during interventional therapy for peripheral anerial disease (PAD). Methods The clinical data of 5 patients with VVRs from 128 PAD patients undergoing interventional therapy so as to analyse the causes for the misdiag-noses and then render pertinent treatment. Results The VVR in the 5 patients belonged to mixed type and all cas-es occurred after intervention and initially misdiagnosed. All patients were recovered well without adverse reaction. Conclusion Intervention therapy for PAD may be susceptible to VVR. Prompt management may bring in a good prognosis when VVR happens.
5.The expression and significance of KLK9 in liver cancer

Objective To investigate the expression of kallikrein 9 (KLK9) in liver cancer and to determine its significance.Methods The expression of KLK9 in liver cancer was detected by immunohistochemistry and RT-PCR techniques.Results The rate of expression of KLK9 protein in liver cancer tissues was significantly higher than paracarcinoma tissues and normal liver tissues (P < 0.05).The expression of KLK9 mRNA in liver cancer cells was significantly higher than normal liver cells (P < 0.05).The expression of KLK9 was related to metastasis,size of tumors,degree of malignancy and clinical staging of the liver cancer (P ≤ 0.05),but there was no associated with age,HbsAg and sex (P > 0.05).Conclusions KLK9 may play an important role in the occurrence and development of liver cancer.It may be used as a tumor marker and a prognostic factor.It may also provide a theoretical basis for the diagnosis and biological targeted therapy of liver cancer.
6.Value of the procedure of percutaneous catheter drainage guided by ultrasound in treatment on deep perianal abscess

Objective To discuss the best method to treat deep perianal abscess and to investigate the clinical efficacy and value of the procedure of percutaneous catheter drainage guided by ultrasound in treatment.Ninteen patients with deep abscess underwent the procedure from Jan.2012 to May.2013.Methods Clinical data of 19 patients were analyzed retrospectively,including 16 male cases and 3 female cases.Results The average age is 39.8.All patients were cured without complications.The average length of stay is 7.8 days,a mean follow-up of 6.6 months after discharge,no patient with recurrence or fistula.Conclusion No specific performance of colorectal carcinoid,surgical resection is the most effective method of the treatment of colorectal carcinoid,the procedure should be individual treatment.However,the long term prognosis need to be explored in the future.

In order to adapt to the demand of culturing high quality talents and to strengthen student's comprehensive quality,pathophysiology department of medical college of Qinghai University carried on the reform on the pathophysiological experiment examination system.After researches and practices in recent years,we established a set of relatively sound examination system including experiment operating and experiment report in normal study,report for designable experiment and experiment skill and theory exam.Investigation results demonstrated that examination system after reform functioned well in training the students' operation ability,observation ability and ability to analyze and solve questions.
8.Homocysteine promotes rat VSMCs proliferation and phenotypic transformation

AIM:To provide evidence for the molecular mechanism of homocysteine (Hcy) as an independent risk factor in atherosclerosis (AS) by investigating the effect of Hcy on phenotype transformation and proliferation of vascular smooth muscle cells(VSMCs) in rats.METHODS:After treated with different concentrations of Hcy for 24 h,the cultured VSMCs were assayed for cell proliferation rate by MTT method,cell cycle by flow cytometry,the expression of SM22-? mRNA by semi-quantitative RT-PCR and the observation of morphological characteristics and the phenotype transformation by transmission electron microscopy.RESULTS:Hcy increased the cell proliferation rate and gradually reduced the proportion of the cells in G0 /G1 phase.Hcy down-regulated the expression of SM22? mRNA and the most significant effect was observed at concentration of 1 000 ?mol/L.The observations of transmission electron microscopy revealed an abundant endoplasmic reticulum,Golgi's complex,loose nucleus and puffy chromatin in VSMCs treated with high concentration of Hcy.CONCLUSION:Hcy promotes the proliferation and phenotype transformation of VSMCs simultaneously.
9. Sedative effect of non-intravenous administration dexmedetomidine in pediatric patients underwent lower abdomen and limb surgery

