1.An initial report of transplantation of the adductor brevis and gracilis to treat scissors gait of spastic cerebral palsy

Objective To evaluate the effects of transplant adductor brevis and gracilis to correct scissors gait in spastic cerebral palsy. Methods From July 2000 to December 2004, 73 patients with scissors gait of spastic cerebral palsy were treated with transplantation of adductor brevis and gracilis, 47 were males and 26 were females, with the average of 12.06 years old. According to Ashworth's standard, the muscle tension was evaluated before surgery. 12 cases were in degree Ⅰ, 38 cases were in degreeⅡ, 18 cases in degree Ⅲ and 5 cases in degree Ⅳ. The end of adductor brevis was cut off and sutured with tensor fasciae latae through the tunnel under the skin, the end of gracilis was cut off and transplanted to the lateral condylus of femur through the back of the knee. Some certain surgery completed at the same time, the semitendious and biceops muscle was transplanted to the front of the knee(37 cases) and extended(13 cases), the soft tissue was released in hip joint(55 cases), the gastrocnemius cut off (32 cases), the Achilles's tendon extended(30 cases) and tibialis anterior lateral transplantation (31 cases), only peroneus longus lateral transplantation (8 cases). After operation the lower limbs was fixed with plaster cast for 6 weeks, then the plaster cast was removedassociating with functional exercise. The orthopaedic results were evaluated on the function of hip, knee, foot-malleoious and the gait, then classified four grades. Results All patients were followed up an average of 24.4 months. The scissors gait was corrected completely, 61 cases were excellent, 10 cases were good, 2 cases were fair. The majority symptom of whem was the knee flexure, but can be relieved by medicine. There were significant differences compared pre-operation with post-operation in four groups, and also between degree Ⅳ group and degree Ⅰand Ⅱgroup(P

AIM: To investigate the feasibility to inhibit the expression of MHCⅡ by special siRNA targeting class Ⅱ major histocompatibility complex (MHC Ⅱ) transactivator (CⅡTA), which might regulate MHC Ⅱexpression for suppressing immune rejection. METHODS: Five different siRNA were designed, synthesized and transfected into freshly isolated rat corneal keratocytes. At 24 hours posttransfection, the changes of MHC Ⅱexpression were detected by flowcytometry, and the mRNA abundance of CⅡTA and MHC Ⅱ was measured by FQ-PCR after inducing with recombinant rat interferon-gamma (IFN-?). Type 1 diabetes mellitus(T1DM), which is an autoimmune disease, mostly affect people with hereditary susceptibility under the joint participation of environmental factors.It is known to coexist with other autoimmune disorders such as celiac disease(CD). The prevalence of CD is higher in patients with type 1 diabetes compared with the general population.Most patients affected with both T1DM and CD are atypical for CD at diagnosis, and may be misdiagnosed and subsequently suffered from the delayed treatment.These patients are more likely to develop symptomatic hypoglycemia, or poor blood glucose control, leading to an increased risk of chronic complications and seriously affecting the quality of their life.Thus, screening for CD is recommended in patients with T1DM.Anti-tissue transglutaminase antibody is the preferred biomarker for CD screening in children and adolescents over 2 years of age, with a sensitivity of 95% to 100% and a specificity of 94% to 100%.And a low-carbohydrate diet is often combined with a gluten-free diet.
7.Health status of senior high school graduates living at urban or suburban areas in Beijing

Objective To compare health status of senior high school graduates living as urban vs suburban areas in Beijing.Methods Wilcoxon rank sun test,Ridit analysis and fuzzy mathematics synthesis were used to evaluate body shape index,physical function index and health defects indicator of 39 982 senior high school graduates in Beijing.Results A total of 9 117 urban boys,9 299 suburban boys,10 380 urban girls and 11 186 suburban girls were studied.For the boys,the urban was superior to the suburban areas in body shape index (Rurt=0.518,Rsub=0.482,P<0.05) and physical function index (Rurt=0.520,Rsub=0.480,P<0.05),and the urban was inferior to suburban areas in health defects indicator (Rurb=0.501,Rsub=0.554,P<0.05) and the composite index (Ruurb=0.484,Rsub=0.516,P<0.05).For the girls,the urban was superior to the suburban areas in physical function index (Rurb=0.562,Rsob=0.442,P<0.05),and the urban was inferior to the suburban areas in health defects indicator (Rurb=0.473,Rsub=0.523,P<0.05).The differences between urban and suburban areas tended to be of no statistical significance in body shape (U=5.79,P>0.05) and composite index (U=5.73,P>0.05).In fuzzy mathematics synthesis,the scores of urban boys,suburban boys,urban girls and suburban girls were 68.37,69.13,69.83 and 69.79,respectively.Conclusion Our investigation identified some differences in body shape index,physical function index and health defects indicator among senior high school graduates living at urban vs.suburban areas,although the differences of synthetical health were not statistically significant.
8.The effects and mechanism of IL-6 on the epithelial to mesenchymal transition of pancreatic cancer cells