Objective: To investigate the application of non-intravenous dexmedetomidine (DEX) in the pediatric patients underwent lower abdomen and limb surgery, and to observe the sedative effect of DEX in this procedure. Methods: Sixty patients undergoing the general anesthesia for lower abdomen and limb surgery were selected and randomly devided into ropivacaine sacral block (RS) group, intranasal DEX + ropivacaine sacral block (ID) group, ropivacaine + DEX sacral block (DS) group, 20 cases in each group. The children in ID group received intranasal DEX 1 μg · kg-1 30 min before operation and the children in RS and DS groups received physiological saline. 1 mL · kg-1 propofol was infused intravenously in the children who could not smoothly enter into the operating room as well as the intolerance to oxygen mask or sevoflurane inhalation while induction. The children in RS and ID groups received 0.25% ropivacaine 1 mL · kg-1, and the children in DS group received the same dose of ropivacaine mixed with 1 μg · kg-1 DEX, and the total volume of drugs was 20 mL. The general information of each child was recorded; the sedation status when separated from their parents and induction period mask and sevoflurane acceptance scores were assessed; the satisfaction of separation with parents, oxygen mask and sevoflurane inhalation were recorded; the time of operation, induction, extraction of laryngeal mask and anesthesia awake were recorded; delayed awakening, laryngismus and awakening period agitation score were recorded. The scores of anesthesia recovery and the dosage of propofol were recorded; the sedation scores 4, 8, 12, 16, 20, and 24 h after operation were recorded. Results: Compared with RS and DS groups, the sedation scores of the children when they were separated from their parents and mask induction and sevoflurane inhalation acceptance, the satisfaction degree of separation, mask and sevoflurane acceptance in ID group were increased (P<0.05); the dosage of propofol in ID group were decreased (P<0.05). The time of operation, extraction of laryngeal mask and anesthesia awake had no significant differences between three groups (P>0.05), the induction time of children in ID group was shorter than those in RS and DS groups (P<0.05). There was no delayed awakening in three groups, and the laryngismus and the awakening period agitation score in RS group were higher than those in ID and DS groups (P<0.05). There was no differences in the consciousness, respiration, activity scores and the scores of anethesia recovery between three groups (P>0.05). The sedation scores in the three groups were less than 3 points 4 h after operation. Compared with RS group, the sedation scores in ID and DS groups were decreased 8 h after operation (P<0.05). Compared with RS and ID groups, the sedation scores in DS group 12, 16 and 20h after operation were decreased (P<0.05). There were no significant differences in the sedation scores between three groups 24 h after operation (P>0.05). Conclusion: When non-intravenous DEX is used in the pediatric patients underwent lower abdomen and limb surgery, the children can quietly and co-operationly enter into the operating room and quickly and smoothly complete the induction process; the incidence of revival restlessness is significantly reduced, and it can play a role in the early postoperative sedation.
10.Clinical experience of single port VATS procedure in treatment of thoracic disease

Objective Summarize the experience of single port video-assisted thoracoscopic surgery (SPVATS).Evaluate the value of SPVATS for thoracic diseases.Methods 32 patients underwent SPVATS from March 2011 to May 2011,including 28 males and4 females,with an average age of (26.5 ±9.2) years (ranged 18-72 years).There were twenty-seven cases of spontaneous pneumothorax ( including 2 cases of single stage bilateral surgery),three cases of unknown pleural effusion,one case of bilateral pulmonary nodules,and one case of pulmonary metastasis tumor.Results All procedures were successfully accomplished without conversion to conventional multi-port VATS or open approach.There was no major complication or operative death.All procedures were using self-developed specialized surgical instruments and two-incision joint protection.The procedures inluded resection of lung bullae in 27 cases,purulent drainage fiber stripping pleural lavage in 3,parietal pleural biopsy in 1,and pulmonary wedge resection in 1.The operating time ranged from (22.0 ± 9.5 ) min.The average intraoperative blood loss was ( 14.5 ± 8.8 )ml.The mean chest tube duration was (3.1 ± 0.6 ) days.The average postoperative hospital stay was (6.8 ± 1.6 ) days.Conclusion SPVATS is a new approach for treatment of thoracic diseases.This technique is preferred as a minimally invasive and cosmetic procedure with decreased operative time,quickened postoperative recovery,few complications,and satisfactory short-term outcome.