Objective To investigate the effects and mechanism of IL-6 on the epithelial to mesenchymal transition of human pancreatic cancer cells.Methods IL-6 was added into the culture media of human pancreatic cancer cells Capan-2,SW1990,and STAT3-siRNA-SW1990.Cell growth was measured by MTT assays.STAT3,p-STAT3,Snail,Twist,and E-cadherin mRNA and protein expression were examined using real-time fluorescence quantitative polymerase chain reaction (RT-PCR)and Western blot,respectively.The invasion abilities of SW1990 and Capan-2 cells were determined by a cell invasion assay in vitro.Results Our results showed that 100 μg/L of IL-6 significantly promoted the growth and invasion abilities of Capan-2 and SW1990 cells (P<0.05).The use of IL-6 not only markedly increased the protein expression of P-STAT3 and Snail,but also greatly decreased the mRNA and protein expression of E-cadherin.The use of IL-6 can not change the mRNA and protein expression of Snail and E-cadherin.Conclusion Activation of the STAT3 signal transducer pathway with IL-6 can promote the epithelial to mesenchymal transition of pancreatic cancer cells in vitro through up-regulation of Snail and down-regulation of E-cadherin expression.Therefore the STAT3 signal transducer may provide a novel therapeutic target for the treatment of pancreatic cancer.
9.A clinical model to estimate risk factor of early BPF after pneumonectomy for NSCLC

Objective To evaluate prognostic factors for early bronchopleural fistula after pneumonectomy with non small cell lung cancer,and establish a validated clinical model to estimate the risk of early-BPF.Methods We reviewed the medical records of 429 patients who underwent pneumonectomy for NSCLC at our institution.We used univariate and multivariate analysis to identify potential independent risk factors for early-BPF after pneumonectomy for NSCLC.A model to estimate risk of early-BPF was developed by combining independent risk factors.Results The rate of early-BPF after pneumonectomy for NSCLC was 6.5% (28/429).Three factors were independently associated with early-BPF:neoadjuvant therapy (HR:2.406),bleeding (HR:2.171)and diabetes (HR:1.144).A scoring system for early-BPF was developed by assigning 2 points for each major risk factor (neoadjuvant therapy and bleeding) and 1 point for each minor risk factor(diabetes).Scores were grouped as low (0-1),intermediate (2-3),and high (3),yielding the rate of early-BPF was 14%,27%,and 43%,respectively.Conclusion This clinical model is established on the basis of independent risk factors.This model can be used as a predictive tool for early-BPF after pneumonectomy for NSCLC.
10.Clinical study on peripheral T lymphocyte subsets in children with hand-foot-and-mouth disease

Objective To evaluate the dynamic changes of T lymphocyte subsets in children with hand-foot-and-mouth disease(HFMD)and to provide new evidence for the therapy and prognosis.Methods Peripheral venous blood samples of 346 HFMD cases in acute stage who were hospitalized in Beijing Ditan Hospital from May 1,2008 to August 31,2008 were collected and T lymphocyte subsets were assayed by flow cytometer.Meanwhile,T lymphocyte subsets of 67 HFMD cases in recovery phase were also detected.The pathogens were determined by reverse transcriptionpolymerase chain reaction(RT-PCR)using pharynx swab samples from 99 cases.Different samples were compared by independent-sample t test,paired t test or variance analysis.Results The average levels of T lymphocyte subsets of HFMD children in different agc groups were all lower than reference levels of healthy children in according age groups.In severe cases.T lymphocyte(TL)/lymphocyte (L)ratio in all age groups,helper T cell(Th)/L ratio in children older than 1 year,TL,Th and Th/suppressor T cell(Ts)ratio in children of 1-2 years old were all lower than those in common eases (P<0.05).The Th/L ratio tended to increase with the disease progression.Ratios of TL/L and Th/L in common cases were increased in recovery phase(TL/L:56.3±8.6 vs 61.1±9.1,t=2.56,P<0.05;Th/L:30.2±7.2 vs 34.9±7.9,t=2.90,P<0.05)and all indices of severe cases except Ts/Lratio and Th/Ts ratio increased apparently in recovery phase(P<0.01).TL[(1.738±0.976)×10~6/Lvs(2.696±1.946)×10~6/L,t=2.17,P<0.05],Th/L ratio(25.9±7.0 vs 30.2±7.2,t=2.34,P<0.05),Th[(0.864±0.550)×10~6/L vs(1.459±0.879)×10~6/L,t=2.90,P<0.01]and L[(3.352±1.458)×10~6/L vs(4.664±2.435)×10~6/L,t=2.32,P<0.05]of severe cases in acute phase were all lower than those of common cases(P<0.05),while those were not significantly different in recovery phase between two groups(P>0.05).The T lymphocyte subsets of enterovirus(EV)71 positive cases were lower than EVT1 negative cases,but there was no significant difference between these two groups(P>0.05).Conclusion T lymphocyte immune responses may be correlated with HFMD onset and progression.
